Immunosuppressive factor from the spleen in gastric cancer patients.

The spleens of gastric cancer patients were examined to determine whether this organ releases an immunosuppressive factor. Compared to serum from peripheral blood, serum from splenic venous blood of advanced gastric cancer patients had greater suppressive activity to normal lymphocyte responses to phytohemagglutinin (PHA); it also contained significantly more immune complexes. Furthermore, the supernatant from spleen cell cultures from advanced gastric cancer patients significantly suppressed normal lymphocyte responses to PHA. This immunosuppressive activity was enhanced when the supernatant was from cultures of separated nonadherent subpopulations. These observations were not detected in patients with early gastric cancer. The blastogenic response to PHA and the percentage of T-cells among spleen cells was not affected by the cancer stage. These results suggest that the spleen of patients with advanced gastric cancer participates in the induction of serum immunosuppressive factors, possibly due to a change in splenic lymphocyte subsets.

Clinical effects of total-body hyperthermia combined with anticancer chemotherapy for far-advanced gastrointestinal cancer.

The authors treated 17 patients with far-advanced gastrointestinal cancer with extracorporeally induced total-body hyperthermia (TBHT) combined with anticancer chemotherapy. Although all patients' tumors were clinically resistant to prior chemotherapy with 1-(2-tetrahydrofurlyl)-5-fluorouracil or 5-fluorouracil and mitomycin C, three patients had an objective response to TBHT plus the same anticancer agents with or without cyclophosphamide. That is, a partial response was obtained in 2 of 12 patients with recurrent gastric cancer and in 1 of 5 patients with recurrent large bowel cancer. However, their survival time was not markedly prolonged. As a characteristic complication of TBHT, the authors noted reversible weakness of the muscles of the lower extremities. Two patients died due to hepatorenal syndrome; preoperatively these patients had manifested hepatic or renal dysfunction. Therefore, in patients with hepatic or renal dysfunction, the application of TBHT must be considered carefully.

A new stable prostacyclin analogue OP41483 (15-cyclopentyl-omega-pentanor-5(E)-carbacyclin).

Effects of a new stable prostacyclin analogue OP41483 (15-cyclopentyl-omega-pentanor-5(E)-carbacyclin) on platelet aggregation, plasma thromboxane B2 levels, hemodynamic parameters, and clinical safety were studied. Normal volunteers received intravenous infusions of OP41483 at doses of 2.5 (n = 3), 5.0 (n = 3), 10 (n = 3), and 20 (n = 5) ng/kg/min for one hour. During infusion, ADP (adenosine diphosphate)-induced platelet aggregation decreased significantly, and blood pressure decreased slightly, but plasma thromboxane B2 levels did not change significantly. The intravenous infusion of OP41483 at the dose of 10 ng/kg/min for one hour over three successive days was performed. During the each infusion, platelet aggregation and mean blood pressure decreased significantly.

[Clinical evaluation of sisomicin following intravenous drip infusion in respiratory tract infections].

The efficacy, safety and utility of sisomicin (SISO) followed intravenous infusion were evaluated in 35 cases with various respiratory infections. For many cases, SISO was given at a daily dosage of 100 mg, and a single dose was infused over about 1 hour. Clinical efficacy was evaluable in 28 cases including pneumonia (14 cases), bronchitis (8 cases), bronchiectasis (4 cases), pulmonary suppuration (1 case) and pulmonary abscess plus pyothorax (1 case). Almost cases had diagnosis of serious infection associated with various diseases. Clinical efficacy was evaluated as "excellent" in 2 cases, "good" in 15 cases, "fair" in 5 cases and "poor" in 6 cases, and efficacy rate in total case was 60.7%. Efficacy rate stratified by disease was calculated as 57.1% in pneumonia, 87.5% in bronchitis, 50.0% in bronchiectasis. Responses against pulmonary suppuration or pulmonary abscess with pyothorax were little or not. Bacteriologically, organisms isolated from sputum cleared in 7 out of 15 evaluable cases, thus the responses rate was 46.7%. Adverse reaction probably due to treatment observed in 2 cases with hepatic dysfunction. Blood levels of SISO at the end of infusion were ranged from 2.1 to 6.4 micrograms/ml, and no tendency of accumulation in blood after repeated infusion was showed.

