Hypertrophy and hyperplasia during goitre growth and involution in rats--separate bioeffects of TSH and iodine.

Goitre growth was investigated in rats receiving a low iodine diet (less than 0.1 microgram iodine/g) and either 1 g/l KClO4 or 1 g/l propylthiouracil (PTU), or a combination of KClO4 or PTU with 50.82 nmol/1 T3 in tap water for 3 weeks. To investigate goitre involution, rats with iodine-deficient goitres were treated for 3 weeks either with T3 (0.5 microgram T3/day = 0.768 nmol/day), iodide (0.5 or 2.7 micrograms KI/day) or a combination of T3 with both iodide doses. Histology together with total DNA distinguished between hypertrophy and hyperplasia of the gland. During goitre growth there was highly significant correlation between goitre weight and TSH serum level (r = 0.93, P less than 0.001). Thyroid total DNA, however, was only weakly correlated to TSH but was inversely related to the degree of iodine deficiency. During goitre regression, TSH levels were normalized, histological signs of hypertrophy had disappeared, and thyroid weight was nearly normalized in all therapy groups. Total DNA, however, was normalized only with 2.7 micrograms KI/day (95 +/- 18 micrograms DNA/gland), and still elevated in all other groups. The highest DNA levels were found under T3 therapy (143 +/- 21 micrograms DNA/gland) and under 0.5 microgram KI/day (161 +/- 19 micrograms DNA/gland). Reduction of total DNA was independent of TSH, but followed replenishment of the iodine content of the glands. We conclude that TSH mainly induces hypertrophy, whereas thyroid hyperplasia is mainly regulated by the intracellular iodine content.

[Contrast enhancement of the nipple in nuclear magnetic resonance tomography of the breast with Gd-DTPA].

Contrast enhancement around the nipple has been evaluated retrospectively in 51 of 70 patients in respect of the underlying breast disease. 2 of the patients had to be considered separately because of malignant disease of the nipple or areola itself. The remaining 49 patients showed no close relation between the underlying breast disease and the amount of contrast enhancement in the nipple. This phenomenon of a non-pathological significant contrast enhancement of the nipple appears important to avoid false positive diagnoses in MRI of the breast with Gd-DTPA.

[Squamous cell carcinoma of the scalp and non-Hodgkin's lymphoma].

This is a case report on a 70-year old male patient with a huge ulcerative squamous cell carcinoma of the scalp as well as numerous palpable lymph nodes in the neck. Tumour extirpation and bilateral conservative neck dissection were performed. Histological examination of the excised lymph nodes did not reveal a metastatic infiltration but an additional neoplasm -- a low-grade malignant lymphoma (lymphoplasmocytic immunocytoma).

[Use of Gd-DTPA in the nuclear magnetic resonance study of the breast].

55 patients with altogether 60 breast masses have been examined postoperatively by plain MR, MR with Gd-DTPA and mammography at our institution. All breast carcinomas and fibroadenomas did enhance significantly. Non-proliferative dysplasia, normal breast tissue and scar tissue did not enhance significantly. Borderline enhancement has been found in cases of proliferative dysplasia. The diagnostic accuracy has been improved by MR with Gd-DTPA. Advantages have been an improved visualization of tumors in dense breasts, an improved differentiation between irregular dysplasia and carcinoma and an improved differentiation between post-therapeutic changes (after radiation, surgery or silicon implants) and carcinoma. Nevertheless both technical improvements and further clinical experience is necessary.

Prolonged reversal of diabetes in the rat by transplantation of allogeneic islets from a single donor and cyclosporine treatment.

Islets were isolated using neutral red as a dye to exclude lymphatic and acinar impurities in the transplant preparation. In a one donor-one recipient model, intraportally transplanted isogeneic islets survived indefinitely. Mean graft survival could be prolonged from 5 to 90 days, using a 3-dose rejection prophylaxis with CsA in allografts of (DA [RT1a]----Lew (RT 1l). Histological examination revealed intact islets in the grafted normoglycemic animals. Immunohistochemical staining of these islets showed B cells with insulin-rich cytoplasma. Blood glucose levels and the results of i.v. glucose tolerance tests 50 days after transplantation are presented. The different outcomes of our experiments are discussed.

