[Late results in transplantation of free or pedicled radical flaps].

The free and pedicled forearm flap is well suited to cover large skin defects on the head, neck and extremities. It offers advantages in terms of size, shape, colour, skin thickness and a wide range of usage with combined tissue transfer. 31 patients with free or pedicled flaps were reexamined. The parameters of reexamination were blood flow, pulse, capillary return, and colour of skin. In half of the cases there was no restriction of blood flow; only one parameter showed signs of less blood flow. Every flap revealed pulse and good capillary return. Esthetic parameters were colour, hairgrowth and skin niveau differences. In 22 patients the results were esthetically satisfactory. The donor area was evaluated with respect to hand circulation in which the selected parameters were cold sensitivity, claudication, and the results of thermography.

[Microvascular anastomosis of the iliac crest transplant in mandibular reconstruction].

In 12 patients with extensive lower jaw defects following tumor removal or after radioosteonecrosis, a free osteocutaneous iliac crest flap with microvascular anastomoses was used to reconstruct the lower jaw in a single session. The results are reported in brief for this group, in many of whom conditions were not optimal for transplantation because of previous surgery, infection, or radiological tissue damage.

[Capsule fibrosis in silicone implants].

The contracture of soft tissue capsule, appearing after implantation of all types of breast prosthesis, is the central problem of interest in the augmented breast. Clinical results were graduated in the Baker classification according to the degree of severity. Frequency rate depends on time lapse between implantation and post-examination, definition of capsular contracture, number of all patients and type of augmentation. Various factors are assumed to be causes of capsular contracture; i.e. haematoma, foreign body reaction, subclinical infections and physical-geometrical characteristics of prosthesis. It is possible to influence fibrosis with steroids and antibiotics.

[Plastic surgery measures in connection with bone transplantation in bone and soft tissue defects].

Free tissue transfer by means of microvascular anastomoses has opened up new dimensions in reconstructive surgery. However, a free microvascular myocutaneous flap should only be used, if a local pedicled flap is impossible. In treatment of complicated soft-tissue and bone defects, there are clear-cut and gradual therapeutic steps: wound debridement; soft tissue repair; bone reconstruction.

[Perioperative high-caloric alimentation with the central venous catheter. Prospective study in 404 patients].

Peri-operative high-calorie nutrition was administered, through a total of 500 catheters introduced into the superior vena cava via the subclavian vein, to 404 patients admitted to hospital for gastro-intestinal cancer resection or other major abdominal operations, some of them taking a complicated course. Most of the catheters remained in situ for 6-20 days; the longest period was 44 days. Catheter insertion was successful in 97.8%. Faulty position of the catheter occurred in 4.4%; in 2.8% it was rectified under fluoroscopic control. In 92.2% there were no complications ascribable to the catheter. A pneumothorax resulted in 1.4% of patients, central thrombosis in 0.4%. Catheter-related sepsis was noted in 6.2%.

[Improvement of suture technic in flexor tendon injuries. Clinical and experimental study].

Multiple causes for the rupture of sutured tendons have been reported. At the time of exploration, loosening of the knot and breakage of the suture have been found to explain failure. A knot with four throws is frequently mentioned in various series as the smallest secure knot which can be made when employing a braided polyester suture material. Experiments have shown that the distance between the ends of the tendons affects the healing process and the tensile strength of the injured tendons. In order to avoid increasing the gap between the sutured ends of the tendons with the thick knot, it can be placed away from the cut surface of the tendons and displaced into the proximal or distal end of the tendons with a small additional incision. Through a longitudinal incision, the suture is easily passed in a criss cross fashion through the tendon and the knot can be buried in the tendon. This technique assures a very delicate and exact coaptation of the tendon ends. Almost 100% of the tendons remain intact during the healing phase. According to this experience, the risk of tendon rupture is reduced as a result of an increase in the tensile strength of the suture and the displacement of the knot away from the cut surface of the tendon.

[Results of the surgical treatment of gallbladder carcinomas].

A retrospective study of 44 patients with carcinoma of the gallbladder is presented. It is noticeable that although the prognosis for advanced carcinoma of the gallbladder is poor, that of the early stages (stage I-III) is relatively better. In no case diagnosis was made preoperatively. In 86% gallstones were found. There has been no valid staging for tumors of the gallbladder or any generally accepted standardized therapy. Attention is drawn to the possibility of improving the results of therapy at the early stages by extended radical operation and to the necessity for early operative treatment of gallstone disease.

[Sensory transplants of the sole of the foot].

The covering of defects of the sole of the foot and the heel with pedicled and free neurovascular flaps such as the dorsalis pedis flap, the forearm flap and the pulp neurovascular flap shows good success because of early pain sensation re-ulceration can be prevented. Clinical cases are demonstrated.

[Forearm flap with its various possibilities of application].

The authors describe the different possibilities of the forearm flap as a free and pedicle flap. The forearm flap is arterialized by the A. radialis, the venous outflow goes through the Venae comitantes of the A. radialis or the big veins of the forearm (V. cephalica and basilica). Sensation can be preserved through the three cutaneous forearm nerves. The advantage of the flap is based on the constant anatomy, the big vessels and nerves, the quality and quantity of the forearm skin as well as the relatively thin layer of subcutaneous fat.

Locus of control and depth of processing in children.

A children's locus of control (LOC) measure was administered to 153 sixth-grade students who were subsequently assigned to one of four processing conditions. Groups were instructed to encode a word list semantically, acoustically, orthographically, or any way the S wished and to retain them for a later recognition test. Criterion variables included total number of words correctly recognized as well as four varieties of distractor choices. Support was provided for the contention that externals tend to encode stimuli in more superficial ways than do internals, although the amount of variance in recognition memory accounted for by the LOC measure was small. Encoding conditions had a marked effect on retention, and false distractor choices appeared to reflect the level at which stimulus words were encoded.

[Thumb replacement by microsurgery].

Our experiences with 11 heterotopic finger replantations and 18 cases of free second toe transfer for thumb reconstruction are reported. The wrap-around flap technique (Morrison) is also described.

Rat major histocompatibility RT1.C antigens of restricted tissue distribution consist of two polypeptide chains with molecular weights of about 42 000 and 12 500.

The RT1.C region has recently been defined genetically as a third region of the rat major histocompatibility system, RT1, and has been shown to code for cell-surface antigens with restricted tissue distribution (present on lymphocytes and absent from red blood cells and liver) and for target antigens of unrestricted cytotoxic T lymphocytes. Immunoprecipitation with C-antigen-specific alloantisera and SDS-PAGE analysis, reveal that C-region products are glycoproteins composed of molecules of 40 500-43 000 and 12 500 molecular weight, respectively. Thus, the RT1.B region, which codes for class II molecules, is flanked by RT1 regions which determine class I molecules (as defined biochemically), which are either expressed ubiquitously (RT1.A antigens) or in a restricted manner (RT1.C products). The homology to H-2Qa antigens is discussed.
