Selenium and vitamin E in blood sera of cows from farms with increased incidence of disease.

Selenium and vitamin E concentrations were measured in the blood sera of 287 cows in 91 herds. Herds with chronic mastitis and muscle diseases had significantly lower selenium concentrations (10.4 +/- 5.26 and 11.7 +/- 8.63 micrograms/litre serum, respectively) than healthy control herds (17.7 +/- 15.00 micrograms/litre). The selenium concentrations in herds with low fertility did not differ from control herds. The vitamin E concentrations were similar in all the herds. Supplementation of the mineral mixture with 20 mg sodium selenite and 2 g vitamin E/kg on 15 farms with a deficient selenium supply increased the selenium concentrations in the blood within two months in 12 of the herds. The vitamin E concentrations were hardly influenced by the supplementation.

Interaction of the cholinergic system and cholecystokinin in the regulation of endogenous and exogenous stimulation of pancreatic secretion in humans.

Pancreatic enzyme secretion is regulated in humans by the cholinergic system and by cholecystokinin (CCK). The interaction between both regulatory systems in response to exogenous and endogenous stimulation was analyzed in the present study using the cholinergic antagonist atropine and the CCK antagonist loxiglumide. A dose-dependent stimulation of pancreatic enzyme output was achieved either by duodenal perfusion of graded caloric loads or by IV infusion of increasing doses of cerulein. Prestimulated pancreatic secretion was inhibited by atropine and loxiglumide. Atropine furthermore almost completely blocked meal-stimulated pancreatic secretion, whereas loxiglumide caused 60% inhibition. The enzyme response to graded doses of exogenous CCK was significantly inhibited by atropine and loxiglumide. Plasma levels of CCK were not altered by atropine but increased with infusion of loxiglumide. This study supports the concept that pancreatic enzyme secretion is predominantly dependent on a cholinergic tone and that CCK modulates the enzyme-secretory response.

Type 1 abomasal ulcers in dairy cattle.

Abomasa from 912 randomly selected cows were examined; specimens were obtained at the local slaughter house on 35 days spread over one year. Abomasal lesions were assessed macroscopically and histologically. Additionally, haematological and blood chemistry (urea, aspartate aminotransferase, potassium, chloride, calcium) evaluations and the determination of rumen chloride concentration were performed. Of the 912 abomasa examined, 187 (20.5%) had ulcerative lesions of the mucosa. Lesions were classified from 1 to 4 based on severity as described by Whitlock (1980). All ulcers were classified as type 1 (erosions and non-perforating ulcers); thus, further division into four subtypes 1 a, 1 b, 1 c and 1 d was carried out. Fifty-six abomasa had minimal mucosal defects which were classified as type 1 a. Deeper erosions combined with local hemorrhage, classified as type 1 b, were observed in 54 abomasa. Type 1 c were crater-like ulcers and were seen in 61 abomasa. Sixteen abomasa had type 1 d ulcers which included two forms: ulcers with radial wrinkles converging on a central point, and ulcers with perforated folds. Types 1 a and 1 c occurred mainly in the pyloric region, and types 1 b and 1 d were observed mainly in the fundic region. Type 1 abomasal ulcus could not be diagnosed based on alterations in haematological or blood and rumen chemistry values.

Ultrasonographic examination of the right kidney in cows.

To determine the position, dimensions, and structure of the right kidney in cattle by use of ultrasonography, the right kidney of 11 healthy Brown Swiss cows was examined 10 times within 2 weeks. A 3.5- and 5.0-MHz linear and convex transducer was placed on the right side of the cow in the lumbar region, in the paralumbar fossa, and in the last intercostal space. The echogenicity of various renal structures differed. The lobulation of the kidney in cattle could be visualized ultrasonographically; however, the cortex and medulla could not be differentiated. The distance between body surface and the right kidney was almost 3 times larger (5.3 +/- 1.71 cm, mean +/- SD) in the lumbar region than in the paralumbar fossa (1.8 +/- 0.52 cm). The vertical diameter of the kidney was remarkably smaller (5.1 +/- 0.47 cm) than the horizontal diameter (9.4 +/- 0.98 cm). In 7 cows, the thickness of the renal cortex and medulla was between 1.9 and 2.1 cm. The medullary pyramids could be visualized when the transducer was placed in the paralumbar fossa. Fourteen of 19 variables measured had a coefficient of variation between 8 and 14%. It was concluded that the ultrasonographic values determined in this study can be used as references for the diagnosis of morphologic changes in the right kidney of domestic dairy cattle.