Circulating immune complexes in gastric cancer patients and their effect on lymphocyte mitogenesis (the first report).

Immune complexes (IC) were measured in 66 gastric cancer patients, using the 3.5 percent polyethylene glycol (PEG) precipitation method. Preoperatively, the IC values in patients with advanced gastric cancer (stages III and IV) were significantly higher than in normal subjects (p less than 0.01). In the presence of 3.5 percent PEG-precipitated IC from sera of gastric cancer patients, the mitogen responses of normal peripheral blood lymphocytes was inhibited; the PHA response revealed a significant negative correlation with the concentration of IC (p less than 0.01). Our data suggest that IC may be a major serum factor exerting immunosuppressive effect in cancer hosts.

Developmental changes in mitochondrial components in liver of newborn rats.

Chronological studies on the respiratory function and membrane components of newborn rat liver mitochondria revealed that perinatal mitochondria retained considerably well-coupled oxidative phosphorylation that functioned in sites II and III prior to site I. The ratio of protein to phospholipid contents (mg/mumol) was low in these mitochondria, in which phospholipids were characterized by their high phosphatidylethanolamine contents (43% of the total phospholipids) and low cardiolipin contents (6% of the total), as compared to those of the adult. The fatty acid composition in major phospholipid species was also different from those of the adult. These characteristics altered with development, and the carbamylphosphate synthetase contents markedly increased from 12 to 20% of the total polypeptides in 10 days postnatal time. From these results structural and functional completion of mitochondria accompanying development is discussed from a mitochondrial biogenesis viewpoint.

Immunocytochemical study of collagen in epidermal growth factor (EGF)-treated osteoblastic cells.

The alteration of collagen components in clone MC3T3-E1 cells by epidermal growth factor (EGF) was investigated immunocytochemically, using antibodies to type I and type III collagens. EGF transformed those cells that had become more slender than those of control cultures. Type I and type III collagens were observed in the same cells in both EGF-treated and control cultures. Type I collagen was decreased by EGF, whereas type III collagen appeared to be increased. However, no cells with only type III collagen were observed, suggesting that EGF influences collagen metabolism in clone MC3T3-E1 cells.

[Serum levels of mitomycin C following a high-dose continuous hyperthermic peritoneal perfusion].

Valid therapeutic means have not been established for treatment of disseminated peritoneal metastasis of carcinoma. Continuous hyperthermic peritoneal perfusion (CHPP) with high-dose of mitomycin-C (MMC) was applied to 26 patients with peritoneal metastasis from gastric, rectal or ovarian cancers. Levels of MMC in the sera and in the perfusate were measured. The results obtained were: 1. Approximately a half of the dose of MMC added into the perfusion fluid was recovered. 2. When perfused with 100 mg of MMC, the maximum serum concentration of MMC was equivalent to 1/3 of the maximum serum level when injected with 10 mg of MMC intravenously. Therefore, MMC which could not be detected in CHPP seemed to be retained in the abdominal cavity. 3. Bone marrow inhibition caused by CHPP was observed in only 2 of 26 patients. 4. The total dose of 300 mg of MMC, consisting of the maximum dose of 100 mg each, is acceptable in CHPP without any severe side effect. 5. CHPP exerts antitumor effects when utilizing hyperthermia and a high-dose of MMC on the disseminated peritoneal foci of carcinoma.

Treatment of implanted peritoneal cancer in rats by continuous hyperthermic peritoneal perfusion in combination with an anticancer drug.

