Effects of orally administered drugs on dynamic viscoelasticity of human nasal mucus.

The effects of orally administered drugs on rheologic properties of nasal mucus were investigated in adult chronic sinusitis patients. The elastic modulus G' and the dynamic viscosity eta' of nasal mucus were determined by an oscillating sphere magnetic rheometer. Both G' and eta' values of the mucus before drug administration were much higher than optimal viscoelasticity for mucociliary transport. Norfloxacin, an antibacterial agent, reduced the G' but not the eta' of nasal mucus. Serratiopeptidase, a proteolytic enzyme, reduced eta' but did not reduce G'. S-carboxymethylcysteine, a blocked thiol derivative of cysteine, did not change either G' or eta'. L-cysteine ethyl ester hydrochloride, a sulfhydryl type of agent, reduced both G' and eta'. The results indicate that some of the orally administered mucokinetic agents can improve the abnormal rheologic properties of nasal mucus in chronic sinusitis.

Effects of myringotomy and orally administered drugs on viscosity and elasticity of middle ear effusions from children with otitis media with effusion.

Clinical and rheological studies were performed on 42 children with otitis media with effusion. Twenty-one patients received S-carboxymethylcysteine (group A), and the other 21 patients were administered Kampo medicine (group B), a Japanese traditional herbal medicine, for 4 weeks following myringotomy. At the end of the 4-week treatment period, 47.2% of the ears in group A and 40.5% in group B were regarded as being free from middle ear effusion (MEE). For rheologically thick MEEs, where the viscosity (eta') and elasticity (G') are much greater than the optimal values for mucociliary transport, post-treatment values of eta' for group A decreased significantly, but those of G' did not, in comparison with pre-treatment values. No such changes in either eta' or G' were observed in group B. Results suggests that the combined therapy for myringotomy and oral SCMC is effective for improving the rheological properties of thick MEE in children.

[A study of microdensitometric evaluation of opacity of maxillary sinuses in patients with nasal allergy].

The opacity of the maxillary sinuses on X-ray film in Waters' view in patients with nasal allergy was evaluated by a microdensitometer. The patients with nasal allergy were diagnosed with the positive results of the following; 1) eosinophilia in nasal secretion, 2) nasal provocation test, 3) intradermal skin test or radioallergosorbent test (RAST). The ratio of density of the maxillary sinuses and orbits was determined by a microdensitometer, and the ratio of the average degree of the opacity of the maxillary sinus to that of the ipsilateral orbit was designated as the M/O ratio. The normal range of the M/O ratio was set over the mean value -2 standard deviations in the normal group. The results are followings. 1) The abnormal shadows in the maxillary sinuses were found out to be 36.9% in patients with nasal allergy. 2) Patients under 15 years old had significantly higher incidence of abnormal shadows, especially in the bilateral sinuses compared with patients over 15 years old. 3) Polyps in the maxillary sinus were recognized in 5.0% of all the sinuses. 4) There were no significant differences for the incidence of abnormal shadow among the patients with nasal allergy caused by different antigens.

[Management of chronic sinusitis in children].

The managements and the results of therapy for children with chronic sinusitis were reported. During the period of 1980 to 1988, 190 patients, aged 3 to 15, were examined at our outpatient clinic. The pathologic conditions of the maxillary sinus were examined by the contrast x-ray study (X-MFT). The mild, moderate and severe lesions in the maxillary sinus evaluated by X-MFT were 22%, 44; and 34%, respectively. Sixty-two per cent of ethmoid sinus was opacified in occipito-frontal view. The degree of patency of the maxillary ostium was evaluated by an application of the constant pressure to the maxillary sinus. Almost all maxillary ostia of these patients were patent. Nasal mucociliary clearance was reduced in 37% of the patients examined after the saccharin method. Antral lavages were performed once in a week for the therapy of 72% of children with chronic sinusitis. The efficacy of this modality was evaluated by the above mentioned items. The maxillary and ethmoid lesions, and nasal mucociliary transit time were significantly improved by the modality after 25 months in average. These results indicate that the antral lavage is an useful therapy for chronic sinusitis in children.

