Lymphocytic and erythrocytic k+, Na+, Ca++, Mg++ content in patients with congestive heart failure.

The content of lymphocytic and erythrocytic K+, Na+, Ca++, Mg++ changed in patients with congestive heart failure, related to clinical treatment and prognosis. The longer the course of disease, the more significant the changes, with no relation to the etiology of heart disease. The changes were partially improved after treatment of the heart failure. The content of electrolytes is not significantly different in patients in NYHA Class III, compared to those in NYHA Class IV. The metabolic changes of electrolytes occur on the myocardial cells, skeletal muscle cells and peripheral erythrocytes in patients with congestive heart failure. However, serum electrolyte content does not reflect precisely overall electrolyte metabolism in the body. Whether similar changes of electrolyte content occur in the peripheral lymphocytes is rarely reported. We studied the lymphocytic and erythrocytic electrolyte content in patients with congestive heart failure, to provide some reference in the clinical treatment and prognosis of congestive heart failure.

[Different expressions of alpha 2 (IV) and alpha 3 (IV) collagen mRNAs in renal glomeruli of IgA nephropathy].

The levels of extracellular matrix protein mRNAs were increased prior to the appearance of histological glomerulosclerosis in isolated glomeruli from several animal models. Striker studied renal biopsies (Seminar in Nephrology, 13:508,1993). We made a comparable study in human renal biopsies to assess the levels of glomerular alpha 2 (IV) and alpha 3 (IV) collagen gene expressions in renal biopsies. Renal biopsy specimens were obtained from 11 patients (7 IgA nephropathy and 4 mesangial proliferative glomerulonephritis). Glomerular lesion was divided into 5 grades. The amount of collagen type IV in glomerular matrix was evaluated by immunoperoxidase staining using a monoclonal antibody to type IV collagen. For gene expression analysis, glomeruli were dissected out from about 10% of each biopsy specimen. The expressions of alpha 2 (IV) and alpha 3 (IV) collagen mRNAs were analyzed by using in situ reverse transcription coupled with polymerase chain reaction. Each PCR reaction contained an amount of cDNA template equivalent to that obtained from 1/10 of a glomerulus, alpha 2 (IV) and alpha 3 (IV) collagen mRNAs were detected in all specimens. All 3 cases of IgAN with grade III glomerular lesion had a more marked increment of glomerular alpha 2 (IV) collagen cDNA than that of the other 3 IgAN patients with grade II lesion (the intensity of PCR product, grade III 0.54 +/- 0.12, VS grade II 0.25 +/- 0.08, P < 0.05). No discrepancies were detected in the level of glomerular alpha 3 (IV) collagen cDNA and the intensity of collagen IV staining between these two groups of IgAN. Interestingly, in MsPGN patients there was no difference in the expression of glomerular alpha 2 (IV) collagen mRNA between grade II and grade III glomerular lesions. The results suggested that the detection of glomerular alpha 2 (IV) collagen mRNA level in IgAN patients may help to define the pathological classification.

[Dynamic observation and significance of autoantibody against ADP/ATP carrier in dilated cardiomyopathy].

The presence and dynamic change of autoantibody against ADP/ATP carrier in patients with dilated cardiomyopathy (DCM) was studied by using indirect micro-solid-phase radioimmunoassay. A significant antibody titre was present in 16 of 48 DCM patients and most of them were in early stage of the disease. The titre of anti-ADP/ATP carrier antibody in DCM patients decreased gradually in a follow-up period of three months. The titre of this antibody in the serum from patients with coronary and rheumatic heart disease were within normal limits. Anti-ADP/ATP carrier antibody may play an important role in the pathogenesis, diagnosis and treatment of DCM.

Divergent regulation of cytochrome P450 enzymes by morphine and pethidine: a neuroendocrine mechanism?

