Production of interleukin-10 in the cerebrospinal fluid in aseptic meningitis of children.

IL-10 is a cytokine that has antiinflammatory properties. We investigated IL-10 using ELISA and a reverse-transcribed polymerase chain reaction in the cerebrospinal fluid (CSF) of children with or without aseptic meningitis. When the patients with aseptic meningitis had meningeal symptoms, IL-10 in the CSF was detectable in 14 of 22 patients (88 +/- 146 ng/L, n = 31). The IL-10 levels decreased as meningeal symptoms disappeared. In 20 of 21 control children without meningitis, CSF samples had no detectable levels of IL-10 (< 10 ng/L). Serum IL-10 levels were lower than the corresponding levels in the CSF from the same individuals with aseptic meningitis. Significant correlations were found between IL-10 levels and mononuclear cell counts in the CSF of the affected patients (r = 0.644, p < 0.001). The IL-10 mRNA was detected by reverse-transcribed polymerase chain reaction-assisted amplification in the CSF cells in four of seven patients with the disease. The culture of CSF mononuclear cells produced high levels of IL-10 (152-485 ng/L) in all of five patients. Cytokine kinetics in the CSF showed that mean IL-10 levels reached the peak on the 2nd to 3rd d of the illness, although all mean levels of IL-6, IL-8, and granulocyte colony-stimulating factor were the highest on the 1st d of the illness. In summary, IL-10 is produced in the CSF in aseptic meningitis, and may increase relatively late compared with the proinflammatory cytokines. IL-10 may play an immunoregulatory role in the meningeal inflammatory network.

Hemagglutination with porcine reproductive and respiratory syndrome virus.

Porcine reproductive and respiratory syndrome (PRRS) virus grown on MARC-145 cell cultures was tested for hemagglutination (HA) with erythrocytes from a variety of species at 4 degrees C, room temperature and 37 degrees C. HA was observed at all temperatures with mouse erythrocytes but not with cattle, sheep, goat, horse, swine, guinea pig, mongolian gerbil, goose and chicken erythrocytes. The HA activity was enhanced by treatment of virus materials with Tween 80 followed by treatment with ether. The HA titer and HA pattern of virus materials treated with Tween 80 and ether (TE) were 4- to 8-fold higher and more clear than those of the virus materials without TE treatment. The optimum conditions consisted in pretreatment of virus material with Tween 80 at a final concentration of 0.06-0.125% (v/ v) for 15-60 min followed by treatment with ether at a concentration of 50% (v/v) for 5-15 min in ice bath with continuous shaking. The curve of active virus production in intra- and extracellular virus samples resembled that of HA production although it rose somewhat earlier in intracellular virus samples. The HA reaction was inhibited by specific antiserum. HI antibody titers of individual pig sera showed a significant positive correlation with their neutralizing antibody titers.

Slow-reacting and complement-requiring neutralizing antibody in swine infected with porcine reproductive and respiratory syndrome (PRRS) virus.

Various conditions were evaluated and modified to enhance the sensitivity of the neutralization (NT) test for detecting antibody in swine infected with porcine reproductive and respiratory syndrome (PRRS) virus. Higher NT antibody titers were consistently obtained by the addition of 10% (v/v) complement, fresh guinea pig serum, to the virus diluent and by the incubation of serum-virus mixture at 4 degrees C for 24 hr. The appearance and persistence of antibodies detected by the modified NT test showed a similar pattern to those of antibodies detected by the indirect fluorescent antibody (IFA) assay, although the antibody titers obtained by the former method were consistently lower than those obtained by the latter method. Slow-reacting complement-requiring NT antibody was detected in sera from pig 2 weeks after infection with PRRS virus. The slow-reacting complement-requiring NT antibody in the early serum samples was sensitive to 2-mercaptoethanol (2-ME), whereas the slow-reacting complement-requiring NT antibody in the late serum samples was resistant to 2-ME. The initial phase may represent the IgM response and the later phase a change to IgG. A NT test was developed in which serum-virus mixtures were incubated at 4 degrees C for 24 hr with complement; this gave an improved sensitivity over the previous incubation at 37 degrees C for 60 min.

Indices of lead-exposure in blood and urine of lead-exposed workers and concentrations of major and trace elements and activities of SOD, GSH-Px and catalase in their blood.