[Therapeutic effects by implantable infusion pump for hepatic malignant tumors].

Chemotherapy and chemoembolization using implantable infusion pump were performed in fifteen cases of hepatocellular carcinoma (HCC) and one case of cholangioma. The following results were obtained; (1) Complete response (CR) was obtained in two cases (12.5%) and partial response (PR) in three cases (18.8%) with HCC. (2) Tumor thrombus in the portal vein disappeared in two of four cases with HCC. (3) The followup period ranged from 54 to 768 days and administration from one to 33 times. These results suggest that good therapeutic effects and quality of life can be obtained by implantable drug delivery system.

[Hepatocellular carcinoma with a complete remission of bone metastasis achieved by arterial chemotherapy].

Reported is the case of a hepatocellular carcinoma with a complete remission of the bone metastasis by arterial chemotherapy. The patient was 60 year old male, with the chief complaint being a tumor at the right side of the chest. The diagnosis was a hepatocellular carcinoma with a bone metastasis at the right 10th rib. The bone tumor showed no decrease in size after an intra-arterial injection of adriamycin, and radiation and or hyperthermia. Therefore, intra-arterial injections of mitomycin C mixed with lipiodol were instituted through the intercostal artery, and no bone tumor was noted after start of this therapy. Although the patient subsequently died, microscopic examination of a specimen obtained at autopsy revealed no malignant cells at the right 10th rib. Intra-arterial injections of anticancer agents mixed with lipiodol therefore are thought to be useful for the treatment of a bone metastasis.

Quantitation of tubotympanal mucociliary clearance in otitis media with effusion.

In an attempt to analyze the tubotympanal mucociliary function in otitis media with effusion (OME), human serum albumin labeled with technetium 99m was instilled into 36 ears with effusion and 16 without. The clearance function of the tubotympanum was measured quantitatively. The viscoelasticity of the effusions was measured and was compared with the clearance rate. The clearance rate was significantly lower in ears with effusion than in those without. A significant negative correlation was observed between clearance rate and dynamic viscosity at dynamic viscosities above 2 poise. It is concluded that ears with effusion have significantly lower mucociliary clearance than those without, and that the viscosity of the effusions plays an important role in the mucociliary dysfunction in OME.

[Brachytherapy of choroidal melanoma using radioactive gold grains--a case report].

A brachytherapy of a choroidal melanoma using 198Au grains has been performed. Procedurally, the sclera was first incised, after which a pouch was constructed and the grains then arranged around the tumor in a circle so as to enclose it, said operative procedure according to the Manchester Technique. Total Gy dosage amounted to 120 Gy. Postoperatively, in due course, the disappearance of the tumor was clearly shown on MRI examination. The small size and the short half-life of the gold grains made it possible to insert the grains into the tissue permanently, and to give an adequate Gy dosage to eradicate the tumor while still preserving the eyeball.

Secondary syphilis of the tonsil.

An 18-year-old man with an ulceration of his right tonsil and cervical lymphadenopathy was examined. The patient also had a papular eruption in his face, chest, abdomen, and upper extremities. There were no signs of genital involvement. Routine serological tests for syphilis were reactive and histological findings of a cervical lymph node biopsy were characteristic for syphilitic lymphadenitis. Spirochetes were also identified in the tissue sections. A diagnosis of secondary syphilis of the tonsil with cervical syphilitic lymphadenitis was made. In recent years, the number of new cases of syphilis has increased in Japan. Since the oral cavity is the most common extragenital site of syphilis, clinicians should bear in mind that oral lesions from an unknown cause might possibly be syphilitic.

Arterial chemotherapy and transcatheter arterial embolization therapy for non-resectable hepatocellular carcinoma.

