[Nephrotoxic effect of gold sodium thiomalate in rats--ultrastructual observations using electronmicroscopy and X-ray energy dispersive analysis].

The purpose of this study is to demonstrate renal injuries by gold sodium thiomalate (G) with ultrastructual changes and gold deposition in kidney tissue using X-ray energy dispersive analysis (XEDA). Twenty-five mg of G containing 12.1 mg of Au was injected into rats intraperitoneally. The rats were divided into 5 groups. Group 1 was sacrificed 6 hours after the injection of G, and group 2 after 24 hours, group 3 after 72 hours and group 4 after 144 hours. Group 5 consisted of the control-rats which were provided with injections of saline. Gold contents in kidneys, liver, lungs and spleen were measured using the flameless atomic absorption method. XEDA was also performed in order to confirm the gold deposition in tissue. Among the organs, only the kidney showed remarkable changes with increased weight. Group 1 already showed marked azotemia which reached to the maximum level in group 3. The amount of gold content in the organs did not change significantly in spite of a marked reduction of serum gold concentration among the 4 groups. Histological examinations revealed marked degeneration and necrosis of pars recta in proximal tubules, although no prominent abnormalities of glomeruli could be observed. Using an electron microscope, many electron dense particles in lysosome were noticed, mainly in proximal tubules. We also found these particles in lysosome of glomerular epithelial cells. Using XEDA, these electron dense particles were demonstrated to be gold, since characteristic energy of gold was found. In conclusion, the kidney was shown to be the most accumulative organ of gold. G caused acute extensive necrosis of proximal tubules. Gold was demonstrated as electron dense particles in lysosomes mainly in proximal tubules, but also partly in glomeruli. Therefore, it was confirmed that a large amount of G had a strong nephrotoxic effect in rats.

Cranial coverage involving scalp, bone and dura using free inferior epigastric flap.

Four cases of successful reconstruction of scalp and skull defects are described. The advantages of the free inferior epigastric flap are that it is large enough to cover massive defects of the head and has an excellent vascular pedicle which allows it to be directly sutured to recipient vessels in the neck without interpositional vein grafts.

Pulmonary deposition and clearance of a coal fly ash aerosol by inhalation.

Wistar male rats were exposed to coal fly ash aerosols at average exposure concentration of 10.4 mg/m3 for 7 hr/day, 5 days/week for 1 month. Some rats were sacrificed just after the exposure, while others were kept for 6 or 10 months clearance time before sacrifice. There were no differences in body weight gain between fly ash exposure groups and controls. The burden of fly ash was estimated by the measurement of aluminum contents in rat organs. Aluminum concentrations in lungs of exposure groups were much higher than those of controls. No apparent deposition of fly ash was observed in the liver, kidneys, spleen, and blood, but lung burdens of up to about 0.7 mg of fly ash were found. The apparent deposition fraction was 5.1% after the 1-month exposure. The clearance rate of fly ash deposited in rat lungs may be very slow.

Characterization of silver-stained nucleolus organizer regions (Ag-NORs) in 16 inbred strains of the Norway rat, Rattus norvegicus.

Silver-stained nucleolus organizer regions (Ag-NORs) in metaphase chromosomes of cultured fibroblasts were compared among 16 inbred strains of the Norway rat, Rattus norvegicus. Ag-NORs were located at the secondary constrictions or juxtacentromeric regions of the short arms of chromosomes 3, 11, and 12. The frequency and relative size of Ag-NORs were found to be strain-specific, providing a genetic marker system useful for characterization of inbred strains. While considerable cell-to-cell variation was observed within a given strain, the strain-specific pattern of Ag-NORs was shown to be consistent in cultured and noncultured cells obtained from different tissues of embryos, newborns, and adults, as well as in successively cultured cells examined up to the 10th subculture generation. The patterns of Ag-NORs in F1 hybrids made between some of the inbred strains were in general agreement with those expected from the parental strains; some unexpected patterns were noted in F1 hybrids of a particular cross, suggesting the possible existence of nucleolar interactions in such interstrain hybrids, although this has to be confirmed.

[Gastric and duodenal ulcers as occupational psychosomatic diseases].

Various stresses are considered to be involved in the development of gastric and duodenal ulcers. We interviewed patients of either gastric ulcer group (GU) or duodenal ulcer group (DU) with regular occupation who were admitted to our Psychosomatic Illness Department in order to examine the differences in the characteristics of their stress and developmental factors of their diseases between the two groups. Developmental factors were scored for comparison. The following results were obtained. Though group DU was generally younger than group GU, it tended to have a greater number of episodes. No significant difference was found between the two groups in living habits, life history, or familial, social and physical environments. However, group DU had greater stress than group GU in the working environment. A significantly greater number of patients in group DU smoked and had problems in personal relations as compared with GU. There was a significant correlation between smoking habit and stressfulness of the working environment in both groups. In group DU, there was a significant correlation between the father-child relationship and personal relations. In group DU, a significant number of those who had problems in the relation with their superiors at work had problems in the father-child relation. These findings show that patients with duodenal ulcer had more problems in the working environment, particularly in personal relations, than those with gastric ulcer, and smoking habit appears to accelerate the development of the diseases. Also, a possible association was suggested between problems of personal relations at work and the history of growth.

Plasma somatostatin response in normal and gastrectomized subjects.

