[Vogt-Koyanagi-Harada disease with cerebellar lesions demonstrated on MRI: a case report].

We reported unique magnetic resonance imaging (MRI) findings of a 57-year-old Japanese man who was diagnosed as Vogt-Koyanagi-Harada disease. This patient presented with complaints of a transient severe headache followed by a bilateral loss of visual acuity and truncal ataxia. Magnetic resonance imaging revealed abnormal contrast enhancement of both the uveas and the cerebellar vermis corresponding to his neurological abnormalities. The distribution and the nature of the resolution of this unusual pattern of contrast enhancement suggested that these MRI findings might illustrate transient destruction of the blood brain barrier or vascular extravasations. Such events might be representative of pathophysiology involving the central nervous system that occurred in patients with Vogt-Koyanagi-Harada disease.

Waveform changes of compound muscle action potential (CMAP) with muscle length.

Changes in finger positions influence the waveform of CMAP recorded from the muscle that moves that finger. The present study suggested that the muscle length, dependent on the finger position, was a main factor affecting the waveform. On shortening the muscle length, the amplitude of CMAP increased with concomitant reduction in the duration, and on lengthening, the amplitude decreased with concomitant increase in the duration. These changes are considered due to changes in propagation velocities of muscle fibers dependent on the muscle length. In nerve conduction studies, it is important to carefully monitor the finger position to distinguish the waveform changes with muscle length from those due to nerve lesions.

[Antimutagenic and bactericidal substances in the fruit body of a Basidiomycete Agaricus blazei, Jun-17].

The fruit body of a Basidiomycete Agaricus blazei, Jun-17 (Himematsutake) was extracted with hexane and chloroform-methanol (2:1, v/v), and the antimutagenic effect of the extracts was examined using an Ames/Salmonella/microsome assay. Both extracts of Agaricus inhibited the mutagenicity of benzo[a]pyrene(B[a]P). The hexane extract was purified by silica gel column chromatography and high performance liquid chromatography (HPLC), and linoleic acid was isolated as a main substance having antimutagenic activity. Fr. IIa, IIb, IIc and IIb, which reduced the number of His+ revertant colonies induced by B[a]P, were separated from the chloroform-methanol extract by silica gel column chromatography and HPLC. An antimutagenic substance in Fr. IIa was linoleic acid. From Fr. IIb, a bactericidal, not antimutagenic, substance was isolated and identified as 13-hydroxy cis-9, trans-11-octadecadienoic acid (13ZE-LOH). Antimutagenic substances in Fr. IIc and IId were not purified. The possible source and mechanism of formation of 13ZE-LOH are discussed.

[Personnel requirements for health examination of infants under 5 months in municipalities in Japan].

Municipalities offer health education, health counseling, health examinations, and home visitation service for infants and their mothers. In order to determine what the personnel staffing requirements were for health examination of infants under 5 months for municipalities in Japan in 1989, a questionnaire survey was conducted in 1990. Questionnaires were sent to all municipalities and 3,198 responses were received for a response rate of 97.9%. Responses from 912 municipalities were considered invalid, leaving a total of 2,286 responses that were analysed. Results showed that 79.2% of the 2,286 municipalities were able to perform health examinations for infants under 5 months independently, while in 6.5% of the municipalities these examinations were performed by the clinical hospitals. Analysis by size of municipalities showed that 85.7% of the cities with populations of 500,000 or more were able to perform health examinations independently. In the large cities designated by the Government, the health personnel system adequately supported these health examinations. However in rural towns and villages with small populations, it was necessary for public health nurses to be assisted by other staff, most of whom were public health nurses from prefectural health centers. For example, in areas with populations of less than 3,000, about 60% of the total volume of the work required to be performed by public health nurses during the health examinations had to be covered by assistance from prefectural public health nurses.

[Working hours of maternal and child health service personnel by type of occupation and municipality].

