[Treatment of reflux esophagitis with sucralfat].

In a multi-centre placebo-controlled double-blind study, the therapeutic effectiveness of sucralfate (aluminium sucrose sulphate, four times 1 g daily) was tested on 66 out-patients with endoscopically confirmed reflux oesophagitis. After 12 weeks of treatment complete healing occurred in 72% of the sucralfate group, and in a further 14% there was improvement. In the placebo group the healing rate was 40%, with improvement in a further 20%. Serious side-effects did not occur. Sucralfate treatment was especially effective in patients with signs of high-grade inflammation (stage III of Savary and Miller) unsuccessfully treated with other drugs. Contrary to previously available drugs, sucralfate is effective also in lesions caused by alkaline reflux and is thus a useful addition to the hitherto unsatisfactory means of treatment.

The roles of self-esteem and self-consciousness in the Wortman-Brehm model of reactance and learned helplessness.

The present experiments were designed to study the conditions under which failure would enhance or inhibit subsequent task performance. Based on the theory of Wortman and Brehm (1975), it was expected that small amounts of failure would produce reactance (manifested by improved performance at a subsequent task), whereas large amounts would lead to learned helplessness (i.e., impaired later performance). It was further expected that individual differences in self-esteem and private self-consciousness would serve as moderator variables for the above effects. In Experiment 1, subjects were exposed to either a small amount of failure or no failure before working on an anagrams task. As predicted, subjects high in self-consciousness, who have shown greater reactance arousal in attitude change studies, performed better on the anagrams task than subjects low in self-consciousness in the small-failure condition, but not in the no-failure condition. Further analyses revealed that this Self-Consciousness X Small Failure interaction was attributable to the performance data of the low, but not the high self-esteem subjects. Experiment 2 was designed to replicate and extend these results. Subjects were pretreated with either a small amount of failure, an extended amount of failure, or no failure before working on the anagrams task. A significant Self-Esteem X Helplessness Training interaction emerged. Relative to the no-failure condition, in which the two self-esteem groups did not differ, low self-esteem participants (low SEs) performed marginally better than did high self-esteem individuals (high SEs) in the small-failure condition but significantly worse than high SEs in the extended-failure condition. The effect of private self-consciousness was considerably weaker in this study, possibly because the sample included few low SEs (who are especially influenced by self-focused attention) who were also relatively low in self-consciousness. Questionnaire data from Experiment 2 were consistent with the notion that enhanced performance reflected reactance, whereas impaired performance signified helplessness.

Effect of cimetidine treatment in the prevention of gastric ulcer relapse: a one year double blind multicentre study.

One hundred and forty six gastric ulcer patients were given open treatment using 1 g cimetidine daily to heal their ulcers. Of 130 who completed the acute treatment period of eight weeks, 112 (86%) had healed ulcers. Of these 112 patients with healed ulcers, 108 entered a one year double blind study to compare the effect of cimetidine maintenance therapy (400 mg at night) with placebo. Of the 84 patients available for assessment at the end of one year, 86% in the cimetidine treated group were in remission compared with 45% in the placebo treated group (chi 2 = 15.03; p less than 0.001). There were similar losses from non-compliance and drop out in both groups. The incidence of untoward effects and significant drug related laboratory abnormalities was low. The results indicate that cimetidine heals nearly 90% of acute gastric ulcers within eight weeks and that subsequent low dose maintenance treatment at night offers a considerable benefit over placebo therapy.

The temporal relation between cigarette smoking and pancreatic cancer.

Temporal trends in US cigarette smoking prevalence rates 1920 to 1978, were related to temporal trends in US pancreatic cancer mortality rates in both sexes. In males, the rise and fall in smoking prevalence was followed by a rise and recent fall in cancer rates. In females, the later rise in smoking prevalence has been accompanied by a rise in cancer rate, and a recent slight decline in smoking rate has been associated with a slowing of the rise in cancer rate.

Heterogeneity in historical cohort studies: a source of bias in assessing lung cancer risk.

Historical cohorts that are heterogeneous with respect to era in which exposure to a hazard began are subject to chronology bias. Those historical cohorts assembled some time after onset of exposure are also subject to selection bias through attrition of the original populations prior to the time of registration. Such cohorts may be called multiserial cross-sectional. The two forms of bias in such cohorts may account for some of the elevated risk of lung cancer reported in occupational studies of this type. Historical cohorts comprising all workers who began exposure in a relatively limited era can be called inception cohorts. Three occupational investigations are reviewed in which both multiserial cross-sectional cohorts and inception cohorts were studied. In all three investigations the inception cohorts showed lower risks of lung cancer than the multiserial cross-sectional cohorts.

