Massive osteolysis of the mandible.

This report deals with a massive osteolysis, which is a rare disease characterized by the progressive dissolution of contiguous osseous structure, occurring in the mandible of a 46-year-old Japanese female. Histopathologic examination of the amputation material revealed that the mandibular body, ramus, condylar and coronoid processes were completely replaced by fibrous tissue, but inferior alveolar nerve and artery in the lesion were intact. A few small islands of osteoid tissue or immature fibrous bone were seen in the lesion. In the transitional area between the lesion and normal bone, active absorption of bone trabeculae was in progress and intertrabecular spaces were occupied by densely packed fibroblastic cells, however, angiomatous proliferation of vessels was not observed. A few bone trabeculae were surrounded by osteoclasts, but osteoclasts might not play a primary etiological role in massive osteolysis.

Minor salivary gland swelling in patient with Sjögren's syndrome.

A case of Sjögren's syndrome with an unusual minor salivary gland swelling is reported. The patient was a 63-year-old female with a chief complaint of palatal swelling and dry mouth. Clinical examination revealed diffuse swelling of the hard palate and bilateral parotid regions, and sialographic examination of the parotid gland showed evidence of sialoangiectasia. Histopathological findings of the palatal and parotid lesions revealed intense lymphoid cell infiltration replacing the salivary gland parenchyma, and scattered epimyoepithelial islands throughout the lesions. Although it is well known that major salivary gland swelling caused by marked lymphoid cell infiltration is one of the common features in patients with Sjögren's syndrome, a similar swelling in the region of the minor salivary glands is not usually apparent.

Diffuse pulmonary ossification associated with chronic bronchitis.

A thirty-three-year-old man with diffuse pulmonary ossification associated with chronic bronchitis is described. Chest roentgenograms of the patient showed diffuse reticulo-nodular shadows partially super-imposed by calcified reticular opacities. Marked declines in forced vital capacity and air-flow limitation were associated with decreased diffusing capacity of the lung for CO, hypoxemia, and hypercapnia. In the postmortem examination, diffuse dendriform ossification was found mainly in the alveolar space. Neither interstitial fibrosis nor capillary congestion was found histopathologically in the lung.

[Clinical experience of tegafur-uracil (UFT) against bladder cancer and renal cell carcinoma].

The safety of prolonged administration of UFT in which tegafur and uracil were mixed in a ratio of 1:4 in molar fraction was studied in 44 cases of bladder cancer and 10 cases of renal cell carcinoma. Daily doses of UFT were 300-600 mg, and average total doses administered were 102.0 g for bladder cancer and 116.6 g for renal cell carcinoma cases. Incidence of adverse effects were 25.0% in bladder cancer and 18.5% in renal cell carcinoma cases. Anorexia, nausea, vomiting and decrease in WBC were observed, but rates of having discontinued the administration of UFT were very low, being 9.1% in bladder cancer and 10.0% in renal cell carcinoma. Thus, UFT was considered to be tolerable during prolonged use in bladder cancer and renal cell carcinoma and also a drug in which more usefulness is expected in multidisciplinary treatments in future.

Relationship between mutagenic potency in Salmonella typhimurium strains and the chemical structure of nitro biphenyls.

Most of the positional isomers of mono-, di-, tri- and tetranitrobiphenyls were synthesized and assayed for their mutagenicity in Salmonella typhimurium strains TA98, TA98NR and TA98/1,8DNP6 in the absence of S9 mix. In mono- and dinitrobiphenyls, the structure requirements favoring mutagenic activity are the presence of a nitro group at the 4-position and its absence at the 2-position. TA98 and TA98/1,8DNP6 were reverted by 2-position-free 4-nitro analogues, but TA98NR was not reverted. The results suggest that direct-acting mutagenicity involves the reduction of the nitro group by bacterial nitroreductase but does not involve specific esterification enzymes. Some of the tri- and tetranitrobiphenyls e.g. 3,4,3'-, 3,4,4'-, 3,4,3',4'- and 3,4,2',4'-derivatives reverted not only TA98 and TA98/1,8DNP6 but also TA98NR. Those derivatives commonly have 2 nitro groups at an adjoining position (3,4-dinitro group), whereas 2,4,2',4'-tetranitrobiphenyl, which has strong potency not only in TA98 and TA98/1,8DNP6 but also in TA98NR, possesses 2 nitro groups at the 2-position of each benzene ring.

Cell-surface hydrophobicity of Candida species as determined by the contact-angle and hydrocarbon-adherence methods.

Cell-surface hydrophobicities of six Candida species were studied by two methods: measurement of the contact angle, and partitioning with aqueous-hydrocarbon (n-octane, n-hexadecane and p-xylene) mixtures. C. tropicalis, C. glabrata and C. krusei adhered better to the hydrocarbons than did C. albicans, C. stellatoidea and C. parapsilosis. Contact angles for the less adherent species were smaller than those for the more adherent species. Thus the two methods gave results that were similar overall and indicated that C. tropicalis, C. glabrata and C. krusei have greater cell-surface hydrophobicities than C. albicans, C. stellatoidea and C. parapsilosis.

