[Two cases of percutaneous transluminal angioplasty (PTA) for the treatment of subclavian steal syndrome].

Recently percutaneous transluminal angioplasty (PTA) has been used to treat atherosclerotic lesion of the brachiocephalic arteries. We treated two patients with subclavian steal syndrome successfully by PTA. The first patient was a 52-year-old man complaining of vertigo and ischemic symptoms of the left arm. Blood pressure was 110/90 mmHg in the left arm and 140/92 mmHg in the right. On the angiogram, about 80% stenosis was found in the proximal portion of the left subclavian artery, and retrograde filling of the left vertebral artery was observed. At first, a carotid-subclavian bypass surgery was performed, but unfortunately failed because of obstruction of the artificial arterial graft. Then we performed PTA using Grüntzig balloon dilatation catheter. The dilatation was carried out successfully. Immediately after PTA, to-and-fro and antegrade flow of the vertebral artery was observed, and one month later it became normal flow. Neurological symptoms gradually disappeared within two months. The second patient was a 57-year-old man suffering from vertigo on effort and TIA. Blood pressure was 130/78 mmHg in the left arm, and 152/82 mmHg in the right. Angiogram showed more than 95% stenosis of the left subclavian artery, and retrograde filling of the left vertebral artery. We also observed bilateral severe stenosis of the MCA. Carotid-subclavian bypass was dangerous because of the low perfusion of the ipsilateral MCA area. PTA was performed as the first choice, and the dilatation of the stenosis was sufficient. In two months natural antegrade filling of the left vertebral artery was obtained. We followed these patients for 12 months and no restenosis was observed neurologically and radiologically. We performed PTA with the Grüntzig balloon catheter using the Seldinger's method through the right femoral artery. Diameter of the balloon was 6 mm and its length was 4 cm. After setting the balloon to the stenotic lesion, we inflated the balloon with 5atm (75 psi) pressure for 30 minutes. Monitoring the blood pressure and arterial pressure wave at the tip of the catheter, we repeated inflation of the balloon 4 times. We used continuous venous infusion of low molecular dextran and heparin during PTA procedures. No complication occurred and neurological symptoms disappeared gradually. PTA is a safe and effective method, so it should be the first choice in the treatment of subclavian steal syndrome caused by severe stenosis of the subclavian artery.

Dietary sardine oil increases erythrocyte membrane fluidity in diabetic patients.

The effects of dietary sardine oil rich in eicosapentaenoic acid, C20:5 (EPA), on erythrocyte membrane fluidity and membrane and plasma lipids were investigated in diabetic and control subjects. Before consumption of this oil, the levels of erythrocyte membrane fluidity were lower in the diabetic subjects, as noted in our previous work (Diabetes 1983; 32:585-91). Decreased membrane polyunsaturated fatty acid contents were evident. Daily consumption of 2700 mg of sardine oil for 8 wk increased erythrocyte membrane fluidity, as determined by electron spin resonance using the 12- or 16-stearic acid label. This increase was seen after 4 wk, and the level remained elevated for 8 wk. Membrane EPA of phospholipid acyl-chains significantly increased after 4 wk and was even more apparent after 8 wk. Membrane-free cholesterol to phospholipid molar ratios significantly decreased after 8 wk. Both the diabetic and normal subjects responded to the sardine oil in the same way. After feeding with sardine oil, there no longer were differences in erythrocyte membrane fluidity between the normal and diabetic subjects. We propose that improvement in membrane fluidity may contribute to the amelioration of altered cell membrane functions in diabetic patients.

Phosphorylation of AMP by inorganic phosphorylating agents.

AMP was phosphorylated by inorganic phosphorylating agents: cyclo-triphosphate and diphosphonate, in aqueous solution (70-80 degrees C, pH 6-12). The molecular structures of phosphorylated products were established by use of phosphorus-31 NMR and high-performance liquid chromatography (HPLC). The OH groups on AMP were phosphorylated by both phosphorylating agents to form 2'- or 3'-phosphate but an OH group on dAMP was not phosphorylated. Phosphorylation of OH group proceeds in two steps: formation of hydrogen bond between OH group and phosphorylating agent; subsequent nucleophilic attack of OH group on a phosphorus atom. Phosphate group on AMP was phosphorylated by diphosphonate but not by cyclo-triphosphate. The difference in the reactivities is explained in terms of charge repulsion between AMP and agents.

