[A case of disseminated intravascular coagulation (DIC) in a cow with endometritis and fetal death].

Acute disseminated intravascular coagulation (DIC) was diagnosed in a 3 1/2 year old cow of the Simmental breed. The cow was little less than 6 months pregnant and was admitted to the clinic because of severely disturbed general health. The most important clinical findings were increased heart and breathing rate, rectal temperature of 39.9 degrees C, nosebleed and petechiae on the nasal mucosa. Additionally, the cow showed petechiae on the vaginal mucosa, haemorrhage from the rectum lasting several hours after rectal examination and severe haemoglobinuria. Haematological and biochemical examinations showed increased liver enzymes and severe changes in all coagulation parameters (platelet count, PT, PTT, thrombin time, fibrinogen, fibrin degradation products). Based on the mentioned findings the diagnosis DIC was made. Possible causes were severe necrotic endometritis and placentitis combined with fetal death. High counts of Escherichia coli and Clostridium perfringens were determined in liver, lung and abomasal contents of the aborted fetus as well as in the placenta. Uterine secretion contained Actinomyces pyogenes besides.

Therapy and clinical progress of cattle with dilatation and torsion of the caecum.

One hundred and eleven heifers and cows with caecal dilatation and torsion were examined and their subsequent progress monitored. Using various criteria it was decided whether the animal was to be slaughtered or treated conservatively or surgically. Conservative treatment consisted of a continuous drip infusion containing neostigmine and of purgatives such as liquid paraffin, sometimes in combination with sodium sulphate. Surgical treatment consisted of laparotomy on the right flank with emptying and sometimes partial resection of the caecum. Five animals had to be slaughtered before or during surgery and another five which developed severe peritonitis were slaughtered after surgery. Fifty-nine animals underwent surgery once without resection of the caecum and 20 with. Another 14 animals needed surgery twice.

Haematological and biochemical findings in cattle with dilatation and torsion of the caecum.

A biochemical examination was made of the blood and rumen fluid of 111 heifers and cows suffering from caecal dilatation, with or without torsion. Haematological values were normal in the majority of cattle. Concentrations of chloride were normal in the rumen fluid of 83 per cent of the animals and higher in the remainder. Nine cows that had to be slaughtered had higher bile concentrations than those which recovered. Twenty-eight per cent had increased blood urea concentrations probably due to dehydration.

Clinical findings in cattle with dilatation and torsion of the caecum.

Dilatation and torsion of the caecum is usually an acute illness with signs of ileus and this paper presents the clinical findings from 111 heifers and cows with the condition. On admission 61 per cent of the animals had been ill for less than 24 hours; the general condition was disturbed in 58 per cent of the animals, 39 per cent showed signs of colic; the rectal temperature was abnormal in 55 per cent, the heart rate in 40 per cent and the respiration rate in 52 per cent of the animals; rumen motility was reduced or absent in 85 per cent. One or more tests for foreign bodies were positive in 58 per cent, and swinging auscultation and percussion auscultation on the right side, either alone or in combination, were positive in 83 per cent of the animals. On rectal examination it was possible to palpate a pathologically distended, displaced or twisted caecum in 95 per cent of the animals. In 87 per cent defecation was reduced or absent. The most reliable diagnostic investigations were rectal examination and swinging and percussion auscultation on the right side.

Reflux of bile acids from the duodenum into the rumen of cows with a reduced intestinal passage.

The purpose of this study was to find out whether the concentration of bile acids in the rumen of healthy cows differs from that of cows with a reduced gastrointestinal passage. The investigations were performed on 277 heifers and cows of the Swiss Braunvieh, Simmental and Swiss Holstein breeds. Forty of these had traumatic reticuloperitonitis, 30 had right or left abomasal displacement, 14 had an ileus of the duodenum, 36 had an ileus of the jejunum and 54 had a caecal dilatation. The other 103 animals had disorders which did not affect the digestive system and were used as controls. The mean (+/- se) bile acid concentration in the rumen fluid of the control animals was 1.0 +/- 0.30 mumol/litre. The bile acid concentrations in the rumen fluid of the cows with a duodenal or jejunal ileus were significantly higher than those of the other groups, demonstrating that bile acids may reflux from the duodenum into the rumen of cows with an ileus in the small intestine.

Purification of the 22 kDa protein substrate of botulinum ADP-ribosyltransferase C3 from porcine brain cytosol and its characterization as a GTP-binding protein highly homologous to the rho gene product.