Xanthogranuloma of the oral cavity in adult. A case report and review of the literature.

A case of xanthogranuloma arising in the palatal gingiva of the maxilla in a 38-year-old Japanese male was investigated. Histopathological findings of the excised lesion were indistinguishable from the xanthogranulomas in infants and children (juvenile xanthogranuloma), and like those in the young, the present case was not associated with abnormalities of serum lipids and was not associated with other lesions. A review of the English literature yielded only two histopathologically documented cases of xanthogranuloma in the oral cavity, arising in young patients. A case of xanthogranuloma of oral cavity in an adult was not documented previously.

Depression of delayed-type hypersensitivity in mice with hypothalamic lesion induced by monosodium glutamate: involvement of neuroendocrine system in immunomodulation.

Delayed-type hypersensitivity (DTH) was depressed in mice that had been treated with monosodium glutamate (MSG) in their suckling period. Analysis of the DTH depression by use of the macrophage migration inhibition assay showed dysfunction of DTH effector T cells. The neuronal loss of nuclei in the hypothalamus, which elaborates the corticotropin-releasing factor and the hypersecretion of adrenocorticotrophic hormone, was observed in the MSG-treated mice. Therefore, DTH response may be modulated by the neuroendocrine system.

[Intracranial hemorrhage associated with pregnancy--4 case reports and a review of the literature].

Four cases of intracranial hemorrhage associated with pregnancy are reported. They were a ruptured anterior communicating aneurysm, a ruptured AVM of cerebellar hemisphere, a lt-thalamic and putaminal hemorrhage and brain stem hemorrhage. As the mode of hemorrhage, one case was a subarachnoid hemorrhage, one case was a subarachnoid hemorrhage combined with a cerebellar hemorrhage and other two cases were intraparenchymal hemorrhage. The mean age of these 4 cases was 29.5 years of age from 25 to 35. One case was occurred the intracranial hemorrhage during the first trimester but other 3 cases was occurred during the third trimester. It was, thus, considered the intracranial hemorrhage associated with pregnancy was more frequently occurred during the third trimester. Primigravida and multigravida were equally occurred on each two cases. It was not related to parity. Consciousness levels were drowsiness; one case, semicoma; two cases and deep coma; one case. A case of ruptured anterior communicating aneurysm underwent neck clipping of aneurysm through the rt-fronto-temporal craniotomy on the first day of the attack. This case fully recovered, but the other three cases of severely impaired consciousness level were dead. Maternal prognosis was related to consciousness level. We mentioned the preferred neurosurgical and obstetrical management to the intracranial hemorrhage associated with pregnancy.

Morphological and immunohistochemical evidence for the presence of subcortical target cells of N-ethyl-N-nitrosourea-induced cerebellar tumors in rats.

Morphological and immunohistochemical studies of cerebellar tumor induction with neonatal administration of N-ethyl-N-nitrosourea (ENU) were conducted in four strains of rats and their hybrids, i.e., noninbred Wistar, Fischer (F344), Long-Evans, Wistar/Furth, and hybrids of Long-Evans and Wistar/Furth. Neonatal s.c. injection of 40 mg ENU/kg body weight produced 53 cerebellar tumors in 46 (8.4%) rats among 550 animals. There was no sex difference in the incidence (male = 9.6%; female = 7.0%). Histological examination showed that most of the tumors (83%) were oligodendrogliomas and the neoplastic cells were positively stained immunohistochemically with anti-Leu-7 monoclonal antibody. In examining the location of cerebellar tumors, 22 (42%) were located in the vermis, 11 (21%) in the hemisphere, 9 (17%) in the flocculus, 6 (11%) in the peduncle, and 5 (9%) in other sites. When their origins were examined in relation to their location to the internal granular layer of the cerebellum, 40 (75%) tumors were found just under the internal granular layer (subcortical region) and 9 (17%) in the white matter or cerebellar nuclei. Only 2 (4%) subependymal tumors were observed. Ontogenic study of the rat cerebellum revealed the presence of an aggregation of primitive glial cells in the subcortical region during the neonatal period, and the [3H]thymidine pulse-labeling index of these cells was 13.7%. Electron microscopic study showed the primitive nature of these cells and they reacted positively with anti-Leu-7 monoclonal antibody. These results indicate that cerebellar tumors are induced in an appreciable incidence with neonatal injection of ENU in rats and that cerebellar target cells in the subcortical region are present after ENU carcinogenesis.

Relationship between mutagenic potency in Salmonella typhimurium and chemical structure of amino- and nitro-substituted biphenyls.

