[The behavior of trypsin-like proteases in sulcus fluid].

The present study shows results aimed at determining whether levels of a trypsin-like protease in gingival crevicular fluid are associated with disease activity as assessed by the level of gingival inflammation and probing attachment loss. Maximum enzyme level was significantly elevated by groups with gingival inflammation and periodontal destruction. In a long-time study we find a decrease of enzyme activity with clinical therapy by scaling and root planning.

Effect of the new immunostimulator HAB 439 on cell-mediated immunity against intracellular bacteria.

The isoxazoline derivative HAB 439 was tested for its enzyme inhibiting potency and was found to be an inhibitor of aminopeptidase B (IC50 = 22.5 micrograms/ml). In further immunopharmacological experiments its efficacy to stimulate cell-mediated immunity was evaluated. HAB 439 was shown to stimulate DTH-reaction against Salmonella typhimurium and Listeria monocytogenes. HAB 439 protected animals against infection by reducing the bacterial load in livers and spleens and by decreasing the mortality rate. Treatment with the antibiotic ampicillin induced a decreased DTH-reaction in mice which was demonstrated to be due to a reduction of the antigen to be presented to the immune system and not to immune suppression. HAB 439 restored the impaired immune response to S. typhimurium and L. monocytogenes in a dose-dependent way. Restoration of DTH was shown to lead to an improvement of protection in ampicillin-treated mice which were challenged with the intracellular bacteria.

Energy metabolism of lactating dairy cows treated with prolonged-release bovine somatotropin or energy deficiency.

Two experiments were carried out with lactating Simmental x Red Holstein Friesian dairy cows. In the first experiment, 8 cows each received six injections at 4-wk intervals of either saline or 640 mg of prolonged-release bST. In the second experiment, 14 of 20 cows received 20 MJ NEL less than their daily requirement for 3 wk. Digestible and metabolizable energy were not affected by the treatments. Mobilization of body energy compensated the bST-induced increase of milk energy and supported milk production of underfed cows by two-thirds of the net energy deficit. The utilization of mobilized body energy and metabolizable energy from the diet for maintenance and milk production was unchanged. The bST and energy deficiency increased N utilization for milk production, because bST increased milk N secretion at the same N intake, and energy deficiency partly compensated the reduced N intake by N mobilization. Milk yield oscillated between the bST injections. Mean milk yield increased by 2.8 kg at constant milk composition. During energy deficit, milk yield dropped by 1.7 kg with decreasing milk protein content.

[Initial experience with a new blood pump].

A new type of blood pump was tested in calves for 6 hours. The pump consists of a rigid housing with a trochoidal internal surface, an inlet and outlet, and two lateral walls. A two-corner piston rotating on an eccentric shaft, describes a trochoidal path, thus creating a gap seal, the gap measuring a constant 10-35 microns. The pump is driven by a watercooled DC motor. For right ventricular assist, a cannula is inserted into the right ventricle through the right atrium, and into the left ventricle for left ventricular assists. From a total of 10 experiments, two left ventricular assists, two right ventricular assists, and three biventricular assists were evaluated. The pump produced a pulsatile flow of 31 at 70 rpm. Energy requirements were 2.19 watts for left, 2.06 for right, and 7.26 for biventricular assists. Plasma hemoglobin remained as low as 10 mg/dl during monoventricular, and increased during biventricular assists to 20 mg/dl after 3 hours, and returned to 16 mg/dl after 6 hours. From these preliminary results it is concluded that this new rotary blood pump may be suitable as a circulatory assist device.

[Classification and documentation of malignoma of the inner nose and paranasal sinuses].

In the fourth edition of the UICC guidelines for TNM classification (valid since January 1st, 1987), the organ "maxillary sinus" has been indicated for the first time within the category "head and neck". This decision made by UICC to consider the maxillary sinus alone is insufficient and contradictory--the reasons are discussed. The AO (Working Group for Clinical Oncology)/ORL will therefore maintain its classification considering the functional unity of the organ "inner nose and paranasal sinuses", which has proved its great worth in 1363 cases treated during the period from 1972 through 1987. There is no reason to abandon the classification of malignomas of the inner nose and the paranasal sinuses and to replace it by a UICC guideline only valid for carcinomas of the maxillary sinus which cannot convince. The practicability of classification and documentation has certainly be improved since the introduction of organ-specific questionnaires (first ascertainment, subsequent ascertainment).

