Magnetic resonance myelography in brachial plexus injury.

We used magnetic resonance (MR) myelography in ten patients with injuries to the brachial plexus and compared the findings with those obtained by conventional myelography and postmyelographic CT (CTM). In the presence of complete nerve-root avulsion (seven cases), a post-traumatic meningocele was detected by MR myelography. In injuries to the upper roots (three cases) MR myelography showed abnormal findings with a high signal intensity in the nerve root, obliteration of the damaged nerve root, or enlargement and obliteration of the root sleeve. No pseudomeningoceles were detected in these upper-root injuries by MR myelography and CTM. The overall accuracy of detection of damaged nerve roots or root sleeves was better with MR myelography than with conventional myelography and was similar to that of CTM. MR myelography is non-invasive, relatively quick, requires no contrast medium, provides imaging in multiple projections, and is comparable in diagnostic ability to the more invasive, time-consuming techniques of conventional myelography and CTM.

Functional anatomy of the triangular fibrocartilage complex.

The functional anatomy of the triangular fibrocartilage complex (TFCC) was investigated in 20 fresh cadavers. Dynamic changes in the TFCC during rotation were also examined from both the proximal and distal aspects. In our analysis, the TFCC was separated into three components. The distal component was stable, functioning like a hammock to suspend the ulnar carpus. The proximal component was the fan-shaped triangular ligament, the true radioulnar ligament. This was found to originate from the fovea of the ulna in a vertical fashion and was easily twisted during rotation. The third component, the ulnar collateral ligament, also twisted during rotation. When analysed by coronal section, the internal portion of the TFCC was found to be loose, and probably serves as a cushion to absorb local deformities in the TFCC during rotation.

Eleven-year survey of safety and efficacy of endoscopic injection sclerotherapy using 2% sodium tetradecyl sulfate and contrast medium.

We conducted a retrospective 11-year survey to evaluate the post-treatment course in 285 patients with esophagogastric varices following administration of endoscopic injection sclerotherapy as an emergency, elective, or prophylactic procedure using freshly prepared 2% sodium tetradecyl sulfate not containing benzyl alcohol. These agents were injected into the varices and the supplying veins under fluoroscopic observation, usually in a single treatment. In all patients the variceal size was greatly reduced following one treatment. The amount of sclerosant necessary to fill the varices and the supplying veins varied widely among the patients. Acute variceal bleeding was controlled in 80 (96.4%) of the 83 patients, and the risk of rebleeding during the first month was 0.0548 in the emergency procedures. The serious complication of perforation was observed in one patient. The cause of death was established in the 122 patients who died and included esophageal variceal bleeding in eight (6.6%) and gastric variceal bleeding in one (0.8%). The overall 50% survival period was 5 years and 4 months. Multivariate analysis disclosed that the factors with the greatest negative effect on survival were poor hepatic status and the presence of hepatocellular carcinoma. The method of preparation and the procedure itself may be considered safe and effective in the treatment of esophagogastric varices.

Immunohistochemical study of cellular events in lesional skin during common virus infections.

Determination was made of epidermal Langerhans cell (LC) distribution and infiltrating cellular events in lesional skin during varicella zoster virus (VZV) infection, and the results were compared with those for herpes simplex (HS), measles, and rubella by immunohistochemical staining with cell surface markers. CD1a positive epidermal LCs increased in number, particularly in measles and rubella. The number of LCs was within the normal range or slightly increased in the epidermis of VZV infection. In herpes zoster (HZ) and varicella, HLA-DR positive epidermal cells were present in the basal part of the epidermis. In measles, HLA-DR positive cells aggregated in papular lesions. In measles and rubella, the number of HLA-DQ positive epidermal cells appeared to increase. In HS cases, CD11b (OKM1) positivity of the upper epidermal keratinocytes was quite pronounced, but not in the basal layer. CD8 positive suppressor/cytotoxic cells extensively infiltrated the dermis of HZ and varicella. Dermal infiltrates were identified as CD8 positive cell dominant in measles, HZ, and varicella. These results provide a partial explanation as to why cellular events in skin lesions act immunosuppressively.

Accumulation of hydroxyapatite in the kidney of streptozotocin-induced diabetic rat fed a low-zinc diet.

Calcification occurred in the kidney of streptozotocin (STZ)-induced diabetic rats fed a low-zinc diet. The deposits were stained by the von Kossa method and were detected intracellularly in the tubular cells, mainly in the cortico-medullar region. The deposits were found to be a heterogenous substance on electron microscopy. There were various sizes of deposits, and the surfactant was very much distorted. Many deposits grew up to bind small particles, and the core-like substance was observed in the center of the deposit. The component of the deposit was analyzed by an X-ray microanalyzer, and was found to be calcium and phosphorus. The ratio of calcium to phosphorus was 2.159, which coincided with the ratio of standard hydroxyapatite. From these observations, the deposit is believed to be hydroxyapatite. It is thought that the core is formed at first, many particles are then bound to the core, and deposits grow up.

