[The value of hepatic arterial infusion chemotherapy for liver metastases--discussion from viewpoints of prevention of hepatic death and QOL].

The value of hepatic arterial infusion chemotherapy for liver metastases was discussed from the viewpoints of the prevention of hepatic death and the negative effect on QOL, based on the results of repeated hepatic arterial infusion chemotherapy for 128 patients with unresectable liver metastases employing implantable catheter and port system. In 70% of patients, hepatic death due to metastases was prevented by this therapy, and the negative effect on QOL by this therapy was limited. Thus, in spite of the limited effects on the prolongation of survival compared with systemic chemotherapy, there is a reasonable basis for the use of hepatic arterial infusion chemotherapy as a treatment for unresectable liver metastases.

A nationwide survey on the occurrence of Vogt-Koyanagi-Harada disease in Japan.

A nationwide survey was carried out to estimate the frequency and distribution of Vogt-Koyanagi-Harada disease in Japan. A questionnaire was mailed to the Departments of Ophthalmology in hospitals with 200 or more beds. Responses were received from 489 out of 1,077 institutions, or 45.5%. The annual prevalence was estimated to be 15.5 per million and the incidence was 6.5 per million, based on the number of reported cases and the response rate per prefecture. It was estimated that 1,800 patients were treated for this disease annually and that there were about 800 new cases each year. Geographical distribution showed no special characteristics such as those seen with Behçet's disease. The results of the analysis of 1,833 reported cases showed that females were slightly more often affected than males. For both male and female patients, the age distribution on the first visit showed a peak in the fifth decade and a median age of 42.3 years.

Healing of chronic gastric ulcer depends on gastric mucosal prostaglandin synthesis.

In the present study, the role of endogenous prostanoid synthesis in gastric mucosa in the healing of chronic gastric ulcers was investigated. Nineteen patients were divided into two groups in accordance with healing state after one month of treatment with cimetidine only: "healed group" and "unhealed group". Biopsy specimens taken from the mucosa around the ulceration (damaged gastric mucosa) and at a distance from the ulceration (normal gastric mucosa) at endoscopy prior to treatment were homogenized, and the mucosal prostanoid synthesis was determined using [14C]arachidonic acid. The mean value of prostaglandin E2 synthesis in the normal gastric mucosa of the healed group was 60% higher than in that of the unhealed group, but the difference was not significant. However, prostaglandin E2 synthesis in the damaged gastric mucosa of the healed group was 117% higher than in that of the unhealed group. The same tendency was observed for prostaglandin D2 and 6-keto prostaglandin F1 alpha synthesis as for prostaglandin E2. In our study it was demonstrated that there is a good correlation of prostaglandin synthesis in the damaged mucosa with healing of chronic gastric ulceration. Furthermore, our study indicated that prostaglandin synthesis, especially in damaged mucosa, might be important in the healing of gastric ulceration.

[Prostatic carcinoma with bilateral testicular metastasis: a case report].

A 77-year-old man was referred for the treatment of urolithiasis. The patient noticed the swelling of bilateral scrotal contents 2 weeks previously and had been suffering from bladder symptoms for several years, dysuria and pollakisuria. Therefore, he was admitted for the treatment of stones and further examination. By transrectal examination, prostatic carcinoma was suspected. Biopsy of the prostate and bilateral orchiectomy revealed the prostatic carcinoma and the metastatic adenocarcinoma of the bilateral testis. The histological or immunohistological examination indicated that the metastatic adenocarcinoma was prostate in origin.

Infection of reovirus type 3 in Mongolian gerbils (Meriones unguiculatus)--lesions in pancreas and brain.

