[A case of pancreatic carcinoma coexist with pancreatic pseudocyst].

Report of Pancreatic carcinoma coexist with Pancreatic pseudocyst is rare. We have experienced a case of pancreatic carcinoma which was diagnosed and resected by coexisted pancreatic pseudocyst. The patient aged 53 years, complained of epigastric distension and visited our hospital. There was no past history of abdominal injuries and he didn't drink much. A cyst 7cm in diameter was found at pancreatic tail lesion by our examination. By blood chemistry tumor marker CA19.9 was 310 and he complicated diabetes mellitus. We suspected pancreatic carcinoma coexist with pancreatic pseudocyst, so distal pancreatectomy and splenectomy was performed. We found a solid tumor sized about 3cm in diameter at just proximal of pancreatic cyst. Since perioperative histological examination revealed adenocarcinoma, we performed lymph node dissection. The tumor was highly differentiated adenocarcinoma which invaded retroperitoneum in some lesion. The patient died 10 months later by recurrence. We concluded that we must take into account not only laboratory finding but their clinical course in such cases.

[Subacute subdural hematoma--report of 4 cases and a review of the literature].

Subdural hematoma is divided roughly into two types acute and chronic. The two show an entirely different mode of illness. The authors have encountered 4 cases of subacute subdural hematoma in which characteristics of both types coexisted. These cases are characterized by the following. 1) The disease develops in the elderly persons with a history of trauma unknown or after minor head injury. 2) There is a relatively long period of clear consciousness and they visit a hospital when they are in the subacute stage, 3) They have a history of drinking alcohol heavily as a habit and there is a high risk of hypertension and diabetes. 4) Brain CT findings sometimes reveals mixed density hematoma. 5) Hemorrhage from the cortical artery is occasionally noted as the operative findings. Hematoma membrane is absent. 6) The outcome is generally poor because of systemic complications. As described above subacute subdural hematoma was similar to chronic subdural hematoma in the clinical course and CT findings. But operative findings of this disease indicated acute subdural hematoma. Repeated minor hemorrhage, related to coagulation disorder and brain atrophy would be important as the mechanism of subacute subdural hematoma. The effectiveness of perforation craniotomy as radical operation was low and removal of hematoma by major craniotomy was needed. The concept of subacute subdural hematoma is considered important in deciding on a therapeutic policy.

Nucleotide sequence of a gene which enhances the activity of glyoxalase I in Saccharomyces cerevisiae.

A gene whose product enhances glyoxalase I activity in yeast Saccharomyces cerevisiae cells was cloned as a 6-kbp DNA fragment in the vector YEp13. The DNA fragment was subcloned in approximately 2 kbp and its base sequence determined. An open reading frame with 318 nucleotide pairs (encoding 106 amino acids) was found. According to Northern blotting analysis, the yeast transformed with the hybrid plasmid harboring the 2-kbp fragment yielded two different transcripts. From the DNA sequence of the fragment, it is deduced that the gene product would contain seven cysteine residues, five of which would be located near the carboxy terminus of the peptide.

[A decision analysis model in laboratory medicine--appropriate usage of microbiological examination in women with dysuria].

Decision analysis is a noteworthy methods to solve complex medical problems and select best alternative under uncertainty, and its practical applications in laboratory medicine are anticipated for appropriate usage of laboratory resources. From the view of classical microbiology, it is necessary to identify bacteria causing an infection and select sensitive antibiotics to this pathogen. Decision analysis was used to estimate the clinical and economic implications of microbiological testing for urinary tract infections at the initial visit for all women with acute dysuria. A strategy of initial culture was compared with a strategy involving the use of culture only for patients with symptoms persisting 3 days after initial therapy (wait 3 days strategy). Expected direct cost and medical care days were 10,748 yen and 7.12 days for initial culture strategy and 6872 yen and 7.24 days for wait 3 days strategy. Sensitivity analysis showed that initial culture strategy was not cost effective when the frequency of antibiotics resistant organisms would be increased or required days to obtain microbiological tests results would be decreased.

Circadian rhythm disturbances in neurological patients with chronic hypothermia.

