[Testicular trauma and its significance for male fertility].

The clinical evaluation of 8 patients after surgical treatment of unilateral testicular trauma showed a decreased testicular volume in 3 patients. The serum endocrine profiles were within normal range--except 1 patient with atrophy of one testicle. In all ejaculates investigated (n = 6) a moderate to high asthenozoospermia was shown. The density was normal in 5 of 6 patients, but 4 patients had a teratozoospermia. The possible causes of pathological spermiogram after rupture of the testis is discussed.

[Postoperative course following interventions for "septic kidney"].

It is reported on 61 patients who have been operated on for a "septic kidney". In 30 patients a primary nephrectomy was performed. One of these patients died immediately after the intervention in septic shock. In 31 patients a kidney sparing procedure was carried out, was successful in 80% (during an average observation period of 5.1 years). In most of these cases a transitory nephrostomy was performed. In this group also one patient died postoperatively of urosepsis. The presented data should facilitate the decision to an organ sparing procedure in case "septic kidney".

[Dermatoglyphics in families with Potter type III polycystic kidney degeneration].

Several dermatoglyphic studies of patients suffering from genetically determined disorders which are not manifest at birth, indicate an early direct or indirect influence of the causative factors on the formation of dermatoglyphics. On the basis of these observations the present study gives an analysis of the quantitative and qualitative dermatoglyphic characteristics of fingers, palms and soles, as well as body and head measurements and morphological traits of the head and face of nine patients with adult polycystic kidney disease (APCD) type III and of their first and second degree relatives. No malformations nor any significant variation of these variables were observed, i.e. no differences were recorded between patients and controls or patients and their healthy relatives. Yet some interesting trends appeared in dermatoglyphic ridge counts when special methods for intrafamilial comparison were applied. Thus, APCD type III ridge counts on fingers and palms were somewhat lower when plotted against their mid-parent values than those of their healthy sibs. Nevertheless, only studies of additional family material may permit the conclusion that such a constellation is due to type III APCD, and not merely a spurious finding.

[Initial experiences with the neodym YAG laser in urology].

The present study reports the therapeutic results obtained by using neodym-YAG laser for the coagulation of tumours of the external genitals (n = 30), as well as for endoscopic laser treatment of tumours of the bladder (n = 32) and the urethra (n = 2). Despite the small number of cases, laser treatment of bladder tumours (observation period 6-33 months) appears to reduce the orthotopic recurrence rate, which is in aggrement with other publications. However, randomized studies will be necessary to prove the advantages of the above-mentioned method as compared with electroresection. Neodym-YAG laser also appears to be a valuable extension of the urological therapeutic armamentarium for external application, especially in penile carcinoma at stage T1 + T2, N0, M0.

The accuracy of transrectal aspiration biopsy of the prostate: an analysis of 452 cases.

This study compares cytological findings obtained by transrectal aspiration biopsy of the prostate with histological findings obtained by simultaneously performed transperineal punch biopsy. All interventions were performed on outpatients. Complications such as hematuria, urinary infection, or epididymitis occurred in 4.4% of the cases, the majority of them being attributed to punch biopsies. Exact correspondence was found in 78.8% of 433 usable specimens, the accuracy for carcinoma amounting to 71.1%. Specimens cytologically judged either false "negative" or "suspicious" were histologically mostly well-differentiated (G1 and G2) carcinomas (96.1%). The value of fine needle biopsy of the prostate as nonstaining diagnostic procedure will be discussed.

[Demonstration of bacteria in the ejaculate of subfertile men, with special reference to Chlamydiae].

Bacterial cultures were obtained from ejaculates of 87 infertile males with pathologic spermiogram (according to the classification of Elliason), and the presence of chlamydia was also examined. In no case chlamydia could be proved. In only 8% bacteria could be found in a pathogenic number (greater than 10(4)). The importance of asymptomatic bacteriospermia in infertility is discussed.

[Significance of the incidental carcinoma of the prostate].

Incidental carcinoma of the prostate was detected in 58 patients after suprapubic prostatectomy for benign hypertrophy carried out in 942 men between 1962 and 1982. A 5-year follow-up was possible in 34 cases. The survival rate was assessed according to staging and grading of the carcinoma whereby the tumours were retrospectively reclassified according to Jewett. The therapy of incidental carcinoma of the prostate and 5-year survival rate are discussed on the basis of a survey of the literature.

[Drug prevention of recurrence in bladder tumors. Intravesical adriblastin instillation following transurethral resection of non-infiltrating bladder tumors].

29 patients were treated with intravesical adriamycin-instillations after transurethral resection of non-infiltrating bladder tumor. 50 mg Adriblastin were administered every four weeks during one year. In no case there were severe side-effects. Endoscopic controls were performed every three months. 12-28 months after beginning of therapy 6 recurrences (20,7%) were noted.

[Therapy of urinary tract infections with dibekacin].

The use of a new aminoglycosid antibiotic (dibekacin = Orbicin) in the treatment of severe urinary tract infections in a urological clinic is presented. After having given their written informed consent, 50 patients were treated with a dosage of 150-225 mg dibekacin, administered in three single daily doses. From a total of 41 mono-infections, asepsis could be attained in 70,7%, in 17,1% no effect could be observed and in 12,2% it came to a change of bacteria. From a total of 9 mixed infections, asepsis was observed only twice after the end of therapy. 87,5% of the patients reported about a far-reaching improvement or complete disappearance of clinical symptoms. In 2 cases allergic exanthema was observed. The local toleration was good. Disturbances of the otovestibular function were not registered.

