[Finger replantation in children].

The possibility of replantation is discussed for children's amputation injuries. The use of this technique is also indicated for multiple amputations and severe crush injuries. More than half of the replanted fingers healed successfully. Since the existence of the Replantation centre of the Klinikum Rechts der Isar, over 600 operations of separated limbs were performed. The healing rate of totally and subtotally separated limbs of adults was 87%. The attempt was made to replant separated digits of 14 children, aged from 1 to 8 years, with microvascular anastomoses. The grade of injuries and the minimal size of anatomic structures demand a particularly high standard of the operative standard. Replantation was also indicated for badly squashed and contaminated limbs.

[Recurrence of Crohn's disease after surgical treatment].

The probability of recurrence was determined in 58 patients after intestinal resection for Crohn's disease using the statistical method of death tables. Within the first 10 years after resection a recurrence must be expected in 84.1% of patients. In 41.6% of cases a second resection becomes necessary. Age, sex, and disease duration preoperatively had no influence on the recurrence. Curative treatment of Crohn's disease is not possible by surgery and the indication for operation should thus be limited primarily to life-threatening complications.

[The effect of methylprednisolone on the course of the experimental tourniquet-shock in dogs (author's transl)].

In experiments in dogs rubber tourniquets were tied around both hind limbs as close to the hip as possible, and a 4-h acute ischemia was induced. With the removal of the tourniquets the tourniquet-shock was produced. In a control group no therapy of the shock was carried out. The animals were observed for 6 h. In a second group, methylprednisolone 40 mg/kg BW was given immediately after revascularisation. In a third group, methylprednisolone 40 mg/kg was given prophylactically 10 min before the tourniquets were applied and therapeutically as soon as the blood circulation was opened. Hemodynamic parameters and renal function were measured to examine the pharmacological effect of methylprednisolone in acute tourniquet-shock. The mortality rate in the control group was 90%, while it was 50% by therapeutic and/or prophylactic application of methylprednisolone. The prophylactic application of methylprednisolone reduced development of metabolic acidosis, hemoconcentration, and hyperkaliemia during ischemia so that a mitigated course of shock was observed.

Successful replantation of a totally avulsed scalp.

A case of a totally avulsed scalp was replanted successfully by microvascular anastomosis. Though the avulsed part was heavily crushed, which resulted in a central wound, the appearance of a "necrotic" area, haematomas and persistent serous exudation after the restoration of circulation, the whole scalp survived.

[Islet cell carcinoma causing Verner-Morrison syndrome (author's transl)].

Predominant symptoms of the Verner-Morrison syndrome are diarrhea and hypokalemia; further symptoms can be impaired carbohydrate tolerance, hypercalcemia distension of the intestine and skin alterations. Diagnosis can be confirmed by computer tomography, angiography and demonstration of increased plasma levels of VIP and GIP. This syndrome is due to islet cell carcinoma of the pancreas in about one third of all cases observed. Total duodeno-pancreatectomy has to be performed, if surgery is possible at all. Patients who cannot be operated upon may receive combined cytostatic therapy.

[Dye dilution test to determine leakage during regional perfusion in the treatment of malignant tumors of the extremities].

During regional perfusion of the extremity in the dog, the dye Evans blue (T 1824) was used as an indicator to control leakage from the isolated region into the systemic circulation. The rate of elimination of the dye was 10% +/- 2% per hour. A simulated leakage from the perfused region into the systemic circulation was determined with a sensitivity of 1,67%.

[Experiences with replantation of large limbs].

From November 1975 to August 1978 26 limb replantations were performed. All 5 lower extremities had to be reamputated. Of the 21 replanted arms 15 healed without major complications, 9 have regained sufficient function up to now; 3 others show signs of reinnervation.

Early experiences in organising and running a replantation service.

In the 10 months since the establishment of a 24-hour replantation service, the centre at the Klinikum rechts der Isar of the Technical University of Munich has treated 162 amputation injuries; 132 underwent replantation surgery and 117 of these healed successfully. Surgical technique, medication and postoperative treatment with physiotherapy are described. Special emphasis is placed on the difficulties associated with crush and avulsion injuries; in most of them the only solution is an intermediate vein graft to bridge the defect in the vessels.

[Comparative study of energy metabolism of the skeletal muscles in man, dogs and rats during long-term ischemia].

A comparative investigation was made on muscle tissue of man, dog and rat during ischemia of five hours duration. The content of energy rich phosphates was measured. The ATP level decreased after two hours ischemia to 68% in man, to 45% in the dog, and to 23% in the rat. By five hours it had fallen further to 25% in man, 9% in the dog and only 1% in the rat. It is concluded that human muscle has a higher tolerance to ischemia.

[Preoperative reduction of intestinal bacteria through orthograde lavage of the large intestine].

The orthograde colon lavage, suggested by Crapp, is a new method for the preoperative preparation of the colon. On the day prior to surgery, the sitting patient receives for 4 h through a stomach probe, 101 of physiological saline solution warmed up to 37 degrees C. The result is a complete evacuation of the bowel. If a bactericidal antibiotic with a broad spectrum of activity (neomycin) is added, the suppression of the microflora amounts to 92%. The regeneration of the intestinal flora could be observed after a few days. A complete evacuation of the colon is guaranteed by this procedure, so that all operations can be performed under optimal conditions. From the clinical as well as from the microbiological point of view this method is without any risk. -- The orthograde colon lavage is indicated in every case of colon, rectum and anal surgery, but it is contraindicated in patients with highly stenosing tumors.
