Tritium concentration in Japanese rice.

Polished rice samples harvested in 1985 were collected from 25 prefectures throughout Japan. Concentrations of both tissue free water tritium (TFWT) and organically bound tritium (OBT) were determined. Nearly uniform distribution of the TFWT and OBT concentrations was observed in Japan, taking into account a relatively large counting error. The average values for all Japanese polished rice were 2.69 +/- 0.74 Bq/L for the TFWT concentration, 2.01 +/- 0.78 Bq/L for the OBT concentration and 0.83 +/- 0.32 for the specific activity ratio. The TFWT concentration was within the range of tritium concentration of the land water reported by other researchers. The specific activity ratio below unity also confirmed by analyses of the 4 Chinese polished rice samples suggests that OBT in the rice is not equilibrated with TFWT or HTO in the environment because of isotope discrimination.

An in vitro evaluation of adrenergic action on rat pancreatic amylase secretion: lack of stimulatory effect.

To assess direct evidence of adrenergic stimulation in pancreatic amylase secretion, effects of catecholamines on amylase release and intracellular cyclic AMP accumulation were examined with rat dispersed pancreatic acini. We first carried out control studies with CCK-8 and carbamylcholine to evaluate the usefulness of the material for the examination of amylase secretion, and examined VIP-induced cyclic AMP accumulation to assess the agonist evoked intracellular response. As a result, significant effects of CCK-8, carbamylcholine and VIP were observed, which confirmed that dispersed pancreatic acini used in this study were useful in examining exocrine pancreatic secretion. However, catecholamines failed to stimulate amylase release from pancreatic acini, although a significant increase in intracellular cyclic AMP accumulation was observed. Thus the present study strongly suggests that direct involvement of catecholamine is unlikely in pancreatic amylase secretion, in contrast to results reported previously.

Photoinactivation of photosystem II and degradation of the D 1 protein are reduced in a cytochrome b6/f-less mutant of Chlamydomonas reinhardtii.

The effect of unoccupancy of the QB site by plastoquinone on the photoinactivation of reaction center II in a Cyt b6/f-less mutant of Chlamydomonas reinhardtii, B6, was investigated. In these cells the oxidation of plastoquinol generated by electron flow via RC II to plastoquinone and thus the turnover of PQH2/PQ via the QB site are drastically reduced. Reaction center II of the mutant cells was resistant to photoinactivation relative to the control cells as demonstrated by measurements of light-induced destabilization of S2-QB- charge recombination, rise in intrinsic fluorescence and loss of variable fluorescence. These parameters relate to functions involving the reaction center II D 1 protein. The light-induced degradation of D 1 in the mutant cells was also considerably reduced, with a t1/2 value of 7 h as compared, under similar conditions, to about 1.5 h for the control cells. These results indicate that the photoinactivation of RC II and turnover of the D 1 protein are related and require occupancy of the QB site by PQ and its light-driven reduction.

Amino acid substitutions in the D1 protein of photosystem II affect QB- stabilization and accelerate turnover of D1.

Isogenic strains of Synechococcus PCC7942 were genetically engineered so that copy I of the gene psb A was mutated at specific sites. These mutations resulted in replacements of Ser 264 by Gly or Ala and of Phe 255 by Tyr or Leu in the D1 protein. The mutants were resistant to herbicides inhibiting electron transfer in photosystem II. All mutants exhibited alterations in the stability of QB- as demonstrated by a temperature downshift, to various extents, of the in vivo thermoluminescence emission. Measurements of the light-dependent turnover of D1 showed a marked decrease in the t 1/2 of this protein in the mutants as compared to wild-type, under low to medium light intensities. A correlation was found between the degree of perturbation in the QB- stability and the rate of acceleration in the turnover of D1. These data provide a direct evidence for the overlapping binding sites for the plastoquinone B and herbicides in the D1 protein. In addition these data indicate a close link between QB- destabilization in reaction center II and the mechanism controlling the light-dependent turnover of D1. Based on these results and previous work we suggest that destabilization of the semireduced quinone, facilitates a light-induced damage in D1 which triggers its degradation.

