[Arterial switch operation for simple transposition of the great arteries in infancy].

Five infants (22-42 days of life) underwent arterial switch operation for simple transposition of the great arteries under deep hypothermic cardio-pulmonary bypass. Three babies required prostaglandin E1 infusion to keep ductus arteriosus opened widely before surgery. Balloon atrioseptostomy was necessary in 0-10 days of life in all babies because of poor condition. Left to right ventricular peak pressure ratio ranged from 0.75 to 0.86, preoperatively. Four of the five survived the operation, and one died of coronary insufficiency because of kinking of the implanted coronary artery. Hypothermic circulatory arrest was used in three (38-41 minutes). Aortic cross clamp time was 70-100 minutes, and cold crystalloid cardioplegia was given only one time just after aortic clamp in 3 babies. Single dose of cardioplegia protected left ventricular muscle well in babies with transposition of the great arteries as same as multiple dose method used in those with ventricular septal defect.

Computer-assisted retention prediction system for oligonucleotides in gradient anion-exchange chromatography.

A technique is presented for the retention prediction of single-stranded homo-oligonucleotides under gradient elution conditions in anion-exchange chromatography. The prediction system is based on the theory proposed by Jandera and Churacek. In the present system, the theory was modified by the extrapolation method, utilizing the linear relationship between the log of the capacity factor and the number of nucleotides. The modified theory allowed the prediction of retention times for high-molecular-weight oligonucleotides, which could not be calculated from the original theory. Oligoadenylate was used as a standard oligonucleotide to demonstrate the accuracy of the prediction system. By use of this system, the retention times of oligoadenylates up to 25-35 bases in chain length were predicted within 8% errors under both binary-linear and binary-convex gradient shapes.

Comparison of arachidonic acid metabolism between myofibroblasts and fibroblasts isolated from rat palatal mucoperiosteum.

Myofibroblasts were cultured successfully from experimental wound tissue in rat palatal mucoperiosteum. Arachidonic acid metabolizing activity in cultured myofibroblasts was compared with that in fibroblasts cultured from normal mucoperiosteum. Prostaglandins biosynthesized from [14C]arachidonic acid in cell-free homogenates of both myofibroblasts and fibroblasts were prostaglandins D2, E2 and F2 alpha, and the activity producing each prostaglandin was not significantly different between the myofibroblasts and the fibroblasts, whereas smooth muscle cells, which are histologically similar to myofibroblasts, produced mainly 6-ketoprostaglandin F1 alpha, and relatively small amounts of prostaglandin E2. The release of arachidonic acid from cells prelabeled with [14C]arachidonic acid was compared among three types of cell. The calcium ionophore A23187 strongly enhanced arachidonic acid release in all three cell types. Bradykinin, 5-hydroxytryptamine and prostaglandin F2 alpha affected the stimulation of arachidonic acid release in the fibroblasts but were less or not effective in the myofibroblasts and smooth muscle cells. In addition, prostaglandin E2 biosynthesized in response to several stimuli was measured by radioimmunoassay. The content of prostaglandin E2 correlated closely with arachidonic acid release. In this study, we showed homogeneity between the myofibroblasts and fibroblasts in prostaglandin synthesizing activity and similarity in response to various stimuli between the myofibroblasts and smooth muscle cells, from the standpoint of arachidonic acid metabolism.

The effect of atrial natriuretic peptide on acid-base balance in rats with chronic renal failure.

We explored the effects of 12-hour infusion of atrial natriuretic peptide (alpha-rANP:rat, 1-28) on arterial acid-base balance, using 5/6 nephrectomized rats with chronic renal failure. Before the infusion, nephrectomized rats had a higher mean arterial blood pressure, greater urine volume, and lower creatinine clearance than the normal controls, but they did not show a significant difference in arterial hydrogen ion concentration (pH), plasma bicarbonate concentration (HCO3-), partial pressure of carbon dioxide (PCO2), plasma base excess (BE), or plasma ANP concentration. alpha-rANP infusion produced a continuous blood pressure reduction in both nephrectomized and control rats. Urine volume and urinary sodium and potassium excretion tended to increase at 2-hour infusion, but not at 12-hour infusion. In the controls alpha-rANP significantly increased pH from 7.47 to 7.50, and decreased PCO2 by 14%. In contrast, in nephrectomized rats alpha-rANP significantly decreased pH from 7.48 to 7.44, HCO3- by 13%, and BE from -0.07 to -3.22 meq/l. Rats with chronic renal failure had greater reduction in HCO3- than the controls (p less than 0.05). There was no difference in plasma ANP level between the two groups. Thus, it is indicated that the long-term infusion of alpha-rANP reduces pH in rats with chronic renal failure, thereby adversely affecting the acid-base balance.

