HLA frequencies and haplotypes in children with idiopathic thrombocytopenic purpura (ITP).

26 patients with an acute reversible ITP and 6 with chronic ITP were tissue typed, together with their healthy first-degree relatives. The HLA frequencies of the different groups were compared with those of a normal control population. The only significant difference between the groups was an increase in the frequency of Aw32 in acute ITP patients. HLA-Aw32 was present in 26.9% of patients, but in only 0.8% of the controls (corrected P = 0.000027). The possible importance of associations between antigens of the HLA-A locus with certain diseases are discussed. Family analyses and haplotype determinations proved to be unproductive because no familial clustering of ITP was found.

Studies on the orientation of brush-border membrane vesicles.

Orientation of rat renal and intestinal brush-border membrane vesicles was studied with two independent methods: electron-microscopic freeze-fracture technique and immunological methods. With the freeze-fracture technique a distinct asymmetric distribution of particles on the two membrane fracture faces was demonstrated; this was used as a criterion for orientation of the isolated membrane vesicles. For the immunological approach the accessibility or inaccessibility of aminopeptidase M localized on the outer surface of the cell membrane to antibodies was used. With both methods we showed that the brush-border membrane vesicles isolated from rat kidney cortex and from rat small intestine for transport studies are predominantly orientated right-side out.

[Comparison between combination therapy with clofibrate and beta-pyridylcarbinol and clofibrate monotherapy (author's transl].

In order to determine whether the lipid-lowering effect of combined treatment with clofibrate and beta-Pyridylcarbinol exceeds that of clofibrate monotherapy, a double-blind crossover study was performed. 17 patients with primary hyperlipoproteinemia of Type IIa and 10 patients with primary hyperlipoproteinemia of Type IIb received either Lipofacton (1.000 mg clofibrate and 50 mg beta-pyridylcarbinol per day) or clofibrate (1.500 mg per day) for a period of 6 weeks each. Before beginning therapy and between both periods of medication, placebo was administered for 14 days. In both total plasma and in the LDL fraction the cholesterol level was lowered by less than 10% for both substances. In patients with hyperlipoproteinemia of Type IIb, the triglyceride levels were lowered by about 40%.

[Worth and unworth of occlusion therapy in the treatment of squint (author's transl)].

The most commonly used methods for the treatment of amblyopia are based on dosed obstruction of the eye with the better function. Occlusion therapy is still the treatment of choice for severe (eccentric fixation) amblyopias. A possible impairment of the binocular neurones must be accepted in doubtful cases with the objects of a possible normalization of monocular function (visual acuity).

Properties of brush border vesicles isolated from rat kidney cortex by calcium precipitation.

Brush border membrane vesicles were isolated from rat kidney cortex by differential centrifugation in the presence of 10 mM calcium. Their properties were compared to brush border vesicles isolated by free-flow electrophoresis. By the calcium precipitation method membrane vesicles were obtained in a shorter time with a similar enrichment of brush border marker enzymes (11- to 12-fold for alkaline phosphatase and maltase), with a similarly reduced activity of the marker enzyme for basal-lateral plasma membranes and an almost identical protein composition as revealed by polyacrylamide gel electrophoresis in sodium dodecyl sulfate. The transport properties of the two membrane preparations for D-glucose, L-phenylalanine, and phosphate are essentially the same; there is some indication for a lower sodium permeability of the vesicles prepared by the calcium precipitation method. The latter vesicles were also shown to exhibit sodium gradient stimulated uptake of L-glutamate.

[Results of cüppers "fadenoperation" (author's transl)].

The so called "Fadenoperation" Cüppers was performed in 233 patients = 40% of the cases who had been treated by muscle surgery at the University Eye Clinic Hamburg in 1975. The results in the different indication groups are presented. The most frequent indications are: Increased objective squint angle during near fixation, incomitant binocular movements in the horizontal plane, unstabile objective angles, nystagmus compensation (block-) syndrome, variation of Kestenbaum's nystagmusoperation. The result in 2/3 of the cases was satisfying, further surgery being not necessary.

[Diagnostic value of radionuclide bone imaging in monitoring skeletal metastases (author's transl)].

In typical cases with pelvic metastases a close relationship is demonstrated between nuclear medicine and radiologic findings depending on the kind of therapy. For this reason total body radionuclide studies should not only be recommended for screening but also for therapeutic monitoring. Positive radionuclide findings should be supplemented by roentgenologic studies of the areas selected in that way.

[Management of bone metastases. Indication for radiotherapy and surgical internal fixation (author's transl)].

Radiotherapy of bone metastases can achieve in up to 78% of patients complete relief of pain. Recalcification avoids pathological fractures and beginning compression of the spinal cord. Depending on size and site of the osteolytic process a prophylactic surgical stabilisation will be necessary. Bone metastases are a sign of generalized disease. Systemic therapy should be provided in addition to local therapy.

Amino acid transport in brush-border-membrane vesicles isolated from human small intestine.

Uptake of L-alanine and L-phenylalanine by purified bursh-border-membrane vesicles isolated from human small intestine was investigated by using a rapid-filtration technique. L-Alanine entered the same osmotically reactive space as D-glucose, indicating that transport into the vesicle rather than binding to the membranes was being observed. The uptake rate for L-alanine was higher in the presence of a Na+ gradient than in the presence of a K+ gradient. In the presence of a Na+ gradient, the lipophilic anion SCN- caused an increase in L-alanine transport, whereas the nearly impermeant SO42- anion decreased the uptake of L-alanine compared with its uptake in the presence of Cl-. The uptake of L-phenylalanine into the brush-border-membrane vesicle was also stimulated by Na+. The results indicate co-transport of Na+ and neutral amino acids inthe human intestinal brush-border membrane.

[Length of effectiveness of calcium-containing and calcium-free antacids. A double-blind study].

The effect of a Ca-containing and Ca-free antacid on meal-stimulated acid secretion was examined by intragastric titration with pH being measured extragastrically. The Ca-containing antacid was more effective in neutralisation of the intragastric content (pH greater than 5,8) and increasing the pH (p less than 0,002). The acid secretion was reduced (p less than 0,04) by the Ca-free antacid and remained constant after administration of the Ca-containing antacid. There were no significant differences between acid secretion before and after the administration of the Ca-free and the Ca-containing antacid.