[Value of bladder washing cytology in the diagnosis and follow-up of bladder tumors].

It is reported on the accuracy of the cytological examination of freshly voided urine of 247 patients with 318 urothelial bladder tumors, and the results are compared with the evaluation of bladder washing cytology in 82 patients with 105 bladder tumors. If voided urine is used for examination, there is no evidence in tumors with 0 and 1 grade of malignancy, whereas the bladder washing cytology of grade 1 tumors shows a correlation for the histological findings in 26.3%. In tumors with malignant grades GII and GIII, the bladder washing cytology shows a significantly higher accuracy (81.4%) than in examination of voided urine (67.2%). In both methods the accuracy of cytology rises with infiltration and differentiation grades of the tumor.

Pregnancy rate after homologous insemination of pathological ejaculate both with and without the addition of kallikrein.

As part of a gynecological-andrological cooperation programme for the treatment of infertile marriages made by the 1st Department of Obstetrics and Gynecology and the Department of Urology, University of Vienna Medical School, the pregnancy rates achieved through 1,269 homologous inseminations of pathological ejaculates (according to Elliason's classification) both with and without addition of kallikrein was analysed retrospectively. In the case of 131 married couples, only 5 pregnancies occurred without the addition of kallikrein, and where kallikrein was used, there were also 5 pregnancies in 75 couples (6.66%). No conception occurred when cryoejaculate was used in addition. There were altogether 11 cases of spontaneous pregnancies. Conception could only be induced if an ejaculate of relatively high quality with a spermatozoa count of more than 12 million/ml and a 2-hour motility of more than 28% was used for insemination. The problems associated with homologous insemination with pathological ejaculate are being discussed.

[Therapy of idiopathic oligozoospermia with bromicriptine. Results of a prospective controlled study].

The effect of bromocriptin on semen parameters and endocrine profiles in 25 males presenting idiopathic oligozoospermia was evaluated in a prospective controlled clinical trial versus 25 patients treated with placebo. Following results were obtained: a) Serum concentrations of prolactin decreased significantly during bromocriptin treatment while FSH, LH and Testosterone concentration remained unchanged. b) Sperm count, motility and morphology were not significantly affected by bromocriptin versus placebo. c) Conception rate was not significantly different between the treatment and placebo group. From the results of this study it can be concluded that bromocriptin is no suitable agent in the treatment of idiopathic, normoprolactinemic oligozoospermia.

[Complications after suprapubic prostatectomy (author's transl)].

From 1962 to 1980 a number of 856 suprapubic transvesical prostatectomies were performed at the Urological University Hospital, Vienna. An analysis of the patients showed a postoperative mortality rate of 2.21% and decisive differences between the first (4.37%) and second (0.74%) decade. Death was primarily caused by pulmonary embolism. In about 9% of the cases a reduced renal function was determined, which, however, did not affect the rate of complications. In more than 9% of the patients early postoperative complication was most frequently determined by the incidence of epididymitis.

[Correlation between serum FSH and histological findings on testicular biopsy in men with azoospermia and severe oligozoospermia].

In 51 infertile men with azoospermia or severe oligozoospermia the histological findings on testicular biopsy were compared with the preoperative serum FSH level. In cases of absence of spermatogenesis on testicular biopsy the FSH showed the expected highly significant "feed back" increase, whereas in cases of functioning, but highly pathological spermatogenesis the serum FSH level gave no indication of the type of dysspermatogenesis. Normal FSH levels were found in over 90% of men showing normal spermatogenesis with obstructive azoospermia. Hence, complete inspection of the testis, with attempted demonstration of spermatozoa in the epidydimis, is essential in these cases during testicular biopsy in order to avoid an unnecessary secondary operative refertilizing procedure (epidydimovasostomy) when the site of obstruction is the rete testis.

[Spermatological changes after ligature of the spermatic vein for idiopathic varicocele].

Spermatological examinations were performed on 61 patients before and after varicocelectomy. Sperm morphology remained unchanged, but the sperm density increased in 67.2% cases and in 54.1% the sperm motility rose over a period of up to 18 months following the treatment. In 72.1% of the cases an improvement in at least one ejaculate parameter was noted. Of 52 patients who underwent the operation because of infertile marriage, the patient's wife became pregnant in 23.1% following varicocelectomy. Pathogenesis of the varicocele and the problems involved in ligaturing the spermatic vein for male subfertility are discussed.

[Therapeutic application of kallikrein in idiopathic oligo- and asthenozoospermia (author's transl)].

The addition of kallikrein to frozen semen after thawing increases the sperm motility in cases of moderate asthenozoospermia. In native semen maximal increase of motility is found two hours after the addition of kallikrein. In vivo an increase of sperm motility was noted in cases of oligozoospermia and asthenozoospermia during treatment with 600 KE kallikrein/day. The wives of 9 patients with asthenozoospermia became pregnant during kallikrein therapy.