We have studied the effects of opiates on cytochrome P450 (CYP) expression in rats and, intriguingly, found that these compounds have divergent effects on this system. Morphine induced a selective down-regulation of hepatic 16 alpha-hydroxylation of androstenedione, which was associated with a reduction in the expression of the 16 alpha-hydroxylation-associated CYP2C11. In addition, other enzymes from the CYP2C, CYP3A, and CYP4A gene families were also suppressed, whereas CYP1A2, CYP2B1, and CYP2E1 were induced. These changes were reflected in the mRNA levels, indicating that they were due to alterations at the transcriptional level. alpha 2-Adrenoceptor blockade with yohimbine augmented the effects of morphine on proteins identified by antibodies against CYP2C11 and CYP3A, indicating an involvement or influence of the alpha 2-adrenergic receptor system. In contrast, pethidine had completely different effects. No CYPs were suppressed by this compound. CYP1A2, CYP2B1, CYP2C6, CYP2C7, CYP3A, and CYP4A1 were all induced by pethidine treatment, whereas CYP2C11 and CYP2E1 were unchanged. Our results demonstrate for the first time the divergent effects of pethidine and morphine on the hepatic CYP system, indicating that an opioid receptor mechanism is not involved. These differences are probably mediated by different influences on growth hormone secretion. The mechanisms by which this may occur are discussed.

Experimental tracheal revascularization and transplantation.

Vascularized tracheal allografts would expand the domain of tracheal reconstructive surgery. The functional capability of vascularized tracheal allografts is closely correlated with immunologic parameters contributing to graft survival, about which little information is currently available. An orthotopic transplant model was developed that allows assessment of allograft function by examining healing at the graft-host junction and graft integrity during normal respiration.

Enhanced interferon-gamma production and B7-2 expression in isolated intestinal mononuclear cells from patients with Crohn's disease.

Many of the in vivo activities of interferon (IFN)-gamma match the changes found in inflammatory bowel disease, but its importance is controversial. Interferon (IFN)-gamma induces the expression of B7-2 costimulatory molecules on monocytes. We measured levels of IFN-gamma production in intestinal mucosa and isolated lamina propria mononuclear cells (LPMC) in patients with Crohn's disease (CD) and ulcerative colitis (UC). We also investigated the induction of B7-2 on the LPMC by flow cytometry. Mucosal IFN-gamma production was higher than the control level in patients with CD, but this was not the case in UC. IFN-gamma production and B7-2 expression in the LPMC of CD were higher than in the LPMC of UC and controls, and high levels of B7-2 expression were observed on the LPMC of CD after incubation with endotoxin. The induction of B7-2 on the LPMC of CD may provide a mechanism for the amplification of T cell proliferation and lymphokine production by IFN-gamma-activated LPMC.

Major histocompatibility complex class I presentation of exogenous and endogenous protein-derived peptides by a transfected human monocyte cell line.

Monocyte/macrophages are professional antigen-presenting cells of the cellular immune system, serving to generate peptides for major histocompatibility complex (MHC) class II-restricted recognition by CD4+ T-lymphocyte effector cells. Antigen presentation by these cells involves the internalization of extracellular proteins and their fragmentation within vacuolar compartments. The resulting peptides become associated with MHC class II molecules. The final destination of exogenous peptide antigens, however, is not absolute in monocytes. Processed peptides, derived from exogenous proteins, can also associate with MHC class I molecules. To study simultaneous presentation of peptides derived from exogenous and endogenous proteins by human leucocyte antigen (HLA) class I molecules, we isolated the peptides from a human immunodeficiency virus nef transfected U937 monocytic cell line. The HLA class I-bound peptides were separated by reverse phase-high performance liquid chromatography. Comparison of the peptide sequence data with protein databases revealed that the peptides derived from extracellular, as well as intracellular, proteins, suggesting that monocytes have a more generalized MHC class I antigen-processing pathway than previously documented.

[Electron microscopic observation of wound healing process of the laryngeal mucosa in guinea pigs].

The ultrastructural changes of wound healing process following abrading of the laryngeal mucosa with a smooth file in 16 guinea pigs were observed by scanning and transmission electron microscopies. The results showed that: 1. the repair of the laryngeal mucosa epithelial defect was initiated from mobilization and migration of residual basal cells in the surrounding tissue onto the denuded epithelial surface; 2. the regenerated epithelium of migration covered small part of the denuded epithelial surface 24 hours after sustaining the injury; 3. five days later, all denuded epithelial surface was covered by the regenerated epithelium; 4. differentiation of the regenerated epithelium began 10 days after abrasion, and was completed 21 days after abrasion; 5. tonofilaments of distributed the minority, permeation, and concentration were seen in those cells of the regenerated epithelium respectively on the 1st, 5th and 10th days after abrasion. Particularness of differentiation of the regenerated epithelium was discussed.

