Serum levels of TA-4, a tumor associated antigen of cervical squamous cell carcinoma, were measured in various gynecologic tumors with a RIA kit (DAINABOT). Immunohistochemical localization of TA-4 was also evaluated in the squamous cell carcinoma of the cervix. Serum TA-4 levels were elevated in 8.3% of patients with CIS, in 25% with stage Ia, in 56% with stage Ib, in 83% with stage II, in 75% with stage III and IV and in 100% with recurrent squamous cell carcinoma of the cervix, respectively. No patients were positive for TA-4 in endometrial and ovarian cancers. Serum TA-4 levels declined distinctively after treatment in accordance with the disappearance of initial tumors in some patients with advanced carcinoma. TA-4 was demonstrated immunohistochemically in 2 out of 8 CIS, 3 out of 8 microinvasive carcinoma, 4 out of 4 keratinizing type, 7 out of 7 large cell type and 3 out of 6 small cell type of invasive squamous cell carcinoma, respectively. In each type of carcinoma, TA-4 was positive in the differentiated cells. Similarly, it was found in the differentiated layers of normal squamous epithelium excluding the immature cells of the basal layer.

The effects of interleukin-2 (IL-2) on natural killer (NK) cell activity were examined in both healthy persons and cancer patients. The NK activity of peripheral blood lymphocytes from 29 healthy donors was augmented by IL-2 treatment in all cases. However, in 14 out of 50 cancer patients NK activity was not augmented by IL-2. The rate of NK augmentation in a group of cancer patients was significantly lower than that in a healthy control group. When the cancer patients were divided into each clinical stage, NK augmentation was as follows: in stage I, 3 out of 3; in stage II, 11 out of 12; in stage III, 10 out of 18; and in stage IV, 12 out of 17. The rate of NK augmentation was significantly lower in cancer patients of stages III and IV. The rate of NK augmentation was also dependent on the performance status (PS) of cancer patients. The NK augmentation of cancer patients of PS 0 to 2 was not significantly different from that of normal controls, whereas the rate of NK augmentation was significantly lower in cancer patients of PS 3 and 4.

The correlations with maturity of gametes and fertilizability was studied. Specimens of semen were treated by several methods for induction of capacitation and acrosome reaction (AR). Follicular oocytes were recovered with endoscopic aspiration procedures, and divided to two groups: 1, maturing in vivo (preovulatory) and 2, immature. Immature oocytes were divided further into two subgroups: 1, germinal vesicle breakdown (GVBD) and 2, first polar body formation (FPB), after maturation culture. The sperms treated with the centrifuge-trypsinization-layering method showed a higher incidence of in-vitro fertilization (69.2%) and cleavage (51.3%) of human preovulatory eggs compared with other capacitation inducing methods. It was found that human spermatozoa that have undergone the acrosome reaction prior to zona attachment are capable of fertilizing eggs under the in vitro conditions. On the estimation of fertilizability with reference to egg maturity, 52.5% (21/40) of IMIV-GVBD and 64.7% (22/34) of IMIV-FPB eggs were fertilized. However, only 12.5% (5/40) of IMIV-GVBD and 11.8% (4/34) of IMIV-FPB eggs cleaved. These results showed that IMIV eggs had fair fertilizability, but no developmental capacity after the first cleavage. This may due to prematurity of the cytoplasm in IMIV eggs.

Tissue polypeptide antigen (TPA) was measured by radioimmunoassay in sera from 413 patients with various gynecologic tumors. The distribution of positive values (greater than 102 U/l) in the patients with gynecologic malignancies was such that the percentage of patients with elevated values appeared to increase with advancing stages of disease. Among the cervical cancer patients, elevated TPA values were observed in 11% of preinvasive, 35% of stage I and 67% of advanced cases. Similarly, the TPA values were elevated in 35% of the endometrial cancer patients. Among the patients with ovarian malignancies, serum TPA was elevated in 33% of borderline, 47% of stage I and 86% of advanced cases. However, serum TPA was elevated in 21% of patients with uterine myomas and in 12% of those with ovarian benign tumors. The serial measurements of TPA in sera of the patients with gynecologic malignancies showed that serum TPA levels correlated with the effect of treatment and the clinical courses. The present observations demonstrate that the lack of tumor specificity of TPA limits its diagnostic value in gynecologic malignancies but that serial measurements of this antigen appear to be useful for monitoring of patients.

Despite recent progress in surgical treatment for acquired heart disease, there still remain some problems to be solved. Recently, incidence of the aged and serious cases are increasing. In order to detect the limitation of surgical indication for these cases, a multivariate analysis for prediction of low cardiac output syndrome (LOS) after prosthetic valve replacement was performed. Variables with significant relationship to LOS were different from the types of disease and it seemed difficult to decide indication from single factor such as age or cardiac function alone. Mechanical assist for the postoperative failing heart was carried out for 45 cases. Of these 45, 3 cases underwent clinical application of partial artificial heart (PAH). Two cases weaned from PAH and there was one long-term survival. Duration of cardiac assist in one case who did not wean from PAH was as long as 70 days. Incidence of re-replacement of prosthetic valve due to malfunction was higher in recent 5 years when compared to that in former years. Incidence and type of malfunction of prosthetic valve were different according to the types of prosthesis. Survival and event-free rates in long-term follow-up period were markedly embellished by the types of prosthesis.
