Arteriovenous malformation of the spinal cord and cauda equina fed by branches of the internal iliac artery and associated with vertebral hemangiomas.

A rare case of an arteriovenous malformation (AVM) of the spinal cord is presented. The patient was a 35-year-old man who experienced progressive paraparesis and rectovesical disturbances. In this patient a spinal AVM was fed by branches of the left internal iliac artery and was associated with a sacral hemangioma. An additional hemangioma of the third lumbar vertebral body was also found. On reviewing the literature, only 4 cases, including ours, of spinal AVM with feeding vessels originating from the internal iliac artery were found. One of these patients had an associated vertebral hemangioma.

Protection of ischaemic myocardium with coenzyme Q10.

Protection of the ischaemic myocardium with Coenzyme Q10 (COQ10) following constriction of left anterior descending coronary artery was studied in 38 open-chest mongrel dogs. Left anterior descending coronary artery blood flow was independently reduced by tightening a constrictor around the vessel. Aortic pressure and left ventricular pressure were measured, and the Tension Time Index (TTI) was calculated. Myocardial segment shortening in the ischaemic area was measured with an ultrasonic dimension gauge. After obtaining haemodynamic data, the ischaemic myocardium was biopsied and the tissue sample was frozen in liquid nitrogen for adenosine triphosphate (ATP) analysis. Animals premedicated with COQ10 (20 mg . kg-1 iv) had a significantly higher ATP content in ischaemic myocardium (3.25 +/- 0.35 mumol . g-1 wet weight) compared with that in ischaemic myocardium of control animals (2.96 +/- 0.19) (P less than 0.05), when coronary blood flow was reduced by 50 to 74%. There were failure of ATP preservation by COQ10 with a further reduction in coronary blood flow. Changes in the pattern of myocardial segment shortening in the ischemic area induced by a 50 to 74% coronary blood flow reduction were less in COQ10 treated animals than in control animals. Since there were no haemodynamic differences between control and COQ10 treated animals, these effects were considered not to be due to a decrease in myocardial oxygen consumption secondary to haemodynamic changes.

Mechanism of supraventricular parasystole.

Two patients with supraventricular parasystole (one atrioventricular and one auricular) are reported. In both patients, reentrant extrasystoles appeared to occur as the result of Mobitz type I second-degree entrance block. We believe that when a sinus impulse fell soon after the absolute refractory period of the pathway containing the parasytolic focus, it reached and discharged the focus after marked delay, and thereafter became a reentrant extrasystole. In interectopic intervals containing more than one sinus beat, the number of intervening sinus beats was always even, suggesting the presence of concealed reentrant extrasystolic bigeminy. The observations in the present report and in our previous patients with ventricular parasystole strongly suggest that most cases of parasystole, whether ventricular or supraventricular, or whether intermittent or "continuous," may be governed by second-degree entrance block.

Electrical activity of the infundibulum in relation to ovulation in the chicken.

Electrical activity was recorded from the smooth muscle layer of the infundibulum of unanaesthetized hens during the ovulation cycle. There were low-frequency isolated spike potentials throughout the cycle except for the period near ovulation. There were intermittent bursts of spike potentials about 1 h before ovulation and then an increased frequency of spike potentials near the time of ovulation. These may be related to the activity of the infundibulum engulfing the shed ovum. The frequency of electrical activity tended to decrease shortly after ovulation, but then increased again for a few hours. Premature ovulation induced by progesterone i.m. was associated with similar electrical changes.

Relationship between changes in regional myocardial contraction and energy metabolism following coronary flow reduction.

To investigate the relationship between mechanical and metabolic deterioration in the ischemic myocardium following a decrease of coronary blood flow, the patterns of regional myocardial contraction and myocardial adenosine triphosphate (ATP) content were analyzed simultaneously in canine left ventricles with constriction of the left anterior descending coronary artery (LAD). The patterns of contraction in the ischemic region were monitored by means of ultrasonic dimension gauges implanted in the endocardial surface and were classified into 5 types; Type 0 = normal, Type I = late systolic lengthening, Type II = early and late systolic lengthening with end diastolic length elongation, Type III = systolic bulge with early diastolic shortening, and Type IV = holosystolic bulge. Type I developed in 6/8 dogs with 1-49% LAD flow reduction, Type II in 6/8 dogs with 50-74% reduction, and Types III and IV in all dogs with 75-100% reduction. The myocardial ATP content (microM/Gm tissue) at the sites of the patterns of myocardial contraction measurement was analyzed. No differences in ATP content were noted between Type 0 and Type I (3.70 +/- 0.51 vs. 3.60 +/- 0.36). In contrast, the ATP content in Type II was significantly reduced (2.94 +/- 0.20) compared to Type I (p less than 0.005). In Types III + IV with systolic bulge, the ATP reductions were severer (2.59 +/- 0.36) than in Type II (p less than 0.05). In conclusion, it can be said that as the LAD flow reduction advanced, the patterns of myocardial contraction were transformed from Type 0 to Type IV with accompanying ATP reductions.

