Necrotizing B-cell lymphoma. A case report with a review of the literature.

We describe a case of massively necrotizing B-cell lymphoma, involving both axillary and cervical lymph nodes. Histologically, no vascular lesions were noted, and the necrosis was manifested by death of the tumor cells themselves. These cells were surrounded by macrophages, simulating the features of tuberculous lymphadenitis. Immunostaining was a useful method of defining the origin not only of viable cells, but also of necrotic cells in the lymphoma.

Malignant melanotic neuroectodermal tumor arising from the pineal body.

A case of a melanotic neuroectodermal tumor arising from pineal region of a 4-year-old girl is presented. The tumor had spread diffusely to the meninges, consistent with malignant behavior. Histologically, the tumor consisted primarily of epithelial elements arranged in tubules, cords and nests separated by fibrous vascular tissue in addition to a small neuroblastomatous focus. Melanin pigment was frequently observed in the epithelial tumor cells, and melanin-laden macrophages were also often observed. No teratoid elements were found. Immunohistochemically, tumor cells were positive for neuron-specific enolase but were nonreactive for S-100 protein, epithelial membrane antigen, glial fibrillary acidic protein, vimentin, alpha-fetoprotein and human chorionic gonadotrophin. Ultrastructurally, the epithelial nature of the tumor cells could be easily demonstrated. In addition, melanosomes in various stages in maturation were observed, indicating melanogenesis of the tumor. On the basis of the tumor location and the histological similarities previously observed for the fetal pineal body, it is very likely that this melanotic epithelial tumor could have originated from the fetal pineal gland.

Onuf's motoneuron is resistant to poliovirus.

Onuf's nucleus, located in the anterior horn of the sacral spinal cord, has been shown to innervate the striated muscles of the urethral and anal sphincters and is known to be well preserved in patients who have motoneuron disease. To answer the question whether the nucleus is also safe from motoneurotrophic virus infections, we examined the spinal cords of 3 individuals who had suffered severe poliomyelitis. In 2 of these which were acutely fatal cases, all motoneurons were damaged by neuronophagia, but Onuf's nucleus escaped the inflammatory degeneration. In a chronic case, neuronal loss in the anterior horn with replacement by gliosis was prominent below the thoracic cord down to the sacral cord, but Onuf's nucleus was well preserved. Thus, Onuf's nucleus appears to differ from other motoneurons with regard to susceptibility to poliovirus infection.

Distributional variation of P-450 immunoreactive hepatocytes in human liver disorders.

Implications of P-450 in human hepatic disorders were immunohistochemically examined. We first confirmed that an antibody against P-450-HM1, an isozyme of cytochrome P-450 which was purified from human livers at autopsy, detects only P-450 on immunoblots. In a study of 79 consecutive autopsied livers using the avidin-biotin-peroxidase complex method, the antibody reacted strongly with fetal hepatocytes, the reaction being more intense in the left lobe than in the right lobe. In normal livers, immunoreactivity was confined to centrilobular hepatocytes, decreasing in the periportal zone. Enhanced expression was occasionally found in scattered hepatocytes and in hepatocytes surrounding sublobular veins; this enhancement was related to longterm steroid therapy. No specific induction was observed in patients with toxic hepatitis. In patients with fibrosis, cirrhosis, or regenerative nodules, however, P-450-positive hepatocytes were observed in the periportal and middle zones as well as in the central zone. In contrast, hepatocellular carcinomas were devoid of P-450 immunoreactivity. These results suggest that P-450-HM1, which is abundant in the fetal liver, is reexpressed in regenerating hepatocytes but not in cancers.

Immunohistological studies of fibrous pseudotumor of the testicular tunica vaginalis.

A fibrous pseudotumor of the testis from a 46-year-old man was examined histologically and immunohistochemically. The tumor, situated in the tunica vaginalis, appeared as multiple round nodules up to 2 cm in diameter, and was composed largely of fibrous tissue with scanty inflammatory cell infiltration. Besides mesothelial cells covering part of the tumor, foci of piled-up cells were found among the fibrous tumor tissue. The cells in the foci were spindle and polygonal in shape, and contained oval nuclei. Immunohistochemically, these cells were positive for vimentin and keratin, but negative for CEA. Similar features have been found in pleural mesothelioma, supporting the concept that testicular fibrous pseudotumor could represent neoplastic growth of the mesothelium of the tunica vaginalis.