[A case of chronic subdural hematoma in a hemodialyzed patient].

In proportion to a rapid increase of dialysis patient, death caused by neurological complications is increasing annually among long-term hemodialyzed patients. A case of chronic subdural hematoma during long-term hemodialysis is presented. A 35-year-old male had undergone hemodialysis three times weekly for four years. He had marked changes in body-weight and blood pressure between hemodialysis. In March 1983, he had a headache, vomiting, and left hemiparesis. The CT scan showed a right subdural hematoma. He was admitted to our hospital 10 days later because of progressive hemiparesis and speech disturbance. The neurological examination showed left hemiparesis with sensory deficit and dysarthria. The CT scan showed an increase in the size of the subdural hematoma. Bleeding time was over 10 minutes. A right-sided burr hole was made and altered blood was removed and irrigated. After operation, headache and weakness rapidly subsided, but the next morning, attacks of convulsion occurred. The CT scan showed the rebleeding in the subdural space. After correcting the level of serum potassium by hemodialysis, a right parietal craniotomy was performed. Hematoma of about 100 g was removed and the capsule of the hematoma showed organized tissue histologically. Postoperatively, although attacks of convulsion occurred temporarily, he gradually improved. The levels of serum potassium and BUN were controlled by several treatments of hemodialysis. He was discharged with only mild hemiparesis. Subdural hematoma caused by hemodialysis is a very important complication. Chronic subdural hematoma is sometimes very difficult to differentiate from dysequilibrium syndrome or dialysis dementia. The CT scan is a very valuable examination to rule out subdural hematoma.

[Histological types of depressed early gastric cancer and roentgenographic findings].

Characteristic findings of depressed early gastric cancer were summarized by neoplastic histological type according to the roentgenographic findings as follows: Undifferentiated cancer: The depressed surface shows many irregular changes and granular shadows of various sizes. The depressed edge is clear and shows a coarse or small wavelike pattern. The mucosal folds of the stomach are often interrupted by the depressed edge or show sharp thinning. Differentiated cancer: The depressed surface shows very few irregular changes, and is rather squamous compared with undifferentiated cancer. The depressed edge is fine and shows a thornlike pattern. The margin shows spindle or low IIa-like protuberance findings. The mucosal folds show slight thickening at the margin and gradual thinning at the depressed site. Thus, the folds are rarely interrupted at the margin.

[Clinical experience of methylcobalamin (CH3-B12)/clomiphene citrate combined treatment in male infertility].

Infertile men (excluding azoospermia) were divided into three groups to test methylcobalamin (CH3-B12) + clomiphene citrate (Clomid) treatment. Group M: CH3-B12 (1,500 micrograms/day, daily for 8 to 60 weeks), Group C: Clomid (25 mg/day for 25 days followed by a 5-day rest period, for 8 to 48 weeks), and Group MC: CH3-B12 + Clomid (CH3-B12: 1,500 micrograms/day, daily and Clomid: 25 mg/day for 25 days followed by a 5-day rest period, for 12 to 24 weeks semen analysis was conducted before and after these administrations. Judging by our criteria, 60.0% (24/40) of the cases in Group M, 71.4%(25/35) in Group C, and 66.7% (8/12) in Group MC were improved. However, in patients whose sperm concentrations were over 10 X 10(6)/ml, 61.9% (13/21) of the cases in Group M, 69.2% (9/13) in Group C, and 80.0% (4/5) in Group MC were improved. Therefore, CH3-B12 + Clomid treatment was successful for patients whose sperm concentrations was over 10 X 10(6)/ml.

[Employment conditions and a system of occupational health administration for disabled workers].