[Pulsed Doppler echocardiography in a case of coronary artery-pulmonary artery fistula: analysis of flow pattern within the main pulmonary artery and detection of a small shunt].

Flow pattern within the main pulmonary artery was analysed by pulsed Doppler echocardiography in a case of coronary artery-pulmonary artery fistula. An asymptomatic 32-year-old female was referred to our hospital for evaluation of a continuous murmur in the second interspace at the left sternal border. Other physical findings, chest roentgenograms and electrocardiograms were normal. Two-dimensional echocardiography did not play an important role in the diagnosis. However, pulsed Doppler echocardiography revealed an antegrade turbulent flow along the antero-lateral wall of the main pulmonary artery from early diastole to early systole. The direction and distribution of this abnormal flow was judged to be different from that of patent ductus arteriosus. Coronary arteriography showed a fistula from the both coronary arteries to the proximal portion of the main pulmonary artery. However, its shunt was so small that it could not be detected by oxymetry or by hydrogen-platinum electrode method. This case suggests that Doppler method is very sensitive in shunt detection and possibly useful for non-invasive diagnosis of coronary artery-pulmonary artery fistula.

[Scotomatous defects in the central visual field, associated with pituitary tumor--a case report (author's transl)].

A rare case which showed scotomatous defects in the left central visual field was presented. A 51 year-old man was admitted to our clinic because of the visual deterioration. In neuroradiological examinations the left carotid angiography showed elevation of both A1 portions and CT suggested pituitary tumor. On February 5, 1980 radical operation was carried out, in which it was confirmed that part of the optic nerve was concave and split longitudinally with a compression of the pituitary adenoma. Postoperative course was uneventful and recovery from the defects in the central visual field was quickly. In this case longitudinal splitting of unilateral optic nerve, caused by gradual compression of supra- or parasellar tumor, was considered as one cause of scotomatous defects in the central visual field. In addition, visual status of forty six cases of pituitary adenoma which had been operated at our clinic during the past fourteen years was analysed both pre- and post-operatively, compared with the case described above.

Diurnal variations in plasma LH, progesterone, testosterone, estradiol, and estrone in the Japanese quail.

Plasma luteinizing hormone (LH), progesterone, testosterone, estradiol, and estrone of the male and female quail were determined by a radioimmunoassay. Progesterone and testosterone or progesterone and LH were determined continuously in ovulating or nonovulating female quail and male quail at 3 hr intervals throughout the day. In another set of experiments, plasma LH, progesterone, estradiol, and estrone were determined simultaneously in ovulating quail, which were autopsied at 3 hr intervals. The plasma progesterone concentration showed a peak 3 hr before ovulation and reached basal level at the time of ovulation, whereas no peak could be observed at any time in the nonovulating or male quail. The LH showed a peak 3 to 6 hr before ovulation and reached the minimum at the time of ovulation, whereas in males and nonovulating quail no specific trend could be observed. Plasma testosterone, estradiol, and estrone levels peaked at 6 hr before ovulation in ovulating quail, whereas testosterone in male and in nonovulating quail showed a peak at 1400 hr.

Effect of tiopronin on senile cataracts. A double-blind clinical study.

The therapeutic effect of tiopronin on incipient senile cataracts was evaluated in 150 cases (275 eyes) by a double-blind study using nonactive placebo. A more favorable tendency to visual acuity was noted in the tiopronin than in the placebo group. The slit-lamp microscopic findings showed a statistically significant difference between the tiopronin and the placebo groups. Integrated judgement revealed tiopronin to be significantly effective as compared with the placebo. A side effect of fever with systemic eruption was observed in 1 case in the tiopronin group but disappeared completely when administration of the drug was discontinued.
