[A case of cystic pheochromocytoma].

A case of cystic pheochromocytoma is reported. A 64-year-old man was referred to our hospital because of right flank pain and gross hematuria. Excretory urography showed an inferior displacement of his right kidney, and right suprarenal mass was suspected. CT and ultrasonographic findings suggested that the most likely diagnosis was cystic adrenal tumor. Right adrenal tumor was removed through a transverse abdominal incision on October 15, 1986. During the procedure, the blood pressure rose to 260/100 mmHg at the time of tumor manipulation. The histological diagnosis was pseudocystic pheochromocytoma. Pathogenesis of cystic adrenal tumor is discussed briefly.

[A case of pheochromocytoma associated with von Recklinghausen's disease].

A case of pheochromocytoma associated with von Recklinghausen's disease is reported. A 53-year-old man consulted the Department of Dermatology of our hospital because of Dermatoptosis in the breast and abdomen. He asked to resect of the mass. Blood bio chemistry findings were almost within normal limits but only catecholamines in serum and urine were remarkably higher. With a result of other detailed examinations, he was diagnosed as pheochromocytoma. He was referred to our clinic. Laparotomy was carried out under the strict monitoring of the circulation. We diagnosed his clinical status to non-hypertensive type. The tumor was 2.5 x 1.7 x 1.7 cm in size and weighed 10 g. After operation, serum and urine catecholamines returned to all within normal limits. a operation for the dermatoptosis was performed afterwards at the Department of Dermatology.

[A case report of brain metastasis due to an advanced gastric cancer].

In April 1984, a 49-year-old male underwent a partial gastrectomy for advanced gastric cancer. At that time, many metastatic foci were found, for example, in the rib, the lumbar vertebrae, the cervical vertebrae, and in the abdominal lymph nodes. Approximately one year later, he suddenly became disturbed and lost consciousness. A metastatic brain tumor was diagnosed. To destroy this tumor the patient received a course of irradiation therapy with a total dose of 45 gys directed toward head. Several months later, however, recurrence of the tumor was detected at the same intracranial region. Thus, a course of therapy, employing a combination of FT-207 and ACNU, was initiated against the recurrent tumor. Though the tumor was not eliminated, the patient manifested few of the prior symptoms. This state has continued for several months, with the patient having survived for three years after his gastrectomy. The following report will present the details of this case.

[Primary adenocarcinoma of the ureter associated with hypocalcemia: report of a case].

A case of primary adenocarcinoma of the ureter, associated with severe hypocalcemia in a 73-year-old man is reported. The patient had undergone left nephrectomy for pyonephrosis due to a ureteral stone, 29 years previously. On November 4, 1986, he visited the hospital with dysuria and overflow incontinence as the chief complaints. He was admitted with suspicion of prostatic carcinoma, but prostatic stones and chronic prostatitis were proven. However, urine cytology showed adenocarcinoma cells, and a ureteral tumor was found at the right ureterovesical junction. On December 10, right partial ureterectomy, total cystectomy and right ureterocutaneostomy were performed. Moderately differentiated, invasive adenocarcinoma was found at the right ureteral end, 1.5 cm in length and in part poorly differentiated transitional cell carcinoma was present continuously to the adenocarcinoma. Hypocalcemia was cured easily with activated vitamin D3. The patient, 9 months after the operation, is apparently free of the ureteral tumor.

[Cerebral lateralization in two cases of crossed dextral aphasia with right-hemisphere arteriovenous malformation].

This paper reports two cases of crossed dextral aphasia. The first patient was a 60-year-old right-handed male with no family history of sinistrality. He experienced sudden onset of left hemiplegia and loss of consciousness. A CT scan showed high-density area in the right fronto-parietal region. An angiography revealed an arteriovenous malformation (AVM) in the right parietal lobe. It was fed by a branch of the middle cerebral artery and drained through a cortical vein. Neuropsychological examination one week after the surgery showed severe defects of all language moderalities. He was alert and cooperative, but completely mute. He recognized common words by auditory and visual stimuli, but could not perform simple command. He wrote some meaningless letters when asked to write his own name. Auditory and reading comprehension gradually improved thereafter, but Broca's type of aphasia with non-fluent hesitant and effortful output was still present four months after the surgery. The second patient was a 38-year-old right-handed male. All members of this family are right-handed except for one sister who is left-handed. He suddenly suffered left hemiparesis and loss of consciousness. A CT scan disclosed a right parietal intracerebral hematoma. And an AVM which was fed by the angular artery and drained through a cortical vein was angiographically demonstrated in the same area. Postoperatively the left hemiparesis rapidly disappeared, but left homonymous hemianopsia and anomic type of aphasia still persisted. His speech was fluent and daily communication was possible in spite of circumlocutory paraphasic output.(ABSTRACT TRUNCATED AT 250 WORDS)

Chemistry and biochemistry of metallothionein.

A wealth of chemical, spectroscopic and structural data attest to the uniqueness of the metallothioneins as a group of novel bioinorganic structures. Their earmarking feature is the arrangement of "soft" metal ions in complexes with cysteine side chains to form discrete metal-thiolate clusters. In this review an account is given of the chemical characteristics of the 52 metallothioneins whose primary structures are now known completely or in part. Also included is an up-to-date summary of the spectroscopic properties and of the spatial structure models derived from X-ray diffraction crystallographic analysis and from two-dimensional nuclear magnetic resonance spectroscopy.

Sex difference of acetohexamide reduction in rat liver.

The acetohexamide reducing activity in hepatic 10,000 X g supernatant was significantly higher in male than in female rats. Evidence obtained in this study suggests that the microsomal carbonyl reductase may contribute to the sex difference in the reductive metabolism of acetohexamide in rats.

[Quantitative study of diffuse increased 99mTc-methylene diphosphonate accumulation of both kidneys].

Patients with diffuse increased accumulation in both kidneys on 99mTc-MDP bone scintigram were evaluated quantitatively. Among 1,100 cases, 7 (0.63%) showed this finding. The accumulation density ratios of kidneys/lumbal vertebra in A/D exchanged images of these patients were compared with control group, so that these ratios of patient group were higher than control group statistically. In conclusion, the accumulation density ratio was made the index of diffuse increase accumulation in both kidneys on 99mTc-MDP bone scintigram.