An assessment was made on the therapeutic effects of arterial chemotherapy and transcatheter arterial embolization (TAE) therapy on 378 cases with non-resectable hepatocellular carcinoma (HCC). For the 191 cases who had undergone arterial chemotherapy, 22% had a 1-year survival rate, 8.9% survived for 2 years, and 4.0% for 3 years. Of these, for the 128 cases who were compatible with our criteria for patient selection, the three survival rates were 31.4%, 12.2% and 5.9% respectively. However, for the other 63 cases, who were incompatible with our criteria, the 1-year survival rate was 1.6% and it was worse for the cases who had received supportive care alone. For the cases who had undergone arterial chemotherapy, the highest survival rates were obtained by the alternate administration of different anticancer agents, and the three survival rates were 39.0%, 13.1% and 4.9% respectively. For the 187 cases who had undergone TAE therapy, the 1-year survival rate was 66.2%, the 2-year survival rate 36.5%, and the 3-year survival rate 21.9%. For the 124 cases with a tumor progression rate of less than 20% in the liver (E1), the survival rates were 77.8%, 50.1% and 30.8% respectively. The peripheral venous drug concentrations of mitomycin C and adriamycin were lower, but were maintained for a longer period in TAE therapy than in arterial chemotherapy. These results suggest that consideration of the criteria for patient selection and the alternate administration of anticancer agents are necessary in arterial chemotherapy, and that the best therapeutic effects can be obtained by TAE therapy combined with chemotherapy for cases of non-resectable HCC because of the chemotherapeutic and ischemic effects on the tumors.

Prognosis of secretory otitis media in relation to viscoelasticity of effusions in children.

Both elasticity (G') and viscosity (eta') of middle ear effusions (MEEs) were measured with an oscillating sphere magnetic rheometer and compared with continuance of fluid retention in 93 ears of 69 children with otitis media with effusion (OME). The ears were divided into four groups according to the viscoelastic properties of MEEs at the first myringotomy. Eight-four percent of the ears in group 2 were free from effusion within 4 months; the difference from the other groups was statistically significant. These results indicate that the mucociliary clearance function plays an important role in the process of recovery from OME.

[Chemotherapy with Cisplatin-Phosphatidyl-choline-Lipiodol (CPL) suspension in unresectable hepatocellular carcinoma].

Twenty-seven patients with unresectable hepatocellular carcinoma (HCC) were treated with Cisplatin-Phosphatidyl-choline-Lipiodol (CPL) suspension. PR was obtained in two of ten cases (20%) by one shot therapy. AFP decreased in 9 of 10 patients by one shot therapy with a 62.1% rate of decrease. In all of 13 patients by TAE, AFP decreased and the rate of decrease was 64.8%. The concentration of CDDP in the peripheral venous blood was lower and continued longer than that of CDDP on the market. Nausea, vomiting and fever were noted in most cases as adverse effects, but they were slight. These results suggest that CPL agents were very chemotherapeutic for unresectable HCC.

[Assessment of three-year survival cases with unresectable hepatocellular carcinoma treated by arterial chemotherapy].

The survival rate was studied in 218 patients with unresectable hepatocellular carcinoma (HCC) in whom arterial chemotherapy was performed by one-shot injection of anticancer agents (one-shot therapy) was studied. Tumor factors and clinical features of 6 out of 218 cases with more than 3-year survival, were also studied. The following results were obtained; (1) One-year survival rate of the cases which were compatible with criteria for one-shot therapy in our department was 35.6%, and the three-year rate was 7.0%. On the other hand, one-year survival rate of the cases not compatible with same was 1.5%. The latter survival rate was worse than that for conservative care. (2) The longest survival time was 6 years and 5 months after one-shot therapy. (3) The tumor progression rate was above 40% in 4 out of 6 cases which survived more than 3 years, and tumor thrombus in the portal vein was noted in 3 cases. Clinical stage was I-II. (4) One-shot therapy was performed repeatedly after initial treatment in these 6 cases. In 3 of the 6 cases, TAE or percutaneous ethanol injection therapy was performed for recurrence or progression of HCC during observation. The results suggest that criteria for one-shot therapy and multidisciplinary treatment during observation are necessary.

