Epididymal lymphangiectasis.

We report on a 59-year-old man with epididymal lymphangiectasis. The patient had noticed somewhat intermittent intrascrotal painless swelling. The interstitium of the right caput epididymis exhibited a number of large, dilated lymphatic vessels forming irregular channels among and around the epididymal ducts. The afferent epididymal ducts showed dilatation similar to that of the lymphatic vessels except for focal cuboid epithelial linings. Spermatogenesis in the right testis was preserved. Lymphangiectasis in the epididymis is infrequent and needs to be differentiated from other intrascrotal lesions.

The pedicled arterialized venous flap.

The arterialized venous flap, in which arterial blood flowing through a vein returns to the venous system through the pedicle, was devised to solve the problems of increasing flap size and raising the overall success rate. This flap can obtain satisfactory blood inflow and pressure. The flap was clinically applied to reconstruct skin defects in three cases, with two complete successes and one partial superficial necrosis. This technique provides a flap to cover a relatively large defect, with no shunt formation, and a high success rate.

Investigations of the run and the attachment of the lateral pterygoid muscle in Japanese.

Studies of the run and the attachment of the lateral pterygoid muscle were made on 79 samples from 41 cadavers in the collection of the Department of Anatomy of the Tokyo Dental College. Origin shapes were classified into 3 types: type I, in which there is a clear boundary between the muscle venter adhesion for the upper muscle venter and the lower muscle venter; type II, where the boundary between them is unclear; and type III, where the difference between the two cannot be distinguished at all. When this classification was applied to the specimens, all 21 of the 1-venter specimens fell into type III; 37 (69.8%) of the 2-venter specimens fell into type I; 10 (18.9%) of the 2-venter specimens fell into type II, and 6 (11.3%) of the 2-venter specimens fell into type III. All 5 of the 3-venter specimens were type II. From the results of this studies, we saw that, regardless of which investigation method was employed, the number of venter runs and the origin configuration did not correlate at all.

A new free thenar flap.

While previous pedicled procedures yielded good results in a small traumatic volar defect of the fingers, a large volar defect presents a difficult reconstructive challenge. We devised a new free thenar flap utilizing the superficial palmar branch of the radial artery which we used to treat two patients with volar defects of the index finger. This free flap provides an excellent alternative in coverage of a large long volar defect of the fingers with little sacrifice of the donor site.

[The effects of hyperventilation upon the spinal pain modulating system].

The purpose of this study is to investigate the effects of hyperventilation on the wide dynamic range (WDR) cell of the dorsal horn of the feline lumbar spinal cord. Cats received midcollicular decerebration and lumbar laminectomy. The spinal cord was transected at T12-L1. WDR cells were sampled with microelectrode at the depth of 2,000 microns from the cord dorsum. Following the control period with PaCO2 of 35-45 mmHg, respiratory rate and tidal volume were adjusted to make a level of hypocapnia as low as 20-25 mmHg. The recovery of cell activity was followed when normocapnia was restored. Hypocapnia with PaCO2 of 25-30 mmHg and 20-25 mmHg suppressed the activity of WDR cell significantly. At PaCO2 of 25-30 mmHg, the spontaneous activity was suppressed for about 20% and evoked activity for 27%. At PaCO2 of 20-25 mmHg, the spontaneous activity was suppressed for about 50% and evoked activity for 33%. The results suggest that the hyperventilation has suppressive effects on single-unit activity of WDR cell.

[A case of Wernicke-Korsakoff syndrome caused by gastrojejunostomy--specific findings of MRI and SPECT].

A 63-year-old woman with Wernicke-Korsakoff syndrome, despite the absence of alcoholism and malnutrition, is reported. She had undergone gastrojejunostomy for ileus 30 years ago, and this operation was thought to be an important risk factor for her vitamin B1 deficiency. Brain MRI showed symmetrical high intensity areas on T2-weighted images in the periaqueductal region and bilateral dorsomedial nuclei of the thalamus. On single photon emission computed tomography (SPECT) using 99mTc-hexamethylpropyleneamine oxime, bilateral frontal perfusion was reduced, which was attributed to thalamo-cortical diaschisis due to injury to the dorsomedial nuclei of the thalamus. Presumably this phenomenon explains the Korsakoff psychosis. A history of gastrojejunostomy, even if normal intake is possible, is a risk factor for vitamin B1 deficiency, and should be considered in the differential diagnosis of Wernicke-Korsakoff syndrome.