[Severe generalized atherosclerosis in a 14-year-old boy].

A case of severe generalized atherosclerosis in a 14-year-old boy is reported; all the large arteries, including the coronary vessels, were involved. A high degree of stenosis of the abdominal aorta very close to the origin of the renal arteries was thought to be the reason for the accompanying considerable systemic hypertension. Laboratory investigations were not able to produce evidence of the cause of the disease, and there were no apparent risk factors. Finally, it was concluded on the basis of the family history of the patient that a genetic component involving unknown pathogenetic mechanisms was responsible for the early manifestation of the disease.

[Immunohistochemical studies on the association between hepatitis B surface antigen and primary hepatocellular carcinoma (author's transl)].

In many geographical areas especially in African and South-east-Asian countries hepatitis B virus infection is considered to be a major etiological factor in the development of primary hepatocellular carcinoma. 107 autopsy- and 15 biopsy-specimens were studied by means of immunohistochemistry [peroxidase-antiperoxidase-(PAP-)method] to demonstrate the association between primary hepatocellular carcinoma and hepatitis B surface antigen (HBsAg). HBsAg was found in 8 of 107 tumour specimens (7.4%) and liver cirrhosis in 102 of the 107 autopsy specimens with hepatocellular carcinoma (95%). 10 of the 15 biopsy-specimens showed neoplastic and non-neoplastic liver tissue, and in 2 of these 10 cases HBsAg was found. Liver cirrhosis could be seen in 9 of those 10 specimens. HBsAg was also studied in 90 cases with liver cirrhosis and was found to be positive in 2 of them (2,2%). HBsAg associated with primary hepatocellular carcinoma was only found in non-neoplastic liver cells of cirrhotic livers. Our studies indicate that in our geographical area the association of HBsAg with primary hepatocellular carcinoma is much less conspicuous than in Asian, African and even Southern European communities.

Coffee consumption and pancreatic cancer: temporal and spatial correlation.

An examination of available data on per caput consumption of coffee and pancreatic cancer mortality in the United States since 1950 shows a temporal association. A rise and fall in coffee consumption was followed by a rise and fall in the incidence of pancreatic cancer with roughly a 10-year lag. Nevertheless, there were inconsistencies in this relationship was also found between the consumption of coffee and pancreatic cancer mortality in 13 countries. While this relationship suggests an association, major inconsistencies case doubt on the possibility that it is one of cause and effect. This may be due to confounding, particularly by cigarette smoking and cancer of the pancreas is much more consistent with a causal relationship.

Epidermolysis bullosa acquisita and inflammatory bowel disease.

Immunologic studies on lesional and nonlesional "normal" -appearing skin of a 32-year-old man with epidermolysis bullosa acquisita (EBA) revealed the following: (1) Noninflammatory subepidermal bullae formation in the periodic acid-Schiff (PAS)-positive basement membrane; (2) linear deposits of polyclonal IgG, Clq, C4, C3, properdin, and factor B at the dermoepidermal junction; IgA and IgM were not found; (3) broad subbasal lamina electron-dense amorphous material (EDAM) lacking amyloid features by electron microscopy; EDAM was not associated with anchoring fibers; (4) cleavage plane below the EDAM. Gastrointestinal endoscopy showed inflammatory erosions in the duodenum. Microscopy of the involved duodenum showed a mixed inflammatory infiltrate, subepithelial edema, microvesiculation, and granular deposits of IgG and C3, but no EDAM. However, EDAM and immune reactants were found in the esophagus. EBA is a distinct entity characterized by the presence and location of EDAM ultrastructurally, and it may represent a gastrointestinal-cutaneous syndrome with an immune pathogenesis.

Epidemic curve of respiratory cancer due to chloromethyl ethers.

In 1962, suspicion arose that an excess of lung cancers was developing in a chemical plant. A prospective cohort study of 125 male workers was begun, and the group was followed from January 1963 to the end of 1979. A small epidemic of respiratory cancer evolved, including 14 cases of lung cancer (13 were the small cell type) and 2 cases of laryngeal cancer among 91 men exposed to chloromethyl ethers in the 17-year period, as compared to 2 cases of lung cancer among 34 unexposed men. The lung cancer epidemic peaked 15-19 years after onset of exposure and began to subside thereafter.