To study the feasibility of combined hyperthermic and anticancer drug treatment for peritoneal cancer, we devised a continuous hyperthermic peritoneal perfusion system in combination with mitomycin C. The model uses i.p.-transplantable rat ascites hepatoma 100B cells. Hyperthermic peritoneal perfusion alone or combined with mitomycin C was performed after i.p. inoculation of the tumor cells into rats. In rats treated with combined peritoneal perfusion (41.5 degrees) and mitomycin C, the mean survival times were significantly prolonged as compared to those of rats treated with peritoneal perfusion at 41.5 degrees alone. Our results suggest that combined hyperthermic peritoneal perfusion and mitomycin C treatment may represent a therapeutic and prophylactic treatment for peritoneal metastasis after gastric cancer surgery in humans.

[Intra-hepato-arterial administration of cis-diammine-dichloroplatinum II (CDDP) for primary or metastatic hepatic cancer].

To study the effectiveness of CDDP for hepatic cancer, intra-hepato-arterial administration of CDDP (0.8-1.0 mg/kg/week) combined with 5-FU (250 mg/body/day) was performed for 10 patients with primary or metastatic hepatic cancer. As the results, partial response was obtained in 6 of 8 evaluable patients. However, the adverse effect was very high rate; leucopenia, thrombocytopenia and vomiting (nausea) were observed in 9, 6 and 6 of 10 patients respectively. The dose limiting factor of CDDP was not a nephrotoxicity but a bone marrow depression in this series. Though a noticeable antitumor effect of this modality was obtained, some improvements should be applied to it to decrease the adverse effect.

The effects of total-body hyperthermia combined with anticancer drugs on immunity in advanced cancer patients.

As total-body hyperthermia (TBHT) therapy may lead to a reduction in the immune response of cancer patients because of the immediate effect of heat on lymphocytes, the authors studied the immunity of advanced cancer patients receiving combined TBHT and anticancer chemotherapy. A decrease was found in their lymphocyte blastogenesis, skin reactions to PPD and PHA, lymphocyte rosette formation, IgG, and C3c component. These parameters returned to their pretreatment levels at 1 week after completion of TBHT therapy. This result indicates that there is no necessity for giving special consideration to a reduction of cell-mediated immunity in TBHT therapy.

Immunosuppressive acidic protein (IAP) in gastric cancer patients.

Immunosuppressive acidic protein (IAP) in the serum from peripheral, splenic and regional venous blood of gastric cancer patients was assayed. Peripheral venous blood was collected before, and for several months after the surgery. The IAP values of peripheral venous blood, increased with advancing cancer stage; in patients at the same cancer stage, the IAP values of venous blood draining from the cancer lesion were the highest. In curatively operated patients, the IAP values increased during the first postoperative month and decreased thereafter. In patients with non-resectable cancer and in patients subjected to palliative surgery, these values remained high; they increased sharply just before death. Determination of the serum IAP values in gastric cancer patients may be useful in estimating the immunological state of gastric cancer patients and for monitoring the postoperative immunological status.

[Short-term intensive chemotherapy (STIC) combined with intravenous hyperalimentation in advanced stomach cancer].

A short-term intensive chemotherapy (STIC) consisting of large dose of mitomycin C (MMC), tegafur (FT-207), streptococcus preparation picibanil (OK-432) and Krestin (PSK), combined with IVH was performed in 72 patients with advanced gastric cancer. Thirty-three patients were unresectable and 39 patients were recurrent. In this modality, an intensive chemotherapy was able to carry out, maintaining, or increasing in host immunologic parameters and body weight in spite of their disturbed condition in oral food take. Partial response was obtained in 10 of 51 evaluable cases (19.6%) and the survival period on STIC was superior to that on controls. However, no significant difference was observed between the two groups. Four patients had become to be able to take full oral food again by STIC and resulted in the removal of IVH. Hyperglycemia and septicaemia induced by IVH were observed in 9 of 72 patients (12.5%), one died of septicaemia. Full attention should be paid in the management of IVH for patients with advanced cancer.
