Evaluation of patients undergoing extra-intracranial arterial bypass surgery based on CT-scanning and neurophysiological studies.

In order to evaluate short-term effects of extra-intracranial arterial bypass procedure (EIAB) on cerebral function, we performed neurophysiological studies, combined with CT-evaluation, on 17 patients undergoing EIAB, pre- and postoperatively. CT helps to define, on a morphological basis, the extent of ischemic brain damage, and is, in some cases, a valuable tool in preoperatively assessing chances for a recovery. EEG shows, 2 weeks after surgery, in 50% of the patients after completed stroke, worsening of ischemic lesions as a result of reduced intraoperative cerebral perfusion and/or temporary occlusion of the recipient cortical vessel.

[Exposure to radiation in percutaneous cervical cordotomy (author's transl)].

During eleven high cervical percutaneous cordotomies using the image intensifier, the amount of exposure to X-radiation was measured at various sites on both the patient and the operator. The values lay below the maximum doses allowed by the radiation protection regulations (Röv; section 33. Para 1).

[Total percentric inversion of chromosome 9 as positive proof of in the legal determination of paternity].

In an anthropological and genetic paternity report, a total pericentric inversion of chromosome 9 was found in the child and in the putative father. This inherited variant was not found in the mother. In view of the unequivocal detectability and the quite well known frequency of the marker, special attention could be given to the cytogenetic finding in the overall genetic evaluation in the present case.

Classification of tumours in the sellar region using CT and encephalotomography.

Tumours in the sellar region may still present diagnostic problems. We therefore compared encephalotomographic and CT findings in sellar tumours and 91 patients were examined by both methods. For clinical purposes and operative treatment the degree of compression of the hypothalamus is of great importance. Encephalotomographic and CT findings are analysed and demonstrated. In cases of density differences between tumour and brain tissue shown by contrast enhancement, more information on the inner structure of a tumour (cystic or solid) can be obtained by computerized tomography. But if these are no density differences or/and contrast enhancement, tumour diagnosis is made easier by encephalotomography because of the great efficiency of this method.

[Recognition of female carriers of haemophilia A (author's transl)].

The detection rate for female carriers of haemophilia A was investigated by comparing the one-step method of Hardisty and Macpherson for biological activity of factor VIII with quantitative immunoelectrophoresis (after Laurell) for factor VIII-associated antigen. Certain or probable positive results were obtained in 25 of 27 female carriers by the quantitative immunoelectrophoresis, but only in 18 when measuring factor VIII activity.

Limitations of computerized tomography in diagnosis of subdural hematoma.

CT findings in 51 patients with subdural hematomas are studied. Direct signs (hyperdensity, hypodensity) and indirect signs (any form of mass lesion) are distinguished. CT is limited with regard to determining correlations between time and the attenuation values. The factors which influence density of the hematoma are discussed. For a rational approach with subdural hematomas, which present diagnostic problems and reveal the limitations of CT diagnosis, a diagnostic scheme is proposed.

High cervical percutaneous cordotomy in intractable pain.

The authors report on 200 cervical percutaneous cordotomies in 146 patients who were suffering from intractable pain. In order to assess the success rate and the incidence of relapse, the neurological findings immediately post-operatively were compared with the results of a later follow-up. The prospect of success in the treatment of pain appears to depend on the anatomical representation in the spino-thalamic tract of the region of the body which is involved. The incidence of complications and the mortality are compared with the observations of other authors and with the results of open cordotomy.

[Correlation of encephalotomography and computer tomography in space occupying lesions in the midline and cerebellopontine angle (author's transl)].

The diagnostic value of encephalotomography and computer tomography in neuroradiology of midline tumors is discussed. Characteristic differences of both methods are summarized. The following conclusions of clinical importance are derived from our experiences: 1. Computer tomography is the most valuable method in the diagnostic procedure of midline tumors. Encephalography is not necessary is cases with density differences of tumor and surrounding brain tissue or contrast enhancement. 2. Encephalotomography should be performed in cases with dislocation of cisternes and ventricles without density differences or contrast enhancement. 3. Encephalotomography is the method of choice in patients with unclear computer tomography and contradictory clincial symptomatology.

[Test of combination of the agents coumarin and troxerutin for embryotoxic and teratogenic side-effects in Göttingen miniature pigs (author's transl)].

Göttingen miniature pigs were used to test a combination of the active principles coumarin and troxerutin (Venalot) for teratogenic and embryotoxiic side-effects. The experimental group consisted of 17 mother pigs with 95 fetuses, the control group of 6 untreated sows with 45 fetuses. The combination of coumarin and troxerutin was administered orally at a level of the 100-fold therapeutic dose (i.e. 25 mg coumarin + 150 mg troxertutin/kg/day) from day 6 to day 30 of gestation. The pathological and histological examinations of the uteri and fetuses revealed no indications of any embryotoxic or teratogenic damage caused by the tested combinations. Morphological and clinico-chemical investigations did also not result in any findings definitely attributable to the medication.

[Computer tomography in intracranial hemorrhage (author's transl)].

For the detection of acute intracranial and intracerebral hemorrhage computer tomography C.T.- examination is of greatest importance. The diagnostic value exceeds angiographic findings. Especially in cases of urgency C.T. should be used as routine examination.

[Lumbar myelography using dimer-x in the diagnosis of lumbar disc lesions (author's transl)].

The pre-operative myelograms performed using Dimer-X (dimeglumine-iocarmate) of 219 patients were re-evaluated post-operatively without knowledge of the operative findings. In 77% the myelographic interpretation was in agreement with the surgical diagnosis. The interpretation was difficult and unreliable when only one or few radiographic signs were present. In order to achieve optimal benefit from the investigation the history and clinical findings should also be considered as equally important. Myelography is only justified in clinically unclear cases. In view of this and possible complications a stringent indication in necessary.

[Percutaneous chordotomy].

The authors review 134 cases of percutaneous cervical chordotomy in 95 patients; 86 patients suffered from malignant tumors, while the pain was caused by benign growths in only 9 cases. The immediate postoperative findings are compared with the clinical results of the follow-up medical examination. Chronological and numerical data follow on permanent sucesses and recurrence rates. The author's own morbidity and mortality rates are contrasted with those if other authors, and compared with the risk involved in open chordotomy.
