[Effect of combination therapy of clofibric acid and nicotinic acid derivatives on fatty acid metabolism in hyperlipoproteinemia].

71 patients (42 males and 29 females at the age of 45-76 years) with primary hyperlipoproteinaemia were treated with clofibrin acid and derivatives of nicotinic acid during 3 years. Gas-chromatographic analyses of the composition of fatty acids of their serum cholesterol ester and triglyceride fractions showed an increase of linoleic, linolenic, arachidonic, eicosapentaenic acid as well as a decrease of palmitinic, palmitoleinic, stearic, oleic and eicosatrienic acid under treatment. The changes were more efficient than under monotherapy. Selective competitive inhibition of unesterified fatty acid, blocking of lipolysis, influence on the hepatogenic metabolism of fatty acids and on the LCAT as well as better utilization of the alimentary repeatedly unsaturated fatty acids were discussed as possible mechanism. The increase of the polyunsaturated fatty acids with their interrelations to the prostaglandin metabolism must be seen in the sense of a vasoprotective effect.

[Trace elements and lipid parameters in arteriosclerotic circulatory disorders by polyene fatty acids].

In 106 patients with peripheral arteriosclerotic obturating disease low values of serum magnesium as well as an increase of the relation of vasoaggressive to vasoprotective lipoproteins could be established. Under a polyene fatty acid diet (40 ml sunflower oil or linseed oil a day) the serum copper values significantly decreased, whereas of the indices of arteriosclerosis only in the linseed oil group of apolipoprotein-B/A-quotient, in the sunflower oil group the triglyceride level decreased statistically significantly. Therefore the favourable influences of physical conditioning as well as of pharmacotherapy on the disturbed lipoprotein metabolism of patients with vascular diseases should additionally be used.

[Modification of special blood-level forming lipid metabolites by long-distance running in men 20-60 years of age].

The lipid metabolism and the lipoprotein metabolism was examined in 44 long-distance runners (people's athletics) and a group of 65 untrained persons as well as in 32 runners before and immediately after a 38-km and a 40-km-run, respectively. High density lipoprotein cholesterol (HDLC) was significantly increased in the runners, whereas very low density lipoprotein cholesterol (VLDLC) was found decreased. The quotient LDLC/HDLC (quotient of arteriosclerosis) is clearly decreased in runners. Total cholesterol and low density lipoprotein cholesterol (LDLC) of runners (people's athletics) and untrained persons were statistically not different. The post-heparin-lipase (pHL) and the rate of lipolysis pHL appeared clearly increased in persons participating in people's athletics. There were parallelities between the lipid values and the disc-electrophoretically established percental proportions of lipoproteins. The proportion of alpha-lipoprotein (Lp) of the runners was increased, the beta-Lp unchanged and the pre-beta-Lp decreased. The increase of the alpha-Lp was concomitant with a shift of the spectre of the alpha-Lp-subfractions. After the running (38 and 40 km) the triglycerides and VLDLC were increased, the pHL-activity and the rate of lipolysis pHL were decreased. In the other cholesterol parameters no significant shifts appeared after load. While Apo A does not change under the influence of the running, like HDLC, Apo B clearly increased. The results were discussed, taking into consideration the results of other authors.

[Clofibrate therapy--the status quo].

Clofibrinic acid (Regadrin) is the effective substance of clofibrate. Clofibrate is one of the lipid-decreasing medicaments; which are used for the longest time and which are most effective. It has an influence above all on the metabolism of triglycerides, but also on the metabolism of cholesterol. The reduction of coronary morbidity and mortality by clofibrate was proved in a number of follow-up studies. An increased incidence of cholelithiasis may be a side-effect of the clofibrate therapy. Prefered indications for clofibrate are hypertriglyceridaemias in HLP type IIb, III, IV and V an (in combination with other drugs) therapy-refractery hypercholesterolaemias in HLP type IIa and IIb.

[Serum markers in Portugal: C3, Gc, Hp and Tf].

The phenotypes C3, Gc, Hp and Tf were determined in 426, 1000, 1000 and 426, respectively, unrelated individuals living in Portugal. The calculated gene frequencies are: C3S = 0.7981, C3F = 0.2019; Gc1 = 0.715, Gc2 = 0.285; Hp1 = 0.430, Hp2 = 0.570; TfC = 1,000.

[High association between properdin factors BfF1 and HLA-B 18 in a Portuguese family (author's transl)].

The HLA and the Bf systems were studied as part of a family investigation carried out in Portugal. In four generations the rare phenotype BfF1 could be determined four times and the BfF1S phenotype six times, HLA-B18 being simultaneously positive in all cases. Since the frequencies of the individual factors F1 and B18 do not differ essentially from those obtained in Central Europe and no inbreeding situation was present, the high-grade linkage disequilibrium between F1 and B18 may also be presumed for the Portuguese population. Neither among the F1/B18 homozygous nor among the heterozygous subjects could one discern any morbid state (e.g., insulin-dependent diabetes mellitus) from which an association with the F1/B18 haplotype could be deduced. Finally, the rarity of Bf factors, such as F1 and S0.7, is discussed from the standpoint of selection vs. mutation.

[Effect of clofibric acid on fatty acid metabolism in HLP patients in the 2d half of life].

