Long-term follow-up results of 97 finger replantations.

From a total number of 664 finger replantations (DII-DV) we were able to carry out a follow-up of 97 postoperative cases using the follo-wing assessment criteria: mobility, sensitivity, type of amputation, mechanism of injury, level and extent of amputation. Subjective criteria were also taken into account. Using the results of the study we have been able to formulate an evaluation as to the absolute indication for operation of fingers DII-DV.

The forearm flap.

We present our experimental and clinical experiences with the free neurovascular forearm flap. The flap is based on the radial artery, one of the great veins of the forearm (cephalic, basilic, or interconnecting vein), and one or two cutaneous forearm nerves (ulnar, median, or lateral). Because of the standard anatomy, the large caliber of blood vessels, the good sensory supply, the quality and quantity of the forearm skin, and the thin layer of subcutaneous fat, the free forearm flap is a technically easy and safe flap for reconstruction of soft-tissue defects, especially those in the head and neck and those areas of the extremities where sensitive skin is desired.

Serologic and cellular characterization of products of a new major histocompatibility gene region, RT1.C, of the rat; possible homology to mouse H-2 Qa.

The A region of the major histocompatibility system of the rat, RT1, codes for ubiquitously expressed class I antigens and the RT1.B region determines class II (Ia) molecules. A third region, RT1.C, adjacent to B, has been described recently by histogenetic methods. Data are now presented on the analysis of RT1.C region products by the use of alloantisera and cytotoxic T lymphocytes, both prepared in two, C region congenic strain combinations. C region antigens have been detected serologically by complement-dependent cytotoxicity assays on the majority of B and T lymphocytes, but not on erythrocytes or liver and brain tissue by absorption tests. With the typing sera available only three "alleles" could be identified among 13 different rat strains, so that the RT1.C polymorphism appears to be restricted. Cytotoxic T lymphocytes could be induced against C region products after in vivo immunization and in vitro restimulation; killing was not restricted by the A and B regions. The concept that RT1.C is homologous to H-2 Qa is discussed.

[Rational EDP-basic documentation in the surgical clinic].

The establishment of a clinical basis documentation has approved because of the co-operation with the service-computer-centre of the clinical administration by linking the administrative and clinical data of the patients. The most essential data like diagnosis, operation, complication, operating surgeon as well as the eventual case of death will be registered in a protocol by the ward physician. These data will be registered by a documentary assistant according to the international keys. The evaluation of the data is done by a SPSS-program. The present results demonstrate that this kind of computer assisted clinical documentation has been successful.

[Reimplantation of peripheral parts of the extremities. 808 cases and functional results].

The Authors present 808 replantations by microsurgery in patients aged between 16 months and 70 years. The functional results were controlled in 369 cases after 10 to 20 months. The less satisfactory function was found in patients over 50 years and following combined injuries of the hand. The replantation of the thumb achieved the best results. The Authors point out that, despite a poor prognosis, some replantations should still be attempted because every case needs to be evaluated individually.

[Isolated tumor of the rectum as primary manifestation of Burkitt lymphoma (author's transl)].

Burkitt tumors are malignant lymphoblastic lymphomas occurring predominantly in Africa in children and young adults and involving in many cases the intestinal tract; these tumors occur rather seldom outside Africa. A case report is given of a white 21 old female suffering from this disease; that first manifestation was an isolated ulcerating tumor of the rectum without involvement of other organs of lymph nodes. Diagnosis was confirmed by rectum biopsy. X-ray therapy, cytostatic therapy and amputation of the rectum did not prevent a fast progression of the disease and final death, which is the usual course of the disease when it occurs outside Africa.

[The neurovascular forearm island flap].

We describe a neurovascular island flap of forearm skin based on the radial artery at the wrist level with reversed arterial blood flow. The flap may cover almost any skin and soft tissue defect of the hand. The flap is drained via venous anastomosis between one of the forearm veins and a suitable vein of the hand. Sensation is provided through a microneural anastomosis of one of the sensitive nerves of the forearm to a corresponding nerve of the hand.

Syntheses and hypoglycemic activities of ethyl esters and various amides of omega-guanidino fatty acids with medium chain length.