[Effect of HCG administration in the gossypol-treated male rats].

The effect of gossypol acetic acid (GAA) on the pituitary-Leydig cell axis was studied. Adult male Wistar rats were administered orally 5 mg/kg/day GAA for three weeks, 10 mg/kg/day GAA for three weeks or 20 mg/kg/day GAA for two weeks. Each group was treated for four days with saline or 500 IU human chorionic gonadotropin intraperitoneally injected. None of the rats died. The mean body weight in each group was increased, but the mean body weight gain in the 20 mg/kg group was significantly lower than that in the controls (p less than 0.01). There was no significant change in the weight of testes or epididymis in any group. The serum level of lactate dehydrogenase, follicle stimulating hormone and testosterone was significantly lower in the animals treated with 20 mg/kg/day GAA for two weeks than in the controls (p less than 0.05). No change was seen in other groups. The serum testosterone level after HCG stimulation (HCG 500 IU intraperitoneal injection for 4 days) was significantly lower in animals treated with 20 mg/kg/day GAA for two weeks (p less than 0.01). GAA is suggested to affect the pituitary-Leydig cell axis and directly inhibit Leydig cell function.

[Pharmacokinetic and clinical studies of ceftizoxime in obstetrical and gynecological field (2)].

Pharmacokinetic and clinical evaluations of ceftizoxime (CZX) in 3 obstetrical and gynecological clinics have substantiated the therapeutic usefulness of this drug in the relevant specialty. The results are summarized below. 1. The peak serum CZX concentration after drip infusion of 2 g given over 60 minutes was 115.3 micrograms/ml. The peak CZX concentration in the pelvic dead space exudate was 34.10 micrograms/ml which was attained 2.02 hours after beginning infusion. Half-lives of CZX in the serum and in the pelvic dead space exudate were 1.64 hours and 3.65 hours, respectively. 2. The passage of infused CZX to the umbilical cord serum was satisfactorily rapid, as evidenced by figures reaching 10 micrograms/ml or higher at 1 hour and 15 minutes and still as high as 3 micrograms/ml or higher at 6 hours after administration. 3. The passage of CZX to the milk of mothers receiving this drug was low, and no untoward effect on the infant was likely. 4. CZX would effectively prevent infections due to premature ruptures of membrane.

[A case of pseudohypoparathyroidism with intracerebral calcification].

Intracerebral calcifications, especially in the basal ganglia, are observed in many kinds of diseases. A 41-year-old man is reported, who suffered from an acute epidural hematoma and underwent surgery to remove the hematoma. We detected very extensive intracerebral calcification on CT. Laboratory findings revealed hypocalcemia and hyperphosphatemia. General physical examination revealed characteristics typical of pseudohypoparathyroidism. The patient was diagnosed as having pseudohypoparathyroidism type I by the Ellsworth-Howard test. Since the advent of CT, the incidence of basal ganglia calcification has increased. CT is 5 to 15 times more sensitive than skull radiography in the detection of intracerebral calcification. Although many pathological states can cause basal ganglia calcification, most of the calcifications which are recognized on CT scans are physiological. But in cases in which basal ganglia calcifications are recognized also on plain radiographs, various kinds of symptoms including ones of basal ganglia origin are often recognized, and calcifications often extend to regions other than basal ganglia, eg. cerebellum, thalamus, etc. Pseudohypoparathyroidism is a rare disease which presents hypocalcemia, some characteristic physical appearances, and dementia. It is important to decide whether further examinations are necessary or not, when basal ganglia calcification is recognized incidentally on CT scan.

[Three-dimensional analysis of maxillary alveolar process in bilateral cleft lip and palate patients].