Responses of pineocytoma to radiation therapy and chemotherapy--report of two cases.

Pineal parenchymal tumors are so rare that their responses to radiation and/or chemotherapy are not well known. Two cases of pineocytoma, which responded well to radiation therapy and chemotherapy, are reported. A 45-year-old female received radiation therapy in a total dose of 5000 rads to the tumor. Three months after the completion of therapy, computed tomography (CT) showed complete disappearance of the tumor, and she remains well as of 1 year after treatment. The second patient, a 6-year-old girl, underwent four cycles of intravenous infusion of ACNU (25 mg) over 10 months. CT demonstrated complete disappearance of the tumor and, 18 months post-operatively, there was no evidence of tumor recurrence. The results in these two cases indicate that postoperative radiation therapy and/or chemotherapy should be strongly considered for patients with pineal parenchymal tumors.

Surgical correction and osteoplasty for forward dislocation of temporomandibular joint.

Surgical correction and osteoplasty were carried out in one case of prolonged and three cases of recurrent forward dislocation of the temporomandibular joint, two of the patients in the latter cases having mental disorders and one having a cerebral lesion. A hockey-stick-shaped preauricular incision was made, ending in the hair-bearing part of the temple. Then, the temporal fascia and muscle were divided anteriorly from the temporal bone. In two aged patients, a fragment of the zygomatic arch was pushed out and down, joining under the articular tubercle. In two younger patients, the cortical bone of the mandible was separated by sagittal split-ramus osteotomy for mandibular prognathism, and two graft blocks removed from the iliac bone were implanted between the root of the posterior zygomatic arch and the anterior cut-edge of the articular tubercle. In 3 of the 4 patients, the fragments or grafts were fixed by means of ligation using wire so as to make contact with the root of the posterior zygomatic arch. There was no recurrence of dislocation during a period of 2-5 years after surgery.

[Endoscopic needle bladder neck suspension for female stress incontinence].

Thirteen operations of endoscopic bladder neck suspension (Raz method) were performed for 12 patients with stress incontinence between October, 1986 and August, 1988. In all patients, except for one with small bladder capacity, the operations were successful. Disappearance of incontinence was maintained in 10 patients 5-27 months (mean 15.3 months) after the operation, and the success rate of this operation was 77% (10/13) at this follow up point. The operative technique was easy except for the adjustment of pulling up strength of the thread for suspension. We believe that the endoscopic needle bladder neck suspension will be performed more widely because this operation can be done safely in a short time without major complications and can produce very good results.

[Comparative study of in vitro diagnosis by the skinned fiber test and clinical diagnosis of malignant hyperthermia].

Skeletal muscles from twenty patients suspected of malignant hyperthermia during general anesthesia were examined with the skinned fiber test for abnormality in the Ca2+ release function of their sarcoplasmic reticulum, and the result was correlated with the classifications of the patients in accordance with the clinical criteria now used in Japan. Among them, fourteen patients were diagnosed as clinical malignant hyperthermia syndrome (fulminant syndrome: 4, abortive syndrome: 10). Three out of four fulminant cases showed potentiated Ca-induced Ca release mechanism of the sarcoplasmic reticulum in their skeletal muscles. The others including six patients not diagnosed as malignant hyperthermia were normal in the sarcoplasmic reticulum function. This suggests that the skinned fiber test has comparatively high sensitivity to fulminant malignant hyperthermia syndrome. The present paper discusses, based on the present results, the problems resulting from the difference in diagnostic concept between in vitro criteria by the skinned fiber test to detect a specific pathogenetical factor in the skeletal muscle of a single abnormality and clinical criteria to pick up a series of relatively non-specific symptoms and signs of a clinical syndrome.

[Perioperative management of cesarean section in a patient pregnant with twins complicated by hyperthyroidism].

A 30-year-old twin pregnant woman complicated with hyperthyroidism was scheduled to receive anesthesia for cesarean section. During the early months of pregnancy, her thyroid status was stimulated, being positive for TSH-binding inhibitor immunoglobulins (TBII). Because maternal positiveness of TBII correlates with neonatal hyperthyroidism, she was treated with antithyroid drugs (methimazole). Her thyroid status was suppressed gradually and her TBII level became negative. On the 38th week of pregnancy, she underwent cesarean section by lumbar spinal anesthesia and twin girls were delivered with Apgar scores of 9. The thyroid statuses of the twins were measured, showing that their thyroid functions to be almost equal, although they were dizygotic twins. T3 and free-T3 were below the normal limits for adults, but they did not require any therapy. Maternal level of TBII is considered to be a useful index for management of patients with hyperthyroidism during the perianesthetic period for cesarean section. In this type of patients the thyroid status of both the mother and fetus should be carefully followed.