The omega-guanidino fatty acids C6-C12 were prepared by amidination of the corresponding omega-amino acids. omega-Amino acids C7-C10 which are not available commercially, were obtained by use of Hofmann degradation of the next higher dicarboxylic monoamid monoethyl esters. For use in biological tests, the omega-guanidino fatty acids were converted into ethyl esters, dimethylamides, sometimes also into methyl- or diethylamides. In isolated fat cells these compounds inhibit glucose oxidation. The inhibition increases with increasing chain length. For example, glucose oxidation (control 100%) is diminished by the dimethylamides of 9-guanidinononanoic acid to 70%, of 10-guanidinodecanoic acid to 62%, of 11-guanidinoundecanoic acid to 17% and of 12-guanidinododecanoic acid to 12%. The same compounds -- except the ethyl esters--depress the blood glucose levels in mice after intraperitoneal injection. Blood glucose levels between 30 and 10 mg/100 ml are reached and convulsions are observed. In the mice fall test, ca. 15-30 min post inj., the mice fall down; the blood glucose values of the fallen mice are hypoglycemic. The toxicity of the compounds examined is remarkably high; lethal dose per mouse (20 g) is 3-5 mg for the dimethylamides. It is obvious that a relationship exists between the inhibition of glucose oxidation in adipocytes and the depression of blood glucose level. The stronger the inhibition, the stronger the blood glucose lowering effect.

Insulin-like partial effects of agmatine derivatives in adipocytes.

In previous investigations, we described insulin-like effects of agmatine [(4-aminobutyl)guanidine] in vitro. In the present work we have examined whether these effects of agmatine can be enhanced by variation in chain length (C3 and C5 forms) and by alkylation. Propyl, butyl, pentyl, hexyl, octyl, isobutyl and isopentyl groups were introduced into C4- and C5-agmatine by hydrogenation of the corresponding azomethines. Alkylation of C3-agmatine was carried out by addition of alkylamines to acrylonitrile, followed by hydrogenation and amidination. For the biological assays, isolated fat cells from rat epididymic adipose tissue were used. N4-Butyl- and N4-pentyl-C4-agmatines lead to a two-fold, N4-hexyl-C4-agmatine to a three-fold enhancement of glucose oxidation in adipocytes. Alkylated C4- and C5-agmatines induce a three-fold increase in lipogenesis compared to agmatine. Alkylation of C3-agmatine does not increase its potency in this test. In our test system, insulin decreases adrenalin-induced lipolysis to 40% of the control value (100%). Agmatine and alkylated C4-agmatines yield very similar values (37% and 27-44% respectively). The alkylated C3-agmatines also exert strongly antilipolytic effects (25-35%), while the effects of the alkylated C5-agmatines are weaker. The synthesized agmatine derivatives were injected intraperitoneally into mice. Tolerable doses do not cause any significant reduction in blood glucose levels.

[Parenteral feeding in outpatients. Indications, technic, experiences].

Home parenteral nutrition is indicated in the short bowel syndrome, chronic inflammatory bowel disease and as an adjunct to cancer therapy. For venous access the cannulation of a collateral of the subclavian vein and the central venous silastic catheter have proved helpful. Our own experience with 19 patients shows, that with careful supervision home parenteral nutrition can be performed with low risk.

[Primary transpositions of fingers, results (author's transl)].

12 cases of finger transposition during replantation using microvascular techniques are reported, all of which were performed in the Department of Plastic and Reconstructive Surgery, Klinikum Rechts der Isar der Technischen Universität München (Head of Department: Prof. Dr. med. Ursula Schmidt-Tintemann). In multiple injuries of the hand, it is sometimes necessary to reconstruct fingers depending on their functional importance, when it is not possible to make a precise anatomical reconstruction owing to the destruction of the various amputated parts. The functional results following finger transposition are comparable to the results following 500 finger replantations in our hospital.

[Late results after thumb replantation].

72 cases of thumb amputation and replantation were studied, 50% of which resulted from saw injuries and 20% from avulsions. 64 (88,9%) of the thumbs survived. The functional results in 52 available for review show that 95% achieved a good or satisfactory result. This demonstrates clearly that a thumb amputation is always an absolute indication for a replantation. In a multiple amputation case, where the thumb is destroyed or absent one should do a primary finger transfer. In such cases also we achieved similar good results.

[Late results after surgery in chronic pancreatitis (author's transl)].

122 patients were operated upon because of chronic pancreatitis within 15 1/2 years. Mortality during and immediately after surgery was 9.8%, later mortality was 4.9%. 104 patients were still alive at follow up. In 40 patients most of the pancreatic parenchyma was left during surgery. Late results were good or satisfactory in 67.5%; carbohydrate tolerance was impaired in 35%, and exocrine insufficiency of the pancreas was present in 32.5% of these cases. In another group most of the pancreas was removed; 86% of these patients improved permanentely, 46% had impaired carbohydrate metabolism, 83% exocrine insufficiency of the pancreas. Good results can be achieved, if impaired carbohydrate metabolism, 83% exocrine insufficiency of the pancreas. Good results can be achieved, if indications are carefully choosen. Personality traits of the patients have to be taken into account when choosing the surgical approach. Advantages of a two session surgical approach combined with conservative treatment are discussed.