Seventy male factory workers were studied. The lead concentrations in their blood (Pb-B) were 16.55 +/- 11.53 micrograms/100 ml (range 1.5 to 50.2 micrograms/100 ml). The subjects were divided into three groups according to Pb-B (in microgram/100 ml): group A, Pb-B < or = 10 (n = 22); group B, 10 < Pb-B < or = 20 (n = 30); group C, Pb-B > 20 (n = 18). The mean +/- S.D. in each group was 5.57 +/- 2.53, 15.02 +/- 2.75, and 32.52 +/- 9.49 micrograms/100 ml, respectively. Pb in plasma was 0.011 +/- 0.010, 0.017 +/- 0.033, and 0.021 +/- 0.021 microgram/liter, and Pb in the RBC was 0.281 +/- 0.246, 0.701 +/- 0.325, and 1.626 +/- 0.861 micrograms/g Hb, respectively. In addition to Pb concentration, the concentrations of 34 elements in the plasma or in the RBC were determined. Se concentrations in RBC in each group were 0.618 +/- 0.139, 0.670 +/- 0.207, and 0.728 +/- 0.200 microgram/g Hb, and the mean values were significantly different between groups A and C (p < 0.05). For Se concentration in plasma, the mean +/- S.D. in each group was 0.132 +/- 0.035, 0.130 +/- 0.031, and 0.126 +/- 0.021 microgram/ml, respectively, and there was no significant difference between groups. On the other hand, when the activities of total SOD, Mn-SOD, Cu, Zn-SOD, and catalase in the plasma and the activities of GSH-Px both in the plasma and in the RBC were assayed, some differences were found. The activities in GSH-Px in RBC were 17.19 +/- 5.03, 17.59 +/- 3.95, and 15.25 +/- 3.18 mumol/g Hb/min, and those in plasma were 0.069 +/- 0.032, 0.081 +/- 0.023, and 0.080 +/- 0.028 mumol/ml/min. In group C, GSH-Px activity was lower in the RBC and higher in the plasma than those in group A, and it was observed that the Se concentration was higher in RBC, and that there was no remarkable change in the plasma. Catalase activity in group C was 3.58 +/- 0.81 mgH2O2/ml/30 min, which was significantly higher than that in group A (2.81 +/- 0.90 mgH2O2/ml/30 min). Further investigation is necessary in order to explain the above results. The regular indices used for evaluating lead exposure, showed significant correlations with Pb-B: r = -0.786 vs delta-Aminolevulinic acid (ALA) dehydratase activity in blood, r = 0.927 vs. inhibition rate, and r = 0.339 vs. ALA in urine.

Reliability of a questionnaire used to survey allergic disease in school.

The reliability of a questionnaire which contained 21 items concerning asthma and allergies of the nose, skin, and eyes and other questions was evaluated by test-retest method. The questionnaire was the same as one used two years ago in a survey of allergic diseases found among elementary, junior and senior high school students in Shizuoka Prefecture, Japan. In order to evaluate its reliability, calculations were made of proportions of agreement, Cohen's kappa values and intraclass correlation coefficients. Both the proportions of agreement and the kappa values were fair, and all the intraclass correlation coefficients showed high values. The results suggest that there might be a slight effect of age, and that the articles which showed higher kappa values tended to be easy to answer, while the items in which kappa values were lower tended to consist of questions of multiple answers. This questionnaire can be regarded as useful for our original purpose of investigating allergic diseases. Among the kappa values of items, the values regarding diagnosis by doctor tended to be the largest. It was suggested that doctor's diagnosis was strongly convincing for patients. The reliability of this questionnaire survey can be regarded as satisfactory.

Recent topics in Japan.

In Japan, the concern about ethical issues in preventive medicine, especially in epidemiological investigation, has been gradually increasing in recent years. In this paper I introduce the following four topics: 1. privacy protection and the computer, 2. informed consent and publication, 3. the attitudes toward ethics among epidemiologists, 4. the attitudes toward epidemiological investigation among examinees. In my opinion, Japanese epidemiologists should give more attention to general ethical principles (Respect for persons, Beneficence) and to the practical methods to apply them in their research works.

Generalized pustular psoriasis in Japan: two distinct groups formed by differences in symptoms and genetic background.