Changes in spermatozoa due to large doses of pyridoxine (vitamin B6).

To investigate the changes of spermatozoa by high doses of vitamin B6 (B6), the alterations in spermatozoa and testis of rats after the administration of high doses of B6 were evaluated quantitatively and morphometrically. Wistar rats of 11 weeks of age were intraperitoneally injected with 63, 125, 250 and 500 mg/kg of B6 daily 5 times per week for 6 weeks. Using the spermatozoa taken from the epididymis and ductus deferens, the number, motility and nuclear morphology of spermatozoa were examined. After preparing 7 parameters for the nuclear morphology, the morphometry was performed by an IBAS version 2 (Zeiss) image analysis system. The number of spermatocytes and spermatids in representative stages of spermatogenesis was counted per Sertoli cell histologically. Mild deformation of spermatozoa nuclei occurred in the 63 mg or more exposure groups. In the 125 and 250 mg groups, the decrease in number as well as motility of spermatozoa together with slight decrease of spermatids in late maturation phase (mature spermatids) and the delay in spermiation appeared. Phagocytosis of mature spermatids by Sertoli cells was clearly increased in the 250 mg group. The alteration and the decreased number of spermatozoa are suggested to have mainly resulted from alteration of mature spermatids and the increased phagocytosis of mature spermatids by Sertoli cells. Computer-assisted morphometry of spermatozoa nuclei was useful not only to evaluate morphological changes objectively but also to discern them early.

Complex formation of human T-cell leukemia virus type I p40tax transactivator with cellular polypeptides.

We examined cellular components which associate with p40tax, the viral transactivation molecule of human T-cell leukemia virus type I. Such molecules were searched by immunoprecipitation with polyclonal and monoclonal antibodies specific for p40tax. Two cellular proteins with molecular masses of 95 kDa (p95) and 60 kDa (p60) were specifically coprecipitated with p40tax from extracts of all p40tax-producing cell lines but not from p40tax-negative cell lines. The p60 component was also shown to associate with p40tax in vitro, by using radiolabel-chase experiments. Rabbit antisera specific for p60 and p95 were prepared by immunization with electrophoretically purified molecules. While anti-p95 antiserum coprecipitated p40tax, no p40tax could be identified in immunoprecipitates by using a polyclonal anti-p60 antiserum. The partial amino acid sequence of p60 demonstrated that p60 is identical to the human 60-kDa heat shock protein (a member of the chaperonin family of proteins). Although the biological significance of the complex formation of p40tax with p95 and p60 has yet to be determined, it may be that the complex formation is one of the mechanisms by which the biological activity of p40tax can be regulated.

A study of the internal structure of the Japanese mandible.

The thickness of the substantia compacta and the volume and width of the trabecular bone of the substantia spongiosa were investigated by soft X-ray photographs made of thin sections of many kinds of dentulous and edentulous mandibles. The results were: 1) The thickness of substantia compacta is especially thick on the lingual side in the incisor region and thickest in the molar region in all materials. At the base of mandible, the thickness is essentially the same in all regions, but, the thickness of dentulous materials is greater than of edentulous ones. 2) On the upper part, the volume of trabecular bone is less in the premolar region in both edentulous and dentulous material than in other regions, however, there is relative more in edentulous than in dentulous. In the central part, there is almost the same amount of trabecular bone in the dentulous and edentulous material. 3) The width of the trabecular bone in the substantia spongiosa is greater in the incisor region than in other regions in all materials. In the upper and central part, it is smaller in the premolar region in both edentulous and dentulous material but, it increases in the molar region in the edentulous materials.

New mutant gene (transthyretin Arg 58) in cases with hereditary polyneuropathy detected by non-isotope method of single-strand conformation polymorphism analysis.

