[Pharmacokinetics of cefoperazone concentration in human lung tissue].

In 11 patients, the concentration of cefoperazone (CPZ) in the lung tissue were studied after a 2 g intravenous bolus injection. Samples of lung tissue and blood were taken during surgery at 15, 30, 60, 120, 180, 240, and 300 minutes after administration of CPZ. All samples were assayed by the paper disc method using Micrococcus luteus ATCC 9341 as the indicator strain. All assay results of lung tissue were corrected for the amount of blood contained in the tissue samples by the hemoglobin content in the homogenate of lung tissue. The CPZ levels in the lung tissue were 116.2 micrograms/g at 15 min., 94.1 micrograms/g at 30 min., 74.6 micrograms/g at 60 min., 67.9 micrograms/g at 120 min., 54.6 micrograms/g at 180 min., 29.2 micrograms/g at 240 min. and 22.7 micrograms/g at 300 min. after administration, respectively. These values were higher than MIC90 values of CPZ for Pseudomonas aeruginosa, Streptococcus pneumoniae, Haemophilus influenzae and Enterobacteriaceae. The level of CPZ in the lung tissue was higher than 50% of the level in the serum until 180 minutes after administration of CPZ.

[A case of malignant lymphoma of the remnant stomach].

A 46-year-old woman was transferred to our facility with the complaint of epigastric discomfort of more than four months duration. She had undergone partial gastrectomy with Billroth I anastomosis 19 years prior to the admission. An upper GI series revealed multiple, variously sized elevated lesions scattered over the remnant stomach. Fiberscopic examination showed the gastric mucosa replaced by several elevated lesions and giant fold embellished by erosion, injection, and necrotic debris. Pathological examination revealed malignant lymphoma of the large cell type whose main growth was in the mucosa and submucosa. This case was significant in that malignant lymphoma occurred in the remnant stomach and that the tumor converted M-protein in the serum, which was confirmed as IgM-k in the cytoplasma immunohistochemically.

Aggressive surgical approach for locally invasive papillary carcinoma of the thyroid in patients over forty-five years of age.

Aggressive radical resection was performed on 18 of 21 elderly patients with locally invasive-papillary carcinoma of the thyroid. Diagnostic studies, including computed tomographic and endoscopic examinations, were useful in allowing accurate determination of tumor extent. Radical resection usually consisted of total or subtotal thyroidectomy and neck dissection combined with excision of the affected organs, which in one case included a laryngotracheoesophagectomy (reconstruction of the digestive tract with a forearm free flap), a sleeve, or a window resection of the trachea in nine cases (immediate end-to-end anastomosis in all but one), a partial esophagectomy in two cases, and resection of the outer layer of the trachea or esophagus in six cases. After these operations, one patient died of rupture of the carotid artery, one died of cancer in 4 years, and one other died of unrelated disease. The other 15 patients are alive and well 1 to 5 years postoperatively, although additional exploration for nodal metastases was required in three patients. Aggressive resection therefore appears to be justified for control of locally advanced, differentiated carcinoma of the thyroid.

Effect of diarachidonin on prostaglandin E2 synthesis in rabbit kidney medulla slices.

The effect of diarachidonin on the synthesis of prostaglandin E2 in rabbit kidney medulla slices was examined. The addition of diarachidonin stimulated prostaglandin E2 production in a dose-dependent manner. At three concentrations (10, 50 and 100 microM), increases in prostaglandin E2 formation induced by exogenous diarachidonin were 2-fold greater than those induced by exogenous arachidonic acid. Diacylglycerol or phosphatidic acid from egg lecithin had little or no effect on prostaglandin E2 production. Moreover, EGTA failed to inhibit diarachidonin-stimulated prostaglandin E2 formation, indicating that the stimulatory effect of diarachidonin is not mediated through the activation of endogenous phospholipase A2 (including phosphatidic acid-specific phospholipase A2). These results are discussed in the light of our former hypothesis that arachidonic acid release from kidney medulla phospholipids might occur through the sequential action of a phospholipase C coupled to diacylglycerol and monoacylglycerol lipases [Fujimoto, Akamatsu, Hattori & Fujita (1984) Biochem. J. 218, 69-74].

