A device to facilitate bathing by the laryngectomee is described. The Aquamat is made up of equipment routinely used in anaesthesia and underwater diving. An airway link is made between a watertight tracheostoma and the mouth such that the patient can breath between nose and trachea. Special precautions are necessary for swimming.
In 100 excisions of the rectum at the Chirurgische Universitätsklinik Erlangen the height of the tumor determined preoperatively by rectoscopic diagnosis was compared directly to the real tumor location of the fresh and unfixed specimen. In up to 58% of the cases we found a distinct discrepancy between the height determined rectoscopically and the real location of the tumor. This result shows that the decision between excision and resection must never be made on the preoperative rectoscopic information, but that the indication must always depend on the intraoperative findings. This is especially true for the disputed cases of the middle third of the rectum.
A methodological concept for measuring anxiety states and changes in anxiety states in clinical studies with anxiolytic substances is presented: Effectivity should be evaluated on several levels which include emotions, objects of anxiety, psychophysiology, biochemistry, and observed behavior. This test concept is based on new knowledge from behavior psychology, learning theory, psychology of testing, psychophysiology, biochemistry, neuroendocrinology, and psychiatry. In diagrams, important mechanisms of thr origin of experimental, stress-induced and pathological anxiety states are depicted. The point is stressed that anxiety is situational and fluctuates.
Our results later many years after 231 TEA in femoro-popliteal segment are based to examinate the indications of this reconstructive operation and to compare with other methods of arterial reconstruction. Resulted: the TEA is always a good method of biological worth without risk. This method authorized to indicate the reconstruction of chronical arterial occlusions. However the method is not to apply to all arterial occlusions without criticism only to selected patients.
Renal transplantation was performed 3 to 64 months after the onset of dialysis in 6 patients with histologically ascertained malignant nephrosclersosis and malignant hypertension, as well as terminal renal failure. Three patients were nephrectomized on both sides before and one after the transplantation. 24 to 105 months after the transplantation all patients were normotensive. Only one patient required low-dosage antihypertensive medication. There was evidence for marked improvement of cardiovascular complications, and no evidence for recurrence of malignant nephrosclerosis in the transplanted kidney.

The NADP-dependent microsomal kidney enzymes, 3alpha- and 20beta-hydroxysteroid dehydrogenase (HSDH), which exhibit considerable sex differences in their activities (male:female activity ratios, 16:1 and 30:1 respectively), were investigated after interference with the pituitary-gonad and pituitary-adrenal systems. Prepubertal gonadectomy as well as hypophysectomy of mature male rats led to a decline in HSDH activity to almost that found in the normal female rat, whereas activities in female rats were unaffected. Testosterone induced typical male 3alpha-HSDH activity in both gonadectomized and hypophysectomized rats of either sex. Administration of 5alpha-dihydrotestosterone (5alpha-DHT) or 5alpha-androstane-3alpha, 17beta-diol to hypophysectomized male rats was equally effective in restoring full 3alpha- and 20beta-HSDH activities whereas 5alpha-androstane-3beta, 17beta-diol was less effective and dehydroepiandrosterone was ineffective. Simultaneous administration of cyproterone acetate did not block the inductive action of 5alpha-DHT. Administration of chorionic gonadotrophin, pregnant mare serum gonadotrophin or a combination of luteinizing hormone and follicle-stimulating hormone to hypophysectomized male rats all led to parallel increases in the weight of the seminal vesicles and in both renal enzyme activities; administration of growth hormone, prolactin or thyroid-stimulating hormone was ineffective. Adrenalectomy of gonadectomized, but not of hypophysectomized male rats, caused a further drop in activity to the normal female level. Adrenalectomy of otherwise intact rats did not affect either enzyme activity. The hypophysis was involved in the regulation of the two NADP-dependent renal HSDH activities through its gonadotrophic function in male rats; adrenal secretions were of little physiological significance.
There are 29 cases of vesiculodeferentography (VDG) (1962-1975) presented. The very indication of VDG was the evaluation of azoospermia due to occlusion of the vas deferens, in addition to other urologic diseases. Especially from the andrologic point of view the various forms of occlusion-azoospermia are classified and analyzed. It is pointed out, that the rete testis region is not to be elucidated by VDG.
The effect of antiaggregating agents - acetylsalicyclic acid (ASA) and dipyridamol - on the rejection of allotransplanted canine kidneys in presensitized animals were studied. Blood flow dropped more markedly in the control group than in the group treated with ASA and dipyridamol. 24 hours after transplantation the blood flow in this group was significantly higher than in the control group (p 2,5 %) and in the third group treated with dipyridamol alone. The cellular rejection started earlier and was more pronounced in the control group. Only in this group vascular rejection and microthrombi were demonstrated. Urine output ceased 24 to 60 hours after transplantation. The immunosuppressive, antiphlogistic and platelet antiaggregating effect of the combined ASA and dipyridamol therapy is thought to be responsible for the better results in this group. In this experimental model the combined treatment of ASA and dipyridamol results in a later onset of cellular rejection, in better blood-flow of the kidney and later onset of anuria.
Among 835 cases of female breast cancer in the years 1951-1971 we found 44 histological rare cancer (comedo-carcinoma 21, carcinoma cribosum 1, gelatinous carcinoma 8, squamous cell carcinoma 3, mixed carcinoma 11). In agreement with the literature we are able to underline the favourable prognosis of the slow growing forms of the neoplasms like gelatinous carcinoma, comedo-carcinoma and carcinoma cribrosum opposite to squamous cell carcinoma and mixed carcinoma.

In order to elucidate the role of the hypophysis in the regulation of cholesterol 7alpha-hydroxylase activity, male and female rats hypophysectomized on day 50 of life were treated with different hypophyseal hormones and tested on day 85 of life. Only a crude extract from human hypophysis and ovine prolactin was able to enhance the enzyme activity, thereby restoring the level of intact controls.
In 1820 the first malignancies ascribed as due to occupational arsenic exposure were reported as scrotal cancers among smelters. A century later the causal relationship between chronic occupational, environmental or medical arsenical exposure and skin carcinogenesis was firmly established. From 1948 to 1975, nine out of eleven epidemiological studies have shown, initially or upon review, significant excess mortality from respiratory cancer among diverse occupations exposed to various inorganic arsenicals. Two of the nine studies have shown concommitant, significant excess mortality from lymphatic cancer, and another, from skin cancer. Additionally, two such studies have revealed a dose-response relationship between arsenical exposure and lung carcinogenesis. In the first, reported in 1969, the relationship was semi-quantitative, with a possible interactive role by sulfur dioxide or other contaminants. The other demonstrated a dose-response which was quantitative for arsenic per se. Upon our reinterpretation, this dose-response also demonstrated an increased lung cancer mortality risk apparently at arsenic concentrations above 1 mug/M3, calculated as the 8-hour TWA daily exposure over a 40-year working life. However, these and related data do not reveal a definite no-effect exposure level. Thus, in the absence of data documenting a cancerigenically safe level of occupational exposure and because of the environmental ubiquity of arsenic, the conclusion is drawn that the arsenic body burden of workers should not be occupationally increased above that produced by the ambient level.
