A clinical study of 99mTechnetium dimercaptosuccinic acid uptake in obstructed kidneys: comparison with the creatinine clearance.

We studied 17 hydronephrotic kidneys owing to stenosis of the pyeloureteral junction. Preoperative uptake of 99mtechnetium dimercaptosuccinic acid was compared to early postoperative unilateral creatinine clearance, measured by urine collection from the nephrostomy tube. An excellent correlation was found. Therefore, uptake of 99mtechnetium dimercaptosuccinic acid represents a reliable parameter of renal function even in the presence of severe urinary tract obstruction. However, 99mtechnetium dimercaptosuccinic acid uptake should be measured more than 24 hours after injection of the tracer.

An open comparative trial of three doses of ciramadol used intravenously in renal colic.

In an open investigation, ciramadol, a partial agonist opioid, was found to be a potent analgesic when given intravenously in patients experiencing renal colic. Single intravenous doses of 20, 30 and 40 mg of ciramadol were given to 11, 11 and 12 patients with renal colic, respectively, and good pain relief was obtained in 10, 11 and 10 patients, respectively. Vomiting and nausea occurred in 9 patients, 4 in the 20 mg group, 2 in the 30 mg group and 3 in the 40 mg group. This adverse effect was related to standing and walking following the ciramadol injection.

Effect of sludge ice cooling on renal function and renal histology in the dog.

The effect of sludge ice surface cooling on the compensatory hypertrophied dog kidney was investigated. Renal function was measured prior to and on days 1, 3 and 7 after the cooling procedure by means of inulin clearance, PAH clearance and sodium excretion capacity during normal hydration and after volume expansion. No alteration in renal function was shown. No freezing lesions or thromboses were seen on histological examination.

European experience with 1-stage urethroplasty with free full thickness skin graft.

A series of 57, 1-stage urethroplasties with a free full thickness skin graft is reported. Results were good in 90 per cent of the cases, fair in 2 per cent and bad in 8 per cent. The bad results were noted in elderly men, suggesting that decreased viability of the surrounding tissues is a contraindication to this type of operation.

Avascular nephrotomy by means of thermography.

Experience with thermography to demarcate the different arterial areas of the kidney is reported. Thermography allows the performance of nephrotomies in an avascular plane with minimal bleeding and subsequent minimal renal parenchymal destruction. It is a safe and simple method which is preferred to the intraarterial injection of dyes which is done for the same purpose.

Prevention of vasospasms during extensive renal surgery. An experimental study in rats.

Six methods for prevention of renal arterial vasospasms, due to extensive surgical trauma are compared in an experimental study in rats. The 'proximal transit time' was chosen as a measure for renal arterial flow 1, 5, 10 and 15 min after a standard surgical trauma. Xylocaine infiltration around the renal pedicle was least effective. Hydration of the rat, preoperative mannitol perfusion, and preoperative phenoxybenzamine treatment gave comparable effects and were significantly better than no treatment. The topical application of papaverine (1 mg/kg) around the renal pedicle did significantly better than the above-mentioned methods. The most effective treatment, however, was the combination of hydration, mannitol perfusion and papaverine application around the renal pedicle.

Unbloody management of penile zipper injury.

The foreskin entrapped by a zipper fastener can be easily released by transversally cutting the lateral material of the zipper where it pinches the foreskin. The method is described.

Clean intermittent self-catheterization in the chronical management of the neurogenic bladder.

30 patients with neurogenic bladder dysfunction were treated with intermittent self-catheterization. Since the application of this method, urine loss has disappeared almost completely and the urinary infection rate has decreased. In some cases there was radiological improvement of the upper urinary tract. In all the other patients no kidney deterioration was found on intravenous pyelography. In most cases the method brought a real improvement in social life.