[Comparison of lisinopril and captopril in treatment of severe heart failure (NYHA III-IV) in high risk patients. Preliminary results of the trial].

We present preliminary data of a study comparing captopril, a short acting, with lisinopril, a long acting ACE-inhibitor in 8 of 12 projected patients with severe chronic heart failure (NYHA III-IV) and one additional risk factor (e.g. diabetes mellitus, renal failure). The 8 patients were treated in a cross over design for 12 weeks with either drug. While lisinopril improved NYHA-class in all patients, captopril reached this goal in only 3. Renal function was stable in all patients. Captopril influenced hormones (renin, aldosterone, norepinephrine, epinephrine) and microalbuminuria less than lisinopril. The number of adverse reactions was smaller in lisinopril treated patients. These preliminary data demonstrate at least an equal efficacy of lisinopril compared to captopril in high risk patients with severe chronic heart failure.

[An unnoticed dislocation of the dens axis in a patient with primary chronic polyarthritis undergoing intensive therapy].

Involvement of the cervical spine is seen in 40%-60% of all patients with rheumatoid arthritis. Consequences are instability of the upper cervical column with pain and neurological deficits, in some cases tetraplegia, and sudden death. From this reason special care has to be taken in the management of those patients, even when they are comatose or anesthetized, to avoid sudden spinal cord compression with irreversible neurological deficits. We report a 49-year-old female with a history of rheumatoid arthritis for more than 10 years. Because of an adhesive ileus complicated by septicemia, she underwent abdominal surgery twice followed by prolonged mechanical ventilation under high doses of sedative drugs. After reduction of the tranquilizer doses tetraplegia with respiratory insufficiency was found. Neurophysiological and X-ray examinations showed spinal cord compression due to dislocation of the odontoid process, a rare but typical complication in patients with rheumatoid arthritis. It was not possible to determine the date of the dislocation, but it might have been caused by intubation or respositioning. Although the patient underwent immobilization and surgical fusion of the upper cervical spine, there was no improvement in the neurological status and she died 5 months later. In patients with advanced rheumatoid arthritis a detailed medical history, clinical examination, and radiography are necessary before general anesthesia or intensive care with intubation is considered. If an unstable cervical spine is suspected, intubation should be performed by fiberoptic technique under light sedation. Regional anesthesia should be preferred over general anesthesia.(ABSTRACT TRUNCATED AT 250 WORDS)

[Differential diagnosis of a low echo thyroid gland].

Hypothyrosis as a result of inflammatory disease of the thyroid gland, often is a diagnostic problem because of the insidous onset and its lack of specific symptoms. In the rather large amount of literature about sonography of the thyroid no papers with a systematic approach to this entity are found. In a prospective study we evaluated 36 proven cases and found a characteristic sonographic pattern of the thyroid with regard to its size, contour and structure.

Cholecystokinin is a regulator of intestinal phase-stimulated PP release.

The purpose of this study was to determine the role of CCK during the intestinal phase of pancreatic polypeptide (PP) release in man. We first compared the PP response to exogenous caerulein infusion in the presence or absence of either loxiglumide (a specific CCK antagonist) or atropine in six healthy subjects. In the second part of the study, a meal was perfused to the duodenum with and without either loxiglumide or atropine. Both loxiglumide and atropine completely abolished the PP response to exogenous or endogenous stimulation (P less than 0.05). We conclude that CCK participates in the intestinal phase of PP secretion.

[Hoflund syndrome due to anterior functional stenosis in 20 cows].