Single-strand conformation polymorphism (SSCP) was analyzed to detect a mutation in the transthyretin (TTR) gene from the mother and son showing polyneuropathy with carpal tunnel syndrome. DNA segments containing TTR coding sequence were amplified by polymerase chain reaction, heat denatured and electrophoresed on a neutral polyacrylamide gel. The single-stranded DNA fragments in the gel were transferred to a nylon membrane and hybridized with biotinylated TTR cDNA probe, followed with chemiluminescent DNA detection. The mobility shift was found in the fragments of exon 3 from the patients' DNA. Sequencing analyses of the exon 3 confirmed a T----G base change, resulting in a Leu 58----Arg substitution. TTR Arg 58 is the first mutant TTR gene that has been detected by SSCP analysis. The rapid and sensitive detection of new mutations at various sites on the TTR gene is hereafter possible by the present method in the facilities for non-radioactive experiments.

Preventive effects of methylcobalamin on the testicular damage induced by ethylene oxide.

In this study, the effects of methylcobalamin on testicular damage induced by ethylene oxide (EtO) were studied. When Wistar male rats inhaled EtO at 500 ppm, 6 h a day, 3 days a week, for 6 weeks, testicular damage was observed histopathologically and by some other parameters. Subcutaneous injection of methylcobalamin at 500 micrograms/kg, 5 times/week was found to ameliorate the damage. However, the degree of the methylcobalamin effect differed among the parameters examined in this study. Decrease in testicular weight due to EtO exposure was completely prevented by methylcobalamin, and decrease in testicular mature spermatid count and LDH-X activity was fairly well prevented. The degree of prevention of alteration in the epididymis, such as epididymal weight, epididymal sperm count and sperm abnormality rate, was significant but not complete. EtO caused apparent alterations in glutathione metabolism in the testes, but methylcobalamin did not affect such alterations induced by EtO. From these results, it has been determined that methylcobalamin has definite preventive effects on testicular toxicity of EtO.

Testicular damage by high doses of vitamin B6 (pyridoxine) in rats: a light and electron microscopical study.

Male Wistar rats were administered daily intraperitoneal injections of 125, 250, 500, and 1000 mg/kg/day of vitamin B6 for 2 and 6 weeks and the histogenesis of the testicular damage was investigated. A reduction of germ cells was not prominent in the 2-week groups, whereas a delay in spermiation, degeneration of elongated spermatids, and Sertoli cell alterations were observed in the 500- and 1000-mg groups, although generally, these were relatively mild. Ectoplasmic specializations (ES), tubulobulbar complexes, and endoplasmic reticulum (ER) in the apical processes of Sertoli cells were irregularly arranged and their disappearance was also retarded. The Sertoli cell cytoplasm was often retracted and condensed. In the 6-week groups, no histological change in the testis was noted with the 125-mg dose. The retardation in spermiation and Sertoli cell alterations similar to those in the 500-mg dose 2-week group were observed in the 250-mg group. In the 500- and 1000-mg groups, germ cells were generally degenerated and markedly reduced in number. Multinucleate germ cells were mingled with anisocytotic germ cells, and openings of intercellular bridges were occasionally found. Sertoli cells also showed more severe alterations, such as focal disappearance of ES in earlier than ordinary stages, marked dilation of the ER, and markedly condensed or electron-lucent cytoplasm. These results suggest that the Sertoli cell damage may induce diverse germ cell degeneration in which retardation of spermiation occurs first.

Absorption, biliary excretion, and metabolism of a new cholelitholytic agent, ursodeoxycholyl N-carboxymethylglycine and its esters in rats.

Intestinal absorption, biliary excretion and metabolism of a calcium gallstone dissolving agent, [11,12-3H]ursodeoxycholyl-N-carboxymethylglycine (UDC-CMG) and its monoethyl, diethyl and dipivaloyloxyethyl esters (UDC-CMG-Et, UDC-CMG-Et2 and UDC-CMG-PV2) were studied in bile duct cannulated rats. Biliary recovery of [3H]-labeled UDC-CMG, UDC-CMG-Et and UDC-CMG-Et2 after intraduodenal administration were 65%, 80%, 98%, respectively. Radio-thin layer chromatography analysis of the bile revealed that UDC-CMG didn't undergo any biotransformation during administration and excretion. About 80% and 20% of radioactivity recovered in the bile was identified as UDC-CMG-Et and UDC-CMG, respectively, after intraduodenal administrations of [3H]UDC-CMG-Et2 and [3H]UDC-CMG-Et. The administered intact UDC-CMG-Et2 was not found in the bile. Intraduodenally administered [3H]UDC-CMG-PV2 was rapidly recovered in the bile. The total recovery rate was 78% within a 24 h period. More than 80% of the radioactivity recovered in the bile was found as UDC-CMG. Lesser amounts of the monopivaloyloxyethyl ester of UDC-CMG were also found, but intact UDC-CMG-PV2 was not detected in the bile as in the case of UDC-CMG-Et2. Among the esters of UDC-CMG investigated in the present studies, only UDC-CMG-PV2 was excreted in the bile mainly as the perhydrolyzed form, UDC-CMG. These results suggest the usefulness of UDC-CMG-PV2 as the pro-drug in calcium gallstone dissolution therapy.