A multi-center study for investigating generalized pustular psoriasis was carried out in Japan in order to clarify the prevalence, the etiology, and the standard therapy. Through questionnaires, 541 cases of patients with generalized pustular psoriasis and related disorders were collected, and the history, the precipitating factors, the symptoms and the laboratory findings were analyzed from medical records. The statistic analysis revealed that 902 (+/- 102.2) patients with generalized pustular psoriasis were expected at the time, and that the prevalence of generalized pustular psoriasis in Japan was 7.46/million. Of 541 patients with generalized pustular psoriasis, 208 cases had the recurrent episodes of symptoms with generalized pustules which had been referred to von Zumbusch type pustular psoriasis or to the acute form of generalized pustular psoriasis. Two hundred and eight patients were further subdivided into two groups: one with a preceding history of ordinary psoriasis (pso+ generalized pustular psoriasis; 65 cases) and another without a psoriasis history (pso- generalized pustular psoriasis; 143 cases). Subdivision into the two groups defined that the onset of pustular outbreak was earlier in the pso- generalized pustular psoriasis group. It was also pointed out that the precipitating factors were different, as the pso- generalized pustular psoriasis group was more frequently affected after infections, and that the pso+ generalized pustular psoriasis group was more frequently affected by preceding corticosterold therapy. HLA analysis of 92 cases with von Zumbusch type generalized pustular psoriasis confirmed that A2, B14, B35 phenotypes were weakly correlated, and when 92 cases were subdivided into the two groups, the pso+ generalized pustular psoriasis group revealed a statistically significant correlation with A1 (p < 0.01), B37 (p < 0.02) and DRw10 (p < 0.05), which was closely related with psoriasis vulgaris patients in Japan. Both clinical surveillance and genetic analysis have disclosed the heterogeneity of von Zumbusch type generalized pustular psoriasis, as there exist two types; one is closely related to psoriasis and the other not.

Carcinoembryonic antigen and related antigens expressed on keratinocytes in inflammatory dermatoses.

Carcinoembryonic antigen (CEA) is recognized as being important not only as a tumour marker, but also as a cell adhesion molecule in the metastasis of malignant tumour cells, and in the development of inflammatory disorders in some organs. However, the role played by this antigen in inflammatory dermatoses has not been studied previously. To investigate the expression of CEA in inflamed skin, we performed immunohistochemical studies to detect the antigen in both frozen and formalin-fixed, paraffin-embedded biopsy specimens of inflammatory dermatoses, using five different monoclonal antibodies and one polyclonal antibody against human CEA or CEA-related molecules. CEA and CEA-related antigens were expressed by epidermal keratinocytes and by some infiltrating inflammatory cells in inflamed skin. The expression was observed primarily in the upper part of the malpighian layers of the epidermis. On the basis of the distribution of this expression, we suggest an additional role for CEA-related molecules, differing from their roles as tumour markers and adhesion molecules, namely that these molecules may be associated with the state of differentiation of epidermal keratinocytes. Unexpected expression of CEA-related antigens was seen on normal follicular keratinocytes. Thus, it is conceivable that CEA-related antigen may also play a role in the biology of hair follicles.

[CT arteriography for evaluation of hepatic metastases from colorectal cancer: diagnostic efficacy of a combination of CT arterial portography and CT arteriography].

The sensitivity of computed tomographic arteriography (CTA) for the detection of hepatic metastases from colorectal cancer was compared with that of computed tomographic arterial portography (CTAP). And the diagnostic efficacy of the combined use of CTAP and CTA (CTAP-CTA) was evaluated for further improvement of detecting the lesions. Both CTAP and CTA were performed sequentially in 71 patients with hepatic metastases from colorectal cancer, of whom 29 patients underwent hepatic resection and 42 patients underwent chemotherapy or clinical observation. CTAP via the superior mesenteric artery was firstly obtained, and CTA via the proper or common hepatic artery followed, employing an interventional CT system, a newly developed angiographic unit combined with a CT scanner. In all metastases, the rate of sensitivity of CTAP, CTA and CTAP-CTA were 93.3%, 93.8% and 98.8%, respectively. In small metastatic lesions of less than 1 cm in diameter, the sensitivity rates of CTAP, CTA and CTAP-CTA were 92.2%, 92.0% and 98.3%, respectively. The difference between the sensitivity of CTAP-CTA and that of CTAP or CTA alone was statistically significant (P < 0.01). In cases of resected metastatic lesion, the sensitivity rates of conventional CT (con.CT), CTAP, CTA and CTAP-CTA were 77.0%, 86.9 %, 90.2% and 91.8% respectively. The difference between the sensitivity of CTAP-CTA and that of con. CT was statistically significant (P < 0.01). Subsequently, the addition of CTA to CTAP was useful in 16 of 71 patients (22.5%). In conclusion, CTAP-CTA showed higher diagnostic efficacy than CTAP alone. Thus CTAP-CTA is recommended for precise pretherapeutic evaluation of hepatic metastases from colorectal cancer.