[Vinyl chloride induced hepatocellular carcinoma].

On the occasion of a hitherto unique observation of three hepatocellular carcinomas in workers of the same industrial plant within 7 years following long-term exposure to vinylchloride, the characteristics are discussed of a chemical carcinogenesis leading to two different malignant tumours: haemangiosarcoma and hepatocellular carcinoma. This carcinogenic sequence has been predicted by animal studies. It is not known why the transformation of hepatocytes into carcinoma is far rarer than of sinusoidal cells into sarcoma. Whereas the hepatocellular carcinoma predominantly develops in association with cirrhosis, vinyl chloride is able to cause cancer directly without other known co-carcinogenic agents. This hepatic carcinogenicity is dose-dependent. After the introduction of industrial prevention measures, a new initiation of the tumour is improbable. Nevertheless, because of its long latency period, estimated between 5 and 20 years, clinical manifestations are still possible. An early diagnosis by sonography and computertomography, possibly combined with puncture, in exposed persons or those formerly at high risk is conceivable, while laboratory data, even tumour markers are unreliable. Its fulminant course does not differ from that of other hepatocellular carcinomas and has until now hindered successful treatment.

[Recurrent pulmonary hemorrhage with normal kidney function].

A case of Goodpasture's syndrome with normal renal function is described. Clinical, morphological and prognostic features of the diagnosis are compared with the findings in Goodpasture's syndrome with progressive renal failure and with the findings in idiopathic pulmonary hemosiderosis.

Preparation of porcine thyroid follicles with preserved polarity: functional and morphological properties in comparison to inside-out follicles.

One of the main problems in establishing isolated thyroid follicles in vitro is their tendency to form inside-out follicles. The reason for this change in polarity is unknown. We describe here a method for the preparation of stable thyroid follicles with preserved polarity for at least 6 days. Isolated follicles were obtained by infusion of collagenase (1.5 mg/ml) dissolved in minimal essential medium into the artery of intact thyroid glands. The morphological and functional properties of these follicles were compared to inside-out follicles. These inside-out follicles were obtained by digestion of minced thyroid tissue in a collagenase (1 mg/ml) solution. The polarity of follicles was proved by morphological criteria. Follicles with preserved polarity did not change polarity for at least 6 days in the presence of 1% or 5% fetal calf serum. As the culture conditions for both preparation methods were identical, we conclude that the preparation method rather than the culture condition is responsible for the preservation of cell polarity of isolated thyroid follicles in our system. Increases in cyclic AMP levels induced either by bovine thyrotropin (10(-3) U/ml) or by isoproterenol (10(-6) M) as well as iodide uptake and organification were rapid and significant in right-side-right follicles but not in inside-out follicles. Therefore the TSH receptor and the beta-adrenergic receptor appear to be exclusively located at the basal membrane of follicular cells. In addition, iodide uptake apparently is unidirectional.

[Diagnostic value of cervical lymph node biopsy in suspected nasopharyngeal cancer].

More than 90% of nasopharyngeal malignancies are carcinomas. Early symptoms are rare and usually ignored by the patients. The most common symptoms present are enlarged cervical nodes. More than half of all patients with nasopharyngeal carcinoma (58%) treated in the ENT-Clinic of the University of Munich had been admitted for further investigation because of cervical lymph node enlargement. Enlarged cervical lymph nodes can be removed easily. Thus the value of a lymph node biopsy for early diagnosis of nasopharyngeal carcinoma was examined. Forty-five cases of occult nasopharyngeal carcinomas have been reviewed. In all cases a cervical lymph node had been removed as a first diagnostic procedure. The histological findings were compared to nasopharyngeal biopsies taken after different time intervals. Our clinico-pathological review has shown that, especially in undifferentiated carcinoma, the histological features of cervical metastases is always identical to the very characteristic primary lesions in the nasopharynx. Thus, the characteristic feature of this tumour type in a jugulo-digastric lymph node strongly suggests the presence of a primary nasopharyngeal carcinoma.

