[Evaluation of tumorous parotid gland diseases by sonography].

Ultrasonic evaluation was done on 62 patients with a palpable tumor in the area of the parotid gland. Morphological characteristics of the individual conditions are demonstrated. Chronic inflammations, abcesses, cysts and Warthin's tumor can be diagnosed correctly. These alterations can be distinguished from the pleomorphic adenoma. A differential diagnosis of this tumor cannot always be done, however, as it cannot be distinguished morphologically from a benign enlargement of the intraglandular lymph nodes. Malignant tumors in our patients were diagnosed correctly. Because of the small number of cases (7), however, we are not able to state whether it is possible to distinguish between benign and malignant tumors in all cases. Ultrasonic criteria of malignancy are described.

Hemodynamic effects of acebutolol and propranolol in hypertensive patients during exercise.

Acebutolol 400 mg vs. propranolol retard 160 mg were investigated in a multicentral, double-blind, cross-over study for their action on blood pressure and heart rate in outpatients during ergometer exercise. A 2-week single-blind placebo control phase, which served as a wash-out and to eliminate placebo responders, preceded the 6-week treatment phase with propranolol (or acebutolol) followed by a further 6-week phase with acebutolol (or propranolol). At the end of each treatment phase a bicycle ergometer test was carried out, the values for 18 patients (from 26 completed investigations) were carried forward to the evaluation of the ergometry. The treatment groups were shown to be comparable. Acebutolol and propranolol produced a similar fall in systolic and diastolic blood pressure and heart rate at rest. During ergometer exercise, acebutolol led to a significantly lower increase in both systolic and diastolic blood pressure and heart rate than propranolol. The possible role of the intrinsic sympathomimetic activity (ISA) and the relative cardioselectivity of the test agents and the clinical impact of the hemodynamic findings with respect to morbidity and mortality are briefly discussed. Questioning at the end of the study revealed that significantly more patients preferred the treatment with acebutolol.

[Artefacts in the CT scan caused by surgical clips and central venous catheters and the use of new materials].

When metal clips and venous catheters have been used the computerized tomography shows artifacts, which make it computerized tomography shows artifacts, which make it difficult to have a reliable and accurate diagnosis, because the X- raying of the metal clips and the venous catheter gives radiating lines, so called streaking. We have tested new materials for the metal clips and a new polymer to replace the substance of the venous catheter with the intention that the materials should have the same density as the normal tissues. With a new metal for the clips and a new polymer for the venous catheters, mixed with metal ions, the computerized tomography does not show streaking.

Subchronic inhalation toxicity of dimethylformamide in rats and mice.

Fisher F344 rats and B6C3F1 mice were exposed to concentrations of 0, 150, 300, 600 and 1200 ppm of dimethylformamide (DMF) for 6 hours a day, 5 days a week for 12 weeks. Detailed clinical observations were obtained weekly and body weights biweekly on all animals. Clinical chemistry and hematology evaluations were made on all rats and approximately half the mice at terminal sacrifice. Gross necropsy examinations were made on all animals. Histopathologic evaluations were conducted on selected tissues of animals of both species at all dose levels. Few overt signs of toxicity were seen in either rats or mice. There was a dose related depression in body weight gain in rats that was significant at the 1200 ppm level from the second week of study onwards. A total of 11 mice died or were sacrificed moribund during the study, 8 from the high dose and 2 from the 600 ppm dose level. Both clinical chemistry (in rats only) and gross necropsy observations, and histopathology of tissues indicate the possibility that liver may be the target in specific organ toxicity. The no-effect DMF dose was below the 150 ppm level for both rats and mice and the maximum tolerated dose was below the 600 ppm level.

[The EEG of the human hippocampus--summary and discussion of results published till now on patients with primary neurologic diseases].

