Preliminary clinical experience with dezocine, a new potent analgesic.

The analgesic effect of a single intravenous dose of dezocine at various dosage levels was studied in 41 patients suffering from acute renal colic. Twenty milligrams was found to be a very effective dose. Out of 25 injections an excellent response was recorded on 21 occasions, with a mean duration of effect of 3.5 hours. In a further 3 patients, pain relief was considered as fair and of shorter duration. Pronounced dizziness was a nearly constant side-effect of the drug. There was no overall effect on blood pressure or heart rate.

Antireflux in patients with neurogenic bladder.

The results of ureteral reimplantation in 8 neurogenic bladders with a high outflow resistance and severe trabeculation are presented. Operation was successful in all cases. The conditions for a successful intervention in these cases are: (1) previous or simultaneous treatment of the high outflow resistance: intermittent catheterization was found to be an excellent method for this purpose, and (2) the submucosal tunnel should be created preferably upon the trigone which is the only place free of trabeculation. High outflow resistance and severe trabeculation should no longer be considered as contraindications for antireflux surgery in the neurogenic bladder.

A plea for antireflux operations in adults: review of 50 cases.

The results of antireflux operations in 41 cases of primary and 9 cases of secondary vesicoureteral reflux in adults are presented. Reflux was eliminated in all cases and there were no postoperative complications. Most patients had a long history of urinary tract infection preoperatively, which was eliminated in 64.1 per cent of the cases after surgical correction of the reflux. A relapse of infection occurred in 30.8 per cent of these cases. Chronic pyelonephritis remains active in 5.1 per cent of the cases. There were 6 patients with serum creatinine levels higher than 1.8 mg. per cent who underwent a successful antireflux procedure without an increase of serum creatinine levels. An antireflux operation should be preferred over long-term conservative management in most cases of symptomatic vesicoureteral reflux in adults less than 50 years old.

The concentration of doxycycline in human prostate gland and its role in the treatment of prostatitis.

The concentrations of doxycycline were measured in 36 enucleated, enlarged prostate glands. After administration of 200 mg of doxycycline intravenously significant tissue concentrations (mean value 1.79 mug/g) were rapidly obtained. The mean concentration of the drug 14 hours after a repeated oral dose of 100 mg was 1.56 mug/g. The ratios of tissue and serum concentrations were 0.610 and 0.568, respectively. As there are only a few antibiotics which penetrate into the prostate, the importance of these data for the treatment of bacterial or mycoplasma prostatitis is obvious.

The concentration of sulphamethoxazole and trimethoprim in human prostate gland.

Sulphamethoxazole and trimethoprim concentrations were measured in 19 enucleated human prostate glands. Good concentrations of sulphamethoxazole were found after 4 days of treatment: the ratio of plasma concentration to that of prostate varied from 1-25 to 3-29. The trimethoprim concentrations were 2 to 3 times higher in the prostate than in the plasma. These high concentrations were reached very rapidly after the first dose of trimethoprim. The results are compared with date from the literature. The combination sulphamethoxazole-trimethoprim should be of value in the treatment of bacterial prostatitis.
