Biosynthesis of proteokeratan sulfate in the bovine cornea. 2) Isolation of subcellular membrane fragments from bovine cornea cells with keratan sulfate synthesizing activity.

Cornea cells were isolated from bovine corneae after collagenase treatment. Subcellular fragments were fractionated by density gradient centrifugation. The density gradient run was monitored by determination of the marker enzyme activities for mitochondria, plasma membranes, lysosomes and endoplasmatic reticulum, of the enzyme activities involved in keratan sulfate synthesis and of the protein content. The fractions were further investigated by electron microscopy. Two membrane fractions with keratan sulfate-synthesizing activity (UDP-N-acetylglucosamine:keratan-N-acetylglucosaminyl-transferase, UDPgalactose:keratan galactosyltransferase and keratan sulfotransferase) were detected: a heavy fraction separated from the other organells investigated and a light fraction exhibiting the same density as plasma membranes. The activities of the three enzymes were found in the same density gradient fractions with a similar distribution pattern between the fractions, which suggests a joint localization of these 3 enzymes at the same intracellular sites.

Acute vascular rejection treated by plasma exchange.

The vast majority of grafts with acute vascular rejection (AVR) undergoes irreversible loss of function, since this type of rejection usually does not respond to increased immunosuppression. Since 1980 we have treated in 22 patients 27 steroid resistant AVR by plasmafiltration (PF). Per episode 6 x 5 L plasma were filtered and simultaneously replaced by 3.5% Albumin-Ringer-lactate. After each PF 20 g immunoglobulins (Intraglobin(R] was substituted i.v.. In all cases renal function improved following PF. In the long run 1 graft was lost due to chronic irreversible vascular rejection, in 2 patients rejection was not reversed by PF, all others are still functioning, 19 of them 6 months and 8 of them 12 months post PF treatment. They show no signs of rejection. PF with repletion of immunoglobulins after treatment is a simple and safe procedure, which in our hands proved to be quite effectful in reversal of AVR.

[Water uptake of dermatoglyphic skin (author's transl)].

After modification of the usual histologic test method concerning water uptake of dermatoglyphic skin our experiences with washerwomen's hands have to be corrected. The singular wrinkle of washerwomen's hands is the effect of the sum of subepidermal water input. In addition, probably a vasomotoric effect is important to the wrinkling intravitally and early postmortally. After some time of submersion in water many disperse subepidermal water-filled chambers were seen postmortally. The ducts of lost of sweat glands were torn below the epidermal basal membrane. In vivo there will be a balance between uptake, resorption by the hemodynamic flux, and layer resistance. It is mentioned that the width of the epidermis after water penetration takes the minimum part of the wrinkles of washerwomen's hands. There was no hint as to the influence of decay or contraction of elastic and collagen fibers.

[Late death in congenital toxoplasmosis].

In spite of a known toxoplasmosis infection rate of up to 90% of the population of the Federal Republic of Germany published T-cases decrease steadily. T-infection during gravidity may cause more pathologic alterations in the infant cerebrum than is commonly assumed. Medico-legal aspects are very rare. A case of connatal toxoplasmosis in a 19 months old infant was found at exhumation (16 days p.m.). In the cerebrum T-parasites, terminal colonies and calcified spots were observed. Specific morphological and immunohistological findings lead to the diagnosis. The medico-legal problems of this case are discussed.

Quantitative histophotometry analysing significant inductive and alterative effects of aminoglycoside application (gentamicin, tobramycin, amikacin) upon tubular kidney proteins.

In order to evaluate quantitative changes in kidney proteins, computer-assisted histophotometry of tissues was performed. Kidney marker enzyme concentrations were considered to be involved in inductive and alterative developments in the proximal tubule. These effects were caused by the administration of aminoglycosides. Indicator enzymes of the proximal tubule such as membrane-bound alkaline phosphatase (AP) and alanine-aminopeptidase (AAP) as well as lysosomal beta-glucuronidase (beta-Gl) were stained in kidney tissues. Graphic monitoring and digital display of kidney cortex sections were registered by electronic image analysis using the 'Micro-Videomat' 2 system. As an experimental design, 160 kidneys of Wistar rats were studied. Their kidneys were analyzed after one, two and three intravenous applications of gentamicin, 25 mg/kg/day, or tobramycin, 25 mg/kg/day, or amikacin, 50 mg/kg/day, on consecutive days. In a fourth test group, the kidneys were examined after the third application and a 5-day recovery period. Concentrations of tubular marker enzymes were significantly increased (2p less than 0.01, Wilcoxon test) after two applications of gentamicin: AP 37.6%, AAP 6.3% and beta-Gl 11.8%. Contrary to these findings, after two injections of tobramycin or amikacin only slight alterations were documented: tobramycin, AP 14.5%, AAP -0.1% and beta-Gl 0.4%; amikacin, AP 6.0% and AAP 4.2%. The studies indicate that tobramycin induces less severe nephrotoxic and inductive reaction than gentamicin and partly than amikacin in doses administered during our experiments. In all cases, enzyme activities studied were nearly normalized after three applications of aminoglycosides and a recovery period of 5 days. Quantitative computer-assisted evaluation of the proteins located in the tubule appears to be a tool for monitoring the degree of alterations caused by enzyme induction, enzyme release and excretion of kidney proteins.

