BIBW22 BS, potent multidrug resistance-reversing agent, binds directly to P-glycoprotein and accumulates in drug-resistant cells.

The expression of P-glycoprotein (P-gp) in tumor cells causes a multidrug resistance (MDR) phenotype. P-gp has been shown to mediate the transport of structurally dissimilar drugs across the cell membrane in an energy-dependent manner. In this report, we show that BIBW22 BS, a phenylpteridine analog, reverses the MDR phenotype of CEM human lymphoma cells in a dose-dependent fashion. Using a photoactive analog of BIBW22 BS {[3H]azido-4-[N-(2-hydroxy-2-methylpropyl)-ethanolamino]-2, 7-bis(cis-2,6-dimethyl-morpholino)-6-phenylpteridine}, we show the photoaffinity labeling of a 170-kDa protein in drug-resistant cells immunoprecipitated with P-gp-specific monoclonal antibodies. The photolabeling of P-gp by [3H]azido-BIBW22 BS was specific and saturable. Furthermore, BIBW22 BS, vinblastine, and verapamil, but not colchicine, inhibited the photolabeling of P-gp by [3H]azido-BIBW22 BS. Drug binding studies showed that membranes from MDR cells bound more BIBW22 BS than parental drug-sensitive cells, and this binding was inhibited with vinblastine and, to a lesser extent, with uridine. However, drug transport studies demonstrated that BIBW22 BS is not a substrate for P-gp efflux pump. Interestingly, BIBW22 BS was shown to accumulate more in resistant cells. Also, BIBW22 BS accumulation in drug-sensitive and -resistant cells was not energy dependent. These results are in contrast with the observed decrease in accumulation or enhanced efflux of [3H]vinblastine seen in the same MDR cells. A comparison of [3H]azido-BIBW22 BS or [3H]azidopine photolabeled P-gp by Cleveland mapping with Staphylococcus aureus V8 protease showed differences in the photolabeled peptides. Taken together, the results of this study show that BIBW22 BS is a potent MDR-reversing agent that binds directly to P-gp but is not effluxed from drug-resistant cells.

Transgenic mice containing the human erythropoietin receptor gene exhibit correct hematopoietic and neural expression.

The erythropoietin receptor (EpoR), known for its role in the proliferation and differentiation of erythroid cells, has been detected in nonhematopoietic tissues. We have reported previously that in addition to hematopoietic expression, EpoR is expressed at high levels in embryonic mouse brain and decreased to nondetectable levels by birth. While using transgenic mice to characterize human EpoR expression, we observed that a 15-kb human EpoR transgene (expressed in hematopoietic tissues but at a reduced level) also exhibited in the embryonic brain low levels of expression that persisted through adulthood. To examine further the basis of tissue and developmental specificity of the human EpoR gene, we produced additional transgenic mice using an 80-kb human EpoR genomic fragment isolated from a P1 phagemid human library. We found that this transgene is expressed appropriately in hematopoietic tissues (including yolk sac, fetal liver, adult spleen, and bone marrow) at levels comparable to the endogenous murine EpoR. The 80-kb transgene also provided high-level expression in the embryonic brain that paralleled the levels of the endogenous murine EpoR and was no longer detectable after birth. These data suggest that the high level of embryonic brain expression may be relevant to the human EpoR gene and that the transgenic mouse with an 80-kb fragment is a suitable model for studying the regulation and possible functional importance of human EpoR expression in the developing embryo.

Effect of protection and repair of injury of mitochondrial membrane-phospholipid on prognosis in patients with dilated cardiomyopathy.

We have already proved that the mitochondrial membrane-phospholipid (MMP) injury changes of peripheral lymphocytes in patients with heart failure can be used as an injury indicator of myocardia, and are related to the long-term prognosis. In the present study, MMP localization of the peripheral lymphocytes was performed by modified Demer's tricomplex flocculation method, and we compared the changes, after classification, between the pre-treatment and the 12-week post-treatment, of coenzyme Q10 (Co.Q10) and captopril in 61 hospitalized patients with dilated cardiomyopathy (DCM). They were followed up for 16.1 +/- 7.8 months (mean). The results showed that compared with the placebo, Co.Q10 and captopril could significantly protect against and repair MMP injury and improve the heart function of patients with DCM after 12 weeks, and the 2-year survival rate rose significantly by 72.7% for Co.Q10, and 64.0% for captopril, vs 24.7% for placebo. As for Longrank test, X2 equals 4.660 and 6.318, respectively, with both p < 0.05. The aforementioned results indicate that MMP injury of peripheral lymphocytes can predict the prognosis of the patients with DCM, thus the protection and repairment of MMP injury can improve the life-quality and prolong the life-span of the patients.

