Successful treatment of malaria tropica with acute renal failure and cerebral involvement by plasmapheresis and hemodialysis.

A non-immune, 31-year-old woman developed an acute infection with Plasmodium falciparum after travelling to Kenia. The parasites proved resistant to chloroquine and sulfadoxine/pyrimethamine. The course of the disease was complicated by acute renal failure, hepatocellular damage, disorders of blood coagulation, thrombocytopenia, hemolysis and cerebral involvement. Despite a very high level of parasitemia (50% parasitized erythrocytes) a rapid clinical improvement was achieved by plasmapheresis and hemodialysis. Our experience shows that plasmapheresis and hemodialysis are excellent additive methods which rapidly improve clinical symptoms and may reduce morbidity and mortality in severe malaria tropica.

[Berolina iubilans: Berlin physicians as Halle doctoral candidates (VI). Curt Schimmelbusch (1860-1895), Halle doctoral candidate of 1886].

The history of asepsis is closely connected with the name of Curt Schimmelbusch. The physician who was awarded the doctor's degree in Halle in 1886 as co-worker of the histologist and pathologist Eberth at first dealt with research in the field of thrombosis. He became famous all over the world by his investigations concerning the development and scientific fundamentation of mechanical sterilisation methods which he performed under Ernst von Bergmann in Berlin. In this field the name of Curt Schimmelbusch is rooted in the special terminology.

[Berolina iubilans: Berlin physicians as Halle doctoral candidates (V). Marcus Herz (1747-1803) and Berlin Jewish physicians].

A number of Jewish medical students from Berlin went in for the formalities for taking a doctor's degree at Halle university in the 18th century. Among them there were several physician-personalities whose names are held in high esteem in the history of medicine up to now. In particular Marcus Herz who graduated in Halle in 1774 came into prominence by outstanding achievements in teaching and research. Under his management at that time a whole generation of Jewish medical students in the Jewish hospital found the occasion to a practice-relevant preacademic education.

[Berolina iubilans: Berlin physicians as Halle doctoral candidates (IV). Johann Ulrich Bilguer (1720-1796), Halle doctoral candidate of 1761].

The physician Johann Ulrich Bilguer (1720-1796) born in Chur in Graubünden obtained high posts in military medicine in Prussian services. He had particular merits in the organization of military hospitals. With his doctoral thesis on indication and contraindication of amputations submitted in Halle in 1761 he became the "Father of the conservative surgery" as he was called. During his stay in Berlin for many years Bilguer wrote a number of treatises, in which he expressed his opinion to many medical scientific problems and again to questions of an improved treatment of patients.

[Berolina iubilans: Berlin physicians as Halle doctoral candidates (III): Carl Ludwig Willdenow (1765-1812), Halle doctoral candidate of 1789].

Carl Ludwig Willdenow (1765-1812) who was trained as a pharmaceutical chemist and awarded the degree of Doctor of Medicine by the Faculty of Medicine of Halle University in 1789 belongs to the great botanists in an era in which the special subject represented by him finally disengages from the complex of medicine. The plant systematizer Willdenow worked on the traces of Linné, trying to note down the complex of species enlarged by him. Under his management the Botanic Garden in Berlin gained a world-wide renown. The Halle dissertation which was remarkable by its details was the intermediate station of Willdenow's professional career.

[Autoimmune phenomena after silicone implantation].

Eleven years after silicone implantation into the breasts a now 42-year-old woman developed arthralgia with a serological autoimmune phenomenon: antinuclear antibodies of the mottled type (titer 1:320 to 1:1,280), complement consumption, cytoplasmic thyroid antibodies (1:400), and constant elevation of erythrocyte sedimentation rate, concomitant with signs of silicone mastitis (angiotensin-converting enzyme 108 U/min; computed tomography demonstrated an infiltration in the right breast implant). As the patient has only quite minor symptoms she has so far refused removal of the implant.

[Berolina iubilans: Berlin physicians as Halle doctoral candidates (II): Ernst Ludwig Heim (1747-1834), Halle doctoral candidate of 1772].

Ernst Ludwig Heim (1747-1834), born in the Thuringian village Solz, took his medical education in Halle between 1766 and 1772. After the following educational journey he became medical officer for the municipal and rural districts of Spandau and Osthavelland. From there he changed for Berlin in 1783, where, by his outstanding knowledge and abilities he became the prototype of the faithful family physician, as "Old Heim" the symbol figure for always responsible medical activities.