The working hours of maternal and child health service personnel were surveyed to describe the characteristics of maternal and child health activities by municipality in 1989. Questionnaires were sent to 24 health centers and the 122 municipalities they represent, and responses from 19 health centers and 103 municipalities were received. The results were as follows: 1) Of all health service personnel, the working hours for public health nurses per 100 births per year was the highest, followed by maternal and child health volunteers, office workers, nurses and physicians. 2) Total working hours increased with number of births per year. 3) No significant correlation was found in total working hours of personnel in health centers with those in the municipalities they represent. Total working hours of personnel of health centers and the municipalities they represent decreased as population size increased. 4) The number of types of occupation involved in maternal and child health service increased as the size of population of the municipality increased. 5) Of the total working hours for all types of occupations, the proportion of working hours for public health nurses averaged about 40% to 50%. 6) The proportion of working hours for infant health examinations increased with increasing population size of the municipality. However, the proportion of the hours for planning and evaluation decreased with increasing population size of the municipality. From this survey, it appears that maternal and child health activities by municipalities differed according to the population size of the municipality.

Monoclonal antibody G6K12 specific for membrane-associated differentiation marker of human stratified squamous epithelia and squamous cell carcinoma.

Monoclonal antibody (G6K12) specific for differentiated keratinocytes was developed using in vitro immunization against the SCC-25 cell line. G6K12 only recognized stratified portions of cultured SCC-25 cells. Immunohistochemical examination using normal human oral mucosa showed that specific G6K12-reactivity was limited to the lower spinous-cell layers, while this antibody weakly bound to cells in basal-cell layers as well as in the upper spinous, granular and cornified-cell layers. G6K12 was also reactive to keratinocytes in most moderately- and well-differentiated SCC tissues. Immunoelectron microscopic examination further demonstrated that the G6K12-immunoreactive area was at the outer surface of the entire plasma membrane, including the microvilli of stratified SCC-25. G6K12-binding was reduced 50% by the treatment of native cells with glycoendoceramidase for 2 h. These results suggest that G6K12 recognizes a plasma membrane-anchored glycoconjugate which is specific for differentiated keratinocytes.

[A survey of goals and the utilization of numerical goals in the health planning of 47 prefectures].

Health plans of 47 prefectural governments based on the Medical Law of 1985, were evaluated regarding four health planning factors: final goal, implementation plan, development of resources, and evaluation planning. Results of the evaluation showed that 8.5% of the 47 prefectures had defined future level of health based on numerical goals. On the other hand, all of the prefectures had developed numerical goals specifying the number of hospital beds in each medical district. A major component of health planning is improvement in the accomplishment of the implementation plan. There were 14 prefectures which had numerical goals for the upgrading of facilities and for manpower development to meet future health care activities. The following factors are required in the health planning to meet the needs for improving the health status of the population. 1) Including in the future health goals, in addition to the number of medical bed in regional areas, objective levels of health that should be attained. 2) Development of practical plans for evaluation of effectiveness of health planning.

[Health personnel staffing requirements for health examination for three year old children in municipalities in Japan].

Based on the Maternal and Child Health Law, municipalities offer health education, health counseling, health examination, and home visitation service for three year old children and for their mothers. In order to determine personnel staffing requirements for the health examination of three year old children for municipalities in Japan for 1989, data from a questionnaire survey in 1990 conducted by the Ministry of Health, was analyzed. Questionnaires were sent to all municipalities and there were 3,198 responses for a response rate of 97.9%. Results showed that in 1989, only 7% of the 3,198 municipalities were able to perform health examinations for three year old children independently. However 83% of the cities which had populations 500,000 or more were able to perform the health examinations independently. In both the large cities designated by the Government and those cities which have Health Centers, the health personnel system adequately supported these health examinations. Due to the lack of health personnel, rural towns and villages with small populations required public health nurses to be assisted by other staff, most often public health nurses from prefectural Health Centers. For example, in those areas with populations less than 3,000, 43% of the total volume of the work required to be performed by public health nurses during the health examinations had to be covered by assistance for prefectural public health nurses.

[Correlates of cigarette smoking among junior and senior high school students in Japan].