The effect of a combined approach to schistosomiasis control on the transmission of Schistosoma japonicum in Xingzi of Poyang Lake area, China.

The impact of a combined approach to schistosomiasis control from 1987 to 1989 and mass chemotherapy from 1992 to 1994 was studied in a rural community in Xingzi county in the northwest corner of Poyang Lake in Jiangxi Province, China. Humans, cattle, buffalo and pigs were known potential reservoirs of Schistosoma japonicum. Transmission occurs during water contact on and around seasonally flooded marshes that are used for grazing, the harvesting of grass and fishing. Humans and livestock underwent yearly selective mass chemotherapy, and snails were eliminated through ploughing and compacting of the marshland in the spring of 1988. Transmission was monitored through the determination of annual re-infection rates in samples of the human population, the annual examination of piles of feces from animals and humans in the marshland, the annual collection and examination of intermediate snail hosts, and the exposure to potentially polluted water and subsequent examination of sentinel mice. Schistosomiasis prevalence among humans and animals declined sharply as soon as mass chemotherapy was implemented. Snail density decreased even before mollusc control was started, possibly indicating a high variability of this indicator. The infection rates of snails and sentinel mice reached zero after a single application of mollusc control. The results underline the importance of single infected water buffalo for the transmission of schistosomiasis. Since the impact stopped for two years (1990-1991), the schistosomiasis prevalence rose quickly. Mass chemotherapy was an effective means to curb the prevalence of schistosomiasis in this area, but the effects were only maintained for one or two years in the marsh zone.

Multichannel flow electrophoresis and its applications in purification of proteins, enzymes, and antibodies.

Multichannel flow electrophoresis (MFE) is a novel preparative electrophoresis technique designed for continuous separation of proteins based on their different isoelectric points. A computer controlled MFE apparatus is developed and the effects of operation parameters such as electric field strength, buffering pH, and sample introducing flow rate on MFE separation efficiency are investigated. The application of MFE is first exemplified by continuous separation of BSA and HBB mixture. Then continuous purification of urokinase from urine extracts is conducted, in which 10 x 10(4) IU urokinase product whose specific activity is higher than 40,000 IU/mg is obtained per hour. Finally, MFE is applied in the continuous purification of anti-urokinase mouse IgG from mouse serum and yields 2.1 mg mouse IgG per hour with high purity demonstrated by SDS-PAGE. The preliminary results presented have confirmed the workability of MFE and pointed out its high application potential in biochemical process.

Studies on prophylactic effect of artesunate on schistosomiasis japonica.

OBJECTIVE: To examine the prophylactic effect of artesunate on schistosomiasis japonica by killing schistosomula. METHODS: Mice, rabbits and dogs after infection with cercariae of schistosoma japonicum (S. japonicum) were treated with artesunate on the 7th day at a dose of 300 mg/kg, 20-40 mg/kg and 30 mg/kg respectively once a week for 4-6 weeks. A double-blind test was used. A total of 864 persons in highly endemic areas for schistosomiasis were administered either artesunate at a dose of 6 mg/kg once a week for 8 weeks or identical placebo with the same dose-schedule during transmission season for S. japonicum. Four weeks after the last dosing, fecal examinations for S. japonicum eggs and miracidia were carried out to evaluate the prophylactic effect. RESULTS: Worm reduction rates in mice, rabbits and dogs were 77.50-90.66%, 99.53% and 97.10% respectively. All of the 467 residents in 2 trials were free from eggs or miracidia upon stool examinations whereas in the control groups with placebo, 15 out of 218 (6.9%) and 26 out of 179 (14.5%) were stool eggs and/or miracidia positive in the first and second trial, respectively. Side effects were mild. No significant changes in routine blood and urine tests, electrocardiogram, hepatic and renal functions were observed after artesunate administration. CONCLUSIONS: Animal experiments and field trials have demonstrated good efficacy of artesunate on killing schistosomula with little side effects. Thus, the drug is suggested to be used on the population in endemic areas for prevention of schistosomiasis.

[Single-dose intramuscular methotrexate for treatment of ectopic pregnancy].