1) We investigated the circadian rhythms of BT and sleep-wakefulness in 8 neurological patients who showed chronic low BT. Their etiologies were varied. 2) Their thyroid functions were normal, but their hypothalamic functions were disturbed endocrinologically to various degrees. 3) Their temperature-elevation system was able to function when they suffered from infectious disease. 4) Six patients had circadian rhythm disturbances of BT and sleep-wakefulness. This fact suggests that there is a close relationship of BT regulation and control of its rhythm, and also a close association of 2 pacemakers, namely, that of sleep-wakefulness and that of BT.

[Infarction in the territory of the anterior choroidal artery due to embolic occlusion of the internal carotid artery--report of two cases].

A case with infarction in the territory of the anterior choroidal artery (AChA) due to embolic occlusion of the internal carotid artery (ICA) is rare. We described two cases and investigated the mechanism of the territory of the AChA. Case 1 was a 69-year-old man. Case 2 was a 71-year-old woman. The neurological examination in both cases showed left homonymous hemianopsia, left facial palsy, left hemiparesis and left hemisensory disturbance. CT scan in these cases showed infarctions of the right uncus, amygdaloid nucleus, genu and posterior limb of the internal capsule, globus pallidus, lateral geniculate body and tail of the caudate nucleus. The right common carotid angiogram showed a complete occlusion of the ICA at its cervical segment in case 1 and at its carotid siphon in case 2. In both cases, the left carotid injection visualized the right anterior cerebral artery and right middle cerebral artery via the anterior communicating artery well, but the right AChA was not visualized. In case 1, the collateral pathways from the right external carotid artery (ECA) and the right posterior communicating artery (PCoA) to the right ICA were not supplied and the precommunicating segment of the right posterior cerebral artery was hypoplastic. [Effects of unilateral temporal lobectomy on verbal dichotic listening test].

Verbal dichotic listening test was conducted before and after anterior temporal lobectomy on 25 patients who underwent the operation because of their medication-resistant epileptic seizures. Their speech dominance by Wada test was all left sided. 1. Preoperatively, patients having the epileptogenic focus in the right temporal lobe (R-TLE) and those having the focus in the left (L-TLE) were, as expected, all right-ear dominant. The mean number of correct responses was fewer in L-TLE than in R-TLE group. 2. Postoperatively, no detrimental effects for recognition of verbal auditory stimuli by the ear contralateral to the focus was observed both in L-TLE and in R-TLE group. 3. Postoperatively, the ear-dominance shifted: to the left in patients with L-TLE, and to the right more conspicuously in patients with R-TLE compared to the preoperative scores. In other words, the recognition ability by the ears ipsilateral to the side of focus, or of resected temporal lobe, was ameliorated. Summarizing, the unilateral anterior temporal lobectomy did not cause "lesion effect" but yielded improvement of verbal auditory recognition by ears ipsilateral to the epileptogenic focus. Diminished epileptic bombardment resulted in by the resection surgery may be a possible explanation.

Comparison of arteriosclerosis obliterans of lower limbs between China and Japan.

Clinical data on 50 patients with arteriosclerosis obliterans (ASO) in China (Group A) and Japan (Group B) respectively were studied for better understanding of these changing trends. The results showed that the incidence of ASO peaked in Group A in the 50 to 69 age range (88%) and in Group B in the 55 to 74 age range (78%). Group A had a lower ratio of smokers and cases of diabetes mellitus than Group B but a higher ratio of hypertension and a higher total cholesterol level. Angiography showed that the prevalence of atherosclerotic lesions in the lower extremities as a whole was similar for both groups. They were different in location, extent and severity of the arteries involved between the two groups. We believe that this study has shown various differences between the two groups but that the causes are probably multifactorial.

[Histopathological observations on orbital fatty tissue in dysthyroid ophthalmopathy].