Physiological role of growth hormone (GH)-releasing factor and somatostatin in the dynamics of GH secretion in adult male rat.

To investigate the physiological role of GRF and somatostatin (SRIF) in GH secretion in adult male rats, we prepared in vitro models using the perifusion system of cultured rat anterior pituitary cells exposed to various combinations of human GRF-(1-44)NH2 (hGRF) and SRIF. We studied the following three models on GRF secretion: 1) pulsatile GRF secreted at 1-h intervals, 2) pulsatile GRF secreted at 3-h intervals, and 3) GRF continuously secreted. When 5-min pulses of 20 nM GRF were delivered at 1-h intervals, the responses to GRF gradually declined. The addition of continuous 20 nM SRIF with short pauses prevented this attenuated response and produced high peaks of GH at 3-h intervals. When 5-min pulses of 20 nM GRF were delivered at 3-h intervals, three high peaks of GH were observed regardless of the addition of SRIF. Pretreatment with GRF pulses enhanced the peaks of GH during SRIF pauses. When 20 nM GRF was continuously delivered, a rapid attenuated response to GRF was observed. Although the addition of continuous 20 nM SRIF with short pauses produced three small peaks of GH, these results were caused by the post-SRIF rebound release. These observations suggest that preexposure to prolonged SRIF can prevent an attenuated response to repeated GRF pulses, and that pretreatment with GRF pulses enhances post-SRIF rebound release.

Mutagenicity modulating effect of quercetin on aromatic amines and acetamides.

The effect of quercetin on the mutagenicity of 32 kinds of aromatic amines and their acetamides were investigated using Salmonella typhimurium TA98 with a mammalian metabolic activation system (S9 mix). Quercetin enhanced the mutagenicity of the tricyclic aromatic amines (aminofluorene, aminoanthracene and aminophenanthrene) and their acetamides by 1.2-5.9-fold. Whereas, quercetin depressed the mutagenicity of aniline derivatives, biphenyl derivatives, and bi- and tetra-cyclic amino derivatives. The modulation of mutagenicity of Trp-P-1, Trp-P-2, Glu-P-1 and Glu-P-2 (heterocyclic amines) by quercetin were liable to be affected by the content of S9 in the S9 mix. It seems that quercetin does not have the same effect as norharman, because quercetin did not enhance the mutagenicity of aniline. It is suggested that the modulation of the mutagenicity of aromatic amines and acetamides is caused by the modulation of the balance between the mutagenic activation and inactivation in the metabolism of these amines and acetamides in the presence of quercetin. In this modulation, quercetin may participate through its effects on the promotion of N-hydroxylation and the inhibition of arylhydroxylation and transacylation. The presence of tricyclic aromatic rings of amines and acetamides is a structural requirement for the mutagenicity enhancement by quercetin.

Clinical course and evaluation of shape and function after reconstruction using various pedicle flaps for oral and maxillofacial defects.

In the past 7 years, reconstruction using various pedicle flaps for oral and maxillofacial defects, was carried out for 40 patients. Pectoralis major myocutaneous, deltopectoral and scalp-forehead flaps for large defects, and sternocleidomastoid, cervical island skin and palatal flaps for moderate defects were indicated. As for results, most of the flaps used at the reconstructed site for the defects of soft tissue repaired the shape and patients were satisfied. However, the recovery of functions concerning speech, swallowing or mastication was not always successful with the larger defects, especially after the reconstruction for defects of tongue, floor of the mouth, cheek and mandible for which mandibular reconstruction was performed. It was pointed out that the main causes for less tongue mobility were due to scar contracture after reconstruction, and difficulty of mandibular reconstruction due to postoperative infection were associated factors. Accumulated survival rate of patients for 5 years was 62.6%, in which 38 advanced malignant tumors were reconstructed following surgery. In the clinical course after reconstruction, patients with primary tumors experienced much better results than those with secondary tumors.