Data from a nationwide cross-sectional survey of the smoking status among junior and senior high school students in Japan conducted in 1990, were utilized to provide sociodemographic correlates of the use of cigarettes. The study was based on data from 57, 189 students. The analyses examined the relationship between smoking status and 9 explanatory variables, using multiple logistic regression for both males and females. The results of logistic regression analyses revealed the following main effects: 1) Smoking by a friend had the strongest relationship to smoking status of students for both sexes. The relative risks for having a smoking experience if a friend smoked were 3.45 for junior high school boys, 5.24 for junior high school girls, 3.65 for senior high school boys and 5.54 for senior high school girls and the risks for current smoking if a friend smoked were 8.54, 15.28, 6.99 and 12.47, respectively. 2) Among both boys and girls, smoking experience and current smoking was significantly more likely if a brother or sister, or mother smoked. Awareness of the harm of smoking was also an important correlate in both sexes. Whether the students enjoyed their school life was significantly related to current smoking status for females, but not for males. 3) The relative risks for current smoking if their mothers smoked were 1.60 for junior high school boys, 1.78 for junior high school girls, 1.61 for senior high school boys and 1.61 for senior high school girls. Fathers' smoking was not selected in this stepwise multiple logistic regression model. Therefore, mothers' smoking appears to be a stronger correlate of smoking status among students than fathers' smoking. 4) The relative risks for smoking experience and current smoking if there were smokers in the family, were higher among junior high school students than senior high school students.

[Nationwide survey of smoking prevalence among school students in Japan].

In 1990, a nationwide random survey of smoking habits among junior and senior high school students was conducted in order to study the smoking prevalence among adolescents in Japan. The self-administered anonymous questionnaires on personal smoking status were sent to all students in sample schools. Seventy junior high schools and thirty-three senior high schools responded with 57,566 questionnaires returned, of which 57,189 were subjected to analysis after excluding incomplete and inconsistent questionnaires. The results were as follows; 1) The percentage of experimenters who had smoked even once was 23.3% among boys and 8.7% among girls in the first grade of junior high school (seventh grade). The percentage increased with school grade and reached nearly half of boys and nearly one sixth of the girls in the third grade of senior high school (twelfth grade). 2) The smoking rate was much higher among boys than girls, and the current smoking rate in the first grade of junior high school was 4.0% for boys and 1.5% for girls. Current smoking rate increased with grade, reaching 25.5% for boys and 4.9% for girls in the third grade of senior high school. 3) While the percentage of regular smokers in the first grade of junior high school was less than 1% for both sexes, it went up to 20.3% for boys and 2.6% for girls in the third grade of senior high school. 4) In current smokers, the proportion of boys smoking 1-4 cigarettes per day decreased and that of those smoking 15-19 cigarettes, and 20 or more cigarettes increased with increasing school grade.(ABSTRACT TRUNCATED AT 250 WORDS)

[Antimutagenic substances in the Armeniacae semen and Persicae semen].

Using the Ames/Salmonella/microsome assay, we examined the antimutagenic effect of the hexane extract of Armeniacae semen (apricot (Prunus armeniaca L.) seed), Persicae semen (peach (P. persica Bat.) seed), and seeds of cherry (P. avium L.), plum (P. salicina Lindle) and almond (P. dulcis Mill). Hexane extracts of Armeniacae semen and Persicae semen inhibited the mutagenicity of benzo[a]pyrene (B[a]P), but those of seeds of cherry, plum and almond did not. The mutagenicities of 3-amino-1,4-dimethyl-5H-pyrido[4,3-b]indole (Trp-P-1) and 2-(2-furyl)-3-(5-nitro-2-furyl)acrylamide (AF-2) were also inhibited by the extracts of Armeniacae semen and Persicae semen. Inhibitory substances in Persicae semen were fractionated by silica gel column chromatography and high performance liquid chromatography, and were identified as oleic acid and linoleic acid. The contents of oleic acid and linoleic acid were 0.7 and 0.4% in the hexane extract of Armeniacae semen, and 1.5 and 0.5% in that of Persicae semen, respectively.

[Relationship between health services for maternal and child health and working hours of public health nurses].