Seventeen specimens of orbital fatty tissue taken from cases of dysthyroid ophthalmopathy at the time of operation were observed by light and electron microscopy, and also by immunohistochemical staining. The pathological changes observed were infiltration of the chronic inflammatory cells, deposit of fibrin, sclerosis of vascular wall, occlusion of the vascular lumen, and accumulation of hydrophilic mucopolysaccharide. Immunoglobulin G was detected in fat cells and lymphocytes, and some of the infiltrating lymphocytes were determined to be T cells, by immunohistochemistry. These findings suggested that the pathology of the dysthyroid ophthalmopathy was closely related to the chronic inflammation induced by the immune reaction. The exophthalmos was considered to occur due to edema secondary to chronic inflammation and accumulation of the mucopolysaccharides.

[Studies of CT images of bronchial tuberculosis, in reference to its pathogenesis].

CT images of 18 patients with bronchial tuberculosis were studied and pathogenesis of the disease was analyzed on the basis of CT findings. Characteristic CT findings, observed in 15 cases, were thickening of bronchial wall and/or swelling of adjacent pulmonary arteries in peripheral part of bronchial tuberculosis lesion. In 11 of these 15 cases, observed pulmonary cavity and/or large tuberculosis lesions at the periphery of the above-mentioned CT findings, were considered to develop to the direct extension from parenchymal tuberculosis. Other 4 cases showed the above-mentioned characteristic CT findings, but without parenchymal lesions, the pathogenesis of the disease remained undetermined. In remaining 3 cases, neither characteristic CT findings nor remarkable parenchymal lesion was noticed. One of them was clarified by bronchoscopic examination.

[Diagnostic usefulness of transbronchial aspiration and bronchial lavage for pulmonary tuberculosis].

Fiberoptic bronchoscopy is a well established methods as a useful tool in the diagnosis of pulmonary tuberculosis with smear negative cases. In order to get the early and definite diagnosis of pulmonary tuberculosis, we performed transbronchial aspiration and bronchial lavage by a fiberoptic bronchoscope in 97 patients. All patients (1) were clinically suspected of having active tuberculosis; (2) showed abnormal chest roentgenogram suggesting tuberculosis; (3) showed negative sputum smears of acid-fast bacilli, or had no sputum. The results of the study were summarized as follows: 1) Final diagnosis of study subjects were 90 patients of active pulmonary tuberculosis, and 7 patients of pulmonary atypical mycobacteriosis. 2) Sputum culture of acid-fast bacilli was positive in 22 out of 90 patients with active pulmonary tuberculosis. 3) Smear and culture examination of acid-fast bacilli of transbronchial aspirates were positive in 9 and 28, respectively out of 90 patients. 4) Smear and culture examination of acid-fast bacilli of bronchial lavage were positive in 12 and 39, respectively out of 90 patients. 5) A rapid and definite diagnosis was made in 16 out of 90 patients by transbronchial aspirates or bronchial lavage. 6) Atypical mycobacteria were detected in 7 out of 97 patients by transbronchial aspirates or bronchial lavage. 7) There were no serious complications such as pneumonia and exacerbation of pulmonary tuberculosis. These results suggested that transbronchial aspiration and bronchial lavage were useful procedures for rapid and definite diagnosis of pulmonary tuberculosis.

[MR images of gliomas].

MR images of 55 gliomas (23 malignant gliomas, 16 Grade I-II astrocytomas, 7 oligodendrogliomas, 5 pontine gliomas, 2 central neurocytomas and 2 ependymomas) were reviewed. Histological diagnosis was obtained in all of these gliomas. Morphologic appearance and signal intensities of each glioma were evaluated on T1 and T2 weighted images. Nearly isointensity areas which correspond to enhanced areas on CT scans were observed in all malignant gliomas, and 19 of 23 malignant gliomas (83%) showed heterogeneous intensities on MR images. On the other hand, 12 of 16 benign astrocytomas (75%), 5 of 7 oligodendrogliomas (71%) and all of 5 pontine gliomas showed homogeneous intensities. All of central neurocytomas and ependymomas were shown as a solid tumor with cysts and heterogeneous intensities. In conclusion, although MR images without Gd-DTPA seemed not to be superior to contrast CT in differentiating malignant gliomas from benign one, it appears significant to know some tissue characteristics on MR images of gliomas in differentiating from the other brain tumors.
