Hepatocellular carcinoma was diagnosed already during life in a 54-year-old workman who had been exposed to high concentration of vinyl chloride due to his occupation for more than twenty years. In addition, typical early changes as seen in experimental vinyl chloride induced angiosarcoma were observed. In recent years permissible room air concentration of vinyl chloride at work have been lowered drastically. However, due to the long latency occurrence of malignant liver tumours may still be expected in persons exposed formerly. Within the search programme for such patients at risk ultrasonography and computed tomography as non-invasive methods with high accuracy are advisable.

56 thyroid gland tumours and non neoplastic alterations were studied for keratin and thyroglobulin staining, using the indirect immunoperoxidase method on serial formalin fixed paraffin embedded sections. Papillary carcinomas showed a strong reaction with anti-keratin serum but a weak reaction with anti-thyroglobulin serum. Follicular adenomas and carcinomas showed virtually no reaction for keratin but a strong reaction for thyroglobulin. Undifferentiated and medullary carcinomas did not react with either antiserum, except for single cells in two undifferentiated carcinomas which reacted with anti-keratin serum. In nodular goiters, hyperplastic follicles showed little or no reaction with anti-keratin serum and strong reaction with anti-thyroglobulin serum. It is suggested that this virtually type-specific staining for keratin or thyroglobulin may be related to different degrees of cellular differentiation and organelle content in the tumour cells.

A microcytotoxicity assay to prove directly against myelin basic protein directed cytotoxic T cells has been developed. Spleen and lymph node cells of myelin basic protein immunized strain 13 guinea pigs were restimulated in vitro with basic protein for 5 days and used as attacking cells. Autologous spleen cells were coated with about 10(7) molecules basic protein by hydrophobic attachment, labelled with 51Cr and used as targets. Concentration dependent lysis (74% corrected lysis) was measured with attackers gained 12 days post immunization. Controls performed using unspecific (PHA) stimulated attackers and non BE coated target showed no lysis. Lysis was shown to be MHC restricted. The test may serve as a tool for the assay of human demyelinating diseases.

This report concerns a 77 years old woman with a solitary metastasis of a bronchiogenic carcinoma to the humerus, which had been involved by previously diagnosed polyostotic M. Paget (ostitis deformans). The distinction from sarcomatous changes in Paget's disease is discussed.
A case of Wolman's disease is described in a German infant who died at the age of 4 months. Hepatosplenomegaly, abdominal distention, gastrointestinal symptoms, dyserythropoietic changes in the bone marrow, but not adrenal calcification on X-ray were present. Stored lipid material could be demonstrated in liver, spleen, intestine, adrenals, thymus, kidneys, blood cells, but not in the central nervous system. Cholesterylesters and triglycerides were markedly increased in liver and spleen. Lysosomal acid lipase was found to be decreased in leucocytes and liver to less than 10% of normal, when measured with synthetic and natural substrates.

PURPOSE: Malignant neoplasms of the salivary glands are extremely unusual in the pediatric age group. METHODS: We report an 11-year-old boy who presented with a mass in the left submandibular region and a second mass in the jugularfacial venous angle. RESULTS: Histologic evaluation determined that this was a squamous cell carcinoma of the submandibular gland with metastasis to a cervical node. Review of the literature was undertaken to identify the rate of malignant salivary gland tumors in children. Malignant salivary gland tumors are extremely rare in children. When malignancy does occur, nearly 90% are present in the parotid gland. The submandibular gland is effected in 7.7% of cases. The most common malignancy in salivary glands of children is the mucoepidermoid carcinoma. Squamous cell carcinoma occurs in less than 2% of cases. CONCLUSIONS: Salivary gland tumors are very rare in children in contrast to adults. The proportion of malignant tumors in children is higher than that observed in adults; however, it is not possible to draw conclusions concerning treatment from the few case reports that exist. A higher rate of local recurrence and cervical lymph node metastasis may be expected in children.
Seventy consecutive patients with a bronchogenic carcinoma, in whom chest radiographs did not allow a sufficient evaluation of primary tumor localization or extension were examined by MR (1T). All diagnoses were confirmed by operation and histopathologic examination (Stage T1: 6 patients, Stage T2: 36 patients, Stage T3: 19 patients, Stage T4: 9 patients). T-staging was correct in 79% of patients. Significant infiltration of major bronchi, of the pericardium, heart, mediastinal fat and chest wall can be visualized with a reasonable degree of accuracy. N-staging based on lymph-node size was correct in 56%. A correlation between lymph-node size and metastatic involvement was not found. MR is limited by the spatial resolution, by the lack of tissue specificity and by artifacts. MR is a useful diagnostic tool in the evaluation of the primary tumor extension, however, especially in borderline cases histopathologic examination is required. A reliable N-staging is not possible based on lymph-node size measurement.
Disorders of the histiocyte-macrophage system in childhood comprise a manifold variety of clinical entities. The basic immunopathological mechanisms have still not been clarified in detail. We report on a 15 months old Yugoslavian boy of non-consanguineous parents, who developed persistent high temperatures and bloody diarrhoea at the age of 11 months, followed by a rapidly progressive dystrophy, hepatosplenomegaly and peripheral pancytopenia. An extensive virological, bacteriological and protozoal diagnostic program failed to reveal a pathological result. A bone marrow aspiration showed an extremely hypocellular marrow without infiltration of pathological cell populations. In spite of extensive supportive care and systemic steroid therapy, the patient died by bleeding complications due to severe untreatable thrombocytopenia. In post-mortem-biopsies a marked infiltration of RES-organs (lymph nodes, liver and spleen) by mature histiocytes, showing marked hemophagocytosis, was seen.
Sixty patients with postoperative scarring with (30) or without (30) silicon implants have been examined by magnetic resonance (MR) with Gd-diethylenetriamine pentaacetic acid. The 60 patients consisted of 28 patients with obvious normal or abnormal findings and of 32 diagnostically difficult patients, who were referred to MR because of uncertain mammographic and/or clinical findings. Scarring older than 6 months postoperatively did not enhance noticeably, whereas scarring younger than 6 months postoperatively enhanced variably (nonsignificant, borderline, or significant). Since all carcinomas larger than the slice thickness enhance significantly (as also confirmed by other studies) contrast enhanced MR allowed an excellent discrimination between scarring older than 6 months and malignancy. Good visualization of the tissue around silicon implants proved to be another advantage of MR. When its use in the diagnostically difficult cases was analyzed, MR proved quite helpful in 23 of 32 cases by excluding or demonstrating malignancy.
