[Secondary hyperlipoproteinemias].

In a survey report pathological aspects and clinical importance of the secondary forms of hyperlipoproteinaemia are dealt with. At first are taken into consideration the relations between hyperlipoproteinaemia and diabetes mellitus. Furthermore is referred to the secondary hyperlipoproteinaemias in kidney diseases, pancreatitis, alcoholism, thesaurismoses, endocrine diseases and liver diseases as well as under different medicaments. Finally the therapeutic problems of the secondary hyperlipoproteinaemias are concerned.

[Lipoproteins and fatty acids--hyperlipoproteinemia and arteriosclerosis].

At first on the basis of literature a short description of the interrelations between lipoprotein metabolism and fatty acid metabolism as well as hyperlipoproteinaemia on the one hand and arteriosclerotic-degenerative diseases of the coronary vessels on the other is given. Finally, the findings of gas-chromatographic analyses of the patterns of fatty acids of the cholesterol ester and triglyceride fractions of the serum of 252 patients with primary hyperlipoproteinaemia are represented. In contrast to persons with healthy metabolism increase of the proportions per cent of palmitic acid, stearic acid, palmitoleic acid, oleic acid, and eikosatrienic acid was shown and decrease of those ones of linoleic acid, linolenic acid, arachidonic acid, and eikosapentaic acid in the cholesterol ester fractions in hyperlipoproteinaemia type IIa and IIb and in the triglyceride fractions in hyperlipoproteinaemia type IIb and IV. The shifts within the patterns of fatty acids in hyperlipoproteinaemia are an expression of an atherogenic risk. Disturbances of the hepatogenic synthesis of fatty acids and oxydation of fatty acids, increased reesterification from the depot fat of mobilised fatty acids as well as a partially increased LCAT-activity are causally discussed.

[Constellation of lipoproteins in patients with hyperlipoproteinemia and in rabbits fed a cholesterol diet with reference to vascular permeability].

By feeding of rabbits with cholesterol a hyperlipoproteinaemia (HLP) was created and the changes of the cholesterol and triglyceride (TGL) level were compared with the changes of the portions of the pre-beta- and beta-lipoproteins. The electrophoresis with a gel of agar-agarose are more suitable for the densitometric evaluation than electrophoresis with cellulose acetate films. Tests with Evan's blue of the angiopermeability of rabbits feeded with cholesterol showed that the permeability of the cutaneous vessels decreased strikingly during test period, while a change of the permeability of the aorta was not to be proved under the conditions of our tests. HLP patients had been treated with clofibrinic acid, nicotinic acid derivatives and metabolism basic diet for 3 years. The cholesterol and TGL values as well as the pre-beta-/beta-lipoprotein quotients have been determined before and after treatment. The pre-beta-/beta-lipoprotein quotient has proved as suitable complementary parameter for pursuit of the therapeutic result with HLP patients. The changes of the quotients corresponded well with the found changes of the HLP types. The decrease of the TGL content of HLP patients of the type IV or IIb being treated with metabolism basic diet or clofibrinic acid may be estimated (with nearly constant cholesterol level) from the per cent decrease of the pre-beta-/beta-quotient.

[Effects of various modes of therapy on the fatty acid spectrum in hyperlipoproteinemia].

Gas-chromatographic investigations of the fatty acid pattern of the fractions of serum triglyceride and serum cholesterol ester revealed characteristic changes under different therapeutic measures in altogether 120 patients with primary hyperlipoproteinaemia of the types IIa, IIb and IV. Already under the treatment with basal metabolic diet, more clearly by medication with clofibrin acid, in the hyperlipoproteinaemia type IV the decrease of the palmitic acid, the palmitoleic acid, the stearic acid and oleic acid and the increase of linoleic acid, linoleneic acid, arachidonic acid and eikopentaenic acid in the triglyceride fraction developed. These fatty acids showed the same behaviour under the therapy with derivatives of nicotinic acids in the cholesterol ester fraction in the hyperlipoproteinaemia of type IIa. The combined treatment of the type IIb resulted in the same tendencies, however not so pronounced, of the spectrum in the cholesterol esters as well as of the fatty acids esterified in the triglycerides.

[Epidermiologic studies on nutritional behavior of adipose subjects].

In 1,000 obese persons and 250 persons with normal weight detailed anamneses of nutrition concerning the average daily intake of calories, carbohydrates, protein, fats, cholesterol, saturated and manifold unsaturated fatty acids were made. Obese persons ingested less food than persons with normal weight, - findings which on the one hand are explained with the different habits of eating in the dynamic phase (the hyperalimentation takes place above all in this phase) and the stationary one of obesity, on the other hand a further differentiation in hypophages, normophages and hyperphages should be performed. Finally frequency and structure of the meals are of importance. Tendencies of age did not occur in females, whereas all nutrients in males, beginning with the fourth decade of life, were ingested in a smaller degree than in adolescence. Longitudinal studies on the behaviour concerning nutrition are desirable.

[Differential diagnosis of malabsorption syndrome].

