Kaposi's varicelliform eruptions during the course of steroid withdrawal in a senile erythroderma patient: cure of regional erythrodermic lesions following infection.

The author encountered a case of Kaposi's varicelliform eruptions on both axillar regions during the course of steroid withdrawal in a 68-year-old male with eythroderma following eczema. Immnohistochemical study gave positive indication of herpes simplex virus type I in epidermal keratinocytes in lesional vesicles. Following cure of the varicelliform eruptions, erythrodermic lesions in the axillar regions cleared up completely. For those at other sites, considerably more time was required for cure, with steroid withdrawal being a factor of this period.

Functional Anatomy of the Interosseous Membrane of the Forearm - Dynamic Changes During Rotation.

The functional anatomy of the interosseous membrane (IOM) of the forearm was studied in 15 fresh frozen cadavers. The IOM consisted of three components: the tendinous part; the membranous part; and the dorsal oblique accessory cord. The tendinous part was a thick and strong complex of three to ten bundles which run from the proximal one third of the radius to the distal quarter of the ulna. The membranous part was recognised as a soft and thin structure, adjacent proximal and distal to the tendinous part. The dorsal oblique cord was found as a single cord on the dorsal side of the IOM from the proximal quarter of the ulna to the middle region of the radius. This cord was distinct from the oblique cord, which existed on the palmar side of the forearm separated from the IOM. The IOM was flat in the neutral position, while it flexed along the rotation axis in pronation and supination. Since the prominence of the curvature in the IOM was almost in the same direction as the fibres of the tendinous part, the tendinous part demonstrated only bending, similar to partial bending of an elastic board. The bending position was changed with the forearm position and the tightened bundles in the tendinous part were altered during rotation.

Systemic hyalinosis of delayed onset.

We report a 46-year-old Japanese male with systemic hyalinosis. The condition had not been present during youth but was clearly evident in the patient's forties. The present case is remarkable because of a delayed onset.

[The application of the endotoxin test for globulin and other blood products].

The application of the endotoxin test for globulin and other blood products were investigated by two different limulus amebocyte lysate (LAL) test methods, colorimetric and kinetic turbidimetric methods, using two endotoxin-specific reagents. By the dilution of the blood products in 40 times or more, spiked endotoxin in the products was recovered accurately showing neither inhibition nor enhancement. The definite difference was not shown between the results obtained by the two LAL test methods. According to the method of the endotoxin test described under General Tests of The Japanese Pharmacopeia (thirteenth edition), JPXIII, the maximum valid dilution (MVD) for these products will be calculated to be 40 or more, so it is capable to measure the endotoxin limit for each product. This study indicates that the endotoxin test is applicable to measure the endotoxin content in globulin and other blood products as an alternative method for the rabbit pyrogen test.

Differences in Thinning Processes of Some Vesicular Foam Films with Different Compositions.

Thinning processes of foam films are observed for various phospholipid vesicle suspensions by the horizontal suspending method using an interferometric microscopy system. It is found that the phospholipid components that construct the bilayer of unilamellar vesicles or micelles affect the thinning process behavior of the plane-parallel liquid film. In the case of vesicle from egg phosphatidylcholine (PC), the plane-parallel foam film made of its suspension shows a gradual but inhomogeneous slow thinning process, which differs from the stepwise thinning process of thin liquid films with silica or micelle suspensions reported in previous investigations. However, when lysophosphatidylcholine was mixed with egg PC, channel formation and coexistence of black thin films and thicker regions were observed. Copyright 1998 Academic Press.

Sea urchin ovoperoxidase: oocyte-specific member of a heme-dependent peroxidase superfamily that functions in the block to polyspermy.

Ovoperoxidase is one of several oocyte-specific proteins that are stored within sea urchin cortical granules, released during the cortical reaction, and incorporated into the newly formed fertilization envelope. Ovoperoxidase plays a particularly important role in this process, crosslinking the envelope into a hardened matrix that is insensitive to biochemical and mechanical challenges and thus providing a permanent block to polyspermy. Here we present the primary structures of two ovoperoxidases as predicted from cDNAs cloned from the sea urchins Strongylocentrotus purpuratus (AF035380) and Lytechinus variegatus (AF035381). We also present a proposed scheme for the post-translational processing of ovoperoxidase based upon comparisons between the cDNA and protein structures and taking into account previously published reports. The sea urchin ovoperoxidase sequences conform to a profile shared by members of a heme-dependent animal peroxidase family, including the mammalian myelo-, lacto-, eosinophil, and thyroid peroxidases. Using in situ RNA hybridizations, we showed that the mRNA of S. purpuratus ovoperoxidase (4 kb) is present exclusively in oocytes, and is turned over rapidly following germinal vesicle breakdown. Taking into account our immunoblot and N-terminal sequencing data along with reports from similar peroxidases, we propose that ovoperoxidases are synthesized in a pre-pro form and proteolytically processed to result in the 70 and 50 kDa forms that are found in the fertilization envelope. The sequence and structural data presented here will facilitate our continuing studies of the biogenesis of cortical granules and the fertilization envelope. Additionally, since ovoperoxidase activities have been reported in a wide range of animals, these cDNAs will be useful in uncovering similar peroxidases used in the fertilization reactions of other metazoan eggs.

Primary healing of physeal separation under rigid fixation.

We observed the healing process under rigid external fixation after Salter-Harris type-1 or type-2 physeal separation at the proximal tibia in immature rabbits. Metaphyseal vessels grew across the gap with little delay; the site of separation then came to lie in the metaphysis and was bridged by endochondral ossification. Union was achieved within two days in all rabbits. Progression of endochondral ossification repaired the separated physis, thus showing 'primary healing of physeal separation'. This depends on accurate reduction and stable fixation to allow the survival of vessels across the gap.

Surgical management of eosinophilic fasciitis of the upper extremity.

Eosinophilic fasciitis is a rare inflammatory disease associated with peripheral eosinophilia, hyper-gammaglobulinaemia and contractures of any joint in the upper extremity. Although conservative treatments are generally advocated, this study reports the results of surgical intervention. Four patients aged from 20 to 48 years underwent fasciectomy followed by oral administration of prednisolone. All presented with contractures of digits, wrist, or elbow due to eosinophilic fasciitis in the upper extremities. Despite one case that required a second operation for recurrence, all patients regained the range of motion of the affected joints a few weeks after surgery. The recovery was much sooner than in previously reported cases treated conservatively, suggesting that surgical management of eosinophilic fasciitis is effective in alleviating symptoms quickly and allows patients to resume activities of daily living sooner.

Mo2+ HLA-DR- monocytosis in varicella zoster virus infection.

Two-color analysis using Mo2+ (CD14) monocytes and HLA-DR class II monoclonal antibodies confirmed monocytosis in varicella zoster virus (VZV) infection in eight cases of herpes zoster and two cases of varicella. Only three cases, one being varicella, showed extensive Mo2+ HLA-DR- monocytosis of more than 12%. Accordingly, monocytosis in VZV infection may not be specific to the VZV antigen.
