Acute suppurative thyroiditis in appearance of unusual neck mass.

We report on three adult cases of acute suppurative thyroiditis due to infection through a piriform sinus fistula. In these cases, the lesion did not exhibit the clinical symptoms characteristic of acute suppuration, but appeared as an unusual neck mass. Repeated barium meal roentgenograms, however, demonstrated the internal fistula in two of the three cases. Acute suppurative thyroiditis may not be so rare even in adults. When acute suppurative thyroiditis occurs in adults with scanty symptoms of acute suppuration, the piriform sinus fistula could easily go undetected. Recognition of acute suppurative thyroiditis in adults per se seems to be important for correct diagnosis.

Eleven cases of nonfunctioning parathyroid cyst--significance of needle aspiration in diagnosis and management.

Eleven cases of nonfunctioning parathyroid cyst are reported. Ultrasonography revealed the cystic nature of the asymptomatic lump in the neck. Clear watery fluid obtained by needle aspiration and the increased parathyroid hormone concentration in the fluid established the diagnosis. Five patients, in four of whom the cysts reappeared after one (3 cases) or seven (1 case) needle aspirations, underwent surgical resection of the cysts. Six other patients were followed conservatively after the aspiration maneuver. In four patients with enough follow-up time for evaluation, the cyst fluid has not reaccumulated 5, 2 and 2 years after only one aspiration, and 1 year after two aspirations. In one patient, the cyst reappeared 9 months after the initial aspiration. Simple percutaneous aspiration may be curative in some cases of nonfunctioning parathyroid cysts.

Transient thyrotoxicosis associated with infarction of a large thyroid adenoma.

We report a 46-year-old male patient with a transient thyrotoxicosis that seems to have been caused by hemorrhagic infarction of a cold thyroid nodule. The serum level of triiodothyronine was markedly but transiently elevated, while the serum thyroxine level remained within the normal range. The resected nodule, measuring 8 x 7 x 5 cm, showed extensive degeneration and necrosis with viable follicles left only at the margin of the nodule. A transient thyrotoxicosis due to acute hemorrhagic infarction of autonomously functioning thyroid nodules has already been reported. This case showed that the phenomenon could occur even in cold thyroid nodules. Its implications were discussed in relation to the high incidence of impaired TSH response to TRH in patients with nodular goiter.

Papillary carcinoma of the thyroid arising from dyshormonogenetic goiter.

A case of thyroid papillary carcinoma associated with dyshormonogenetic goiter is reported. The patient, a 35-year-old male, has been on thyroid hormone therapy since the age of three because of familial dyshormonogenetic goiter. He developed a distinct tumor in the right lobe, which was suggestive of carcinoma upon physical as well as ultrasonographic examination. Total thyroidectomy was performed, since a frozen section disclosed a focus of papillary carcinoma and the possibility of future development of further malignancy in any remaining thyroid tissue was considered. The patient is currently well with complete thyroid hormone supplementation one and a half years after the operation.

Radiolocalization of bovine lymphosarcoma cells in athymic mice, using a monoclonal antibody against tumor-associated antigens.

Mouse monoclonal antibody c 143 was purified and F(ab')2 fragments were generated by pepsin digestion and then radiolabeled with 125I. The 125I-labeled c 143 F(ab')2 fragments were injected into athymic mice bearing bovine lymphoid tumor cells. The fragments became preferentially localized in tumor tissues, but not in normal tissues, as determined by differential counting of tissue radioactivity. The fragments became localized specifically in those tumors that were reactive with c 143 in vitro, but did not become localized in unrelated tumors. Localization of labeled F(ab')2 fragments of a monoclonal antibody of the same isotype directed against Taka virus (a variant of Newcastle disease virus) was not observed in athymic mice bearing bovine lymphoid tumor cells. Tumors were detectable by radioimmunoscintigraphy, using radiolabeled c 143 F(ab')2 fragments, without background subtraction, and by use of silver-grain scattering in light microscopic autoradiography.

Cardiac involvement in a patient with limb-girdle muscular dystrophy.

A case of limb-girdle muscular dystrophy with cardiomegaly, electrocardiographic abnormalities, and paroxysmal ventricular tachycardia was described. Some of the interesting electrocardiographic features were discussed, and the importance of serial electrocardiograms for the detection of cardiac involvement is emphasized, since acute disturbances in cardiac rhythm may indicate that the patient is in immediate danger of sudden death.

[Gastrointestinal polyposis and hereditary large bowel cancer].

Hereditary large bowel cancer is roughly divided into the hereditary gastrointestinal polyposis (HGIP) that is including adenomatosis coli (AC), Peutz-Jeghers syndrome and juvenile polyposis, and non-polyposis familial large bowel cancer (NPF) which is including cancer family syndrome and its related conditions. In the HGIP, AC is most frequent and of highest risk of large bowel cancer, and epidemiological method of approach to patients has been well studied. NPF, however, has been insufficiently studied because of lacking of distinctive marker as seen in the polyposis in spite of that it may be actually present much more frequently than AC. In order to extract NPF from the general large bowel cancer group, the marker or criteria for it has been studied. The factors besides family history presently considered to be useful for detecting NPF are multiple cancers, proximal cancer, association of multiple adenoma and young (50 years or younger) onset of the disease.

[The effect of gastrectomy on serum 5-FU concentrations of patients administered UFT per os].

Ten post-total gastrectomy patients and five postsubtotal patients were administered with UFT 3 capsules per os. Then the concentrations of the serums FT-207, 5-FU and Uracil, and that of 5-FU in the lymph-nodes were measured chronologically. The concentration of serum FT-207 reached to its peak one hour after administration of the regimen for both the total and subtotal gastrectomy patients and gradually lowered. The 5-FU serum concentration, on the other hand, showed a rapid drop after reaching a peak one hour after administration of the UFT for both the post-total and subtotal gastrectomy patients. The concentration of 5-FU in the lymph nodes remained five times as high as the serum concentration after four hours for both the post-total and subtotal gastrectomy patients.

[A study of lymph node metastasis in gastric cancer--a new method for fixing resected specimens with methylene blue formalin].

Lymph node metastasis in 171 cases after a radical resection of the gastric cancer have been studied by means of examining specimens of these lymph nodes fixed with methylene blue formalin. The number of the resected lymph nodes in a single case was 50.3 on the average. By the Appleby method this number was 82.8. The rate of metastasis (the number of cases with lymph node metastasis measured against the number of patients) was 43.9%. The frequency of metastasis (the number of metastasized nodes versus the number of dissected nodes) was 11.3%. When classified by location no difference was noted, but in the group which showed an infiltration of a macroscopic type or an undifferentiated adenocarcinoma, the rate of metastasis and the frequency of metastasis was found to be higher than in the group with a non-infiltration type or a well-differentiated adenocarcinoma. Although the metastatic rate of perigastric nodes (No. 1-6 was higher than in the other lymph node group (No. 7-11), no difference was recognized between both groups in the frequency of metastasis.

Is phospholipid a required cofactor for the activity of mammalian signal peptidase?

To determine whether phospholipid is required for the activity of mammalian signal peptidase, the enzyme was partially purified from porcine pancreas and then extensively freed of phospholipid by SP-Sephadex C-50 chromatography. The delipidated enzyme showed signal peptidase activity, with a low concentration of detergent. Phospholipid was found to release the enzyme from the inhibition due to excess detergent.