On the basis of two cases the development of a severe postoperative malabsorption syndrome after resection of the stomach with predisposition to a gastroenterostomia ileoterminalis. The fatal course in one of the patients is made the occasion to discuss the causes of the operatively conditioned failures and to demonstrate the symptomatology of a gastrointestinal anastomosis with a lower ileum loop. The duration of the disease can be retarded by insufficient examination technique. In postoperative distrubances of the passage as soon as possible the radiological control of the condition of the anastomosis including the whole samll intestine should be carried out.

[On the relations between overweight and age (author's transl)].

There are reported new results on pathogenesis, etiology, and classification of adiposity under consideration of the age. The examinations show, that the postulated hyperphagia of the adipose person cannot be demonstrated so simply, therefore it is necessary to make longitudinal studies in further researches in this field. There are given simple directive rules for classification of the adipose persons to overcome the common uncertainly in this regard. New studies on the motivation of adipose persons for visiting a adiposity dispensary show an age dependence, in which in younger persons mostly esthetic causes, in the elderly sanitary causes are important.

[Medicinal reduction of mechanical hemolysis].

The mechanical resistance of erythrocytes after gas dispersion (oxygen, resp. mixed oxygen/carbon dioxide) and treatment in the hemoresistometer was studied on human conserved fresh blood under in-vitro conditions. Xylocitin (Lidocain) was tested as to memebrane stabilizing capability. It was found that test doses of about 5 mg and 40 mg per 100 ml of heparinized fresh blood result in an increased resistance of erythrocytes against mechanical trauma.

[Magnesium level in the serum and erythrocytes during pregnancy, delivery and puerperium].

Complexometric determinations of serum magnesium levels, moreover partly of Mg concentrations in whole blood, and withal determinations of hematocrit in 489 healthy pregnancies, parturients, puerperants (1 to 10 days post partum), and nonpregnant women were performed. By aid of these values we were able to calculate Mg concentrations in 100 ml erythrocytes (Mg E 100). The apparent decrease of serum Mg during pregnancy may be founded according to the changes of the plasma volume. However, actually the total (absolute) ammount of serum Mg is increased in pregnancy. We suppose some influences on serum Mg due to stimulation of the thyroid gland in pregnancy and further those of Mg being released from the muscular cells of uterus those been destroyed after delivery. The increased erythrocyte Mg during pregnancy and post partum confirm the view that there are no corresponding relations between Mg concentrations of serum and those of erythrocytes. The possible causes of the increase of erythrocyte Mg in pregnancy are discussed in detail.

[Primary hyperlipoproteinemia and cardiovascular diseases in relation to age].

The primary hyperlipoproteinaemia is a frequently appearing disease of the second half of life. Out of 188 patients with hyperliporpoteinaemia 19% were at an age of 40-49 years, 33% at an age of 50-59 years and 51% of 60-69 years. The type IV of hyperlipoproteinaemia was the most frequent one (19%), in the second place was type IIa (21%) and in the third place was type IIb (14%). In the 5th decade of life type IV prevailed in males and type IIa in females, in the 6th and 7th decade of life nearly the same percentage for these types was to be found in both sexes. In 60% of the patients with hyperlipoproteinaemia we found a hypertension, in males in 20% coronary insufficiency appeared already in the 4th decade of life, then it rose to 70%, in females these appearances occurred about 10 years later. 9% of the patients fell ill from myocardial infarction. A general arteriosclerosis appeared in 63% of the males and in 89% of the females beginning with the 5th decade of life, then it still increased. Therefore, adequate preventive and curative measures (diet and pharmaca) are necessary in time.

[The densitometric evaluation of lipoprotein electrophoresis on acetate foil and agar-agarose gel].

By means of cholesterol feeding on rabbits the change of the beta- and pre-beta-lipoprotein relations is observed by densitometric evaluation of lipoprotein electrophoreses on cellulose acetate foils and in agar-agarose gel. At the same time the changes of the cholesterol and triglyceride content of the sera are determined. The agar-agarose-gel electrophoreses prove to be more suitable for the densitometric evaluation compared with the electrophoreses on cellulose acetate foils, since they give more exact results. A reason for this is the worse demarcability of the pre-beta-lipoprotein proportion in the curves of the densitometre of acetate foils. The percental distributions of lipoprotein in patients of different types of HLP are determined and with the help of the results the evidence of the visual judgment of the lipoprotein electrophoreses is discussed.

Differences in the demonstration of C3 polymorphism in high-voltage gel electrophoresis depending on agarose quality. Investigations of rare C3 variants in Behring agarose AGS 082.

Different batches of agarose may lead to migration differences in the demonstration of C3 polymorphism in high-voltage gel electrophoresis. We studied a particular batch (i.e. AGS 082) which caused two basic phenomena concerning mobility and electrophoretic position of C3 phenotypes: 1. C3 variants migrate to the cathode, apparently due to a positive net charge of the protein caused by this agarose batch. 2. The relative mobility of slow and fast bands is virtually reversed. Previously obtained results are controlled and verified by further investigations on rare C3 variants. The possible causes of the net charge differences of the C3 proteins in various agarose batches are discussed.