[Ultracytochemical localization on Na(+)-K(+)-ATPase activity in the guinea pig endolymphatic sac].

It is well-known that Na(+)-K(+)-ATPase plays an important role in active transportion of sodium (Na) and potassium (K) ions across the epithelium. This paper reports the ultrastructural localization of the K(+)-NPPase reflecting Na(+)-K(+)-ATPase activity in the guinea pig endolymphatic sac (ES) with the lead citrate one step method. The ultrathin sections viewed in the transmission electron microscope revealed that the reaction product was restricted to the cytoplasmatic side of the basolaterl plasma membranes of the ES epithelial cells. And the reactivity could be traced all the way up to the level of tight junctions close to the luminal aspect of the cells. On the other hand, the luminal plasma membrane of the epithelial cells showed no enzyme activity. Our findings suggest that the rich Na(+)-K(+)-ATPase enzyme activity is preferentially localized on the basolateral plasma of membrane of the ES epithelial cells. Possible physiological mechanism of Na(+)-K(+)-ATPase in the Na(+)-K+ ion transport of ES epithelial cells is discussed.

An animal model of bullous pemphigoid: what can it teach us?

BP is an organ-specific autoimmune disease characterized by blistering of the skin due to a detachment of the epidermis from the underlying connective tissue layer and by the production of autoantibodies directed against the cutaneous basement membrane zone. A 180-kD epidermal protein, BP180, has been identified as one of the major antigenic targets of these autoantibodies and, interestingly, has been localized to the epidermal hemidesmosome, a cellular structure involved in anchoring basal epithelial cells to the basement membrane. An immunodominant and potentially pathogenic epitope associated with BP has recently been mapped by our laboratory to a site on the extracellular domain of the human BP180 antigen. This antigenic site, designated the MCW-1 epitope, has been shown to be recognized by the majority of sera from patients with either BP or HG, a pregnancy-associated subepidermal bullous disease. Unfortunately, the MCW-1 epitope is absent from the murine BP180 molecule; therefore, human autoantibodies directed against this site could not be tested directly for pathogenicity using the conventional passive transfer mouse model. As an alternative approach, rabbit antibodies were prepared against the segment of murine BP180 that is homologous with the MCW-1 epitope region and were tested for pathogenicity by passive transfer experiments. Remarkably, neonatal mice injected with these antibodies developed a subepidermal blistering disease that faithfully reproduced all of the key immunopathological features of BP and HG--deposition of lgG and complement at the dermal-epidermal junction, inflammatory infiltration of the upper dermis, and frank, subepidermal blistering. These results strongly suggest that the autoimmune response against the human BP180 MCW-1 epitope is relevant in the pathogenesis of blister formation in BP and HG patients. This animal model is currently being used to further dissect the pathophysiology of these autoimmune disorders.

[Reversing effect of dimethyl-4,4'-dimethoxy-5,6,5'6'-dimethylenedioxybiphenyl-2,2'- dicarboxylate(DDB) on the phenotypes of human hepatocarcinoma cell line].

When human Bel-7402 hepatocarcinoma cell line grew in a medium containing 10(-4)M DDB, the secretion of alpha-fetoprotein (AFP) and the activity of gamma-glutamyl-transpeptidase (gamma-GT) were significantly lower than the control cells, whereas the albumin (ALB) secretion and the activity of tyrosine-alpha-ketoglutarate transaminase (TAT) were markedly increased. DDB at the concentration of 10(-4)M could significantly increase the content of cAMP in Bel-7402 cells, and also suppressed the expressions of oncogene c-myc and hepatocarcinoma marker AFP gene and enhanced the anti-oncogene p53 expression. The results of this paper suggest that DDB has some reversing effects on the phenotypes of human Bel-7402 hepatocarcinoma cell line.

[Anti-promoting and anti-mutagenic actions of G9315].

G9315, a complex extracted from Glycyrrhizae inflata Bat (III), consists of 6 flavonoids with significant antioxidant effects. At 1 mg dose it inhibited the mouse ear edema induced by croton oil, and showed strong anti-promoting effects on two-stage carcinogenesis in mouse skin induced by DMBA plus croton oil. The TPA enhanced 32Pi-incorporation into phospholipid fraction in HeLa cells was inhibited, and the micronuclei in mouse bone marrow cells induced by cytoxan was also depressed.
