Effects of angiotensin I-converting enzyme inhibitor (SA-446) on renal function in dogs.

The effects of an orally active inhibitor of angiotensin-converting enzyme (SA-446) on systemic arterial pressure, renal function and renin release were examined in anesthetized dogs. Intrarenal infusion of the larger dose of SA-446 (0.1 mg/min) caused an increase in RBF, urine flow and renin release and caused a fall in blood pressure. The smaller dose of SA-446 (0.02 mg/min) did not affect the blood pressure, but it increased the urine flow. However, the same dose of SA-446 in combination with probenecid caused a significant fall in blood pressure. The potentiation of SA-446 with probenecid may be explained by the elevation in plasma SA-446 concentration via the inhibition of its tubular secretion by probenecid.

Musculoelastosis: a change of small pulmonary arteries found in a case with atrial septal defect and pulmonary hypertension.

Interesting findings were obtained in the lung biopsy of a 46 year old female patient with secundum atrial septal defect and pulmonary hypertension. In the intima of small pulmonary arteries, there were a marked increase in elastic fibers and marked proliferation of longitudinal smooth muscle cells. The vascular lumen was markedly stenotic, but reconstruction of small pulmonary arteries indicated that all vascular lumina were patent. The case was accordingly diagnosed as operable and closure of atrial septal defect was undertaken. Three years postoperatively, the patient is well with much improved condition and lowered pulmonary arterial pressure was demonstrated by cardiac catheterization. This case had apparently serious vascular lesions due to severe intimal thickening which we labeled "musculoelastosis", but it was found that such thickening alone did not cause peripheral pulmonary arterial occlusion. It is therefore thought to be a benign pulmonary vascular condition.

Lung tumor imaging by positron emission tomography using C-11 L-methionine.

This paper described the first clinical study of lung tumor scanning by positron emission tomography (PET) using C-11-labeled L-methionine (11C-L-Met). Tumor images were clearly visualized by high contrast in eight lung cancer patients and also in a patient with a silicotic nodule. Quantitative evaluations of methionine uptake in tumor tissue and normal tissue by comparing differential uptake ratios suggested that the extent to which 11C-L-Met accumulates in a tumor is closely correlated to the tumor's viability such as benign or malignant, viable or necrotic. 11C-L-Met is considered to be an effective tumor marker for PET diagnosis which represents increased amino acid metabolism of tumors in the mediastinum and lung.

[Clinical efficacy of aztreonam in pelvic peritonitis].

Aztreonam (AZT) was evaluated for its clinical efficacy in 6 patients with pelvic peritonitis. In all the cases, AZT was administered by intravenous injection, and the duration of treatment ranged from 4 to 12 days. Daily dose was 2 g in 4 cases, and 4 g in the remaining 2 cases. Surgical treatment was necessary in 2 cases, whereas in another 2 cases, either LMOX or ABPC was administered in addition to AZT. The clinical results was excellent or good in 5 out of 6 cases. Side effects possibly attributed to AZT were diarrhea in 2 cases and slight increase of GOT and GPT in 1 case, although they disappeared promptly afterward.