[Dysontogenetic median maxillary cysts].

Nasopalatine duct cysts, median palatine cysts and nasoalveolar cysts belong to the developmental cysts of the upper jaw. They are situated at the midline in the fusion areas of embryonic facial processes and for this reason are known as "fissural cysts". They are, therefore, not of odontogenic origin and, on account of their relationship to the mouth, the nose and the nasal cavities, are of practical relevance to the otolaryngologist. The various clinical symptoms and the histological findings are described, and a review of the literature is presented.

Pyruvate dehydrogenase activity is not deficient in the brain of three autopsied cases with Leigh disease (subacute necrotizing encephalomyelopathy, SNE).

In autopsied brain tissue from three cases with Leigh disease (subacute necrotizing encephalomyelitis, SNE) and controls, the activity of pyruvate dehydrogenase complex (PDHC) was determined under different conditions. It was found to be at the control level or increased, but not deficient. The activities of succinate dehydrogenase, fumarase, succinate cytochrome c reductase, cytochrome c oxidase, and glutamate dehydrogenase were measured as additional mitochondrial markers and showed no essential differences between SNE and control tissue. The metabolic defect in SNE remains unknown. According to the literature, the defect may be localized to the mitochondrial systems. However, the reported results indicate that it cannot be ascribed to PDHC function. Extensive biochemical studies are necessary for understanding of the pathogenesis in the fatal genetic metabolic disease.

Distribution of lysozyme (muramidase) and alpha 1-antichymotrypsin in normal and neoplastic epithelial tissues: a survey.

Lysozyme and alpha 1-antichymotrypsin are useful in differentiation of histiocytic tumours. Both enzymes, however, also can be expressed in epithelial tissues. In contrast to lysozyme, alpha 1-antichymotrypsin is more often found to be positive in non-histiocytic neoplasias. In general, the activity of parent tissue is retained in tumour cells. In malignant melanoma and in single cases of other epithelial tumours an activity of alpha 1-antichymotrypsin was found which could not be demonstrated in normal parent tissue and which may find its explanation in a dedifferentiation of tumour cells.

Endoscopy in the diagnosis of gastritis. Diagnostic value of endoscopic criteria in relation to histological diagnosis.

Endoscopy of the stomach was performed in 152 consecutive patients alternately by two endoscopists, such that the non-examining endoscopist always followed the procedure via a teaching attachment. A fiberscope with a close-focusing optical system for magnification of the mucosa was used. The interobserver reliability of eleven predefined macroscopic criteria was analysed and these criteria were compared with the histological diagnosis for each gastric region (body, transitional zone, antrum). The percentage of agreement between the two observers ranged between 80% and 97% for all criteria except mucosal erythema (74, 68, 62%) and mucus covering (53, 61, 87%). A consideration of all three gastric regions reveals that only one macroscopic criterion (visibility of submucosal vessels) correlated significantly with the histological diagnosis (i.e. atrophic gastritis). Other macroscopic criteria that correlated significantly with the histological diagnosis were most often observed in the gastric body. Their positive predictive value, however, rarely exceeded 50 percent. Thus, even with the use of modern instruments, endoscopy is of limited value for the prediction of gastritis identified by histology.

Spleen transplantation in the dog as a model for studying immunological feedback reactions.

Modification of the immune response towards the generation of suppressor T cells can be attained using experimental designs promoting immunological feedback mechanisms. Those studies have been carried out in defined inbred animal systems. With respect to the great importance for organ transplantation, an in vivo model using larger outbred animals was developed in order to study such feedback reactions. The following experimental design was used: (1) allogeneic spleen transplantation in the dog; (2) autologous retransplantation of this spleen together with an allogeneic kidney transplantation to the former donor of the spleen after a period of 2-4 days. Although full immunological activity of the transplanted as well as the retransplanted spleen was demonstrated, rejection of the kidney occurred in a fashion identical to that observed in control animals.