Characterization of mouse nasal lymphocytes isolated by enzymatic extraction with collagenase.

A reproducible method for isolation of mouse nasal lymphocytes was developed. The cells were released from tissue fragments of dissected mouse nose by enzyme extraction with collagenase and separated by a stepwise Percoll gradient centrifugation. The partially purified nasal lymphocyte fraction from normal BALB/c mice contained CD4+ T cells (18-23%), CD8+ T cells (7-10%) and B cells (20-38%), when analysed with a FACScan fluorescence analyser. The ratio of T to B cells and that of CD4+ to CD8+ T cells in the nasal cell fraction were about twice as high as those in Peyer's path lymphocytes. The nasal lymphocyte fraction from the mice infected with influenza virus was then characterized. The nasal lymphocytes contained a twice larger number of CD4+ and a three times larger number of CD8+ T cells than those of normal mice 7 days after infection. They produced IFN-gamma which increased after infection. They contained the cells secreting influenza virus-specific IgA and IgG antibodies 4 weeks after infection. Moreover, the nasal lymphocytes from infected C3H mice lysed the virus infected-target cells. These results suggest that this method can successfully be used for investigating cellular dynamics of mucosal immunity in the upper respiratory tract of experimental animals.

Serum anti-Helicobacter pylori antibody and gastric carcinoma among young adults. Research Group on Prevention of Gastric Carcinoma among Young Adults.

BACKGROUND: Helicobacter pylori is recognized as one of the possible causes of gastric carcinoma. There have been few studies on the relationship between H. pylori and gastric carcinoma in patients younger than 40 years. METHODS: Data and sera were collected from the cases (105 hospitalized patients younger than 40 years with gastric carcinoma from 9 hospitals in the Kanto-Shin-Etsu Area in Japan) and controls (102 hospitalized control subjects and 101 screening control subjects) whose sex and age (within 4 years) were matched. The serum anti-H. pylori immunoglobulin G antibody was measured, and the odds ratio (OR) for the association between seropositivity and gastric carcinoma was calculated. RESULTS: The OR (95% confidence interval) was 13.3 (5.3-35.6). For women, the OR was 32.8, whereas for men it was 6.8. The OR for patients with early gastric carcinoma was 20.8, and for patients with advanced disease, it was 10.8. The OR for intestinal-type carcinoma was 18.0, and for diffuse-type carcinoma, it was 12.8. The OR for proximal carcinoma was 11.3, and for distal carcinoma it was 14.8. CONCLUSION: The OR for these young subjects was considerably larger than that for the older subjects in previously published studies. Among those younger than 40 years of age, early stage carcinoma has a stronger association with H. pylori than advanced carcinoma, and intestinal- and diffuse-type carcinomas have an association with H. pylori.

Isolation of porcine adenovirus as a candidate of 5th serotype.

A strain of cytopathic virus, named strain TG/K79, was isolated from the brain of a newborn piglet, pure Hampshire breed, which died shortly after birth. The physicochemical property of virus was considered to be that of the family Adenoviridae. A significant difference between our isolated and 4 reference porcine adenoviruses was demonstrated by cross-seroneutralization test. Differences between TG/K79 and other porcine adenoviruses were also seen in electrophoretic patterns of viral DNA in agarose gel after digested by restriction endonucleases. Two SPF pigs, 2-month-old, experimentally infected via intranasal showed a fever and a hemorrhagic enteritis. A serological survey indicates that at least swine in the farm where the virus was isolated have been highly contaminated.

Salt- and pH-dependent hemagglutination with Kasba virus, a member of the Palyam serogroup of the genus Orbivirus.

Kasba virus grown in BHK21-WI2 cells was tested for hemagglutination (HA) with erythrocytes of a variety of species at 4 degrees C, 25 degrees C and 37 degrees C. HA was observed at all temperatures with cattle, sheep and goat but not with swine, chicken, and goose erythrocytes. The HA was dependent on not only the NaCl concentration but also the pH of the diluent. The HA titer significantly improved by increasing the NaCl molarity to 0.6 M and standardizing pH to 7.5. The HA titer was 16- or 32-fold higher in 0.4 M or 0.6 M solution than in 0.2 M solution of not only NaCl but also several other salts. The HA reaction was inhibited by specific antibody.

