[Reproduction toxicologic studies on rats following oral administration of benzopyrone preparations].

The influence of the benzopyrone preparation Venalot (active substances: coumarin and troxerutin; in the following briefly called CT) on the fertility and teratogenicity as well as on the perinatal and postnatal development of a total of 3 generations was evaluated in a combined study. The 1-, 8-, 64-, and 128fold of the daily therapeutical doses for humans was suspended in tap water and administered orally by gavage to 95 male and 190 female SPF rats (Wistar) (test groups 2 to 5). 23 male and 46 female rats (test group 1) served as controls and were given tap water alone. The male animals were subjected to a pretreatment of 10, the female animals to one of 3 weeks. The treatment was continued during the phase of mating. The animals scheduled for cesarian section received the test substance until the day of the laparatomy (gestation day 20), those selected for littering throughout lactation (day 24 post partum). With the aid of a stepwise histological technique, the teratological examination could also disclose non lethal malformations of the organs. The treatment resulted in a decreased food consumption in the animals of group 5 and in a reduced gain of body weight as well as pathologic-anatomically and histologically demonstrable, definitely dose related hepatic lesions. The test substance had no effect on either the treated P generation nor the untreated F1 generation. As teratogenic effects could also not be demonstrated and the peri- and postnatal development of the filial generations 1 and 2 was undisturbed, the present study does not indicate a reproduction toxicological risk.(ABSTRACT TRUNCATED AT 250 WORDS)

Prenatal diagnosis of a short-rib-polydactylia syndrome type Saldino-Noonan at 17 weeks' gestation.

A case is described in which the family history and the sonographic findings during the 17th gestational week led to the suspicion of a short-rib-polydactylia syndrome of the Saldino-Noonan type and an abortion was induced. The ultrasound image disclosed cystic structures in the topographic area of the kidney, marked ascites and oligohydramnios and indicated a severe fetal developmental disturbance. Due to the oligohydramnios the extremities could not be interpreted sonographically. The pathological-anatomical findings confirmed the suspected short-rib-polydactylia syndrome of the Saldino-Noonan type. Typical skeletal changes, dysgenetic cystic kidneys and pancreatic fibrosis were especially indicative of the syndrome. The complete agenesia of the islets of Langerhans, present in our observation, has, as far as we know, previously not been reported in the case of short-rib-polydactylia syndromes.

CT imaging in lumbar disc disease. The value of multiplanar reconstructions.

A CT/T 8800 (General Electric) CT scanner was used to study lumbar disc disease in fifty patients. In seven patients so-called computed tomography metrizamide myelography was performed, whereas in the remaining cases no contrast enhancement was used. Multiplanar reconstructions were done. Irradiation exposure was measured during the investigations and compared with data during conventional myelography. We found that lumbar CT scanning is not a screening method in cases of lumbar disease, but the aimed use gives further information about the underlying process and its differential diagnosis. Multidirectional reconstructions reduce the number of diagnostic errors. This procedure does not increase the radiation exposure dose in comparison with conventional myelography.

Modern treatment of the symptomatic os odontoideum.

A new surgical procedure for treatment of congenital, symptomatic atlanto-axial instability (os odontoideum ) is described. Two metallic clamps, with an integrated inner spring for maintaining continuous pressure on a cortico-cancellous bone-graft implanted into the interlaminar space, are fixed posteriorly on the vertebral arches C1/2 next to the graft. Because of the initial solid segment-stability external fixation is unnecessary. Posterior interlaminar osteosynthesis prevents late neurological complications caused by a mobile os odontoideum .

[Neurosurgical management of gunshot injuries to the head].

The article describes the pathophysiological mechanisms of injuries caused by shots in the head. In particular, the effect exercised on the cerebral tissue by the impact of rapid projectiles is explained. The review is based on 23 patients treated by the authors. These cases are discussed and critically assessed, using the results communicated in other papers as basis. The requisite surgical measures are considered; the need for effecting complete cleansing of the path of the bullet is viewed with some reserve.

Meiotic segregation in familial reciprocal translocation t(8q;22q).

We studied the chromosomes of a mentally retarded boy with minor anomalies and of his parents using a G-band stained high-resolution chromosome method. This documented dup (8q24.1 = to 8qter) and dup(22pter = to 22q11.2) in the boy due to a maternal balanced reciprocal translocation of chromosomes 8 and 22 and 3:1 disjunction during meiosis I. The karyotype of the boy is 47,XY, +der(22) (22pter = to 22q11.2::8q24.1 = to 8qter). The der(22) was involved in satellite associations and stained positively with AgNO3 in mother and child. The case is compared to similar cases in the literature and the function of the small acrocentric marker chromosome during meiosis is discussed.

Posterior atlanto-axial fusion: a new compression clamp for laminar osteosynthesis.

A new method of dorsal arthrodesis in patients with atlanto-axial instability of different etiology is described. In three patients solid laminar osteosynthesis C1/C2 was achieved by insertion of an autogenous cortico-cancellous bone graft into the interlaminar space and by a bilateral application of the lately developed metal clamp. The technical principles of the clamp guarantee a permanent pressure on the graft and a solid initial stability in the segment. Additional external fixation and immobilization of the patient are not necessary. Complications as known from other procedures of the atlanto-axial dorsal fusion will be prevented efficiently.

Scintiphotographic studies for long-term results in carotid cavernous sinus fistulae.

Scintiphotographic studies as a method for analyzing the brain perfusion in carotid cavernous sinus fistulae before and after surgical treatment with different techniques are presented. Twelve patients have been reexamined long after their operation to prove the sufficient crossover circulation through the circle of Willis while the homolateral carotid artery is impatent. Sufficient perfusion of both hemispheres could be well-documented by this radionuclide method. No neurological deficit was found in clinical examination either.

Cytogenetic findings in T-zone lymphoma.

Five cases of T-zone lymphoma were investigated with histologic, immunologic, and cytogenetic methods. The chromosome analyses were performed on lymphoma cells prepared immediately after removal of the lymph nodes. The chromosomes involved in structural rearrangements were nos. 1, 2, 3, 4, 14, and Y. Numbers 3, 5, 6, and 13 were lost by some tumors, and nos. 3 and 9 were gained. Chromosome 3 was involved most often in structural and numerical aberrations, whereas 14q + markers occurred in only one case. The importance of multidisciplinary studies is pointed out.

Multiple meningiomas. Clinical and computer tomographic observations.

The same histological types of tumor are found in multiple as in solitary meningiomas. Multiple meningiomas cannot be considered in every case to be a forme fruste of von Recklinghausen's disease. The incidence of multiple meningiomas in this paper is higher than had been reported before the introduction of computer tomography (CT) into clinical practice. The number of tumor nodes is determined more accurately by CT than by angiography because tumors are detected in patients without neurological deficit. CT has limitations in the verification of small meningioma nodules near the base of the skull and in the differentiation of multiple meningiomas from meningeal meningiomatosis or primary meningeal sarcomatosis.