The 22 kDa protein substrate of botulinum ADP-ribosyltransferase C3 was purified from porcine brain cytosol by acetone precipitation, CM-Sephadex, octyl-Sepharose and TSK phenyl-5PW HPLC chromatography to apparent homogeneity. ADP-ribosylation of the protein was increased by guanine nucleotides (GTP, GDP, GTP gamma S, each 100 microM) but not by GMP, ATP or ATP gamma S. The purified 22 kDa protein bound maximally 0.9 mol [35S]GTP gamma S and hydrolyzed GTP with the rate 0.007 mol per mol protein. Amino acid sequences were obtained from two tryptic peptides, selected from an in situ digestion of Immobilon electrotransferred, gel purified ADP-ribosylated substrate. The two sequences obtained, cover 23 residues from the corresponding sequences in human rho.

The rho gene product expressed in E. coli is a substrate of botulinum ADP-ribosyltransferase C3.

The ras-related rho A protein expressed in E. coli, was ADP-ribosylated by botulinum ADP-ribosyltransferase C3. C3 also modified the valine-14 mutant rho protein but not the products of H-ras, R-ras, ral, ypt, and rap 1 genes. A ras-rho chimaera consisting of 60 amino acids from the amino terminus of ras fused to 133 amino acids from the carboxy terminus of rho was not modified by C3. Antibodies raised against the porcine brain cytosolic substrate of C3 cross reacted with the rho, valine-14 rho and ras-rho proteins, but not with the gene products of H-ras, R-ras, ral or rap 1. Polyclonal anti-H-ras antibodies cross reacted with H-ras but not with ral, rho, or the C3 substrate purified from porcine brain.

Evaluation of methods for indirect calorimetry with a ventilated lung model.

A combined lung and ventilator model was built, validated and used to test commercial systems for indirect calorimetry. It simulates O2 uptake and CO2 excretion under ventilator treatment conditions. In the model inspiratory gases are diluted with N2 and CO2 to give the desired expiratory concentrations. Minute volume, FIO2, ventilatory pressure, VO2, VCO2 and consequently RQ can be altered to simulate the adult clinical situation. A selected respiratory pattern is maintained by the lung model. Equipment for indirect calorimetry can then be connected to it and the results compared. Reference values are derived from measurements with a mass spectrometer and a Godart spirometer. Three commercially available instruments (Beckman MMC, Horizon MMC and Engström MC) were evaluated with this system. The limits of agreement with the reference values under different conditions (FIO2 0.4-0.7, ventilatory pressure 0-50 cmH2O) were determined. Differences as high as 15% from the true values of VO2 and V CO2 were observed. The pattern of mechanical ventilation and the intrinsic properties of the analyzers in the equipment used for indirect calorimetry influence measurements to a significant extent.

Cerebral and peripheral demethylation of psychotomimetics measured by expired 14CO2.

Demethylation of psychotomimetic compounds was measured by labeling each methyl and methoxy substitutent separately with 14C and injecting it into rats, intravenously and intracerebrally. Expired [14C]CO2 was measured continuously and the resultant multi-exponential curves yielded rates and integral demethylation. 5-Methoxy-N,N-dimethyltryptamine was not demethylated, eliminating one proposed metabolic pathway. 2,4,5-Trimethoxyphenalkylamines were demethylated less in the brain than peripherally, markedly so at the p-methoxy position, suggesting a possible biochemical site for endogenous induction of psychosis.

[Laryngeal and hypopharyngeal carcinoma--the limits and advantages of sonography].

29 patients with carcinoma of the larynx and the hypopharynx were evaluated not only for staging cervical lymph nodes but also for classification of the primary tumour. With appropriate technique it is possible to assess tumour infiltration (T4) of surrounding tissue of the larynx at all levels. Tumour classification T1 to T3 is possible in patients with tumour of the epiglottis (regio supraglottis, UICC). The endolaryngeal tumour T1 to T3 (regio glottis and subglottis, UICC) still remains the domain of the microlaryngoscopy. The limits of the method are apparent in hypopharynx carcinomas where in only 50% of the patients the tumour could be assessed.

[Bone metastases diagnosed by ultrasound].

Osteoclastic bone metastases - mostly associated with soft tissue infiltration - but also osteoblastic metastases present a typically sonographic appearance. These patterns are discussed in two cases. Nearly 50% of skeletal metastases are located in sonographically easily accessible regions. The use of this low-cost and simple method complementary to other diagnostic procedures is discussed.

[A.L. Lavoisier and the anesthetist].

