What are the signs of recent hemorrhage, and what do they mean? Criteria for massive bleeding.

Hematemesis, melena, shock, vague symptoms, anemia, blood examinations and endoscopic findings are reviewed as signs of recent hemorrhage. An assessment of the condition of patients with upper GI bleeding is important for planning treatment. Therefore, in order to be able to evaluate a method of treatment of GI bleeding, reports referring to treatment of GI bleeding must contain a clear record of the severity of the patient's state and of the bleeding source. A common criterion for massive bleeding has been proposed. Specifically, a patient suffering from shock who needs more than 1,000 ml of blood or plasma expander by rapid transfusion within one hour, or more than 2,000 ml within the initial 24 hours, to stabilize his circulation, and whose Hb level is 8.0 g/dl or less, should be graded as having massive bleeding. Gastric ulcers with exposed blood vessels need endoscopic YAG laser hemostasis or ethanol injection therapy as quickly as possible, since 43% of the cases rebleed within 72 hours under conventional drug therapy, and such rebleeding can be prevented by the endoscopic hemostatic methods.

Direct-writing recorder of the frequency dependence of dynamic compliance analyzed from one cycle of breathing and pulmonary resistance: effect of fenoterol on asthmatic subjects.

We developed a new method of direct-writing recording of the frequency dependence of dynamic compliance analysed from one cycle of breathing and pulmonary resistance. Both pulmonary resistance (RL) and frequency dependence of dynamic compliance (Cdyn.f) are calculated by Fourier-series analysis of flow and transpulmonary pressure in a single cycle of breathing. RL was obtained from fundamental harmonics. Cdyn.f was calculated from 1st and 2nd harmonics and estimated by the ratio of Cdyn at 0.5 Hz to Cdyn at zero frequency, C0.5/C0. We used fenoterol aerosol which contained 0.2 mg of fenoterol with one puff of aerosol. Two puffs of fenoterol aerosol were used in each subject and followed changes of RL, C0 and C0.5 as long as 30 min. After fenoterol inhalation RL decreased considerably and C0.5/C0 cont [C0.5/(control C0)] increased. With time, while RL was kept stable, C0.5/C0 cont increased further. We suggest that fenoterol has a potent effect on the small airways which is enhanced with time.

Mechanism of the inhibition of cholesterol absorption by DL-melinamide: inhibition of cholesterol esterification.

In order to elucidate the mechanism of action of DL-melinamide [DL-MA, N-(alpha-methylbenzyl)linoleamide], an inhibitor of cholesterol absorption, the effect of DL-MA on esterification of cholesterol in the mucosa of rabbit small intestine was studied. DL-MA inhibited acyl CoA:cholesterol acyltransferase (ACAT, EC 2.3.1.26) activity in the mucosal microsomes, with 50% inhibition occurring at approximately 0.5 microM. On the other hand, DL-MA had no effect on the cholesterol esterase (EC 3.1.1.13) activity in the mucosal cytosol. Kinetic studies indicate that DL-MA is an uncompetitive inhibitor of ACAT. D-MA, one of the two optical isomers of DL-MA, was found to be a more effective inhibitor of ACAT than L-MA, another isomer. This finding indicates that the inhibition of cholesterol absorption by DL-MA depends on the inhibition of ACAT by this compound, in view of the fact that D-MA is a more effective inhibitor of cholesterol absorption than L-MA.

[Population of Japan in the Meiji-Taishyo era: a reestimation].

After reviewing previous estimates of the Japanese population in the pre-censal years from the 1860s to 1920, the author presents a reestimation for this period based on more recent studies in historical demography and on Coale and Demeny's regional model life tables. "The most important revision in the present estimation [is] that life-table death rates...for the Meiji-Taishyo era are estimated by Gompertz curves with upper and lower limits in place of the former linear curves." [Intra-arterial infusion chemotherapy with anticancer agents for advanced breast cancer--effect and toxicity of mitomycin C and adriamycin].