OBJECTIVE: The efficacy and indication of single-dose intramuscular methotrexate to treat early ectopic pregnancy were explored. METHODS: 27 cases of ectopic pregnancy were treated by single-dose intramuscular methotrexate (50 mg/m2) without citrovorum rescue. The beta-hCG was monitored regularly till it became normal. Twelve of them recieved this regimen in outpatient service. RESULTS: 24 cases (88.9%) were successfully treated. Three failed and switched to operation. There were no significant differences (P > 0.05) in gestational age and size of adnexa ectopic mass between successful cases and failed cases, but highly significant differences (P < 0.01) in occurrence rate of abdominal pain and beta-hCG titers before treatment. CONCLUSION: The early diagnosis and adherence to strict criteria are the keys to successful management. No abdominal pain, ectopic mass < or = 5 cm in greatest dimension and titer of serum beta-hCG < 6000 IU/L were mainly indications of drug treatment.

[Immune-mediated tubulointerstitial injury in hemorrhagic fever with renal syndrome].

OBJECTIVE: To study the presence of Hantan virus RNA and specific immune-complex deposits in the renal tubular epithelium of HFRS patients. METHODS: Renal biopsy specimens from 15 cases of HFRS, 3 cases of ischemic ATN and 5 normal controls (donor of transplantation) were investigated. Human HLA-DR, ICAM-ILFA-1 and CD4+, CD8+, CD8+ cells in the renal tissue section were quatitatively analyzed by immuno-peroxidase staining 4 layers PAP. Histo-morphometric analysis of pathological section was made for each specimen. RESULTS: HFRS presented abundant amount of HLA-DR and ICAM-1 in tubular cells both on cortical and medullary region, while ischemic ATN tended to have such positive results only in cells of cortical region. The intensity and extent of HLA-DR and ICAM-1 LFA-1 staining were closely correlated with the degree of tubulointerstitial damage of HFRS. Interstitial CD4, CD8 and monocytes increased in amount profoundly in HFRS with a preponderance in the medullary region, and particularly of CD8 positive cells. Less marked increment occurred in ATN cases, and no discrepancies were found between cortical and medullary region or cell types. The severity of tubulointerstitial lesion was closely associated with the extent of cellular infiltration and the expression of HLA-DR, ICAM-1 in renal tubular cells. CONCLUSION: The characteristic changes of tubulointerstitial lesions in HFRS suggest that virus infection may be one of the mediators responsible for the immune kidney injuries in HFRS.

[The sequences of intron 50 and 51 of DMD gene and the deletion mechanism of 51st exon].

OBJECTIVE: To investigate the relationship between the structural characters of 50th and 51st introns and deletion mechanisms of 51st exon of DMD gene. METHODS: We cloned the 3.1 Kb-Hind I human genome DNA fragment containing the 50 and 51 introns of DMD gene, sequenced the fragment completely, and analysed the character of the sequences with computer. RESULTS: The length of the fragment was 3179, including the entire 51st exon and partial 50th and 51st intron sequences. There were 36 short tandom repeats, 22 direct and inverted repeats, and 8 homologous sequences in the 50th and 51st introns. A TATA box was found in the intron 50 and it may be another new promotor of DMD gene. CONCLUSION: The hypothesis, which the recombination of repeat sequences may result in the deletion of 51st exon, is put forward.

[The effects of lipoteichoic acid and calcium on the adherence of oral streptococci in vitro].

In this study, the effects of lipoteichoic acid (LTA) and calcium on adherence of oral streptococci in vitro were investigated. The results revealed that the adherence of streptococcus sobrinus to a glass surface was increased by the addition of calcium, in the condition of the same calcium ion concentration, LTA inhibited adherence, suggesting that LTA and the bacterial cell surface LTA competed for the same site of calcium ion, providing some evidence to support calcium-bridging theory. The result also showed that calcium ion not only influenced sucrose-independent adherence, but also infulenced sucrose-dependent adherence. The effect of calcium ion on the adherence has a threshold, over which the adherence was reduced.

[Constant composition study on kinetics of enamel demineralization].

The aim of this work was to investigate the kinetic behaviour of whole enamel demineralization compared with that of powder enamel. Constant composition technique was used to maintain a constant undersaturation degree with respect to hydroxyapatite in the demineralizing solutions. The demineralizing processes were monitored by recording the volume of adding titrant solution. Finally, sections were measured for lesion depth by microradiography, and blocks were tested for microhardness. The results showed that demineralization of enamel powder nearly completed within 40 minutes, while demineralization of whole enamel was much slower and with a constant rate within the experimental time. It was also found there were significant increases of concentrations of calcium and phosphate in solutions during the first 40 hours of whole enamel demineralization. It suggests that the kinetic behaviour of whole enamel demineralization is different from that of powder enamel, which is not only due to the unique transport process in whole tissue, but also possible due to a different phase transformation before and after formation of a surface layer.