A case of gastric carcinoma associated with excessive granulocytosis. Production of a colony-stimulating factor by the tumor.

A patient with gastric carcinoma exhibited an excessive granulocytosis (58,000/microliter) preoperatively, in the absence of overt infection. After resection of the primary tumor, the peripheral leukocyte count decreased promptly to the normal value. In a search for a colony-stimulating factor (CSF), the tumor was transplanted into nude mice. A marked neutrophilia was observed in the tumor-bearing mice, suggesting the production of CSF by the tumor. Media conditioned by the primary culture of the tumor cells revealed the presence of CSF activity as well. CSF-producing carcinomas have been detected in various organs; nevertheless, no cases of gastric carcinoma have hitherto been described. It is of particular interest that in this patient hypercalcemia was not observed, although it often accompanied CSF-producing tumors reported previously. Therefore, it is suggested that this tumor secreted pure CSF and that the CSF produced by the tumor did not necessarily induce hypercalcemia.

Parathyroid carcinoma of the oxyphil cell type. A report of two cases, light and electron microscopic study.

Parathyroid adenomas are subdivided into chief cell and oxyphil cell variants. However, the parathyroid carcinomas described thus far have been only of the chief cell type. Two cases of oxyphil cell carcinoma of the parathyroid gland are reported, with light and electron microscopic study. The patients presented apparent clinical hyperparathyroidism with x-ray finding of generalized fibrous osteitis and palpable parathyroid tumors. Initially, a pathologic diagnosis of parathyroid adenoma was made in both of them. However, in due course, pulmonary metastases developed in one patient and a local recurrence occurred in the other, 5 and 8 years after the primary operation, respectively. Review of the microscopic slides showed that both primary tumors met the criteria of parathyroid carcinoma. A matter of interest in both cases is that the neoplasms were composed principally of oxyphil cells. Electron microscopic study confirmed the existence of typical oxyphil cells packed with numerous mitochondria.

Benign mediastinal teratoma associated with Klinefelter's syndrome.

We treated a patient with a benign mediastinal teratoma associated with Klinefelter's syndrome which was first thought to be a malignant germ cell tumor because of an anterior mass shadow, gynecomastia, atrophic testis and a high level of HCG. Chromosomal analysis showed a total of 47 XXY. Median sternotomy revealed a tumor originating from the right lobe of thymus. The postoperative course and 2 year follow-up have been uneventful.

Prediction of lung cancer spread and functional resectability by 133Xe radiospirometry.

Sixty-six patients with primary lung cancer who underwent thoracotomy were studied to determine the correlations among 133Xe radiospirometry, surgical procedures and histological extension of the lung cancer. Disturbance in the regional perfusion (Q per cent) was more prominent than disturbance of the regional ventilation (V per cent), as the pathological stage and t factor proceeded, while V per cent and Q per cent were disturbed almost equally in relation to the pathological n factor. Lobectomy was impossible in patients with a Q per cent of less than 33 per cent of the total, but low perfusion did not necessarily contraindicate surgery. The predicted postoperative FEV1.0 was calculated according to the equation of (1-b/a x (V per cent or Q per cent)) x (preoperative FEV1.0), where a and b were the number of subsegments in the lung lobes on the involved side and the resected lobe. The predicted and actually measured postoperative FEV1.0 showed significant correlations (less than 0.001) in both equations. We conclude that Q per cent reflects a complex pattern of lung cancer spread more sensitively than does V per cent, and the significance of V per cent and Q per cent in terms of prediction of postoperative EFV1.0 seems to be equivocal.

Identification of estradiol in the ovaries of the silkworm, Bombyx mori.

Estradiol was extracted and partially purified from the ovaries of the silkworm, Bombyx mori. Identification of estradiol was done by use of radioimmunoassay (RIA) and by gas chromatography-mass spectrometry (GC-MS) after derivatization into the ethyldimethylsilyl derivative. Concentration of estradiol in the ovaries was estimated to be 176 pg/g (RIA) and 63 pg/g (GC-MS).
