Blood levels and urinary excretion of thiamin and riboflavin during oral administration of multivitamin tablets to healthy adults.

1. The baseline blood thiamin concentration increased gradually during the long-term administration of TTFD, and the blood concentration was dose-dependent. The cumulative urinary thiamin excretion showed the same dose-dependence as the blood concentration. 2. The baseline blood riboflavin concentration also increased steadily during the administration period, but the increase was less than that of thiamin. Dose-dependence of the cumulative urinary excretion of riboflavin was noted. 3. No side effect were observed.

[Studies on the function of mast cells infiltrating in nasal polyps].

The function of nasal polyp mast cells has not been elucidated despite the large number of these cells observed in tissues. We examined these mast cells histochemically, immunohistologically and functionally. Ninety-three percent of collagenase dispersed cells in a nasal polyp were formalin-sensitive. These dispersed cells released histamine in reaction to calcium ionophore A23187 in a dose dependent manner, but not in response to C5a, Compound 48/80 or Substance P. From these results, dispersed mast cells from nasal polyps were considered to be analogous to dispersed mast cells from the human lung and nasal mucosa but not those from human skin. On the other hand, in the reaction with anti-human IgE, dispersed mast cells from a non allergic nasal polyp could not be seen to release histamine. In only 2 of 7 patients, could histamine release in response to Japanese red cedar antigen, from mast cells sensitized passively with the serum of Japanese red cedar pollinosis, seen. Using small tissue samples from polyps, histamine was released by anti-human IgE in allergic patients but not in non allergic patients. Immunohistologically in allergic nasal polyps, some IgE positive mast cells could be seen, whereas in non allergic polyps these cells were absent. These observations suggest that mast cells which had accumulated in nasal polyps both with and without allergy were capable of functional histamine release, whereas in the nasal polyps of allergy patients but not in non-allergic patients these cells are involved in IgE mediated reactions.

Risk factors and the effects of xamoterol in idiopathic dilated cardiomyopathy.

A long-term follow-up study was performed for 110 patients with idiopathic dilated cardiomyopathy (DCM) for 34 +/- 12 months (range 3-122 months). Thirteen patients died of heart failure, 15 of sudden death and one of non-cardiac death. The 3- and 5-year survival rates were 78 and 62%, respectively. The important factors in predicting the 3-year survival rate were left ventricular end-diastolic volume index (LVEDVI > or = 150 ml/m2 = 66%, < 150 ml/m2 = 93%, p < 0.01), myocardial cell diameter (> 25 microns = 42%, < or = 25 microns = 87%, p < 0.05) and sustained ventricular tachycardia (VT present = 32%, absent = 85%, p < 0.01). In a prospective study, 26 patients with DCM were given a beta 1-partial agonist, xamoterol (200 mg daily) and were followed for 35 +/- 15 months (6-53 months). The cardiothoracic ratio, left ventricular end-diastolic dimension and exercise heart rate decreased, and the exercise duration, fractional shortening and ejection fraction increased after xamoterol therapy. The 3-year survival rate was 83%. These results suggest that the important factors in predicting the survival rate of DCM patients were LVEDVI, myocardial cell diameter and the occurrence of VT. Adjunctive xamoterol therapy in DCM had a beneficial effect on hemodynamics and symptoms.

Effects of human recombinant IL-1 on d-tubocurarine-induced histamine release from isolated rat peritoneal mast cells.

The relationship between d-tubocurarine (d-Tc) and human recombinant interleukin-1 (rIL-1) was studied on the histamine-releasing property of isolated rat peritoneal mast cells. d-Tc induced histamine release in a dose-dependent manner (1 x 10(-4) M-3 x 10(-3) M) from isolated rat mast cells. Human rIL-1 (0.3-10 ng/ml) potentiated the d-Tc-induced histamine release and shifted the dose-response curve to left without changing the maximum histamine release by d-Tc. The potentiation by human rIL-1 was completely blocked by anti-IL-1-antibody. Human rIL-1 neither released histamine nor affected IgE-related histamine release in isolated rat peritoneal mast cells. Human recombinant IL-2, IL-3, and tumour necrosis factor neither released histamine from isolated rat mast cells nor affected d-Tc-induced histamine release. These results suggested that human rIL-1 might hypersensitize d-Tc receptors distributed on plasma membrane of rat mast cells.

[Polarized light irradiation near the stellate ganglion in a patient with Raynaud's sign].

Polarized light irradiation near the stellate ganglion was performed in a 55-year-old female with Raynaud's sign. She was suffering from cold and numb pain in bilateral fingers for 1 year. Stellate ganglion block and low reactive-level laser therapy near the stellate ganglion were not sufficient to relieve this symptom. Polarized light irradiation near the stellate ganglion induced a sting stimulation and warm sensation in her hands. Thermograms revealed a remarkable increase in temperature of her hands. The results imply that polarized light irradiation near the stellate ganglion increases blood flow of forearms and relieves Raynaud's sign.

Synthesis of halogen-substituted 1,5-benzothiazepine derivatives and their vasodilating and hypotensive activities.