[A case of adult-onset spike-wave stupor associated with hypoparathyroidism and hyperostosis frontalis interna (HFI)].

A 34-year-old women without history of seizures was admitted to our hospital because of the diminished responsiveness and the repeated blinking for two days. Her past history showed neither surgery nor inflammation of the thyroid gland. General physical examination was normal. On neurological examination, the patient was blinking frequently and staring without any responsiveness. However, she occasionally became able to respond correctly to verbal orders, such as "open your eyes", "open your mouth", "stand up" and so on, which occurred abruptly just like the switch-on. She was otherwise normal neurologically. The laboratory data showed hypocalcemia, hyperphosphatemia, decreased level of parathyroid hormone and normal renal function, indicating the presence of idiopathic hypoparathyroidism. EEG showed the continuous generalized 2-4 Hz spike and wave complexes with the maximum intensity on frontal lobes. Skull roentgenograms and MRI CT of the head disclosed hyperostosis frontalis interna (HFI) and the compression of superior medial frontal lobes by the HIF. SPECT also showed the diminished circulation and hypometabolism in the superior frontal lobes. Based on clinical and EEG findings, the diagnosis of spike-wave stupor was made. She was successfully treated with valproic acid and ethosuximide. Spike and wave complexes on EEG completely disappeared after administration of alfacalcidole. It is assumed that both hypoparathyroidism and HFI were deeply involved in the development of spike-wave stupor in this adult case.

[Medical treatment of type III dissecting aortic aneurysms: influence of the shape of the false lumen assessed by X-ray CT as a factor in the long-term prognosis].

The future needs of surgical repair for type III dissecting aneurysms among medically-treated patients were estimated according to the influence of the shapes of false lumina assessed by contrast-enhanced CT scans and by the level of blood pressure control at the time of discharge on the long-term prognoses. Thirty-six patients, 30 males and six females, whose mean age was 61 years, underwent enhanced CT scans at their discharge. Events, including deaths and dissection-related surgery were analyzed by a nonparametric actuarial method of Kaplan and Meier. The changes in CT findings were also studied in 33 patients who underwent follow-up CT scans within a mean interval of 3.5 years. 1. Systolic blood pressure was maintained below 140 mmHg (good control group) in 15 patients, and above 140 mmHg (poor control group) in 18 controls. There were no significant differences between the two groups for event-free survival rates. 2. The shapes of false lumina assessed by enhanced CT scans at the time of discharge were categorized in three types; 21 patients (group A) without false lumina of the aorta, or with a small crescentic false lumen in the thoracic aorta (type a), six patients (group B) with intimal flaps and two contrast-material-filled lumina in the thoracic aorta (type b), and nine patients (group C) with expanded false lumina or a false lumen whose margin was convex towards a true lumen in the thoracic aorta (type c).(ABSTRACT TRUNCATED AT 250 WORDS)

[Electron-probe microanalysis examination of acid-proof dentin layer].

To predict the presence and thickness of the acid-proof dentin layer by the method of combining the halogenated methacrylate and electron-probe microanalyzer (EPMA), 2-methacryloyloxyethyl hydrogen chloromaleate (CIMEM) used as a base monomer for a bonding agent and 2-bromoethyl methacrylate (2 BEM), 2-(4-bromophenyl)ethyl methacrylate (BPy1EM), 2-(4-bromo-1-naphthyl)ethyl methacrylate (BNEM) and 2,4,6-tribromophenoxy methacrylate (TriBr-PM) used as tracers were synthesized. The bond strength to dentin treated with 37% phosphoric acid solution for 30 sec. was not statistically different for the bonding agents with and without tracers. The SEM micrographs revealed that the layers, which may be acid-proof dentin layers, were 3-4 microns thick at the resin-dentin interface for all bonding agents. According to EPMA analysis, cps of C1Ka and BrLa increase at the same points and the acid-proof dentin layer thickness was about 4 microns.

