Three cases of duodenal myogenic tumor are described in detail. All the three were surgically treated and the final diagnoses were histologically established. The three consisted of two leiomyomas and one leiomyosarcoma. To make clinical diagnosis, roentgenologic, endoscopic, and angiographic examinations were empolyed. The selective angiography was more reliable than endoscopic biopsy to make a differential diagnosis whether a tumor was benign or malignant. Surgical procedures were required not only for malignant myomatous lesions but also for benign lesions because the latter also produce a high incidence of hemorrhage from ulceration of the tumor. Out of the three cases, one was successfully treated by enucleation but the other two were undergone more extensive surgery such as a Whipple's procedure or wide resection of the duodenal wall with side-to-side duodenojejunostomy. Operative techniques are discussed in detail basing on the biological characteristics, location, and size of the tumor.

The mechanism of protein loss from the gastric wall was investigated in patients with Ménétrier's disease, gastric polyps, and carcinoma of the stomach. In Ménétrier's disease, plasma protein was lost as glycoprotein in the gastric juice secreted from proliferated foveolar cells. Some of the gastric polyps also showed pathologic findings similar to that of Ménétrier's disease. As a treatment for the diffuse type of Ménétrier's disease, subtotal gastrectomy was recommended by presenting enough recovery from hypoproteinemia. A small amount of protein lost from the remaining portion of the stomach may be almost completely digested and reabsorbed from the intestine. In instances of carcinoma of the stomach, the mechanism of protein loss was speculated to be loss of lymph, blood or other interstitial fluids from opened capillaries of the cancerous surface. Complete removal of the carcinoma, therefore, certainly results in recovery from hypoproteinemia.

e-antigen and anti-e were assayed in sera of asymptomatic HBs-Ag carriers and of patients with liver diseases. Thirteen out of 34 (38.2%) asymptomatic carriers were positive for e-antigen, which was in sharp contrast to the reports from USA and Europe. e-antigen was detected to a greater extent in patients with chronic active hepatitis, reversely anti-e in patients with chronic persistent hepatitis. However, e-antigen was found rarely in patients with cirrhosis and never in 23 cases with hepatoma positive for HBs-Ag. HBc-Ag in the liver was detected in 4 out of 8 e-antigen positive asymptomatic carriers and in 4 out of 5 patients with chronic liver diseases with e-antigen respectively, and moreover in 3 out of 14 anti-e positive cases, so that the presence of anti-e did not necessarily mean the negativity of HBc-Ag in the liver. Anti-HBc titer, however, was lower in anti-e positive sera than in e-antigen positive ones. This may implicate the decreased replication of HBV in cases with anti-e. These results emphasize that the investigation of e-antigen/anti-e is mandatory for the evaluation of the prognosis of asymptomatic carriers and of patients with chronic hepatitis.
Computer simulation of the ventricular activation sequence of the WPW syndrome was attempted by producing fusion beats in which localized ventricular myocardium was activated earlier than the "normal excitation". It suggested that the site of pre-excitation was; i) at the posterior or the lateral free wall of the left ventricle adjacent to the A-V groove in type A, ii) at the antero-lateral base of the right ventricle in type B, and iii) at the posterior paraseptal region of the right ventricle in type C. A mild degree of pre-excitation at the left ventricular postero-lateral region will result in type B QRS complexes.
The antibody to hepatitis B core antigen (anti-HBc) is belived to be a marker for natural infection with hepatitis B virus (HBV). In order to study the etiological role of HBV in relation to primary hepatocellular carcinoma (PHC), the anti-HBc in sera of 31 PHC patients was surveyed by the immune adherence hemagglutination method which was about 10 times more sensitive than the complement fixation method. Twenty two out of 31 PHC cases were positive for anti-HBc (71.0%). The is a higher rate of incidence than that of HBs-Ag (51.6%). However, high anti-HBc titer above 2(10) which might reflect current infection with HBV was observed in 15 of these 22 cases. The remaining 7 cases had a titer lower than 2(8); 5 of these patients had neither HBs-Ag nor anti-HBs in their sera. As control, 37 anti-HBs positive blood donors with no definite liver disease were surveyed for anti-HBc titer. Thirty-four of them were positive, but in the majority of cases, the serum titer was less than 2(9), which might only reflect previous infection with HBV. These findings indicate that not only the prevalence of anti-HBc, but also the determination of its real titer is quite important for studying the relationship of HBV to PHC.
The present experiments were undertaken to elucidate the effect of chronic oral administration of clonidine on the endogenous catecholamine metabolism in spontaneously hypertensive rats (SHR). A three month administration of clonidine (20 microgram/kg/day p.o.) produced a slight decrease in blood pressure and heart rate. No significant change was observed in the body weight. A one month administration of clonidine (300 microgram/kg/day p.o.) produced hypotension and weight loss. No significant effect was observed in heart rate. After a two month administration of clonidine (300 microgram/kg/day p.o.), the catecholamine content in the brainstem showed a significant decrease as compared with that in control. 3H-norepinephrine (NE) turnover rate after intraventr culari injection of 3H-NE tended to decrease in the brainstem of SHR while no significant difference was recognized from a view point of uptake, biological half life, elimination rate constant of 3H-NE between control and clonidine administered rats. Under the same experimental conditions, a marked rebound hypertension occurred when medication is discontinued in some cases of this group of SHR.

Echocardiograms of 52 patients with the Wolff-Parkinson-White (WPW) syndrome were investigated. Abnormal patterns of motion were observed in the left ventricular posterior wall (LVPW) and the interventricular septum (IVS). The abnormal LVPW motion was presumably specific for the syndrome and included early onset of the anterior motion which preceded the first heart sound (S1) and a premature peak formation of the anterior displacement which occurred before the second heart sound. The latter was usually followed by a second lower peak. These findings seem to suggest that both contraction and relaxation of LVPW begin earlier than normal due to ventricular pre-excitation initiated at or near the LVPW. The IVS abnormality characteristic of the syndrome was a posterior protrusion, the onset of which preceded S1. Subsequent motion of IVS was either paradoxical or hypokinetic during ejection. These IVS abnormalities were quite similar to those observed in left bundle branch block. The abnormal LVPW motion was observed in all 20 Type A patients and 10 of the 32 Type B patients. The abnormal IVS motion was observed in 10 Type B patients. We feel that echocardiography could be used as an auxiliary noninvasive means of locating the pre-excitation site in patients with WPW.