In 23 cases of primary advanced breast cancer, intra-arterial infusion chemotherapy of Adriamycin (ADR) and Mitomycin C (MMC), which were injected jointly or individually, was performed and its effects and side effects were studied. As for the clinical effects, the response rate (CR + PR) was 73.9% (17/23 cases) and the histological response rate (greater than grade IIb) was 82.6% (19/23 cases). ADR alone (100-150 mg) and MMC (28 mg) + ADR (42 mg) combined regimens were especially superior in both their clinical and histological effects. In metastatic lymph nodes, the histological response rate was 78.9% (15/19 cases). As for the side effects, in the cases treated with MMC, bone marrow suppression such as leukocytopenia and thrombocytopenia was remarkable and took a long time to recover. The above results suggested that the most effective regimen for primary advanced breast cancer using intra-arterial infusion chemotherapy is ADR alone, in single doses of 50 mg up to a total dose of 150-200 mg. Histological examination of the effective cases revealed that the central region of the tumor was more markedly recrotic than the periphery. It was suggested that the grade of the effects on tumor tissues is related to the mechanism of anticancer agents.

[Gastrointestinal hormones and operations for achalasia of the esophagus and sliding esophageal hiatal hernia: their surgical significance].

Experimental achalasia dogs produced with Deloyer's method showed higher resting pressure at the gastroesophageal junction and the increase in LES pressure in response to tetragastrin and cholecystokinin. Dose-response curve of the LES to each dose of tetragastrin in achalasia dog shifted to the left. Resting LES pressure in 11 patients with achalasia was 42.73 +/- 23.31 cm H2O. It increased significantly after intramuscular injection of 5 micrograms/kg of tetragastrin and fluoroscopic observation showed the tonic contraction of the lower esophagus and cardia. After the performance of Jekler-Lhotka operation, LES pressure decreased to lower values sufficient to prevent the gastroesophageal reflux. Comparing 5 kinds of hiatal herniorrhaphies in dogs, LES pressure increased postoperatively in the following order: Nissen, Belsey Mark IV, Stensrud, Hill and Harrington methods. Responses to tetragastrin increased after Nissen and Belsey Mark IV methods. In 12 out of 21 clinical cases of sliding esophageal hiatal hernia who had undergone Nissen-Rossetti method adding fundopexy and posterior gastropexy, preoperative esophageal manometry showed HPZ of 24.98 +/- 8.87 cm H2O in peak value and 5.1 +/- 3.46 cm in length. Seven cases showed the biphasic pattern and 5 cases showed the negative response to tetragastrin. Postoperative manometry showed HPZ of 31.42 +/- 18.46 cm H2O in peak value and 4.5 +/- 1.73 cm in length. One case showed the biphasic pattern and 3 cases showed the negative response to tetragastrin.

Absence of correlation between ACh-induced Ca influx and phosphatidic acid labeling in rat uterus.

Rat uterine smooth muscle was preincubated in Ca-depleted modified Locke-Ringer solution to investigate the correlation between the 32Pi incorporation into phosphatidic acid induced by acetylcholine and the contractile response to acetylcholine induced by the addition of CaCl2 (Ca influx). The results showed that in rat uterine smooth muscle under these conditions phosphatidic acid does not act as a Ca ionophore or as a trigger for opening the Ca channel.

Frequency of criteria-defined borderline patients among the first-visit patients at a psychiatric clinic.

Five trained psychiatrists evaluated 330 first-visit patients aged from 12 to 30 at a psychiatric outpatient facility using the DSM-III criteria (Axis-I and Axis-II). Seven cases were diagnosed as definite Borderline Personality Disorder and six cases as definite Schizotypal Personality Disorder. Hence, we had a total of 13 criteria-defined borderline patients (3.9%). Discussions were held on some methodological problems involved in collecting information and making diagnostic judgments.