[An interesting case of pituitary apoplexy showing abnormality in water-electrolyte before and after surgery].

Cases which present abnormality in water-electrolyte before and after operation of pituitary adenoma are occasionally reported. The authors have encountered a case in which neurological symptoms became aggravated abruptly with pituitary apoplexy after admission, hyponatremia was noted before operation and polyuria, not hypotonic urine was observed after operation. As a result of an endocrinological examination which may have an influence on water-electrolyte (ADH, aldosterone, ANP, etc.) the ADH level in hyponatremia before operation was high at 6.8 pg/ml; so, it was taken as SIADH. According to a study at the time of polyuria after operation, the ADH level was normal at 2.4 pg/ml, the ANP level was abnormally high at 140 pg/ml and the specific gravity of the urine was kept at 1.010 or more. So, polyuria was considered due to abnormally increased content of serum ANP. In polyuria due to abnormally increased content of serum ANP, the osmotic pressure of the urine is maintained relatively well, which is a clinical feature evidently different from diabetes insipidus. After operation for pituitary adenoma, water-electrolyte should be controlled with polyuria due to abnormally increased content of serum ANP in addition to diabetes insipidus taken into consideration.

[A combination phase II study of adriamycin, CDDP and etoposide in small cell lung cancer].

A combination phase II study was performed on 31 patients with previously untreated small-cell lung cancer (19 LD & 12 ED). Patients were treated intravenously with Adriamycin (30 mg/m2 day 1), CDDP (80 mg/m2 day 1) and Etoposide (70 mg/m2 x 3 day 1, 3, 5). This combination chemotherapy was administered over a four- or five-week period. Among 26 evaluable patients there were 4 CR and 17 PR, giving a response rate of 80.8% (68.4% in LD). The median duration of response was 19 (3-123) weeks. Despite the relatively low response rate and CR rate, the median survival times of LD and ED patients were 17.0 (3.1-43.1) and 9.4 (4.4-17.7) months, respectively. The major toxic effect of this regimen was bone marrow suppression. Two patients were excluded from this study in the first course because of severe hematologic toxicity. The renal toxicity of this regimen was minimal and no patients developed any clinical problem. Nausea and vomiting during the treatment were well controlled by high-dose metoclopramide and methylprednisolone. In conclusion, this combination of chemotherapy is effective for patients with small cell lung cancer. However, the advantage of adding Adriamycin to CDDP and Etoposide is still controversial.

Antiinflammatory properties of the new antirheumatic agent 4-acetylaminophenylacetic acid.

The antiinflammatory activity of 4-acetylaminophenylacetic acid (MS-932) was investigated. MS-932 did not suppress the acute inflammation of carrageenin-induced paw edema in rats or of primary swelling in adjuvant arthritic rats. However, prophylactic treatment with MS-932 inhibited secondary inflammation in adjuvant arthritic rats. MS-932 also restored to normal the weight of the spleen and the serum albumin/globulin ratio of adjuvant arthritic rats. In addition to its prophylactic effect, MS-932 had a therapeutic effect on adjuvant arthritis. In in vitro tests, MS-932 did not inhibit prostaglandin E2 biosynthesis from arachidonic acid by sheep seminal vesicle microsomal enzyme or superoxide generation by guinea pig neutrophils stimulated with opsonized zymosan. MS-932 had no analgesic effect in mice and no antipyretic effect in rats. These results indicate that MS-932 suppresses adjuvant arthritis through modulation of the immune system.

[In vitro sensitivity to antiviral agents of herpes simplex viruses isolated from patients with herpetic keratitis].

