[Assessment of therapeutic effects by transcatheter arterial embolization (TAE) in hepatic malignant tumors with degradable starch microspheres (DSM)].

Transcatheter arterial embolization (TAE) with degradable starch microspheres (DSM) was carried out for fourteen cases with hepatic malignancies including twelve cases with hepatocellular carcinoma (HCC), one case with cholangiocarcinoma and one with metastatic liver cancer. DSM combined with anticancer agents were administered through a catheter introduced by Seldinger's method, in ten cases and through subcutaneously implanted drug delivery system (Port-A-Cath) in four cases. The dose of DSM was 900 mg/body and adriamycin 30-40 mg/m2 through the catheter or 12-14 mg/m2 through Port-A-Cath was used for HCC and cholangiocarcinoma. A same dose of DSM and mitomycin C 15 mg/m2 was administered for metastatic liver cancer through the catheter immediately after angiography. Results were as follows: 1) Partial response (PR) was obtained in six cases (50%) with HCC, and there were two other cases with minor response (MR). PR in cholangiocarcinoma and CR in metastatic liver cancer were obtained. 2) There were no adverse effects in four cases with Port-A-Cath. 3) The concentration of adriamycin in the peripheral venous blood was lower than that of one shot therapy, and decreased rapidly within an hour. These results suggested that good therapeutic effects can be obtained by TAE with DSM combined with anticancer agents for hepatic malignancies.

[Current status of diagnosis and treatment of metastatic liver cancer].

Clinical usefulness of liver function tests, tumor markers including CA 19-9, CEA and serum ferritin and image diagnosis was discussed in regard to early diagnosis. Liver function tests were not useful, and tumor markers were highly positive, i.e., CA 19-9 was positive in 70.7% and CEA in 66.7%. However, these were low positive rate, in the early stage. Imaging techniques were very useful in detecting liver metastasis; US positive in 92.5%, CT 87.2% and angiography 93.1%. In the treatment of metastatic liver cancer, arterial chemotherapy and transarterial embolization are the major modality.

[An autopsy case with a combination of hepatocellular carcinoma and cholangiocellular carcinoma].

A 68-year-old male was admitted to the Kurume University Hospital with a diagnosis of hepatocellular carcinoma (HCC). Serum CEA levels ranged from 3.7 ng/ml and 6.8 ng/ml during the course. Serum AFP levels were at normal range at admission and elevated to 13,250 ng/ml 3 months before death. He died of liver failure 17 months after admission. An autopsy disclosed diffuse infiltrative tumors distributed throughout the liver with metastasis to the bilateral lungs. Histologically, tumors of the liver consisted of elements of both HCC and cholangiolocellular carcinoma. Cholangiolocellular carcinoma is believed to occur from the canals of Hering or cholangiole, and this case may provide further information as to the histogenesis of combined HCC and cholangiocarcinoma.

Glycoprotein analysis of middle ear effusions by rectin-conjugated sepharose affinity chromatography.

We analyzed the composition of glycoproteins in serous and mucoid middle ear effusions (MEE) collected from patients with chronic otitis media with effusion (OME), and compared this with plasma glycoproteins, using rectin-conjugated sepharose. The concentration ratio of rectin-absorbed glycoproteins to total protein concentration in serous MEE resembled that in plasma, although mucoid MEE had a higher concentration ratio than that of the serous effusions. By SDS-polyacrylamide gel electrophoresis analysis, the molecular weight pattern of glycoproteins adsorbed to wheat germ agglutinin (WGA) gel in serous MEE was more similar to that in plasma than in the mucoid effusions. Using 2-mercaptoethanol, the basic low-molecular-weight components of the glycoproteins were almost identical in MEE and in plasma. Our findings show that mucoid MEE has a greater amount of plasma and epithelial glycoproteins than does serous fluid. The composition of these latter substances is more similar to that of plasma glycoproteins. The strong disulfide bonds present in glycoproteins may significantly contribute to the physicochemical properties of mucoid MEE.

Possible mechanisms of reduction of nasal mucociliary clearance in chronic sinusitis.

Nasal mucus was collected from 38 patients with chronic sinusitis. Following this collection, nasal mucociliary clearance (ST) was measured by the saccharin granule technique. Nasal mucus was used for in vitro frog palate clearance studies and then compared with the in vivo ST. The mucociliary transport rate of nasal mucus on mucus-depleted frog palate (MTR on frog palate) was 5.93 +/- 1.04 mm/min in the patients whose ST was within normal range (group A), and was 5.54 +/- 1.94 mm/min in the patients whose ST was above the normal range (group B). Significant differences were not observed in MTR on frog palate between groups A and B and no significant correlation was noted between ST and MTR. These results suggest that factors other than the rheological properties of nasal mucus control nasal mucociliary clearance in patients with chronic sinusitis.