Hippocampal theta-activity in several animals (rodents, cat, dog) is often described and well established. But there is an exception in primate animals: the so-called "rhythmic slow activity" seen so often in infraprimates is here essentially lacking. The aim of this paper is to answer the question whether or not there are EEG patterns which are characteristic and specific for the human hippocampal formation under pathophysiological conditions. This question is particularly interesting in view of the hypothesis that the emotional theta-waves in man described by several authors may originate in the hippocampus or other limbic structures. Out of the overwhelming literature about deep electroencephalography in man, some representative papers dealing with the EEG of the hippocampal formation in patients with neurological disorders were compiled. The different results may be summarized as follows: 1. there was a predominance of slow irregular theta or delta-activity, mostly superimposed by fast frequencies, 2. spontaneous rhythmic slow activity in the strict sense was essentially lacking, 3. even under conditions eliciting rhythmic slow activity in animals, rhythmic theta activity was not found in patients. Problems of comparability to results found in animals and possible reasons for the differences to infraprimates are discussed: --limitation of the study of the human brain by ethical considerations and therefore exploration only of (mostly epileptic) patients with potentially pathologic hippocampal EEG activity not comparable to normal animals.(ABSTRACT TRUNCATED AT 250 WORDS)

[Endoscopic tube implantation for the palliative treatment of malignant stenoses in the esophago-gastric region].

Palliative surgical treatment of malignant oesophago-gastric strictures has been disappointing. Resections in an advanced tumor stage are heroic interventions with a high mortality rate. Alimentary fistulas do not release from annoying saliva regurgitation but unnecessarily mutilate the terminal patient. The positioning of a tube under endoscopic control into the constricting neoplasma has the following advantages: 1. deglutition and saliva drainage are restored; 2. morbidity- and mortality rate is low; 3. duration of hospitalization is brief (1-2 days).

Transport of hemolysin by Escherichia coli.

The hemolytic phenotype in Escherichia coli is determined by four genes. Two (hlyC and hlyA) determine the synthesis of a hemolytically active protein which is transported across the cytoplasmic membrane. The other two genes (hlyBa and hlyBb) encode two proteins which are located in the outer membrane and seem to form a specific transport system for hemolysin across the outer membrane. The primary product of gene hlyA is a protein (protein A) of 106,000 daltons which is nonhemolytic and which is not transported. No signal peptide can be recognized at its N-terminus. In the presence of the hlyC gene product (protein C), the 106,000-dalton protein is processed to the major proteolytic product of 58,000 daltons, which is hemolytically active and is transported across the cytoplasmic membrane. Several other proteolytic fragments of the 106,000-dalton protein are also generated. During the transport of the 58,000-dalton fragment (and possible other proteolytic fragments of hlyA gene product), the C protein remains in the cytoplasm. In the absence of hlyBa and hlyBb the entire hemolytic activity (mainly associated with the 58,000-dalton protein) is located in the periplasm: Studies on the location of hemolysin in hlyBa and hlyBb mutants suggest that the gene product of hlyBa (protein Ba) binds hemolysin and leads it through the outer membrane whereas the gene product of hlyBb (protein Bb) releases hemolysin from the outer membrane. This transport system is specific for E coli hemolysin. Other periplasmic enzymes of E coli and heterologous hemolysin (cereolysin) are not transported.

Induction of cell arrest at G1/S and in G2 after treatment of Ehrlich ascites tumor cells with metallocene dichlorides and cis-platinum in vitro.

The influence of in vitro application of the cytostatic agents titanocene dichloride (TDC), vanadocene dichloride (VDC), and cis-diamminedichloroplatinum(II) (DDP) on the cytokinetic behavior of EAT cells was investigated. All three substances induced similar cytokinetic phenomena that are characterized, on the one hand, by an accumulation of cells in the late S phase and in the G2 phase during exposure periods of 8 to 20 h. On the other hand, some cells were apparently arrested at the end of the G1 phase and, after removal of the cytostatic agents, moved through the following S, G2, and M phases as synchronized cell populations. In view of this unusual cytokinetic pattern of multiple effects, it is argued that the striking similarities in cytokinetic behavior after treatment with the three different cytostatic substances indicate analogies in their molecular mechanisms of action. The different alteration patterns observed after in vivo application of the drugs are obviously due to additional processes occurring only in vivo.