[Effect of the intravenous administration of lormetazepam on hemodynamics and arterial blood gases in patients with coronary heart disease].

In 11 conscious patients suffering from coronary artery disease undergoing aorto-coronary bypass operations the hemodynamic and respiratory effects of Lormetazepam were investigated preoperatively. Lormetazepam was administered i.v. in a dosis of 2 mg (average weight 70.4 +/- 5.7 kg) into the right atrium. The maximum of hemodynamic changes was found about 10 min after injection. At that time the mean arterial pressure was diminished about 14%. At the same time total peripheral resistance fell about 15% below the control level. The cardiac index remained unchanged. The hemodynamic effect therefore consisted in a peripheral vasodilation. This was emphasized by the reduction of right atrial pressure and pulmonary capillary wedge pressure. The heart rate was unchanged. The arterial pO2 was reduced to 18% below the control level. The arterial pCO2 was increased about 24%. This finding is possibly due to an additional effect of Morphine and Phenobarbital used for premedication.

[Results of transplantation of kidneys from related living donors].

Between 1973 and 1981 23 transplantations of kidneys from living related donors have been performed in Frankfurt/Main. Donor complications were haematopneumothorax (1 case) and reversible urinary tract infections (3 cases). All donors were discharged after 8-12 days, all have until now (6 months to 8 years postoperatively) normal excretory renal function and normal arterial blood pressure. Patient-survival in the recipients is 5 years postoperatively 96% and graft-survival is 90%. Nine patients received HLA-identical kidneys, all have excellent to fair function of the grafts, 7 patients are off steroids. In 14 cases kidneys were transplanted, although donor and recipient shared only one haplotype. In these patients 5-year graft survival is 84%. Seven patients have normal graft function, whereas 6 patients have a reduced function or have rejected. One patient died. Results of kidney transplantation using living related donors are--even when donors with only one haplotype-identity are accepted--much better than those obtainable when using cadaveric donors. When renal transplantation is considered, patients should be informed about the favourable results attainable with kidneys from living related donors.

[Metabolic effects of diuretics (author's transl)].

Thiazide diuretics and loop-diuretics, often used for treatment of hypertension, interfere with various metabolic reactions: An increase of uric acid in plasma is a regular finding, but gout will not be caused, an elevation of plasma lipoproteins has been observed but did not proceed to pathological values, in a number of patients blood glucose has been raised but without producing diabetes mellitus. In general, these metabolic interference are of minor pathological significance. Special treatment for these side-effects are needed in a few cases only. The indicated use of thiazide diuretics should not be disregarded because of their effects on regulatory mechanisms of metabolism.

[Chemotherapy of non-small-cell bronchial cancer with a combination of Cis-diamminedichloroplatinum (II) and VP 16-213].

In a phase II study 40 patients with non-small-cell lung cancer were treated with 100 mg/m2 cis-diamminedichloroplatinum (II) (DDP) i.v. on day 1, combined with VP 16-213 (VP) in a dose of either 80 mg/m2 daily i.v. on days 1, 2 and 3 or 120 mg/m2 daily by mouth on days 3, 4, 5 and 6. The course was repeated every 3 weeks. In 30 evaluable patients, 10 partial remissions were recorded with a median duration of 3 months. Eleven patients had stable disease and 9 showed progression under treatment. Leukopenia was more pronounced with intravenous administration of VP than with oral VP (median leukocyte nadir 2400/mm3 and 3700/mm3 respectively). Two patients had thrombocytopenia under 50,000/mm3. All patients suffered from moderate to marked nausea and vomiting. All patients had alopecia. Nine patients had serum creatinine elevations over 1.4 mg/dl. Six patients with renal toxicity were treated in one institution with incorrectly applied forced diuresis during DDP administration. DDP and VP are an active regimen for remission induction in non-small-cell lung cancer. Due to cumulative and marked gastrointestinal intolerance this regimen cannot be given over prolonged periods of time.