[Stereotactic intratumoral irradiation of huge craniopharyngioma].

Craniopharyngioma, often with cystic diliatation, is difficult to resect radically. Fifty patients with huge craniopharyngioma (diameter of tumor was over 5 cm) treated with intratumoral irradiation of radioactive isotopes (32P and 90Y) through CT-guided Leksell stereotactic system are reported. The patients were 2 to 69 years of age with the disease of 1- to 12-year duration. Of the 50 patients, 21 had recurrent tumor after craniotomy, 29 without surgery operation before. All tumors were confirmed pathologically. The major clinical symptoms were as follows: visual field defect, headache, vomiting, diabetes insipidus, hemiplegia and growth retardation in juvenile cases. There was no death or serious complications following the treatment procedure. Partial response (> 50% reduction of tumor size on CT scanning with improvement of symptoms and signs) rate of the treatment was 82.0% at one month and 62.0% at 2 to 7 years after treatment.

[Long-term observation of the treatment of blepharoptosis by frontalis-aponeurosis suspension].

During the past 6 years, 78 cases of blepharoptosis were treated with the forked frontalisaponeurosis suspension technique. The main characteristic of this procedure was that the frontalis-aponeurosis was properly cut longitudinally so that the upper eyelid was elevated completely and the elevating force scattered. Thirty-six cases were followed up, with the longest time being 75 months. Most patients were satisfied with the results. We conclude that this procedure is the first choice for moderate or severe blepharoptosis or recurrent blepharoptosis after other operations.

[Inhibition of human poorly differentiated nasopharyngeal carcinoma cell line CNE-3 growth by antisense EB virus LMP-1 gene].

A recombinant retroviral vector containing Epstein-Barr virus (EBV) LMP1 antisense sequences was constructed to evaluate the posibility of gene therapy in nasopharyngeal carcinoma. The vector was packed with PA 317 cells as a pseudoretroviral one. Immunofluorescence examination showed it can evidently suppress the activation of B95-8 cell EBV. The inhibition rate was 71%. We have successfully using it to infect human poorly differentiated nasopharyngeal carcinoma cell line CNE-3, its growth was lowered down at a rate of 70%. The ability of clone formation in soft agar, tumorigenicity in nude mice of CNE-3 were markedly dropped. By Southern blot, there was high level of retroviral vector pZIP neo gene present in the transfection of tumor cells which confirmed that antisense virus LMP1 can suppress the growth of CNE-3 cells and the tumorigenicity in nude mice. Reversly it also confirmed the relationship between EBV and nasopharyngeal carcinoma and provided experimental basis for the gene therapy of the nasopharyngeal carcinoma.

[A study of gingivain from extracellular vesicles of Porphyromonas gingivalis W50: purification and cytotoxic characterization].

Gingivain was purified from extracellular vesicles of Porphyromonas gingivalis W50 with high performance liquid chromatography (HPLC) and its cytotoxic characterization on L929 cell line was examined. The result showed that gingivain might belong to systeine proteinases. The molecular weight of gingivain was about 27,800 unit by HPLC in which there might be one or two peptides being 10,900 unit, the enzyme was activated by Ca2+ and thiol-containing agents such as dithiothycitol and inhibited by Na-4-tosyl-L-lysine chioromethyl and EDTA. The maximum activity of the enzyme were found at pH 7.5, purified gingivain was toxic to L929 cell line at the content of 10 micrograms/ml. The study indicates that gingivain carried by vesicles might be more important in bacterial toxic activities.

[A bacteriological study of frontal deciduous dental caries in children].

Bacteriological factors of caries in anterior deciduous teeth were studied by analysing the flora in 25 children, aged 3 to 4 years. Samples were collected from the caries of labial surfaces of maxillary anterior deciduous teeth, including caries lesions, white spot margins of these lesions and caries-free smooth enamel surfaces. Plaques were obtained from the cervical third areas of the maxillary labial anterior deciduous tooth surfaces in caries-free children and served as controls. The results show the detectable rate of S. mutans and their amount against total streptococci in the 3 sampling sites are both significantly higher than that of caries-free children. This suggests S. mutans may be the major pathogenic bacteria of caries of anterior deciduous teeth.