Based on the "Maternal and Child Health Service Act", public health centers and municipalities offer health education, health counseling, health examination, and home visits for district children and for their mothers. The relationship between the number of the MCH services offered and the working hours of public health nurses at the public health centers and the municipalities was analyzed. Data were taken by a survey in 1989, which was used for evaluating services provided to the children and their mothers in 1988. Questionnaires were sent to 23 health centers and their districts and 100 responses from 8 wards, 20 cities, 47 towns, and 25 villages were analyzed. When total working hours of the public health nurse for MCH are allocated to the four parts-planning, implementation, evaluation and training-86.7% of working hours are for implementation itself. The correlation coefficient between the total working hours of the public health nurse for MCH and the population, and the number of births per year, were significantly positive. A total of 107 hours were supplied for the infant child health examination per 100 births per year, for which 64% was for implementation itself. In the case of the health examination for three-year-old children, total working hours were 143 hours, for with 79% was for implementation itself. The number of mother-child handbooks issued, the number of health education classes and health counselings conducted, various health examinations performed, and home visits made were correlated significantly with the working hours of the public health nurse.

[A case-control study on factors relating to discontinuation of domiciliary care for the bedridden elderly in a metropolitan area].

A case-control study was conducted to examine factors relating to discontinuation of domiciliary care for the bedridden elderly in Shinagawa-ku, Tokyo. Cases were bedridden residents aged 65 years and over who had abandoned home care and applied for admission to live in a special nursing home for the aged between April and September in 1990 after being recipients of welfare allowances for disabled elderly. Controls were bedridden residents who continued to be given home care and matched to cases by sex, age and beginning month of the receiving of allowances. Among 50 cases and 94 controls interviewed, we obtained responses from 31 cases (62%) and 60 controls (64%). The main results were as follows: 1. During the home-care period, ADL (activities of daily living) of cases, especially walking ability, deteriorated more severely than in controls. Night delirium also appeared more frequently in cases. 2. The primary caregivers of cases were older than those of controls. Remarkable differences between cases and controls were observed in the family structure, the number of family members and the number of sub-caregivers. Cases tended to live alone or live with a spouse only, and with smaller number of family members and caregivers. 3. Case lived more frequently in houses with small numbers of rooms and without rooms of their own. 4. As regards utilization of domiciliary care services, cases used dispatch of home helpers more frequently and used day services less frequently.

[A case of humidifier lung associated with BHL].

We encountered a rare case of hypersensitivity pneumonitis associated with bilateral hilar lymphadenopathy (BHL). The patient was a 53-year-old male, who developed dry cough and shortness of breath when using a humidifier since 1982. He was admitted to our hospital for further evaluation in 1987. Chest X-ray films showed BHL and ground glass appearance in the bilateral lung fields. Pulmonary function test indicated disturbance of diffusing capacity. Transbronchial lung biopsy revealed interstitial pneumonitis, and lymph node biopsy by mediastinoscopy showed lymphoid sinus histiocytosis without noncaseating granuloma. Provocation test using the humidifier was positive, and the diagnosis of humidifier lung with BHL was made.

Ultrastructural studies on the plerocercoid of Spirometra erinacei in experimental sparganosis.

The gland cell and the tegument of the Spirometra erinacei plerocercoid following parasite migration in hamsters were observed by electron microscopy. Gland cells released secretory granules mainly into the frontal pit during each stage of migration. The density of the perinuclear secretory granules decreased markedly just after parasite penetration through the host intestinal wall. Therefore, gland cells seem to play an essential role in penetration. During migration in the host, lipid-like droplets appeared in the subtegumental cells and tegument and increased in number and size during migration in the abdominal cavity, but they had disappeared from plerocercoids recovered from subcutaneous tissues. Myelin-like bodies also occurred in the tegument, subtegumental cells and medullary matrix; they were released directly through the tegument into the frontal pit. Discoidal bodies gathered in the distal part of the tegument, where filamentous microtriches appeared in regions from which microtriches had been peeled off during penetration.

Scanning electron microscopic study of four Diphyllobothrium species.

Three-dimensional observation was carried out on plerocercoids and adults of Diphyllobothrium dendriticum, D. ditremum, D. latum, and D. vogeli using scanning electron microscopy. The species-specific differences between plerocercoids were recognized in the shapes of the whole body, scolex, and bothrium and the wrinkle pattern on the body surface. The differences between adult worms were also observed in the shapes of the scolex, neck, and genital papillae around the genital pore and the pattern on the egg surface. The significance of species specificity in the three-dimensional morphology of diphyllobothriid cestodes is briefly discussed.