A survey using a questionaire was conducted to elucidate the employment conditions and a system of occupational health administration status of the disabled workers in private enterprises. The average employment rate of disabled workers was found to be higher in small-scale enterprises, than in large-scale enterprises. Also, the low employment rate was found among cases of spinal cord injuries and hemipregia. The reasons for workers becoming disabled were "private accident and illness" (59.2%) "congenital" (30.6%) and "labor accident" (10.2%). The main physical factor influencing work efficiency of disabled workers was "degree of disability" while "work morale" was found to be the most influential psychological factor. Also it was found that about 50% of private enterprises will accept the reinstatement and employment of spinal cord injury or hemipregia workers. But it will be necessary that the disabled workers are able to do the requested job. However, it is necessary for employers of disabled workers to make various efforts in overcoming situations such as, changes in organization, improvement of occupational health administration staffs and improvements in the working environment and equipment.

Follow-up study on pulmonary function of dust workers: an epidemiological evaluation of discontinued workers of dust operations.

An epidemiologic study on 176 dust workers from various facilities was initiated in 1978. Their mean age was 48.3 years and mean duration of employment was 21.6 years. No workers complained of chronic cough and phlegm. Their chest X-rays were found to belong to Category I of the diagnostic criteria for pneumoconiosis in Japan. Pulmonary function test was performed on these workers, however, 54 workers discontinued taking part in the study by the end of 1979, and 49 workers had discontinued by 1981. A total of 73 workers had a five-year follow-up survey of pulmonary function. Forced expiratory volume in one second divided by forced vital capacity (FEV1.0/FVC) and forced expiratory flow rate at 25% of FVC divided by height (V25/H) were mainly compared by smoking habit, work years, type of job, size of company, and the time when they discontinued their jobs. Predicted percentage values of lung function were used for the adjustment of the influence of workers' ages. Decrement of FEV1.0/FVC and V25/H in the five-year follow-up group were larger than the screening level. The mean values of those functional parameters in the early discontinued group were lower than late discontinued and active working groups. The active working group was considered to be a physically healthy workers' population.

Liposomal membranes. XX. Autoxidation of unsaturated fatty acids in liposomal membranes.

Autoxidation of egg phosphatidylcholines and polyunsaturated fatty acids, arachidonic and linoleic acids, has been investigated in homogeneous and liposomal membrane systems. In order to monitor the very initial stage of the radical chain mechanism of the autoxidation, an improved method using 1,1-diphenyl-2-picrylhydrazyl radical was newly implemented and used with the regular thiobarbituric acid test to determine the peroxides of polyunsaturated fatty acids. Autoxidation of polyunsaturated fatty acids or lipids was significantly enhanced in liposomal bilayers compared to that in a bulk homogeneous solution. In liposomal bilayers, the reaction could be controlled by membrane fluidity, which was confirmed by the fluorescence polarization technique using 1,6-diphenylhexatriene and dansylhexadecylamine. Even vitamin E esters such as the acetate and the pivalate could depress effectively the autoxidation of egg phosphatidylcholines in bilayer systems, which supported Lucy's proposal (Diplock, A.T. and Lucy, J.A. (1973) FEBS Lett. 29, 205-210) about the importance of the side-chain of vitamin E as an antioxidant.

Novel nonnarcotic analgesics with an improved therapeutic ratio. Structure-activity relationships of 8-(methylthio)- and 8-(acylthio)-1,2,3,4,5,6-hexahydro-2,6-methano-3-benzazocines.

Conversion of the 8-phenolic 1,2,3,4,5,6-hexahydro-2,6-methano-3-benzazocines to the corresponding 8-thiophenolic analogues was achieved by three different routes. Diazotization of 8-amino-2,6-methano-3-benzazocine (2) followed by the reaction with CH3SNa afforded 8-(methylthio)-1,2,3,4,5,6-hexahydro-2,6-methano-3-benzazocine (3). Another route using Grewe cyclization was also examined for the synthesis of 3. As the most effective route, Newman-Kwart rearrangement of benzazocines was selected and closely investigated. 8-(N,N-Dimethylthiocarbamoyl)oxy derivatives (6a-e) rearranged to 8-(N,N-dimethylcarbamoyl)thio derivatives (7a-e) in good yields. Reductive cleavage of 7a-e and subsequent methylation or acylations gave the title compounds (3, 8-24). Although analgesic activities of sulfur-containing benzazocines decreased compared to the corresponding hydroxy compounds, the N-methyl derivative (S-metazocine, 8) showed potent analgesic activity.