In an attempt to improve the effectiveness and duration of the action of diltiazem (1), a 1,5-benzothiazepine calcium channel blocker, its derivatives (2) with halogen substituents on the fused benzene ring were synthesized. These compounds were evaluated for their effects on vertebral and coronary blood flows and antihypertensive activity. The structure-activity relationships are discussed. The 8-chloro derivative ((+)-2b), the most potent compound in this series, was selected for clinical evaluation as a cerebral vasodilating and antihypertensive agent.

Pulmonary carcinogenicity of 3,9- and 3,7-dinitrofluoranthene, 3-nitrofluoranthene and benzo[a]pyrene in F344 rats.

3,9- and 3,7-Dinitrofluoranthene (3,9- and 3,7-DNF), 3-nitrofluoranthene (3-NF) and benzo[a]pyrene (B[a]P) were tested for pulmonary carcinogenicity by intrapulmonary implantation of the compounds into rat lung. These chemicals were given in various doses as suspensions in beeswax-trycaprylin and the animals were observed for 100 weeks. The control group received no drugs. The incidences of lung tumors were 19/21 (90.5%), 7/10 (70%) and 1/10 (10%) in rats treated with 200, 100 and 50 micrograms of 3,9-DNF, 4/9 (44.4%), 3/10 (30%) and 0/10 (0%) in rats treated with 200, 100 and 50 micrograms of B[a]P, 12/22 (54.5%) in rats treated with 200 micrograms of 3,7-DNF and 1/20 (5%) in rats treated with 1000 micrograms of 3-NF respectively. No lung tumors were found in control rats. The incidence of lung tumors induced by 3,9-DNF was twice as high as that induced by B[a]P, when the equivalent dose levels of the two compounds were compared. Histologically, most of the tumors induced by 3,9- and 3,7-DNF and B[a]P were squamous cell carcinomas.

Tumor promoting potential in male F344 rats and mutagenicity in Salmonella typhimurium of dipyrone.

For assessment of the carcinogenic potential and the mutagenicity of dipyrone, an antipyretic anodyne, -[(2,3-dihydro-1,5-dimethyl-3-oxo-2-phenyl-1H-pyrazol-4-yl) methylamino]-methanesulfonic acid sodium salt monohydrate, three experiments were conducted using dipyrone A produced in Japan and/or dipyrone B obtained from the Federal Republic of Germany. (i) Carcinogenic potential of dipyrone A for rat liver: 8 week old male F344 rats were pretreated with 0.01% diethylnitrosamine (DEN) in drinking water for 2 weeks and, after 1 week of resting, administered 0.4% dipyrone in drinking water, 5 days a week, for 72 weeks. After an 8 week recovery period, all surviving rats were killed at 83 weeks. Hepatocellular carcinomas developed at a higher incidence in the DEN + dipyrone group (18 of 29 rats, 62%) than in the DEN alone group (9 of 29 rats, 31%), the difference being statistically significant (P less than 0.05). No carcinogenic activity of dipyrone was demonstrated in the groups given 0.4% dipyrone for 72 weeks or 0.4% dipyrone for 25 weeks, followed by 0.05% phenobarbital (PB) for 50 weeks. However, glutathione S-transferase P positive (GST-P+) preneoplastic hepatic foci in these groups were observed at a higher incidence than in the untreated control group (P less than 0.01). (ii) Effect of dipyrone A and dipyrone B on induction of DEN-initiated GST-P+ hepatic foci in a medium-term bioassay system: 0.4% dipyrone A in drinking water and 0.57% dipyrone A or dipyrone B in powdered diet after DEN initiation had similar enhancing effects on the development of GST-P+ foci (P less than 0.001). (iii) The Ames mutation test in Salmonella: both dipyrone A and dipyrone B proved weakly mutagenic for strain TA100 in the presence or absence of S9 fraction.

Arterial tonometry for noninvasive, continuous blood pressure monitoring during anesthesia.

Arterial tonometry is a technique used to measure arterial blood pressure noninvasively. The authors developed a new tonometer system containing an array of 15 piezoresistive pressure transducers, a mechanical positioning system, signal conditioning and multiplexing electronics, and a display and control console. The authors evaluated the accuracy, reliability, and clinical acceptability of this system by comparing tonometric blood pressure measurements with intraarterial blood pressure measurements in 60 anesthetized patients. Blood pressure was measured intraarterially in either the right or left radial artery by a Gould P23XL calibrated transducer, whereas blood pressure was measured by tonometer at the radial artery of the other arm. The tonometric waveform was similar to the intraarterial waveform. Simultaneous tonometer and intraarterial systolic blood pressures of the 60 patients (3,036 data sets) had an overall regression coefficient, r = 0.97, and an equation, regression equation = 0.95X + 5.8. Similar values were obtained for mean and diastolic pressures. Regression analyses of the paired tonometric and intraarterial blood pressure values showed good correlations in both sexes and in ages ranging from 8 to 82 yr (r = 0.94-0.97). Mean absolute values of error (precision) for the systolic, mean, and diastolic measurements did not differ significantly among the five systolic, five mean, and four diastolic pressure groups and ranged from 3.6 to 6.6 mmHg, with negligible bias, with intraarterial pressure used as the reference. Bias for the various pressure groups was small: -0.9-3.6 mmHg for systolic; -3.0-0.7 mmHg for mean; and -2.1-4.5 mmHg for diastolic. The "limits of agreement" (mean difference +/- two standard deviations) were within an acceptable range for clinical anesthesia.(ABSTRACT TRUNCATED AT 250 WORDS)

Changes in nasal metachromatic cells during allergen immunotherapy.