Pelvic crush injuries with occlusion of the iliac artery.

During a 1-year period, three patients presented with acute traumatic thrombosis of the common or external iliac artery concomitant with a massive crush injury to the pelvis. All had vascular compromise of the involved extremity on initial physical examination, but in two patients with open pelvic trauma, exsanguination, major visceral injury, hypothermia, and a coagulopathy precluded emergency vascular reconstruction. Both required hindquarter amputation for adequate debridement. The third patient presented without exsanguination or visceral trauma. Angiography and vascular reconstruction were undertaken, but myonecrosis compounded the initial vascular compromise, and eventually required a hip disarticulation for debridement. It was concluded that: exsanguination and/or major visceral injury takes priority over emergency vascular reconstruction; soft-tissue injury may preclude limb salvage despite vascular reconstruction. If a cadaveric limb exists, early radical amputation, including hindquarter amputation, should be undertaken.

Synthesis and structure-activity relationships in the cefpirome series. I. 7-[2-(2-Aminothiazol-4-yl)-2-(Z)-oxyiminoacetamido]-3-[(su bstituted-1-pyridinio)methyl]ceph-3-em-4-carboxylate s.

7-[2-(2-Aminothiazol-4-yl)-2-(Z)-oxyiminoacetamido]-3-[(s ubs tituted-1-pyridinio)methyl]ceph-3-em-4-carboxylates II are a group of beta-lactam antibiotics with extraordinary high antibacterial activity. The promising member of this group, cefpirome (HR 810, II-1) is a candidate for clinical use. Synthetic pathways to II starting from cefotaxime derivatives I or 7-aminocephalosporanic acid (7-ACA) are described. A preferred method for the conversion of I to II or 7-ACA to precursors III respectively employs iodotrimethylsilane and an excess of the pyridine base. Structure-activity studies reveal an optimum overall activity in the series of pyridines with fused saturated and unsaturated rings or cyclopropyl- and alkoxy substituents. Favorable oxyimino substituents are methyl, ethyl, difluoromethyl and carbamoylmethyl groups. Acidic substituents lead to decreased activity against Staphylococcus aureus SG 511. Introduction of halogen in the thiazole nucleus causes improvement of activity against the K1 beta-lactamase producing Klebsiella aerogenes 1082 E strain.

Synthesis and structure-activity relationships in the cefpirome series. II. Analogues of cefpirome with different 7-heteroarylacetamido and 3'-ammonium substituents.

The synthesis and antibacterial activity in vitro of 7-(2-heteroarylacetamido)-3-[(2,3- cyclopentenopyridinium)methyl]cephalosporins and of some related compounds with different ammonium functions in 3'-position are described. The 7-[5-amino-1,2,4-thiadiazol-3-yl] and the 7-[4-aminopyrimidin-2-yl] analogues of cefpirome and compounds with 3-aliphatic ammoniummethyl functions have excellent antibacterial activity. Cephalosporins with different N-heterocycles other than pyridine in 3'-position are less active than their 3-pyridiniummethyl analogues. Attachment of a pyridinium group to a cephem at C-3 via a thiomethyl or an aminomethyl bridge causes reduction of antibacterial activity.

[Report of experiences (1974-1986) in classification and site distribution of malignant tumors of the organs of the inner nose and paranasal sinuses. A retrospective review of over 1,296 cases].

The 4th edition of the UICC classification of the TNM system came into force on 1st January 1987. New rules have been established for the classification of maxillary sinus tumors. We have compared these to the classification system used by the AO/HNO for all the sinuses and the nose. From a review of the world literature and analysis of 1296 patients registered in the years 1974 to 1986, it became clear that less than 60% of all malignant tumors of the nasal cavity and sinuses are primarily located in the maxillary sinuses. Furthermore, the primary site is often hard to define because of the high rate of T3 and T4 tumors (approx. 70%).