Transcatheter partial splenic arterial embolization in patients with hypersplenism: a clinical evaluation as supporting therapy for hepatocellular carcinoma and liver cirrhosis.

Eleven cases with hypersplenism, one with liver cirrhosis and ten with hepatocellular carcinoma (HCC) associated with liver cirrhosis, underwent transcatheter partial splenic arterial embolization. In four of ten HCC cases, the spleen was accidentally infarcted during the procedure of transcatheter hepatic arterial embolization (TAE). The mean infarcted area of the spleen was 55.7%. An increase in the peripheral platelet count was particularly remarkable and continued over one year after the embolization. High fever and abdominal pain were observed in all cases. The fever was seen for 18.0 days and pain was noted for an average of 12.8 days after the embolization. Other adverse effects such as pleural effusion and ascites were transitorily observed. Splenic embolization is an effective supporting therapy for hypersplenism in patients with cirrhosis or HCC.

[Adenoid cystic carcinoma of the head and neck--a clinical study of 27 cases].

Adenoid cystic carcinoma (ACC) is frequently seen in the salivary glands, but may occur at other sites in the head and neck. We report 27 patients with ACC treated at the Department of Otorhinolaryngology, Mie University School of Medicine, for 15 years. The survival rate dropped from 78% at five years to 36% at 10 years. This suggests that the long-term prognosis of ACC is poor. According to Batsakis, we classified ACC according to histopathology and emphasize that the solid cellular pattern had the worst prognosis.

Mechanism of slowing of mucociliary transport induced by SO2 exposure.

Responses of nasal mucociliary transport mechanisms to exposure to 6 ppm SO2 were studied in chickens in vivo. This model takes advantage of the natural cleft palate which exposes the mucociliated base of the nasal septum. Exposure to 6 ppm SO2 decreased the mucociliary transport rate along the base of the nasal septum. The minimum force required to move an iron particle along this area of mucous membrane by use of a magnetic field in vivo increased significantly after SO2 exposure, while the minimum force required to move an iron particle on a pool of mucus collected from the same chicken and tested in vitro showed no change after SO2 exposure. The elastic recoil distance of mucus was measured both in vivo and in vitro. The in vivo recoil distance decreased significantly after SO2 exposure, while SO2 exposure did not change recoil distance in vitro. It is proposed that exposure of chickens to SO2 results in the formation of multiple points of adhesion of strands of mucus between the acinar gland cells and the emergent extracellular mucus or adhesion of a mucous blanket to the cilia, causing mucociliary transport to be retarded or static.

Rheological properties of middle ear effusion and their role on mucociliary clearance.

The viscoelastic properties of freshly harvested middle ear effusion (MEE) from children were determined by an oscillating sphere magnetic rheometer and compared with transportability of MEE on mucus-depleted frog palate. The elastic modulus (G') at 1 Hz of MEE from 43 untreated ear was ranged from 1.9 to 1,790 dyn/cm2 and the mean value was 28.4 dyn/cm2. The dynamic viscosity (eta') at 1 Hz of the same samples was ranged from 0.2 to 146 poise with the mean value of 3.4 poise. A maximum value of transport was obtained at G' of about 20 dyn/cm2, and below this value there was a significant positive correlation between the transport rate and log G'. Above 20 dyn/cm2, the negative correlation between the transport rate and log G' was significant. A similar significant correlation between the transport rate and log eta' was observed.

The significance of protease inhibitors in the pathogenesis of otitis media with effusion.

Concentration of alpha 1-antitrypsin (alpha 1-AT), alpha 1-antichymotrypsin (alpha 1-AChyT), inter-alpha-trypsin inhibitor (I-alpha-I), and alpha 2-macroglobulin (alpha 2-M) was measured in 27 serous middle ear effusions (MEEs) from 24 adult patients. The presence of protease-inhibitor complex was analyzed by crossed immunoelectrophoresis (CIEP). Mean concentration of alpha 1-AT was 361 +/- 90.0 mg/dl and was higher than that of other inhibitors: alpha 1-AChyT, 80.6 +/- 40.7; I-alpha-I, 21.3 +/- 21.5; alpha 2-M, 59.5 +/- 57.1. Molar concentration of alpha 2-M was the lowest. Most of alpha 1-AT and alpha 1-AChyT in MEEs were unsaturated; free inhibitors. Alpha-1-AT could be saturated by trypsin and elastase immediately, and only alpha 2-M could be saturated by papain (classical thiol protease). Serous MEEs have high anti-trypsin activity attributed to mainly free alpha 1-AT. Since level of alpha 2-M was very low, lysosomal thiol proteases could be one of the major proteases inducing proteolytic damage to middle ear mucosa.
