[New and changed guidelines in the TNM system of head and neck tumors].

Starting on the 1st of January 1987, the 4th edition of the UICC classification of the TNM-system is to be applied. Only slight changes have been made in the rules for the larynx and the skin, whereas the classification of the nodal status has undergone a more profound change. New rules have been established for the classification of salivary gland and maxillary sinus tumours, including a stage grouping for the salivary glands. The R-classification is new and changes have been made in the histological grading. The head and neck part of the edition is presented. The classification for the maxillary sinuses is compared to a personal system covering all the sinuses and the nose: its usefulness has been proven by successful documentation of 1224 cases.

[Initial experiences with interstitial brachycurietherapy (low dose rate I-125 seeds in carrier/vicryl and high dose rate Ir-192 afterloading system) in the palliative treatment of head and neck tumors].

The local control rates for previously irradiated recurrent tumours of the head and neck is dose dependent. High dose percutaneous irradiation alone is associated with a high complication rate. On the other hand it is possible to apply high local doses by interstitial irradiation, whilst sparing the surrounding tissue. In the last 2 years we have used Iodine 125-seeds in carrier (Vicryl) and a high dose rate Iridium 192-source for the afterloading technique. Our first experiences with 12 patients show reasonable palliation, but not effect on survival.

[Traumatology of the midface--diagnostic and therapeutic guidelines for the practicing ENT physician from the viewpoint of the otorhinolaryngologist].

Middle face fractures are classified as follows: fractures of the nose, maxillary fractures (vertical and transverse, Le Fort I-III), lateral middle face fractures, isolated orbital floor fractures (blow out fractures), isolated zygoma fractures and combined multiple fractures. Rhinosurgical indications are discussed with emphasis on osteosynthesis using wire and miniplates.

[Instructions for the documentation of neoplasms in the area of the head and neck following the TNM system. Contribution to the validation of the TNM system].

As the only international classification scheme the TNM-system allows classification of practically all tumours of all organs according to uniform rules. In the interest of international comparison of findings and results the TNM-system alone can be recommended for classification. Checklists are used for extension of the basic documentation for the head and neck. Directions for initial and follow up analysis are given. The TNM-system in this proposed practice allows clear determination of the pre-therapeutic findings, recurrences to be taken into account, the course of the disease to be recorded, and the survival rate to be calculated. The data are suitable for recording on a computer. Finally uniform recommendations for treatment may be deduced.

The use of computerized axial tomography versus peritoneal lavage in the evaluation of blunt abdominal trauma: a prospective study.

Computerized axial tomography (CAT) of the abdomen was prospectively compared with peritoneal lavage (PL) in 19 patients who sustained acute blunt abdominal trauma. All were selected carefully and were deemed stable, never having been in shock, and all required diagnostic PL. Seven patients underwent exploratory laparotomy on the basis of CAT and PL findings. There were no negative findings at laparotomy. Three significant splenic injuries, one hepatic laceration, and two hemoperitoneums were undetected by CAT. All seven cases explored were preceded by a grossly positive PL. Thus no major injury would have been missed if PL had been used alone. There were no complications of PL but one patient aspirated oral contrast medium and one patient developed hypotension during CAT. Open PL required one half the time (20.6 minutes) as CAT (47.4 minutes). The total cost of CAT was approximately eight times that of PL ($900.01 versus $116.38). In our hands, PL would seem to be significantly more sensitive with fewer false negative results than CAT of the abdomen in acute blunt abdominal trauma, a deviation from results of earlier reported series. CAT alone would have added cost, time, some risk, and very little information of use that would not be obtained by PL followed by surgery. Therefore before CAT replaces PL in the evaluation of adult patients with blunt abdominal trauma, we feel that additional prospective studies are needed to better define the accuracy and sensitivity of CAT as compared with PL with regard to intraperitoneal injuries.

[What is new in plastic surgery of the head and neck for the practicing otorhinolaryngologist?].

On examples the plastic facial surgery is shown as an multidisciplinary territory, whereby main points are set. Most important for the oto-rhino-laryngology is the ear and nose correction as whereas the possibility to use collagen injections, the problems of secondary Rhinoplastics, the esthetic corrections in the area of the throat and head (dermabrasio, eye lid corrections, face lifting, hair transplantation) and considerations about education and advanced training. The maxillo facial surgery puts out the dento-alveolic dysgnathies, the ophthalmology the eye lid corrections. The experimental surgery can contribute to the still open questions, whether in the future in plastic operations allogene transplantates can fill the place of autogenous material or not.

[Head and neck cancers. What's new in clinical oncology for the practicing ENT physician?].

An interdisciplinary evaluation is made of the most common forms of head and neck cancer, with emphasis on certain aspects.--In the field of x-ray diagnostics, computed tomography dominates varying importance being given to the individual organs. Nuclear medicine pays particular attention to skeletal scintigraphy and gallium scintigraphy. In radiation therapy, progress has been made especially through the use of optimal radiation planning as well as the use of the after-loading technique and neutron radiation. Antineoplastic chemotherapy is currently being used as palliative treatment, adjuvant chemotherapy and as the first step towards a curative therapy.

[Diagnosis of rhabdomyolysis in alcoholism].

Alcoholic rhabdomyolysis is only considered to be such a rare disease, because it is often not thought of. When increasing CPK and creatinine levels are found a muscle biopsy should be taken despite a sometimes not typical anamnesis. This is illustrated by the presentation of a case report. Early diagnosis and therapy should make it possible to lower the mortality rate of this disease.

[Influence of sport on the cardiovascular system in pregnancy (author's transl)].

Stroke volume and cardiac output were determined in 24 pregnant women at the end of the first and second trimenon. There was a decrease of cardiac output in 7 women (29%) and among these a decrease of stroke volume in 6 women (25%). In this group there were especially untrained women with a high incidence of "health complications" during pregnancy.