[Berolina iubilans: Berlin physicians as Halle doctoral candidates (I): Johann Carl Wilhelm Moehsen (1722-1795), Halle doctoral candidate of 1742].

The celebration of the 750th anniversary of the city of Berlin gave rise to remember life and work of several physicians who after their graduation at Halle university set up their practice in Berlin and here performed remarkable accomplishments in various fields. One of these representatives of an ars medica Berolinensis is Johann Carl Wilhelm Moehsen (1722-1795), graduated at Halle university in 1742, a polyhistor, who dealt with several subjects and with his regional-historical studies initiated a historiography of science including history of medicine.

[A physician and polyhistorian: on the 300th birthday of Johann Heinrich Schulze (1687-1744)].

The 300th anniversary of the birthday of Johann Heinrich Schulze gives rise to the remembrance of life and work of a polyhistorian who is riveted by particular attainments in medicine and several other special branches (orientalistics, graecistics, archaeology, numismatics). The medical historiography considers him one of the outstanding representatives of the 18th century, chemistry calls him the ancestor of photography. The University of Halle honours him as one of the most eminent scientists of its period of foundation.

[Importance of MR tomography for the diagnosis of bone marrow diseases in childhood].

As a diagnostic and follow-up method of bone marrow diseases in children, magnetic resonance imaging (MRI) possesses special importance. It is characterised by high sensitivity in detecting bone marrow lesions, high soft tissue--bone tissue contrast, sharp demarcation of intraosseous lesions and visualisation of vascular structures without application of contrast medium. Thus it is able above all to take on the findings of radionuclide bone scanning, computed tomography and angiography. In the detection of pathophysiological alterations of the corticalis and of calcifications it is less reliable than x-ray images and CT.

[Ars medica Anhaltina (VI): Status, rights and responsibilities of physicians and pharmacists with reference to medico-legal aspects of the 18th and early 19th century].

The medicinal legalities of the Anhaltine territories developing under the influence of Enlightenment and Pietism and then characterized by philanthropism contain numerous progressive and for their age exemplary components. When comparing other relevant decrees their progressive character becomes clear by the accentuation of the necessities for provision of the population above all in rural regions. The in other places still strictly observed borders of the various groups of medical profession are varied according to the necessities. Earlier than in many other countries the representatives of surgery and pharmacy experience a reevaluation of the state leading to the insertion into medicinal-organizational authoritative bodies.

[Ars medica Anhaltina (V): Physicians and scientists of the Anhalt territories in Goethe's circle of friends].

From the view of regional history for the Anhaltine territory several medical and natural scientists are to be mentioned whom a remarkable role can be ascribed in the Weimarian circle round Goethe. The Bernburg physician and pharmaceutist Bucholz became there for many years his consultee for questions of natural sciences; Gren, who also came from Bernburg, became the antipode in the dispute about problems of optics. Goethe visited Gilbert, who was educated in Dessau, in his laboratory in Halle. In old age the poet was supported as physician and friend by Carl Vogel, born in Dessau, who described Goethe's last disease.

[Ars medica Anhaltina (IV): Friedrich Samuel Kretschmar (1730-1793) and public healer organization in the philanthropic principality of Anhalt-Dessau].

In the history of the organisation of public health of the 18th century the territory of Anhalt-Dessau takes a remarkable place. There the ideas of enlightenment and philanthropism induced a model, the details of which were determined above all by the activities of the physician Friedrich Samuel Kretschmar. A carefully worked out system of social welfare was particularly for the benefit of the poor population and included prophylactic measures, which also oriented to the variolisation.

[Ars medica Anhaltina (III): the Anhalt Hahnemann interpretation and the Köthen Homeopathic Institute].

In the years between 1821 and 1835 the domiciliation of Samuel Hahnemann in Köthen induces a consolidation of the homoeopathy in the district of the Anhalt territories. By dynastic order hereby the existing medical laws concerning the self-dispensing of the physicians are cancelled. By the establishment of an institute called Institutum homoeopathicum et magneticum the homoeopathy concentrates for a long time on Anhalt territories also after Hahnemann's retirement. The fight against homoeopathy permanently remains on the programme of the scholastic medicine, which certainly appeals to existing laws, but cannot prevent the development of a prospering business with homoeopathy. The reconstruction of these events became possible by the preserved documents.