This study gives a comprehensive survey on 20 cows with vagus indigestion due to cranial functional stenosis (failure of omasal transport). The most important clinical findings were distension of the entire lateral abdominal wall on the left side and partially of the right ventral area as well, severely distended rumen, reduced or missing appetite and reduced defaecation. Nine animals had bradycardia. The dominant laboratory findings were increased concentrations of total protein and fibrinogen. Sixteen cows were slaughtered at the clinic. Six of these had signs of peritonitis in the cranial part of the abdomen, which had started from foreign body peritonitis. Six cows had abscesses between reticulum and liver, also starting from a foreign body, one cow had one isolated abscess in the liver and one cow severe fatty degeneration of the liver. In 2 cows no pathological changes could be found on the occasion of slaughter.

Clinical, haematological and biochemical findings and the results of treatment in cattle with acute functional pyloric stenosis.

The clinical features and changes in blood and rumen fluid, and the results of therapy are described in 10 cows suffering from acute functional pyloric stenosis. The general condition of the cows was moderately to severely disturbed. The abdomen of most of them was distended on one or both sides and the rumen was excessively full. Defecation was reduced or absent. In most of them there was moderate or severe abomasal reflux-syndrome. Exploratory laparotomy or slaughter revealed a grossly distended abomasum which was filled with ingesta but not displaced. The omasum, reticulum and rumen of most of the cows were dilated secondarily and filled with ingesta. Six of the cows were treated by the administration of a solution of sodium chloride, glucose and potassium chloride intravenously, and metoclopramide intramuscularly. Five cows recovered within a short time, general condition, appetite and defecation were again normal and the abomasal and ruminal function returned within three days.

[Sonography of the lungs and pleura].

Diagnoses were made by ultrasound in 73 out of 80 cases with unclear thorax x-ray. One case was misinterpreted, no diagnoses were possible in 6 cases. Pleural effusion or opacities reaching the pleura are necessary to perform ultrasound examination. Sonographic findings in pleural or pulmonary changes are discussed and correlated with pathological anatomical findings. Additional information can be obtained by sonography in pleural lesions and in pulmonary lesions masked by pleural effusion. Atelectases can be differentiated from pneumonia, and tumours can be identified in peritumorous pneumonia or atelectases.

[Intraoral dental fixation of an endotracheal tube using a cofferdam clamp].

UNLABELLED: Extraoral tape fixation of the orotracheal tube for general anesthesia is a major problem in maxillofacial surgery. First of all, surgical access to the perioral and nasal region is handicapped by the tape covering and distracting the skin, especially in those cases where no nasotracheal intubation is possible; furthermore, blood, saliva, and disinfectant fluid interfere with reliable adhesion of the tape. A method of intraoral dental fixation of the orotracheal tube by a rubber dam fixation clamp is presented. The rubber dam isolates teeth against the oral cavity, providing a dry operative field to the dentist. The set of clamps, each designed according to the individual anatomy of the different teeth, is usual in the dental trade. After intubation the selected clamp is placed on the tooth by means of the forceps. The tube is laid on the clamp and tied on by a silk thread (2 x 0), which is inserted through the clamp's holes. We recommend fixation to the teeth in the mandible to avoid tension load, which could strain teeth in the maxilla; as far as possible only teeth without any impairment (e.g. loosening) should be selected. CONCLUSION: The method of intraoral dental fixation of the orotracheal tube by a rubber dam clamp offers the following advantages: (1) the surgeon, especially the maxillofacial surgeon, has a good view of the perioral region and free access for surgery; there is (2) no skin distraction or irritation by tape; there is (3) reliable tube fixation even for patients with allergy to adhesive materials; there is (4) no solution of tape fixation by blood, saliva, or disinfectant fluid; and (5) silk sutures cannot be subjected to strain when solving tape fixation.

Ultrasonographic examination of the liver in cows.

The size of the liver, as well as the situation and diameter of vessels in cattle were determined by use of ultrasonography. Ultrasonographic examinations of the liver were performed on 10 cows 10 times within 2 weeks, using a 3.5-MHz linear transducer on the right side in the 12th, 11th, and 10th intercostal spaces. Dorsal and ventral margin of the liver as well as localization and diameter of the caudal vena cava and the portal vein were determined in each intercostal space. Furthermore, the angle of the liver in the ventral area between the visceral surface and the diaphragmatic surface, the dorsal margin, and the circumference of the gall bladder were determined. The ultrasonographic values of liver size and localization in healthy cows can be used as reference values for the diagnosis of changes in liver size attributable to illness.