Electron microscopic observations of epithelioid cell granulomas experimentally induced by prototheca in the skin of mice.

Electron microscopic analysis was conducted on epithelioid cell granulomas experimentally induced by prototheca in the skin of mice. In BALB/c mice, epithelioid cell granulomas were easily induced. Each cell had a round nucleus with euchromatin on the margin, abundant cytoplasmic organelles, and particularly abundant rough endoplasmic reticulum (rER). Cells with a plasmacytoid appearance were the most common. Fibroblast-like epithelioid cells were sometimes observed. The whole body and cell membrane of an organism was usually within the cytoplasm of epithelioid cell. In ICR mice, epithelioid cell granulomas were not easily induced. Cells which had ingested whole organisms had spindle shaped and/or oval nuclei with marginated heterochromatin and with abundant dilated rER. Near completion of organism digestion, the cells had irregular and/or indented nuclei with marginated heterochromatin and often one or two distinct nucleoli. The cytoplasm was often replaced with numerous phagocytic vacuoles. At higher magnification, secondary lysosomes and Golgi complexes forming Golgi lamellae could be seen in the cells in which cytoplasmic organelles such as endoplasmic organelles and mitochondria were abundantly present. In contrast to the BALB/c mice, the remnants of digested organisms, split up into many small pieces, could be seen in the cytoplasm. The cells with a fibroblast-like appearance were most frequent, but plasmacytoid cells were sometimes observed. In the present study, the ultrastructural features of epithelioid cells in BALB/c and ICR mice clearly differed. Plasmacytoid cells were the most abundant in BALB/c mice, while fibroblast-like cells were in ICR mice.

Effects of varicella zoster virus or herpes simplex virus type I infection in vitro on response of human peripheral blood mononuclear cells to phytohemagglutinin.

Examination was made of the in vitro response of human peripheral blood mononuclear cells (PBNMCs) to phytohemagglutinin (PHA) following treatment with varicella zoster virus (VZV) or herpes simplex virus type 1 (HSV 1). Cell proliferation was determined by colorimetric assay using 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl-tetrazolium bromide. The response to PHA was depressed in all cases by virus infection of PBMNCs prior to PHA treatment. When the infection with the viruses was after PHA treatment, PHA response differed. For VZV infection, the response increased in four out of six samples, but was reduced in the other two. The response to PHA was depressed in all six samples by HSV 1 infection.

Epithelioid cell granulomas experimentally induced by prototheca in the skin of mice: a light microscopic study.

Prototheca wickerhamii, an achlorophyllous algae, was previously found to induce massive epithelioid cell granulomas in the skin of mice. By means of light microscopy, examination was made of the histological reactions involved in epithelioid cell granulomas induced by intradermal and/or subcutaneous inoculation of Prototheca wickerhamii in BALB/c and ICR mice. Six BALB/c mice showed granuloma nodules while only three of six ICR mice did so. Based on the results of the present and previous studies, BALB/c mice may be considered a strain particularly vulnerable to contracting epithelioid cell granuloma and ICR mice, a resistant strain. In very early lesions at one week following initial prototheca inoculation, cellular infiltration with varying numbers of polymorphonuclear leukocytes, lymphocytes and some macrophages was observed throughout the dermis and subcutaneous fat tissue. In early lesions at one to two months after inoculation, focal granulomas composed of histiocytic cells and/or macrophages were observed. Mast cells were occasionally present among the histiocytic cell infiltrates. In the granulomatous lesions at two to three months, scattered eosinophils and some lymphocytes were seen. Central necrosis, with numerous neutrophils and many endospores surrounded by the granuloma, was often observed. In late stage lesions at six months, massive lymphocyte and plasma cell infiltration surrounding and/or intervening between vacuolated epithelioid cell clusters was evident. Histological reactions in epithelioid cell granuloma and the ultimate course of this disease can be staged from the histological point of view as follows: 1) diffuse inflammation, 2) cell proliferation leading to epithelioid cell formation, 3) hypertrophy of epithelioid cells with consequent formation of cell aggregates and/or organized granuloma and 4) degeneration of granuloma.(ABSTRACT TRUNCATED AT 250 WORDS)

Cytophagic histiocytic panniculitis in systemic lupus erythematosus.

This paper presents a case of cytophagic histiocytic panniculitis in a Japanese woman, who had systemic lupus erythematosus complicated with Hashimoto's thyroiditis and lupus nephritis from the age of 12. The patient had painful multiple purplish subcutaneous nodules on the face, trunk and extremities, high fever and liver dysfunction without coagulopathy. The histological features of the skin nodules were extensive histiocyte and/or macrophage infiltration often with leuko- and/or erythrophagocytosis in the subcutaneous fat tissue.
