Treatment failures and severe complications in children with Hodgkin's disease.

Therapy failures and severe complications in the treatment of 46 children affected with Hodgkin's disease are reported. All children were treated with polychemotherapy (modified MVPP scheme) and radiotherapy (high voltage therapy X-ray 210 KV; involved field radiotherapy). 42 patients came into a complete remission, with a median of 58 months in 37 patients (10--97 months). 6 patients 14.3%) suffered from a recidive, 5 of them came into a second remission. Life-threatening infections appeared in 3 patients. After 9 MVPP cycles an encephalopathy appeared in 2 boys, one of them died. After local radiotherapy a severe laryngitis developed in two patients. An azoospermia existed in 6 adolescent boys examined.

Treatment of lymphocyte depletion Hodgkin's disease in children.

Lymphocyte depletion Hodgkin's disease (LDHD) was diagnosed in 8 of 40 children with Hodgkin's disease treated from July 1969 to September 1976. All of the children received conventional radiotherapy and combination chemotherapy with mustine, vinblastine, procarbazine, and prednisone (MVPP). In LDHD children the disease was associated with severe anemia, splenomegaly presence of mediastinal enlargement, and dissemination at the time of diagnosis. Three children with LDHD did not achieve remission and died with uncontrolled disease. In two others, relapses occurred and led to death in one. Only three children remain in their first remission. Only 3 of 32 patients with non-LDHD relapsed and entered second remission. All 32 patients were free of disease at the time of evaluation, with mean survival of 88.2 months. We conclude that LDHD in childhood is frequently associated with clinically advanced disease and still has a bad prognosis. Alternative approaches to its therapy are needed.

Pyrimethamine in prevention of relapses of meningeal leukemia: report of two cases.

Relapses of meningeal leukemia occurred in two children during hematologic remission of acute lymphoblastic leukemia. Treatment of CNS leukemia consisted of radiotherapy and repeated intrathecal injections of methotrexate and cytosine arabinoside. Intrathecal installations of the drugs were accompanied with increasing neurological side effects which seemed to be lifethreatening. To avoid them, repeated courses of pyrimethamine were introduced. No further relapse of meningeal leukemia occurred after pyrimethamine. Hematologic remissions in both children have lasted for over 7 years. All therapy was stopped 16 months ago.

MVPP chemotherapy combined with radiotherapy in the treatment of Hodgkin's disease in children.

Thirty-four children with Hodgkin's disease were treated during the years 1969--75. After radiotherapy, 7--15 cycles of MVPP were given within 24--53 months. In order to avoid chronic leukopenia, leukocyte counts were made frequently during chemotherapy, and the drug doses adjusted accordingly. A complete remission was obtained in 32 of the 34 children. Two patients died because of progressive disease. Twelve of the 32 survivors have been followed for at least 5 years, and a further 12 for at least 3 years. Three children are still on chemotherapy, whereas the remaining 29 being followed are in continued complete remission.