All positional isomers of mononitro- and monoaminobiphenyls and those of dinitro-, diamino- and aminonitrobiphenyls, which have one substituent on each benzene ring, were assayed for mutagenicity in Salmonella typhimurium by the Ames method. The results suggest that the structural requirements favoring mutagenic activity are the presence of substituents at the 4-position and their absence at the 2'-position. The introduction of an amino group to the 3'- or 4'-position of 4-nitrobiphenyl or a nitro group to 3'- or 4'-position of 4-aminobiphenyl enhanced the mutagenicity. Among the mutagenic compounds, 4-nitro analogues were mutagenic in strains TA98 and TA100 in the absence of a microsomal metabolic activation system. Strain TA98NR was not reverted by the direct-acting mutagens, whereas strain TA98/1,8-DNP6 was as revertible as strain TA98; these results suggest that the direct-acting mutagenicity involves the reduction of the nitro group by bacterial nitroreductase but does not involve specific esterification enzymes.

Possible role of apoptotic cells of the oral epithelium in the pathogenesis of aphthous ulceration.

Electron microscopic examination of the normal oral mucosa of patients with recurrent aphthous ulceration or Behçet's syndrome revealed that the degenerate dark prickle cells are apoptotic cells. Such dark prickle cells showed characteristic findings of apoptosis, such as the shrinkage of nucleus and cytoplasm and the formation of contraction vacuoles. The number of intraepithelial mononuclear cells phagocytosing apoptotic cellular debris increased remarkably at the preulcerative stage of the aphthous lesions. The apoptotic debris also attracted neutrophilic leukocytes in the prickle cell layer at the preulcerative stage but not in the normal oral epithelium. It is speculated that the onset of aphthous ulceration is closely related to phagocytosis of these apoptotic cells by intraepithelial mononuclear cells.

Immediate repair of mandibular defects following surgery for carcinoma of the lower alveolus and gingiva using a pectoralis major osteomyocutaneous flap.

During the two years from 1981 to 1983, a pectoralis major myocutaneous island skin flap, with the 5th rib or sternum, has been used for immediate repair of mandibular defects following surgery in five cases of carcinoma of the lower alveolus and gingiva. In these 5 patients with segmental and hemimandibulectomy, immediate mandibular reconstruction was performed simultaneously using a flap with the 5th rib or the sternum. Thereafter, in 3 of them, the contour and function of the mandible were restored by prosthetic appliances. In the remaining 2 patients prostheses could not be used because of mandibular discontinuity due to removal of a portion of the graft because of postoperative infection and pseudo-arthrosis. Mandibular reconstruction using the osteomyocutaneous flap is advantageous in the restoration of the contour and function of the mandible. Some problems concerning the reconstruction will be reported.

Further studies on the subcellular distribution of Cd2+ in Cd-sensitive and Cd-resistant strains of Saccharomyces cerevisiae.

When a Cd-resistant strain (301 N) and a Cd-sensitive strain (101 N) of Saccharomyces cerevisiae were incubated in medium containing Cd2+, a large proportion of the cellular Cd2+ was found in the cytosol of strain 301 N, but not in that of strain 101 N. Approximately 65% of the cellular Cd2+ was released from strain 301 N after treatment with chitosan, which affects cell membrane permeability. About 80% of the cellular Cd2+ released from strain 301 N by chitosan treatment was detected in a 30 000-10 000 molecular weight fraction prepared by ultrafiltration. The distribution of Cd2+ into the cytosol in strain 301 N was inhibited in the presence of cycloheximide. The proportion of cellular Cu2+ or Zn2+ present in the cytosol after incubation with these ions was similar for the two strains (about 40%).

Melanocytes in odontogenic keratocyst.

Melanocytes in odontogenic keratocyst (OKC) were studied histopathologically. As a result, melanocytes were observed within the basal layer of lining epithelium in five of 47 cases with OKC, with the overall prevalence being 10.6 per cent. Although no conclusion could be drawn from the present examination as to the origin of melanocytes in OKC, its possible histogenesis was discussed.

[Histopathological diagnosis of early stages of myocardial infarction--applications of the improved hematoxylin basic fuchsin picric acid (HBFP) staining method to human autopsy hearts].

In the early stage of myocardial infarction, the mortality rate is very high and its diagnosis is difficult not only clinically but also pathologically. Hematoxylin basic fuchsin picric acid (HBFP) staining method has been controversial on reliability for the morphologic diagnosis of early stages of myocardial ischemia. We applied HBFP staining method improved hematoxylin staining and differentiating solution to 42 human autopsy hearts. In eight cases with diagnosed acute myocardial infarction clinically and/or pathologically and 4 cases that suffered from post-infarction angina and died suddenly at home or outdoors, HBFP stain showed positive. Other cases, including non-ischemic heart disease and sudden death due to non-cardiac disease and ventricular arrhythmia, were HBFP staining negative. In conclusion, improved HBFP staining method is useful in identifying early myocardial ischemia and extent of early infarcts, and explaining for sudden death due to re-attack of myocardial infarction.