[Clinical course and prognosis in medulloblastoma in infants and young children].

From 1962 until 1981 six children with histologically assured medulloblastoma younger than three years of age have been treated at Münster University Clinics. The interval from the onset of symptoms until the clinical admission lasted six weeks, a very short time compared with the admission interval of adults. The survival time after admission was only sixteen days in average. All children died because of central disregulation due to their disease.

Transport of hemolysin across the outer membrane of Escherichia coli requires two functions.

Among a large collection of hemolysis-negative mutants obtained by mutagenesis of the Hly plasmid pHly152 with Tn5, we have isolated two classes of mutants which are defective in the transport of hemolysin across the outer membrane. The two cistrons (hylBa and hlyBb) which are affected in these mutants are located adjacent to each other on the hly determinant but are transcribed from different promoters. Recombinant plasmids were constructed which carry the two functions as combined or separated cistrons. These were shown to complement the two types of transport mutants. Studies on the compartmentation of hemolysin in these two classes of mutants indicate that most hemolysin (greater than 70%) in hlyBa mutants is located in the periplasmic space, whereas in hlyBb mutants a larger portion of hemolysin is associated with the outer membrane fraction. The phenotypic appearance of colonies from hlyBb mutants is that of beta-hemolytic Escherichia coli strains, indicating that a substantial portion of hemolysin has already reached the outside of the outer membrane without being released into the medium. Release was achieved readily when hlyBb mutants were complemented with a recombinant plasmid carrying hlyBb.

[Value of sialography in the diagnosis of parotid and submandibular gland tumors].

40 patients clinically suspected of a tumor of the parotid gland, and 2 patients suspected of tumor of the glandula submandibular, were subjected to fractionated sialography at the Radiological Clinic of Münster University in the course of five years. In 31 cases (74%), the tumor was confirmed. In 11 patients (26%) no suspicion of tumour was possible even on retrospective examination of the x-ray films. Hence, the value of sialography is more in the direction of pre-operative exclusion of other types of disease, especially of stone formation. However, it is to be expected that the introduction of axial CT sialography will significantly improve the accuracy of tumour diagnosis, because this method enables examination and assessment of the glandular parenchyma besides an examination of the parotid duct.

Serum prolactin, gonadotropins, androgens, estrogens, and gestagens in patients with malignant testicular germ cell tumor.

In the serum of five patients with malignant germ cell tumor of the testis, prolactin as well as one or several of the gonadotropic, androgenic, estrogenic, and gestagenic hormones appeared to have the quality of tumor markers so that monitoring their serum level together with that of tissue polypeptide antigen, carcinoembryonic antigen, alpha-fetoprotein, and ferritin was useful. These compounds were determined by radioimmunoassay. Their serum concentration was often elevated although a constant pattern of findings or a relation to the severity of the disease was not observed. A corroboration of these results and an elucidation of their pathogenesis are necessary in a large number of patients.

[Malignant hyperthermia - therapy results with dantrolene. A case report].

A 5-year-old previously healthy girl, received general anaesthesia for performing an appendectomy. After administration of succinyl choline (20 mg, twice repeated) and halothane (increasing to 2 per cent by volume), the following symptoms of malignant hyperthermia became manifest during anaesthesia: rigor, tachycardia, cardiac dysrhythmia, temperature increase to 42.6 degrees C; anaesthesia was effected with 2 litres O2/min, 4 litres N2O/min, halothane 1.5-2 per cent by volume, using the Kuhn system. Cooling reduced the temperature at first to 40.6 degrees C and subsequently, with additional intravenous administration of dantrolene (initially rapidly 20 mg equal 1 mg/kg body weight, then 10 mg/kg body weight X 24 hrs) within an hour to 37.5 degrees C. The postoperative phase was uncomplicated. The pattern of symptoms and therapy are critically reviewed. Basing on the cases described in literature, as known to the authors, the value of dantrolene in respect of treatment of malignant hypothermia in man is reviewed.