Effect of heparin on hemagglutination by Akabane and Aino viruses belonging to the Simbu group of bunyaviruses.

Heparin inhibited the hemagglutinin activity of Akabane and Aino viruses. The minimal inhibitory concentration of heparin required to inhibit 8 hemagglutination (HA) U of Akabane and Aino viruses was 10 U/ml. Goose erythrocytes failed to combine with the HA inhibitory factor of heparin. On the other hand, goose erythrocytes treated with heparinase had greatly reduced agglutinability by Akabane virus. Virus-heparin complex formation was observed by sedimenting heparin with the virus particles.

The incidence of internal malignancies in pemphigus and bullous pemphigoid in Japan.

To evaluate the significance of the association of malignancy with autoimmune blistering diseases, we studied the incidence of internal malignancies in pemphigus and bullous pemphigoid based upon 496 cases of pemphigus and 1113 cases of bullous pemphigoid in Japan. Results showed that (1) an association between internal malignancies and pemphigus was observed in 25 out of 496 cases (5.0%), while that with bullous pemphigoid was seen in 64 out of 1113 cases (5.8%). Such association ratios were significantly higher than that of the controls aged over 70 years old (0.61%); (2) The average ages of pemphigus/bullous pemphigoid with malignancy were 64.7 and 69.2 years, respectively. The association ratio of malignancy with pemphigus increased by age, while that with pemphigoid was not correlated with aging; (3) Lung cancer was most common in pemphigus and gastric cancer in bullous pemphigoid; (4) There were no significant differences in the titers of circulating antibody, the presence or extent of mucous involvement or annular erythema between bullous pemphigoid patients with malignancy and without malignancy. Our results indicated that detailed examination for internal malignancy is essential for those patients with pemphigus or bullous pemphigoid.

The association of smoking and drinking habits with serum pepsinogens.

BACKGROUND: The influence of smoking on serum pepsinogen I has been assessed. However, still to be assessed are the influences of smoking on pepsinogen II and drinking on serum pepsinogens. METHODS: Data were collected from 13,381 employees by questionnaire and serum tests. Multiple regression analyses were done with logarithms of serum pepsinogen I (LPI), pepsinogen II (LPII) or pepsinogen I/II ratio (LI/II) as a criterion variable and as categorized explanatory variables, sex, age, subjective symptoms in the stomach, past history of peptic ulcer, current smoking dose, past smoking amount, drinking habit and current drinking dose. RESULTS: Current smoking dose showed dose-dependent positive associations with LPI and LI/II: Past smoking amount yielded weakly dose-dependent negative associations with LPI and LI/II: Current drinking dose showed dose-dependent negative associations with LPI and LPII. CONCLUSION: Current smoking elevates pepsinogen I and the I/II ratio, and it may be necessary to consider the effect of smoking when pepsinogens are used as markers for gastric cancer. Drinking reduced pepsinogen I and II, but the effect was not so large.

Detection of human papillomaviruses and eccrine ducts in palmoplantar epidermoid cysts.

Although epidermoid cysts of the palms and soles have long been assumed to develop following implantation of an epidermal fragment as a result of a penetrating injury, the pathogenic mechanism is still controversial, and the discovery of a more common aetiological agent is awaited. Clinical, histological, immunohistochemical and molecular biological studies were performed on 119 epidermoid cysts of palmoplantar location, in order to examine the role of the eccrine ducts, and human papillomavirus (HPV), in the pathogenesis of this disorder. Characteristic histological features were found, including intracytoplasmic eosinophilic bodies (ICB: in 14 cases, 12%) in the cyst wall, vacuolar structures (V: in 28 cases, 24%), or parakeratotic nuclei (P: in 85 cases, 71%) within the keratinous mass in the cyst cavity. Ductal structures suggesting eccrine ducts (E: in 63 cases, 53%) were also found in the cyst wall or in the cyst cavity. Either ductal structures or carcinoembryonic antigen expression (66 cases, 55%) were noted in a total of 73 cases (61%). Papillomavirus common antigens were detected in 36 cases (30%) showing one or more of the three distinct histological features, i.e. ICB, V and P. Subsequently, hybridization experiments to detect HPV DNA were performed in 47 cases, revealing an association between cysts showing ICB or V and the presence of HPV 60 DNA sequences. On the basis of our results, we propose that epidermoid cysts in the palmoplantar regions may develop from eccrine ducts, and that HPV and injury may play a role in their pathogenesis.