Gerbils were inoculated with reovirus type 3 (Abney strain), and the pancreas, brain and other organs histopathologically evaluated. Male Mongolian gerbils (Meriones unguiculatus) aged 5-10 days (newborn, 9 animals), 4 weeks (juvenile, 5 animals), and 12 weeks (adult, 5 animals) were used. Gerbils inoculated intraperitoneally with the virus were sacrificed 3, 5, 7, or 10 days later. Tissues were observed by light and electron microscopy. The pancreas of newborns showed inflammatory edema with infiltrates of neutrophilis and mononuclear round cells, degranulation of acinar cells and dissociation of lobules and acini with necrosis at 3 days or later after inoculation but not degeneration of pancreatic islets. Brains of newborns had necrosis of neurons and aggregation of microglial cells in the brain stem and cerebral hemisphere. Azurophilic inclusion bodies were seen in the cytoplasm of some neurons. In electron micrographs, virus particles (60-80 nm in diameter) were observed around the nucleus of neurons in the brain stem. No changes were observed in the juvenile and adult gerbils.

Effect of heparin on hemagglutinin of herpes simplex virus type 1.

Heparin inhibited the hemagglutinin activity of herpes simplex virus (HSV) type 1. The minimal inhibitory concentration of heparin required to inhibit 8 hemagglutination (HA) U of HSV ranged from 0.005 to 0.01 U/ml. Mouse erythrocytes failed to combine with the HA inhibitory factor of heparin. On the other hand, mouse erythrocytes treated with heparinase had greatly reduced agglutinability by HSV. Virus-heparin complex formation was observed by sedimenting heparin with the virus particles.

Varied clinical morphology of HPV-1-induced warts, depending on anatomical factors.

The clinical and histological features of 49 lesions induced by human papillomavirus type 1 (HPV-1) were studied. The majority of cases showed the typical clinical features and location of such lesions. They were usually located on the ventral or lateral surfaces of the hands or feet, and were dome-shaped lesions with a central depression. However, HPV-1-induced lesions located on other body areas displayed different clinical features, such as those of common warts, a digitate wart, and a cutaneous horn. One lesion on a healing burn scar had a herpes-like appearance. As these 49 lesions were induced by the same type of HPV (HPV-1), differences in their clinical features are likely to have been due to host-related factors.

The characteristics of another small DNA virus isolated from a pig with diarrhea.

A small DNA virus was newly isolated from the small intestines of a pig with diarrhea in 1987, in Japan. Concerning the physicochemical properties, hemagglutination and susceptibility of cell culture to the virus, the virus was identical to a formerly isolated small DNA virus, the H-45 strain and also physicochemically similar to the parvovirus group. In a serological test however, the virus was distinctly, antigenically different from the H-45 strain as well as each of porcine, bovine and canine parvoviruses.

Distinction between Aujeszky's disease virus-infected and vaccinated pigs by hemagglutination-inhibition test.

A hemagglutination-inhibition (HI) test was applied to distinguish virulent Aujeszky's disease virus infected pigs from those immunized with a glycoprotein gIII deletion vaccine. The vaccine strain, dlg92/dltk, did not have hemagglutination activity with mouse erythrocytes and the pigs vaccinated five times with the dlg92/dltk strain failed to develop HI antibody, although they developed neutralizing antibody with 128 to 512 titers to Aujeszky's disease virus. On the other hand, these pigs produced HI antibody 1 to 2 weeks after virulent virus inoculation. Thus the animals infected with virulent strain were easily differentiated from the animals immunized with the gIII deletion vaccine.

[A case of gastric diverticulum difficult to differentiate from left adrenal tumor].

We report a case of gastric diverticulum difficult to differentiate from left adrenal tumor. A 58-year-old woman was admitted for the examination of right renal cyst. Ultrasonogram and abdominal CT scan revealed 3 cm mass at the upper pole of the left kidney. Operation was performed with diagnosis of left adrenal incidentaloma, but no evident tumor was explored. Postoperative X-ray of upper gastrointestine revealed the mass as a gastric diverticulum.

[Intermittent hepatic arterial infusion of high-dose 5-FU for liver metastases from colorectal cancer].