Thirty-five clinical isolates of herpes simplex virus type 1 (HSV-1) from 34 patients (35 eyes) with herpetic keratitis were examined in vitro for 5-iodo-2'-deoxyuridine (IDU) and acyclovir (ACV) sensitivity. In addition, the effect of clinical treatment with these two drugs in herpetic keratitis was also investigated. The viral effective dose50 (ED50) was defined as the concentration that inhibited the plaque count by 50% compared to the count of the no drug controls. The viral ED50 of IDU ranged from 0.073 to 0.77 micrograms/ml (0.33 +/- 0.16; Mean +/- SD) and that of ACV from 0.0032 to 0.33 micrograms/ml (0.13 +/- 0.11). No virus with markedly diminished sensitivity to IDU and ACV was found. These results suggest that all HSV-1 strains isolated from patients have good sensitivity in vitro to antiviral agents.

[Lipiodol-CT for the detection of small hypovascular HCC].

To evaluate the diagnostic value of Lipiodol-CT for small hypovascular HCC, we injected 3 ml or less Lipiodol into the hepatic artery of patients with chronic liver disease and small SOL in the liver detected on echogram but not on angiogram. About seven days after injection CT was used to check for accumulation of Lipiodol in the liver SOL. We found that the sensitivity of this method for detection of hypovascular HCC is only 25%. We assume that Lipiodol does not accumulate in small hypovascular HCC lesions because they have little vascular stroma. Lipiodol-CT has high diagnostic value for the detection of small hypervascular daughter HCC lesions, but this method should not be relied on for the detection of small hypovascular HCC.

[Usefulness of dynamic thin section CT in detecting lymph node metastasis of lung cancer].

The diagnosis for metastatic lymph nodes of lung cancer by the conventional CT is done only by the size of nodes, therefore, its diagnostic accuracy is questionable. For the purpose of more qualitative diagnosis in order to elevate accuracy of metastatic nodes, we studied a new CT image (Dynamic Thin Section CT) by combining the Thin Section CT with the Dynamic CT, and reviewed on the image of histopathological positive nodes. Firstly, for the preoperative cases of lung cancer, the conventional CT was performed for the whole chest with 10 mm-thickness and 10 mm-interval. Secondary, among them, the Dynamic CT by bolus-injection at the sites of detected hilar and mediastinal lymph nodes was performed. For this method, the Thin Section CT with 2 mm-thickness was used, and the image-detected nodes were isolated by surgery. We studied 25 cases whose images were compared with the histological findings. By the conventional CT sensitivity were 35.7%, specificity 54.5%, and accuracy 44.0%, in regarding the shorter diameter over 1 cm as positive nodes, then under diagnosis were 36%, over diagnosis 20%. Morphological features were reviewed on the image of metastatic lymph nodes in the Dynamic Thin Section CT, and were (1) lump-like lymph nodes, (2) disappearance of the fatty plane around the lymph nodes or uneven and irregular margins, (3) irregular internal structures of enhanced lymph nodes. Moreover, small lymph nodes were clearly detected, and the size was exactly measured.(ABSTRACT TRUNCATED AT 250 WORDS)

[Herpes simplex virus latency in the cornea].

We tried to identify herpes simplex virus type 1 (HSV-1) latency in the cornea obtained at the time of penetrating keratoplasty from patients with herpetic stromal keratitis in the non-active (subsided) stage. The subject consisted of 8 patients (3 males and 5 females; average age 42.3 years) who were diagnosed as having herpetic stromal keratitis and underwent penetrating keratoplasty during a period without active lesions (subsided stage) between August, 1984 and July, 1988. No infective virus was detected in the centrifugation supernatant following each corneal homogenization. Latent virus was detected from the culture supernatant of sections of the corneas in 4 of the 8 patients. Although the ganglion trigger theory of Hill et al. has been conventionally supported as the mechanism(s) of herpetic keratitis recurrence, our results suggest that HSV-1 proliferation from latency in the cornea (peripheral tissue) might stimulate the ganglion (ganglion and skin trigger theory). The present study is the first to demonstrate HSV-1 latency in the cornea.