Ten patients with primary type IIb hyperlipoproteinemia and one hundred patients with primary type IV hyperlipoproteinemia (sixty-seven men and forty-three women aged forty-three to seventy-four) were treated with clofibric acid (Regadrin) for three years. Gas chromatographic analysis of the composition of cholesterol ester and triglyceride fatty acids in serum were done prior to and at four-month intervals during therapy. During treatment of type IIb and type IV hyperlipoproteinemia there was observed a decrease of palmitic, stearic, palmitoleic, oleic, and eicosane-tetraenoic acids as well as an increase of linoleic, linolenic, arachidonic, and eicosane-pentaenoic acids in the triglyceride fraction. These changes manifest themselves most conspicuously in the effect of clofibric acid upon the hepatogenic fatty acid metabolism (increased synthesis or reduced catabolization of polyunsaturated fatty acids and increased oxidation or reduced formation of monounsaturated and saturated fatty acids, increased esterification of polyenic acids). In addition, these is the possibility of selective displacement of saturated and monounsaturated fatty acids of the unsaturated fatty acids fraction as well as specific inhibition of their esterification with glycerol by clofibric acid. Treatment with clofibric acid over a long period of time may well give rise to additional reactions through influences exerted upon the insulin level.

[Clinical significance of HDL-cholesterol].

In a survey the authors deal with the metabolic role of the HDL and the HDL-cholesterol. Via several mechanisms (LCAT-substrate, esterification of cholesterol and transport of cholesterol, competitive LDL-inhibition) the HDL render possible the efflux of cholesterol from the vessels and its catabolism in the liver. HDL-cholesterol possesses a cardio-protective effect. As it was proved by epidemiological, clinical, angiographical and experimental studies, it is reduced in cardiovascular diseases. Its determination, in connection with the calculation of the LDL-cholesterol renders possible diagnostic and prognostic evidence concerning the coronary risk. For males the standard risk is in a HDL-cholesterol of 35-55 mg/dl (0.9-1.4 mmol/l) and for females of 45-65 mg/dl (1.2-1.7 mmol/l). A series of influence factors and diseases which are cited have influence on the HDL-cholesterol-concentration. By physical conditioning, change of nutrition, reduction of weight and different medicamentous measures it is increased.

[Immune reactivity in patients with disorders of lipid metabolism].

The accidental observation of a selective IgM-deficit in a family with hyperlipoproteinaemia type II caused the working hypothese of close interactions of lipometabolism and immunoreactivity. In 69 patients with hyperlipoproteinaemia the quantitative determination of the immunoglobulins G, A and M was performed by the Mancini-test. All decreased values were repeated in a second determination at another moment. In 9 patients with hyperlipoproteinaemia and deficit of immunoglobulins we determined the T-lymphocytes by means of the sheep erythrocyte rosette test, the B-lymphocytes with the help of the method of direct immunofluorescence and the mouse erythrocyte rosette test. Altogether 14 of the 69 patients with hyperlipoproteinaemia showed reproducible deficit of immunoglobulins, which is statistically significant. The immune defect appeared above all as decrease of IgA (11 patients) isolated 8 patients or in combination with IgM-deficit (2 patients) and IgG-deficit (1 patient). 2 resp. 1 patient showed selective IgG- or IgM-deficit. Clinical symptoms of an immune defect were not recognizable anamnestically and prospectively. In the 9 patients who were immunologically further characterized the T-lymphocytes were in the region of the norm or were increased, the subpopulation of B-lymphocytes which reacted with mouse erythrocytes was normal or increased. The classification to the types of hyperlipoproteinaemia of the altogether 14 deficient patients showed the order IIa, IV, IIb. The original working hypothesis could be supported by the present findings, though an obvious clinical importance is not yet to be recognized. Changes of the membrane lipids of lymphocytes, interventions through the prostaglandin system and effect via suppressor cells must be discussed causally.

[Behavior of serum lipid parameters--results of a longitudinal gerontologic study].

Within the Leipzig longitudinal study from 1968 to 1978 in 158 test persons (46 males, 112 females) cholesterol and the total fatty acid spectre were gas-chromatographically determined in regular intervals. Serum cholesterol as well as proportions of linoleic acid increased significantly. By the significant decrease of the palmitic acid the percentages of the saturated fatty acids altogether decreased despite increase of stearic acid. In these cases the differences between males and females as well as between persons with normal weight and overweight more and more evened up. Ascertained correlations existed between reduction of the relative weight and decrease or more insignificant increase of cholesterol on the one hand and increase of linoleic acid on the other. The latter phenomenon is above all to be traced back to the fat-reduced diet and to the diet rich in polyene fatty acids which was more and more applied mainly by older test persons in the course of the longitudinal study, which thus may lead to antiatherogenic constellations of serum fatty acids.

[Lipoprotein lipase activity in healthy subjects and patients with metabolic disorders].

The activity of the post-heparin-lipase as well as of the cholesterol and triglyceride levels of test persons with normal weight and obese test persons of different age groups was established: 1. In normal persons (males and females) in mature age (30-45 years) a decrease of the enzyme activity takes place which is connected with an increase of the triglyceride content. Above all in older females a repeated increase of the post-heparin-lipase activity was observed, which positively correlates with the triglyceride level. While the decreases of the enzyme activity in mature age may be connected with processes of ageing, a repeated increase of the enzyme activity in older persons is perhaps induced by the increase of the triglyceride level. 2. Obese persons show higher enzyme activities in all age groups examined. Also in obese persons in mature age a decrease of the activity of post-heparin-lipase is to be established. The ageing development of the enzyme activity of the post-heparin-lipase is similar to that of normal persons, takes place, however, at a higher level. 3. The cholesterol and triglyceride levels of the obese persons examined were on an average higher than those of normal persons of the same age group. A dependence of age of these parameters was not observed in the adipose patients.

Properdin factor B polymorphism in Portugal.

The distribution of properdin factor B (Bf) phenotypes and the corresponding gene frequencies in a population sample from Portugal was investigated by agarose-gel electrophoresis and immunofixation. The following gene frequencies were found in Portugal: BfS = 0.6535, BfF = 0.2775, BfS0.7 = 0.0263, and BfF1 = 0.0427.