A phase II study of weekly intermittent hepatic arterial infusion of high dose 5-FU for liver metastases from colorectal cancer was carried out. Thirty-two patients with unresectable liver metastases from colorectal cancer were entered in this study. Nausea (< or = grade 2) was observed in 31% of the cases, mild elevation of rGTP in 13%, biloma in 9%, and hepatic arterial occlusion in 22%. However, no major toxicity occurred. The response rate evaluated by CT-scans was 75% (4 CR + 20 PR/32), and the overall median survival was 22 months. Significant differences of median survival were observed in the extra-hepatic lesions [extrahepatic lesions: (-) 16 months vs (+) 22 months]. This regimen showed high activity without major toxicity and the reduction of pts' QOLs caused by pumps. It confirmed by a study involving a large number of cases, this regimen will become a standard regimen for unresectable liver metastases from colorectal cancer.

[A case of multiple ureteral polyps treated with ureteroscopy].

We report a case of multiple ureteral polyps treated with a rigid ureteroscope in a 27-year-old man with the chief complaint of left lower abdominal pain. The patient, who had left renal stones pointed out on his plain film, was referred for further examination and treatment of left hydronephrosis as suggested by drip infusion pyelography (DIP). DIP and retrograde pyelography (RP) revealed left hydronephrosis and a filling defect in the upper third of ureter. An ureteroscopic inspection and biopsy was conducted. The pathological diagnosis was fibroepithelial polyps of the ureter, so we performed an ureteroscopic polypectomy for the treatment. The postoperative course has elapsed favorably, without ureteral stricture or recurrence of the polyps. Ureteroscopic procedures are considered to be useful for the preoperative diagnosis and treatment of ureteral polyps.

[Characteristics of day service center clients and their families based on their medical conditions].

Clients utilizing day service centers in Hokkaido, and their families were surveyed for the purpose of analyzing present conditions of health, ADL level, family structure, motivation, activities, and changes in physical and psychosocial status. The average age of subjects was 76 years old with women generally being older than men. Most were living with their families. As to disease, cerebrovascular diseases were most common followed by hypertension and spinal-articulation diseases. Women had more hypertension problems compared to men who had more cerebrovascular conditions. Effectiveness of these day service programs were seen in the improvement of psychological functions. Persons with cerebrovascular disease had greater participation in functional rehabilitation training programs, and their ADL skills were improved.

Management of patients with unresectable liver metastases from colorectal and gastric cancer employing an implantable port system.

Between 1985 and 1990, 50 patients with unresectable liver metastases from colorectal cancer and 34 subjects with metastases from gastric cancer were treated by repeated hepatic arterial infusion chemotherapy employing an implantable prot system. A catheter was inserted into the hepatic artery via the left subclavian artery and was connected to the implantable injection port in each patient. 5-Fluorouracil (5-FU) at 330 mg/m2 per week (167 mg/m2 daily given continuously over the initial 3 months for colorectal cancer), Adriamycin (ADR) at 20 mg/m2 every 4 weeks and mitomycin C (MMC) at 2.7 mg/m2 every 2 weeks were given to all 34 patients with gastric cancer and to 31 of the colorectal cancer patients. The remaining 19 patients with colorectal cancer received 5-FU at 1,000 mg/m2 every week. As a rule the treatment was performed on an outpatient basis. The side effects and complications observed included myelosuppression (23%), hepatic arterial occlusion (21%), and gastroduodenal mucositis (12%), although no major toxicity was encountered. The response rate (CR+PR) among the evaluated patients as determined using CT scans was 67% for colorectal cancer and 73% for gastric cancer. The overall median survival was 12 months and 15 months, respectively. Good local control of liver metastases from the colorectal and gastric cancers was achieved by repeated hepatic arterial infusion chemotherapy employing an implantable port system without the need for hospitalization and without producing major toxicity. Thus, the implantable port system is very useful for the management of patients with unresectable liver metastases.