We have investigated changes of nasal metachromatic cell number, nasal symptoms and nasal provocation at the third and sixth month during allergen immunotherapy. Twenty-five subjects with perennial allergic rhinitis (house dust (23), Alternaria (2) were divided into two groups: an immunotherapy-treated group (n = 14) and a control group (n = 11). At the first visit nasal symptom scores, nasal provocation reactions and the number of metachromatic cells in nasal mucosal epithelial scrapings were not significantly different between groups. At the third and sixth month after immunotherapy nasal symptom scores, nasal provocation and the metachromatic cells in epithelial scrapings were significantly reduced (P less than 0.05) compared with the pretreatment values in the immunotherapy group, but unchanged in the control group. These results suggest that the reduction in metachromatic cell number at the nasal mucosal surface may be one of the mechanisms which could explain the improvement of nasal allergic symptoms by immunotherapy.

Blood pressure measurement by arterial tonometry in controlled hypotension.

A newly developed arterial tonometer enabled us to measure the blood pressure waveforms in addition to determining systolic and diastolic pressures noninvasively and continuously. Twenty-eight adult patients undergoing orthopedic surgery under controlled hypotension were studied. Systolic blood pressure was reduced to two-thirds of baseline values with an infusion of nitroglycerin during nitrous oxide/enflurane anesthesia. Intraarterial blood pressures were simultaneously measured in either the right or the left radial artery with a cannula and a Gould P23XL calibrated transducer; tonometric monitoring was performed on the contralateral radial artery using a Colin CBM-3000 instrument. The outputs of the two blood pressure measurement instruments were recorded for later data analysis. The shape of the tonometric pressure waveform was nearly identical to the waveform recorded intraarterially even during controlled hypotension. Regression analyses of 2039 paired tonometric and intraarterial blood pressure values during the hypotensive period showed good correlations (r = 0.78 for systolic, r = 0.81 for mean, and r = 0.70 for diastolic pressures). The accuracy of systolic, mean, and diastolic readings was from 4 to 7 mm Hg with negligible bias and did not differ significantly among six systolic, four mean, and four diastolic pressure groups. Our results indicate that arterial tonometry can provide accurate, reliable, and real-time monitoring of blood pressure even during controlled hypotension.

[Allergic rhinitis nasal epithelial cell conditioned medium stimulates growth and differentiation of basophil/mast cell and eosinophil progenitors from atopic blood].

We examined the conditions, kinetics and cell sources for basophil/mast cell and eosinophil colony stimulating activity (CSA) production by nasal epithelial cells taken from house dust (HD) nasal allergic patients. Epithelial scrapings removed from HD nasal allergic patients were grown to confluence over 7 days as a monolayer of epithelial cells in medium supplemented with fetal calf serum (FCS) on collagen coated microwell plates. CSA in nasal epithelial cells conditioned medium (CM) was assessed with density-gradient separated, nonadherent peripheral blood mononuclear cells in 14-day and 21-day methylcellulose assays. In the 14-day methylcellulose assays, the number of Eo-type colonies in the presence of either 10% or 5% CM was significantly higher than the background number of Eo-type colonies (negative control). Comparison of Gm CSA among 1%, 5% and 10% CM with negative, revealed no significant differences. We also compared the Eo-type CSA in the presence of 10% significant differences. We also compared the Eo-type CSA in the presence of 10% CM in the 14-day methylcellulose assay with 21-day methylcellulose assay. There was no significant difference in the number of Eo-type colony between the 14-day and 21-day methylcellulose assays. We also examined the composition of the cells in the colonies in the 14-day and 21-day methylcellulose cultures. The percentage of metachromatic granule containing cells in a Eo-type colony in 14-day methylcellulose assay was significantly higher than in a Eo-type colony in 21-day methylcellulose assay.(ABSTRACT TRUNCATED AT 250 WORDS)

[Disorder of serum electrolytes following CAP therapy].

Combination chemotherapy with Cyclophosphamide, Adriamycin and Cisplatin (CAP) is one of the most effective chemotherapies for ovarian cancer. Severe vomiting induced by this treatment may cause a disorder of the serum electrolytes. Severe hyponatremia was observed in one patient who complained of convulsions and unconsciousness. Accordingly we studied the disorder of serum electrolytes in 158 courses of CAP treatment (44 patients) during the five years from 1984 to 1988. The serum electrolytes were influenced by the amount of vomiting in this study. Consequently frequent examination of serum electrolytes is necessary during this treatment.