The collapsed maxillary alveolar arch in bilateral cleft lip and palate (BCLP) patients was quantitatively evaluated by three dimensional analysis. Materials were consisted of alveolar models taken from 15 Japanese boys (11 years old) with BCLP. The findings obtained were as follows: 1. Examination of the alveolar displacement in individual BCLP patients disclosed a marked tendency of inward and upward displacement of the lateral segments. On the other hand, premaxilla showed an upward displacement in the vertical dimension with a great deal of variation in the amount and direction of horizontal displacement. 2. Concerning the average amount of horizontal displacement, the premaxilla showed relatively little, but the lateral segment, especially the left one, showed a considerably large amount. In the vertical dimension, both the premaxilla and lateral segment were displaced markedly upward. 3. Patterns of collapsed alveolar arch in BCLP patients were classified according to the amount of horizontal displacement of the premaxilla into the following 4 types; type A (larger displacement towards the inside), type B (moderate or smaller displacement towards the inside), type C (moderate or smaller displacement towards the outside) and type D (larger displacement towards the outside). The order of prevalence of these 4 types was B,C,A,D in order. On the other hand, in the vertical dimension, the direction of displacement of the premaxilla was upward in all patients. 4. The anterior edge of the lateral segments showed the largest amount of displacement both in the horizontal and vertical dimensions. In a comparison of these results with those of Unilateral Cleft Lip and Palate (UCLP), the total amount of horizontal displacement of the maxillary alveolar process was larger in UCLP, while the vertical one was larger in BCLP. A remarkable difference between BCLP and UCLP was found in the displacement of the anterior region due to the premaxilla in BCLP.

Phosphorylation by protein kinase C of a synthetic heptapeptide bearing a lysine residue on the C terminal side of serine.

A peptide, Ala-Ser-Gly-Ser-Phe-Lys-Leu, which corresponds to Ala103-Leu109 of Hl histone, was synthesized and tested as substrate for protein kinase C. The serine residue at position 4 was phosphorylated specifically. Another peptide lacking the lysine at position 6 was not phosphorylated by the same enzyme, indicating the importance of that basic residue as the recognition site for protein kinase C.

Radiotherapy of spontaneous carotid-cavernous sinus fistulas.

Between 1984 and 1986, 7 patients with spontaneous carotid-cavernous fistulas (CCF) were treated by radiotherapy delivering 3,000 cGy (200 cGy, 5 times per week) to the sellar region. Improvements of clinical signs and symptoms were seen in all patients within 6 months of treatment. During a follow-up period of 7 to 35 months, 6 patients remained in good clinical condition and only one patient developed a recurrence. As an adverse effect, one patient developed early menopause, but no other side-effects or complications were seen. Radiotherapy should be considered in patients with spontaneous CCF, when CCF are seen in middle-high aged patients and progressive without relief of symptoms.

Ultrastructural study of hemangiomas. 5. Glomus tumor (Masson).

The fine structure of four cases of glomus tumor was described. All of the cases showed a solid type, and the glomus cells were shown to be typical or modified smooth muscle cells. Our conclusion is that the glomus cells are not derived from pericytes, but from smooth muscle cells of vascular part of the glomus, and are thought to be a specialized smooth muscle cell tumor.

Study of antibody response to 4 streptococcal antigens in rheumatic fever and Kawasaki disease with or without cardiovascular lesions.

Patients with rheumatic fever (RF) and Kawasaki disease (MCLS) were divided into two groups, one with and the other without cardiovascular lesions, and their sera were examined for antibodies to 4 streptococcal antigens: anti-streptococcal polysaccharide (ASP), anti-deoxyribonuclease B (ADN-B), anti-streptolysin O (ASO) and anti-streptokinase (ASK) in order to clarify the relationship between group A streptococci and these two diseases. In RF the frequency of positive levels of ASP at its early stage was lower than those of the other 3 antibodies, but the ASP titer tended to persist at elevated levels. The frequency of ADN-B in patients with rheumatic heart disease (RHD) was higher than that in those without RHD, but with regard to the other antibodies no difference was found between the two. Of the 4 antibodies the frequency of at least one positive level was 100% in all the sera of RF patients within 3 months from onset. The differences in the frequencies of positive levels of all 4 antibodies in both MCLS and the controls did not prove to be significant. The frequencies of ASP, ADN-B and ASO in the sera of patients without cardiovascular lesions tended to be slightly higher than those in patients with cardiovascular lesions.