The activity of the anesthesiologist is centered upon the respiratory organ function; therefore, the question of how the fundamental knowledge of gas exchange was gained seems justified. During the 18th century, the elegant but incorrect phlogiston theory (G.E. Stahl, 1659-1734) dominated all theoretical conceptions about combustion and respiration. Chemistry at that time was a collection of observations without any sound hypothetical background. Our knowledge about oxidation, combustion, and respiration was developed mainly by the work of A.L. Lavoisier (1743-1794). He observed that metals gain weight during their calcination (oxidation) and concluded that air is taken up during this process. In these experiments he also proved that air consists of two elastic fluids (gases), one respirable and the other nonrespirable. After experiments with a sparrow breathing in a closed vessel until it expired, Lavoisier stated that during respiration respirable air (O2) is changed into fixed air (CO2) in the lungs with the mephitic portion of the air (N2) not taking part. These were the first investigations of respiration where accurate and reproduciable methods were applied. Respirable air is also transformed into fixed air by a burning candle. After investigations by which thermochemistry was founded, respiration could be defined as a slow form of combustion. In these and other biologically oriented experiments, Lavoisier developed direct and indirect calorimetry. He named respirable gas oxygen and explained the composition of water. In refuting the phlogiston theory, he established modern chemistry. His fate was sealed during the French revolution: in 1794 he was executed as a consequence of his activities as a farmer.(ABSTRACT TRUNCATED AT 250 WORDS)

[Monitoring the oxygen supply of skeletal muscle and total oxygen uptake in coronary surgery interventions].

In patients undergoing aortocoronary bypass operations, extensive monitoring is used for early recognition of complications that may decrease the oxygen supply of body organs. However, none of the parameters usually monitored during open-heart surgery yield information on the state of oxygen supply to a certain organ. Particularly during and after extracorporeal circulation (ECC), undetected organ hypoxia may occur and lead to an increase in postoperative complications. In order to study whether functionally significant changes in oxygen supply to skeletal muscle occur during extracorporeal circulation, in 14 patients undergoing aortocoronary bypass operations pO2 histograms from local pO2 values in resting skeletal muscle were monitored. Intra- and postoperatively, whole-body oxygen uptake (VO2) and pO2 histograms of skeletal muscle were simultaneously measured to determine whether the observed decrease in VO2 during cooling of the patient or the increase in VO2 during rewarming was reflected in the distribution of tissue pO2 in skeletal muscle. PATIENTS AND METHODS. Fourteen patients (aged 42 to 68; 12 male, 2 female) scheduled for 2-4 aortocoronary venous bypass grafts were studied. Measuring periods of 6 min were undertaken after induction of anesthesia (1), after thoracotomy (2), during ECC (3), after ECC (2), and every hour postoperatively up to 5 h. The following data were measured during each period: pO2 histograms of the biceps brachii muscle using a pO2 histograph VO2 using open indirect calorimetry; arterial (a. radialis) and venous (v. cava superior) blood gases, acid-base balances, and blood pressures; venous lactate (v. cava superior); muscle and rectal temperatures. RESULTS. During ECC mean muscle pO2 in all 14 patients decreased from 25 to 14 mmHg. In 7 of 14 patients pO2 values between 0 and 5 mmHg had the highest incidence (left-shifted pO2 histograms). After ECC mean muscle pO2 increased to the same value observed before ECC (25 mmHg). During the first 3 h postoperatively an intermediate decrease in mean muscle pO2 (range 23%-46%) occurred in each patient. Stable values above 20 mmHg were reattained 4 h postoperatively. The postoperative increase in individual VO2 was correlated to the increase in body temperature (r = 0.84); in 5 patients with severe shivering, VO2 was markedly increased (up to 130% compared to the last intraoperative value).(ABSTRACT TRUNCATED AT 400 WORDS)

[Hypotrichosis and oligodontia, combined with an Xq-deletion, in a calf of the Swiss Holstein breed].

Hypotrichosis and oligodontia associated with a chromosomal anomaly (Xq-deletion) are described in a 11-month old cattle (Simmenthal/Red Holstein cross-breed). This chromosomal anomaly was accompanied with hairlessness and grievous teeth abnormalities. The animal had a very thin haircoat, had only one incisor and between one to three molars per mandible or maxilla. This resulted in reduced food intake, reduced rumination, and retarded growth. Post-mortem examination revealed lesions in the kidneys (bilateral chronic interstitial nephritis), adrenals (hyperplasia), pancreas (focal fibrosis) and abomasum (obstipation and multiple ulcers). Some of these abnormalities are comparable with the human "anhidrotic ectodermal dysplasia" (Christ-Siemens-Touraine syndrome) and supports the hypothesis that there are homologies in the X-chromosome of different mammals.