Surgical results of brain metastasis from lung cancer--prognostic factors.

Twenty-five patients receiving surgical treatment for brain metastasis from lung cancer were retrospectively studied to evaluate the prognostic factors for survival time. Twenty-two patients had died of respiratory distress by April, 1989. Favorable prognostic factors derived from the median survival time (MST) in these patients included; 1) resection of primary tumor (MST 10 months); 2) total or subtotal removal of metastatic tumor (MST 6.5 months); 3) adenocarcinoma (MST 13 months); 4) metachronous onset of brain metastasis (MST 12 months); 5) single metastasis (MST 8 months). These results suggest that therapy for the primary lung cancer is important before surgery for metastatic brain tumor.

Research into changes in canine maxillary trabecular bone orientation after tooth extraction.

This paper described the morphological changes in the trabecular bone orientation of the jaw bone due to reduction in the applied functional pressure. All teeth on the right side of the upper and lower jaws were extracted from canine specimens. Experimental periods were set at 0.5, 1, 1.5, 2, 2.5, 3, 5, 7, 9, 11, and 13 months after tooth extraction. The trabecular bone orientation was measured by an image analyzer as morphometrical parameters. The loss of teeth changed the support system for functional pressure, resulting in the marked disordering of the trabecular bone orientation.

[Effect of transfusion of blood components on gastric cancer patients--special reference to the incidence of postoperative hepatitis and prognosis].

To evaluate the effect of blood transfusion on incidence of postoperative hepatitis and prognosis of gastric cancer, the data of 493 patients having been operated on curatively in our hospital were divided into two groups: those who received transfusion before the beginning of components transfusion and those who received transfusion after that. The data indicated that the incidences of postoperative hepatitis increased from 3.7% to 5.4% after the beginning of transfusion of blood components, but the 5-year survival rates did not differ significantly. The 5-year survival rates of transfused patients and non-transfused patients were 57%, 84%, respectively (p less than 0.001) and the rates decreased according to the dose of whole blood and/or packed red cells. The same results were obtained when limited to stages I + II, But they were not deteriorated by fresh frozen plasma significantly. The result suggests that it is better to refrain from unnecessary blood transfusion and transfuse whole blood in the case of massive transfusion to decrease the incidence of postoperative hepatitis and more better to transfuse leucocyte-free blood to avoid immunological effect.

[A case of Behçet disease with transverse sinus thrombosis and successful treatment with colchicine].

A 64-year-old man was admitted to our hospital because of headache and fever. Four months before admission, he had suffered from thrombophlebitis of the legs, erythema, oral aphtha and genital ulcers, and was diagnosed as having Behçet disease. Several days prior to admission he developed fever and was treated with enoxacine, which caused disorientation and abnormal behavior. These psychiatric symptoms disappeared after enoxacine was discontinued. On admission he was neurologically normal. Fever and headache disappeared spontaneously. However, he complained again of headache, and was suspected to have cerebral sinus thrombosis. Brain CT scan showed no abnormality, but brain MRI and cerebral angiography demonstrated right transverse sinus thrombosis and stasis. He was treated with colchicine, isosorbide, and ticlopidine. Because he had a gastric ulcer, corticosteroids were not indicated. Two months later, brain MRI demonstrated reduction of the size of the sinus thrombosis, and the clinical outcome was favorable. We would like to emphasize the usefulness of MRI and colchicine in the patient with Behçet disease complicated by sinus thromboses.