[Fibrin deposits on the surface modified PMMA IOLs].

Fibrin reaction and fibrin deposits on the surface of intraocular lens (IOL) after implantation were studied experimentally in vitro and in vivo, using various surface-modified PMMA IOLs. PEO-coated PMMA IOLs and heparinized PEO-coated PMMA IOLs prevented deposition of fibrin. The results suggested these IOLs would be useful to prevent accumulation of fibrin deposits.

[Effect of vitamin E-containing liposome on experimental sugar cataract].

The effect of vitamin E-containing liposome on experimental sugar cataract formation in vitro was investigated. The lenses of male Wistar rats aged 6 weeks were prepared by incubating with 55. 6 mM glucose with vitamin E-containing liposome (dipalmitoyl phosphatidylcholine: DPPC). We examined the formation of lens opacity and assayed vitamin E and its related compounds. Incubation with vitamin E-containing liposome prevented sugar cataract formation. In Addition, vitamin E concentration in lens was significantly elevated by incubation with vitamin E-containing liposome. In lenses of the high level glucose group incubated without vitamin E-containing liposome, lipid peroxide (LPO) content was increased, but in lenses of the high-level glucose group incubated with vitamin E-containing liposome the increase was significantly inhibited at each incubation time. Vitamin E had no effect either on the decrease of reduced glutathione (GSH) or the increase of sorbitol content in lens incubated with high level glucose medium. In conclusion vitamin E-containing liposome was transported from medium to lens well and was significantly effective in preventing experimental sugar cataract formation in vitro. The protective effects of vitamin E are probably caused not only by its antioxidative action.

The effect of an orally administered proteolytic enzyme on the elasticity and viscosity of nasal mucus.

We have evaluated the effect of serratiopeptidase (SER), a proteolytic enzyme, on the elasticity and viscosity of the nasal mucus in adult patients with chronic sinusitis. SER was administered in a dose of 30 mg/day orally for 4 weeks. Nasal mucus was collected from the nasal cavities of each patient before (week 0) and 4 weeks after the start of the medication (week 4). The storage modulus (G') and the dynamic viscosity (eta') of each specimen of nasal mucus were determined by an oscillating sphere magnetic rheometer at frequencies of 0.5, 1, 5, 10 and 20 Hz at a constant temperature of 25 degrees C. The dynamic viscosity (eta') of the mucus at week 4 was significantly lower than that at week 0 (at frequencies of 5, 10 and 20 Hz). No significant differences were observed in the storage modulus (G') between the mucus at week 0 and week 4. SER reduced the viscosity but not the elasticity of the nasal mucus. These findings are discussed in relation to mucociliary clearance.

Bactericidal activity of middle ear effusion on a single isolate of non-typable Haemophilus influenzae.

Bactericidal activity of middle ear effusion (MEE) from children with otitis media with effusion (OME) on one strain of non-typable Haemophilus influenzae was measured and was compared with that of normal human serum. H. influenzae culture negative MEEs had a bactericidal activity on one strain of non-typable H. influenzae (P less than 0.01), and the activity of serous effusions was significantly stronger than that of mucoid ones (P less than 0.01). The bactericidal activity of 20% pooled MEE was almost equal to that of 4% normal human serum. Heat treatment (56 degrees C, 30 min) abolished this activity of both pooled MEE and normal human serum. Addition of fresh guinea pig serum (GPS) as a complement source to the heat-treated pooled MEE restored this activity, while its addition to the heat-treated normal human serum failed to do so. These results suggest that complement factor is important in the bactericidal activity of MEE.