The present work was undertaken to investigate a possible contribution of the stomach to plasma concentrations of somatostatin-like immunoreactivity (SLI) in humans. For this purpose, plasma SLI responses were determined in previously gastrectomized subjects during lipid-heparin induced elevation of plasma free fatty acids (FFA), which gave a consistent rise of plasma SLI in normal controls. Fasting plasma SLI levels in both the subjects with antral and total gastrectomy did not differ from the values in normal controls. Acute elevation of plasma FFA resulted in about 5-fold increase of plasma SLI above basal level in the controls. In contrast, in both the gastrectomy groups there was no significant rise in plasma SLI, despite a comparable elevation of plasma FFA levels. In conclusion, these findings demonstrate that the stomach, probably the antral region of the stomach, accounts for the most part of increase of plasma SLI when hyperfatty-acidemia is induced and suggest that following the stimulation by nutrients the stomach makes a greater contribution to plasma SLI levels than does the pancreas or extragastric intestinal tract in humans.

[A case of pelvic arteriovenous malformation].

Congenital pelvic arteriovenous malformation (AVM) is an uncommon lesion. This disorder occurs in female frequently than in male. A 44 year-old male complained of right lower abdominal discomfort and hemorrhoids. He had neither history of abdominal trauma nor abdominal surgery. A pulsatile mass was palpable on rectal digital examination and bruit was heard at right lower abdominal quadrant. Pelvic computerized tomogram showed an abnormal mass with homogeneous density near the right side of the bladder. The mass was equally enhanced by the contrast medium. Digital subtraction angiogram revealed the pelvic AVM which originate from right hypogastric artery. Selective angiogram showed many feeding arteries and a dilated vein. Surgical procedure was performed with transabdominal approach because this AVM was localized. All feeding arteries and aneurysmally dilated vein were completely removed. Post operative course was uneventful. Follow up arteriogram after the surgery showed no further abnormal arteriovenous connections.

Follow-up study on pulmonary function of dust workers: an epidemiological evaluation of discontinued workers of dust operations.

An epidemiologic study on 176 dust workers from various facilities was initiated in 1978. Their mean age was 48.3 years and mean duration of employment was 21.6 years. No workers complained of chronic cough and phlegm. Their chest X-rays were found to belong to Category I of the diagnostic criteria for pneumoconiosis in Japan. Pulmonary function test was performed on these workers, however, 54 workers discontinued taking part in the study by the end of 1979, and 49 workers had discontinued by 1981. A total of 73 workers had a five-year follow-up survey of pulmonary function. Forced expiratory volume in one second divided by forced vital capacity (FEV1.0/FVC) and forced expiratory flow rate at 25% of FVC divided by height (V25/H) were mainly compared by smoking habit, work years, type of job, size of company, and the time when they discontinued their jobs. Predicted percentage values of lung function were used for the adjustment of the influence of workers' ages. Decrement of FEV1.0/FVC and V25/H in the five-year follow-up group were larger than the screening level. The mean values of those functional parameters in the early discontinued group were lower than late discontinued and active working groups. The active working group was considered to be a physically healthy workers' population.

[A simple determination of phenol in urine by high performance liquid chromatography].

A simple analytical method for urinary phenol by high performance liquid chromatography was tested. This procedure consists of the following three steps: hydrolysis of urine with hydrochloric acid, extraction of phenols with isopropylether, separation and determination of phenol by high performance liquid chromatography, under the following operating conditions: column, LiChrosorb RP 18 (250 mm X 4 mm, I.D.); mobile phase, acetonitrile-water (3: 7, v/v) with a flow rate of 0.8 ml/min; column temperature, 40 degrees C; detection, UV monitor at 270 nm. The detection limit (signal-to-noise ratio = 2) was 1 ng. The analytical results by this method were compared with those obtained by another method used for gas chromatography. The analytical results obtained by the two methods were in good agreement (r = 0.998) for 30 normal urine samples tested. The geometrical average value for phenol in normal urine samples (n = 30) was 8.2 micrograms/ml. Therefore this method was proved to be useful for biological monitoring of benzene exposure.

[Skull base metastasis of prostatic cancer presenting as exophthalmos--a case report].

A 72-year-old man was admitted to our hospital complaining of diplopia and right exophthalmos. Craniography and CT scan showed thickening of the right orbital roof and no intracranial lesion. Total acid phosphatase and prostatic acid phosphatase were high. Bone scintigraphy revealed high uptake at that lesion and the right pelvis. Unroofing of the right orbit and opening of the optic canal were performed in order to reduce intraorbital pressure. Microscopic examination revealed a metastatic lesion of prostatic cancer. Postoperatively, the patient was treated with "Honvan" and the course has been good.

Keratoconus with pellucid marginal corneal degeneration.

Examination of 1,625 Japanese patients with keratoconus and 20 patients with pellucid marginal corneal degeneration showed that 17 patients with pellucid degeneration also had keratoconus in the central portion of the cornea. Three patients with pellucid degeneration without keratoconus showed bilateral involvement. Among the 17 cases of pellucid degeneration with keratoconus, eight patients showed bilateral involvement and the others showed unilateral involvement. Pellucid marginal corneal degeneration with or without keratoconus may be a variant of keratoconus or a different manifestation of the same etiologic factor.

Growth patterns of gastric carcinoma detected by mass survey.

Two hundred and sixty-seven cases of gastric carcinoma detected by mass survey included 196 discovered at the first examination (Group A) and 71 noted at the annual sequential examination (Group B) were analyzed in terms of growth pattern and prognosis. The incidence of early carcinoma was 54.9 per cent in Group B higher than 35.7 per cent in Group A. In early carcinoma, the superficially spreading (SUPER) type of carcinoma was more common in Group A than in Group B. On the contrary, the penetrating growth (PEN) type of carcinoma was more frequently found in Group B than in Group A. About eighty per cent of those with an advanced carcinoma, in both groups, was the advanced PEN type. The outcome of the patients in Group B was more favorable. These results support the hypothesis that the majority of advanced carcinoma is derived from the PEN type of early carcinoma. Early detection of the PEN type of carcinoma is mandatory.
